  • The tracheobronchial tree is a system of airways that allows the passage of air to aerate the lungs and entire body. Several pathological conditions can affect this anatomical region. Multidetector CT (MDCT) helps identify and characterize various large airway diseases. Post-processing tools, such as virtual bronchoscopy and automatic lung analysis, can help enhance the performance of imaging studies. In this pictorial essay review, we provide imaging findings of various bronchial lesions manifested as wall thickening and endoluminal nodules on conventional MDCT and advanced image visualization and analysis.

The Comparison of Usefulness between MDCT Angiography and DSA in the Diagnosis of Intracranial Aneurysm (뇌 동맥류 진단에 있어 디지털 감산 혈관조영술과 다중검출기 전산화단층촬영 혈관조영술의 유용성 비교)

  • The purpose of this study was to compare the image between DSA and MDCT Angiography and to examine whether MDCT Angiography could be useful as a screening test for the diagnosis of cerebral aneurysm in patients who were diagnosed with cerebral aneurysm on DSA. Of patients who were diagnosed with cerebral aneurysm DSA at University Hospital, 194 patients who concomitantly underwent MDCT Angiography were enrolled in the current retrospective study. The methods for analyzing cerebral aneurysm were to analyze the presence of cerebral aneurysm on DSA and MDCT Angiography. In cases in which it exceeded 1, the corresponding cases were classified as narrow-neck aneurysms. In otherwise cases, they were classified as wide-neck aneurysms. Thus, a comparative analysis could be performed to ascertain if cases were narrow-neck or wide-neck aneurysms. As compared with DSA, the sensitivity of MDCT Angiography for cerebral aneurysm was measured to be 97.4%. The degree of consistency between narrow-neck and wide-neck aneurysms was 90.2% and the proportion of undetectable an at MDCT Angiography was 2.54%. mean size was 2.4 mm. It is expected that a non-invasive diagnostic modality for a screening test for cerebral aneurysm, MDCT Angiography might be a very useful regimen as compared with an invasive one, DSA.

Image Evaluation of Resolution Parameter and Reconstitution Filter in 256 Multi Detector Computed Tomography by Using Head Phantom (256 다중 검출기 전산화단층촬영에서 두개부 전용 팬톰을 이용한 분해능 파라메터와 재구성 필터의 영상 평가)

  • The purpose of this study was to evaluate of resolution parameter and reconstitution filter in the 256 multi detector computed tomography(MDCT) by using the head phantom. We used 256 MDCT, and head phantom of philips system. We evaluated to image quality by using Extended Brilliance Workspace. The protocol were axial scan method with 120 kVp, 0.5 sec of rotation time, 5 mm of slice thickness and increment, 250 mm of field of view(FOV), $512{\times}512$ of matrix size, 1.0 of pitch, $128{\times}0.625$ mm of collimations. The resolution parameter was applied for 'Standard', 'High' and 'Ultrahigh'. The reconstitution filters were changed to seven type of 'A', 'B', 'C', 'D', 'UA', 'UB', 'UC'. The assesment factors of image quality were the uniformity, the noise, the linearity and 50% and 10% of the modulation transfer function(MTF). Finally The good image quality in 'High' resolution parameter showed at the uniformity, the linearity and 50% and 10% of MTF. The 'UA', 'UB' reconstitution filter showed at the good image quality of the uniformity and the noise and 'C' reconstitution filter showed at the same result of the linearity and 50% and 10% of MTF.

Image Evaluation for A Kind of Patient Fixing Pad in 64 Multi-Channel Detector Computed Tomograph (64 다중채널 검출기 전산화단층촬영에서 환자고정자 재질에 대한 영상평가)

  • The purpose of this experiment intend to evaluate the quality of the image based on the orbit and basal ganglia with high radiosensitivity for the noise, SNR and dose using the five kinds patient fixing pad in brain phantom MDCT(BrillianceTM CT 64 slice, PHILIPS, Netherward). The noise had a higher values in AP than those of others, but the SNR was lower in AP than those of others. The SNR was higher in UP than those of RP, PP, SP and AP. The UP, RP and PP were no statistically significant(p>0.05), whereas it was significant difference between UP, RP, PP and SP, AP(p<0.05). This is causes of the noise difference is generated due to the differences in the radiation absorption dose in accordance with each the component of the absorbed dose level of the detector according to the reference line and each of SOML when the radiation exposured. The CTDIvol(mGy) and DLP of orbit and basal ganglia were 56.95, 911.50, respectively. There is no difference between both mean dose. In conclusion, it is possible to distinguish among a kind of 5 patient fixing pad by using brain phantom MDCT. Overall, patient fixing pad of UP, RP and PP based on a brain phantom MDCT can provide useful information.

