Disturbed Functional Asymmetry of Sensorimotor Cortex in Schizophrenia: A Study with Functional Magnetic Resonance Imaging (정신분열증에서 감각운동피질의 기능적 비대칭성의 장애: 기능적 자기공명영상을 이용한 연구)

  • Purpose : The purpose of this study was to investigate the pattern of cerebral response to motor tasks in patients with schizophrenia compared with normal subjects using functional MRI. Materials and methods ; Nine right handed-schizophrenic patients and six right-handed normal subjects were included. We used right hand movement as task. Series of 120 consecutive echo-planar images per section were acquired during three cycles of task and rest activations. Lateralization index of cortical response was measured and compared between patients and normal subjects. Results ; Right hand motor task was associated with greater activation in left sensorimotor cortex than the right in normal subjects. Schizophrenia patients showed relatively decreased activation in left cortex and increased activation in right cortex compared with normal subjects. In one patient, reversed lateralization was noted. Conclusion : Normal hemispheric asymmetry of cortical response to motor task was found in different pattern in schizophrenia. Our result is consistent with functional disturbance of motor circuitry in this disorder. Functional MRI will play an important role in diagnosis and research of this disorder.

Neural Correlates of Faux Pas Detection: An fMRI Study (헛디딤 탐지의 신경 상관: 기능적 자기공명 영상 연구)

  • The aim of this study was to identify neural correlates underlying the detection of faux pas, a test of theory of mind (ToM), in Korean healthy adults. Using functional magnetic resonance imaging, we compared the brain activities associated with faux pas stories and the activities associated with control stories. Faux pas stories compared with the control stories produced activations bilaterally in the superior frontal gyrus (BA 9) and in the precuneus (BA 7). The left medial frontal gyrus (BA 9), the left superior temporal gyrus (BA 38), the left inferior temporal gyrus (BA 20) and the right inferior parietal lobule (BA 40), the right postcentral gyrus (BA 1), the right lingual gyrus (BA 18), the right transverse temporal gyrus (BA 41) were also activated. The orbitofrontal cortex and the amygdala were not found to be involved in the detection of faux pas. This result suggests that brain activations associated with ToM are dependent on the type of mental state drawn by the task.

  • After proving home-made imaging pulse sequences including tailored RF pulse by phantom, susceptibility-contrast-enhanced MR venograms of cat brain were obtained using tailored RF gradient-echo(TRGE) method. Sagittal MR imaging of the cat brain obtained by TRGE technique shows several veins, for example, dorsal sagittal sinus, straight sinus, vein of corpus callosum and internal cerebral vein, etc., compared with cats anatomical figure. Tailored RF waveform was generated by PASCAL language in ASPECT 3000 computer(Switzland, Bruker). Rectangular-shaped slice profile with bi-linear ramp function as phase distribution in the slice, at which maximum value was 2$\pi$, was fourier transformed to make tailored RF pulse. Experimental MR imaging parameters were TR/TE=205/10 msec, slice thickness TH=7mm, maxtrix size=256$\times$256, in-plane resolution=0.62$\times$0.31mm$^2$, and field of view(FOV)=8cm for both conventional gradient-echo(GE) imaging and TRGE imaging techniques.

Rapid Regression of White Matter Changes in Hypoglycemic Encephalopathy (저혈당성 뇌병증에서 뇌백질 변화의 빠른 퇴행)

  • Purpose : In a previous report, it took several days for white matter lesions to regress in hypoglycemic encephalopathy. We present a case of rapid diffusion-weighted image (DWI) changes in hypoglycemic encephalopathy. Case Report: A 58-year-old male patient was found semi-comatous with the only abnormality in his laboratory tests showing hypoglycemia (44 mg/dL). After rapid correction of glucose level, immediate brain DWI showed bilateral subcortical white matter lesions. After about 5 hours, follow-up DWI showed resolved subcortical white matter lesions, with newly-appeared bilateral fronto-temporo-parietal cortical lesions. Conclusion: Both white matter and cortex involvement in hypoglycemic encephalopathy has been shown in several reports, but rapid regression of white matter changes in hypoglycemic encephalopathy has been rarely reported. It is important to know that MR imaging changes in hypoglycemic encephalopathy can be made as quick as just a few-hour-long.

  • 뇌 손상 후 급성기에 기능의 자발적인 회복이 일어나지만 환자들은 환측의 상지를 잘 사용하지 못하게 된다. 그 결과 원하는 움직임을 억제하는 상황을 발생시키는데 이것을 학습 무사용 증후군(learned nonu se syndrome)이라 한다. 이러한 학습 무사용 증후군을 치료하기 위해 억제-유도 치료(constraint-induced therapy)가 고안되었다. 억제-유도 치료는 연속되는 몇 주간에 걸쳐 매일 많은 시간 동안 건측의 상지를 묶어두고 환측 상지를 사용하게 하여 기능을 반복 학습하게 함으로써 기능을 증진시키는 방법이다. 이미 여러 연구자들이 경두개 자기자극(transcranial magnetic stimulation), 움직임 관련 피질전위(movement-related cortical potential), 기능적 자기공명 영상기법(functional magnetic resonance imaging) 등을 통하여 억제-유도 치료 후 운동피질영역에서의 재조직화를 보고함으로써 기능 증진과 관련된 회복 기전을 뒷받침하고 있다. 억제-유도 치료의 영역은 확대되어 뇌졸중, 척수손상, 고관절 치환술 후로 하지에서의 기능증진을 위하여 연구가 진행되고 있으며 특히 뇌졸중 후 실어증 환자에서 새로운 방법으로 제시되고 있다. 따라서, 억제-유도 치료는 신경학적인 손상 후 움직임의 재활에 있어서 치료-유도를 통한 중추신경계의 회복에 효과적으로 작용할 수 있다.