Analysis of Bone Mellow Density in Adults of Domestic Local Area Using Multi-Detector Computed Tomography: Focus on Corelation About Eating Habits, Lifestyle, Physical Features and Social Characteristics (다중 검출 전산화단층촬영을 이용한 국내 일부 지역 성인의 골밀도 현황 분석: 식습관, 생활습관, 신체적, 사회적 특성과의 상관관계를 중심으로)

  • This study analyzed the correlation between BMD (bone mineral density) value calculated in the MDCT (multidetector computed tomography) and lifestyle, physical features and social characteristics. From July 15 2015 to June 6 2016, we converted from HU (hounsfield unit) value measured by using MDCT to T-score for BMD of 141 patients (male: 63, female: 78) in W medical center. We measured the 2nd, 3rd and 4th lumbar spine and analyzed the correlation between gender differences in BMD and lifestyle, physical features and social characteristics. Statistical significance was validated using independent sample T test with one way Anova. Gender BMD was confirmed that a statistically significant difference (p<0.05). BMD values decreased with increasing age but for the statistically men, there was no significant difference from 20s to 50s, it only showed a significant difference in 20s and 60s (p<0.001). For the statistically women, there was no significant difference from 20s to 40s. but since 50s BMD was decreased rapidly, which showed a significant difference (p<0.001). women showed significant differences for the menstruation and menopause, childbirth, alcohol, cereals and greasy food in bone mineral density (p<0.05) but there were no significant differences in men. The bone mineral density values calculated by the MDCT and lifestyle, physical features and social characteristics correlation analysis method is considered to be used as a basis for estimating the state in BMD and osteoporosis management.

Reducing of Craniofacial Radiation Dose Using Automatic Exposure Control Technique in the 64 Multi-Detector Computed Tomography (64 다중 검출기 전산화단층촬영에서 관전류 자동노출조절 기법을 이용한 두개부 방사선량 감소 정도 평가)

  • The purpose of this study was to evaluate the usefulness of reducing of craniofacial radiation dose using automatic exposure control (AEC) technique in the 64 multi-detector computed tomography (MDCT). We used SOMATOM Definition 64 multi-detector CT, and head of whole body phantom (KUPBU-50, Kyoto Kagaku CO. Ltd). The protocol were helical scan method with 120 kVp, 1 sec of rotation time, 5 mm of slice thickness and increment, 250 mm of FOV, $512{\times}512$ of matrix size, $64{\times}0.625\;mm$ of collimation, and 1 of pitch. The evaluation of dose reducing effect was compared the fixed tube current of 350 with AEC technique. The image quality was measured the noise using standard deviation of CT number. The range of craniofacial bone was to mentum end from calvaria apex, which devided three regions: calvaria~superciliary ridge (1 segment), superciliary ridge~acanthion (2 segment), and acanthion~mentum (3 segment). In the fixed tube current technique, CTDIvol was 57.7 mGy, DLP was $640.2\;mGy{\cdot}cm$ in the all regions. The AEC technique was showed that 1 segment were 30.7 mGy of CTDIvol, 340.7 $mGy{\cdot}cm$ of DLP, 2 segment were 46.5 mGy of CTDIvol, $515.0\;mGy{\cdot}cm$ of DLP, and 3 segment were 30.3 mGy of CTDIvol, $337.0\;mGy{\cdot}cm$ of DLP. The standard deviation of CT number was 2.622 with the fixed tube current technique and 3.023 with the AEC technique in the 1 segment, was 3.118 with the fixed tube current technique and 3.379 with the AEC technique in the 2 segment, was 2.670 with the fixed tube current technique and 3.186 with the AEC technique in the 3 segment. The craniofacial radiation dose using AEC Technique in the 64 MDCT was evaluated the usefulness of reducing for the eye, the parotid and thyroid with high radiation sensitivity particularly.

  • A defect analysis program of the composite motor case was developed to apply the ultrasonic signal processing method on basis of the ultrasonic pulse-echo method and the defects of FRP delamination and FRP/Rubber disbond in the composite motor case could be quantitatively measured. The defects detected in the composite motor case were in agreement with the results measured with the computed tomography and video microscope. This paper was described about the development process of the defect analysis program to convert the ultrasonic test data into the C-Scan image.