MRI Findings of Neuro-Behcet's Disease (신경베체트병의 자기공명영상소견)

  • MR findings in nine patients(three female, six male) with neuro-Behcet's disease were retrospectively analyzed. NeuroBehcet's disease was diagnosed on the basis of typical clinical symptoms. Involved site, pattern, signal intensity, and contrast enhancement pattern on MRI were evaluated. In addition, follow up MR imaging was performed in four patients. The midbrain(7/9), internal capsule(7/9), pons(6/9), thalamus(6/9), basal ganglia (5/9), middle cerebella peduncle(4/9), medulla oblongata(2/9), and subcortical white matter(2/9) are involved on MRI. The size of lesions was 1cm to 3cm and their margin was ill-defined and patchy. Inhomogeneous high signal intensity on the T2-weighted images and low signal intensity on T1-weighted images was seen respectively. In four of nine cases, there was focal enhancement. On follow up MR imaging, improvement or recurrance of the lesions was found. Also in two cases of follow up cases, there was artophy in brainstem and(or) middle cerebellar peduncles. In conclusion, MR imaging with systemic clinical symptoms is useful for diagnosing neuro-Behcet's disease.

  • The study is to automatically or semi-automatically detect the accurate contour of tumors or lesions using active contour models (Snakes) in the MRI images of the brain. In the study we have improved the energy-minimization problem of snakes using dynamic programming and have utilized the values of the canny edge detector by the image force to make the snake less sensitive in noises. For the extracted boundary, the inside area, the perimeter and its center coordinates could be calculated. In addition, the multiple 2D slices with the contour of the lesion wore combined to visualized the shape of the lesion in 3D. We expect that the proposed method in this paper will be useful to make a treatment plan as well as to evaluate the treatments.

  • Purpose : The purpose of this study was to evaluate the image quality, contrast characteristics, and possible clinical utility of Medium Tau Inversion Recovery(MTIR) sequence with white matter suppression in patients with brain cortical lesion. Materials and methods : Two normal volunteers and twenty-one patients with cortical lesion were scanned with MTIR as well as other MR imaging sequences. Gray-white matter contrast was evaluated objectively using region-of-interest calculations, including percent contrast and contrast-to-noise ratio(CNR). MTIR sequence was visually compared with other sequences in 21 patients with cortical lesion including conspicuity and detection rate. Results : MTIR sequence had the highest present contrast and CNR between the gray matter and white matter. In twenty-one cases of cortical lesion including cortical dysplasia, MTIR sequence improved delineation and conspicuity of lesion, but MTIR sequence could not detect new lesions. Conclusion : The MTIR sequence well delineated the cortical lesions, particularly in including cortical dysplasia. It may be used as an adjunctive imaging sequence in case of poor gray and white matter differentiation with conventional T1-weighted sequences.

A Review of Brain Imaging Studies on Classical Fear Conditioning and Extinction in Healthy Adults (건강한 성인에서의 고전적 공포 조건화 및 소거에 연관된 뇌 영역에 대한 뇌영상 연구 고찰)

  • Fear conditioning and extinction, which are adaptive processes to learn and avoid potential threats, have essential roles in the pathophysiology of anxiety disorders. Experimental fear conditioning and extinction have been used to identify the mechanism of fear and anxiety in humans. However, the brain-based mechanisms of fear conditioning and extinction are yet to be established. In the current review, we summarized the results of neuroimaging studies that examined the brain changes-functional activity and structures-regarding fear conditioning or extinction in healthy individuals. The functional activity of the amygdala, insula, anterior cingulate gyrus, ventromedial prefrontal cortex, and hippocampus changed dynamically with both fear conditioning and extinction. This review may provide an up-to-date summary that may broaden our understanding of pathophysiological mechanisms of anxiety disorder. In addition, the brain regions that are involved in the fear conditioning and extinction may be considered as potential treatment targets in the future studies.

Association between Cerebral Small Vessel and Alzheimer's Disease (알츠하이머병과 뇌소혈관질환의 연관성)

  • Cerebral small vessel disease (CSVD) includes vascular lesions detected on brain MRI, such as white matter hyperintensities, lacunar infarctions, microbleeds, or enlarged perivascular spaces. There is accumulating evidence that vascular changes may play an important role in development of Alzheimer's disease (AD), and CSVD lesions detected on brain MRI were reported to be associated with β-amyloid and tau proteins accumulation. As the vascular contribution has therapeutic potential, it is important to understand the association of CSVD with AD and AD biomarkers. This review begins with a brief introduction of AD and AD biomarkers, explains the association between AD and vascular changes, and then details the pathogenesis and MR imaging findings of CSVD. Afterwards, we discuss the association of CSVD with AD and AD biomarkers.