  • Purpose: In maxillofacial surgery, proper preoperative diagnosis is very important in achieving good postoperative results. Although conventional CT scans are useful for visual representations of fractures, they cannot provide direct guidance for reconstructing facial bone fractures. However, the recent technology of multislice scanning has brought many clinical benefits to CT images. Direct correlations can be made between preoperative imaging data and operative planning. The aim of the current study is to evaluate the differences between conventional CT and multidetective three-dimensional CT(3D MDCT) measurements in craniofacial deformities. Methods: From January 2005 to November 2005, MDCT scans of 41 patients were evaluated by comparing them with conventional CT scans. The 3D MDCT images were assessed and reviewed by using a simple scoring system. Results: The 3D MDCT scans offered easy interpretation, facilitated surgical planning, and clarified postoperative results in malar complex fractures, mandibular fractures, and extensive maxillofacial fractures and cranioplasty. However, 3D MDCT images were not superior to conventional CT scans in the diagnosis of blowout fractures. Conclusion: In spite of its limitations, the 3D MDCT provided additional and more comprehensive information than the conventional CT for preoperative assessment of craniofacial deformities. Therefore, the 3D MDCT can be a useful tool for diagnosis and systematic treatment planning in craniofacial skeletal deformities.

Geometry of Resident's ridge with Multidetector-Row Computed Tomograph Image (다중검출기 컴퓨터 단층 영상 분석을 이용한 Resident's ridge의 형태학적 연구)

  • Purpose: The purpose of this study was to report the real geometry of Resident's ridge doing in anterior cruciate ligament reconstruction Materials and Methods: From Jan 2007 to Aug 2007, 48 cases which had normal distal femoral condyle analyzed with Multidetector-Row Computed Tomography. Resident's ridge was defined as change of height above 1 mm in lateral wall of intercondylar notch. Anterior-posterior length of intercondylar notch, length and height of Resident's ridge, distance of Resident's ridge from posterior cortex were estimated with 3-D reconstruction using $Lucion^{(R)}$ program. Results: Cases were $59{\pm}16$ years olds and male was 16 cases, female was 32 cases. 9 cases had no Resident's ridge, anterior-posterior length of intercondylar notch was $25.4{\pm}3.5$ mm, average of length and height of the Resident's ridge was $8.2{\pm}2.6,\;3.5{\pm}1.5$ mm. Distance of the Resident's ridge from posterior cortex was $7.6{\pm}2.6$ mm. Conclusion: Resident's ridge was used as landmark in anterior cruciate ligament reconstruction, which presented in many cases and which had distinct length and height.

Evaluation of Radiation Exposure Dose for Examination Purposes other than the Critical Organ from Computed Tomography: A base on the Dose Reference Level (DRL) (전산화단층촬영에서 촬영 목적 부위와 주변 결정장기에 대한 피폭선량 평가: 선량 권고량 중심으로)

  • In this study measured patient exposure dose for purpose exposure area and peripheral critical organs by using optically stimulated luminescence dosimeters (OSLDs) from computed tomography (CT), based on the measurement results, we predicted the radiobiological effects, and would like to advised ways of reduction strategies. In order to experiment, OSLDs received calibration factor were attached at left and right lens, thyroid, field center, and sexual gland in human body standard phantom that is recommended in ICRP, and we simulated exposure dose of patients in same condition that equal exposure condition according to examination area. Average calibration factor of OSLDs were $1.0058{\pm}0.0074$. In case of left and right lens, equivalent dose was measure in 50.49 mGy in skull examination, 0.24 mGy in chest, under standard value in abdomen, lumbar spine and pelvis. In case of thyroid, equivalent dose was measured in 10.89 mGy in skull examination, 7.75 mGy in chest, 0.06 mGy in abdomen, under standard value in lumber spine and pelvis. In case of sexual gland, equivalent dose was measured in 21.98 mGy, 2.37 mGy in lumber spine, 6.29 mGy in abdomen, under standard value in skull examination. Reduction strategies about diagnosis reference level (DRL) in CT examination needed fair interpretation and institutional support recommending international organization. So, we met validity for minimize exposure of patients, systematize influence about exposure dose of patients and minimize unnecessary exposure of tissue.
