Purpose: The aim of the study was to evaluate the hemodynamic changes after successful bypass surgery in patients with atherosclerotic stenosis in ICA using $^{99m}Tc-ECD$ SPECT. Materials and Methods: Fourteen patients (M:F=8:6, mean age; $60{\pm}9$ years) who underwent STA-MCA anastomosis for unilateral atherosclerotic cerebrovascular disease were enrolled. $^{99m}Tc-ECD$ basal/acetazolamide perfusion SPECT studies were performed before, 10 days and 6 months after bypass surgery. Perfusion reserve was defined as the % changes after acetazolamide over rest image. Regional cerebral blood flow and perfusion reserve were compared preoperative, early-postoperative and late-postoperative scans. Results: The mean resting perfusion and decrease in perfusion reserve in affected ICA territory on preoperative scan was $52.4{\pm}3.5\;and\;-7.9{\pm}4.7%$, respectively. The resting perfusion was significantly improved after surgery on early-postoperative scan (mean $53.7{\pm}2.7$) and late-postoperative scan (mean $53.3{\pm}2.5$) compared with preoperative images (p<0.05, respectively). Resting perfusion did not showed further improvement on late-postoperative scan compared with early-postoperative scan. The perfusion reserve was $-3.7{\pm}2.6%$ on early-postoperative scan, and $-1.6{\pm}2.3%$ on late-postoperative scan, which was significantly improved after surgery. Additionally, further improvement of perfusion reserved as observed on late-postoperative scan (p<0.05). While, in the unaffected ICA territory, no significant changes in the resting perfusion and perfusion reserve was observed. Conclusion: The improvement of resting perfusion and perfusion reserve in early-postoperative scan reflects the immediate restoration of the cerebral blood flow by bypass surgery. In contrasts, further improvement of perfusion reserve showing on late-postoperative scan may indicate a good collateral development after surgery, which may indicate good surgical outcome after surgery.
Radiosurgery treatment in the brain requires detailed information on three-dimensional dose distribution. A three-dimensional treatment planning is a prerequisite for treatment plan optimization. It must cover 3-D methods for representing the patient, the dose distributions, and beam settings. Three-dimensional dose models for non-coplanar moving arcs were developed using measured single beam data and efficient 3-D dose algorithms for circular fields. The implementation of three dimensional dose algorithms with stereotactic radiosurgery and the application of the algorithms to several cases are discussed.
Profound hypothermia protects . cerebral function during total circulatory arrest(TCA) in the surgical treatment of a variety of cardiac and aortic diseases. Despite its importance, there is no ideal technique to monitor the brain injury from ischemia. Since 1994, we have developed compressed spectral array(CSA) of electroencephalography(EEG) and monitored cerebral activity to reduce ischemic injury. The purposes of this study are to analyse the efficacy of CSA and to establish objective criteria to consistently identify the safe level of temperature and arrest time. We studied 6 patients with aortic dissection(AD, n=3) or aortic arch aneurysm(n=3, ruptured in 2). Body temperatures from rectum and esophagus and the EEG were monitored continuously during cooling and rewarming period. TCA with cerebral ischemia was performed in 3 patients and TCA with selective cerebral perfusion was performed in 3 patients. Total ischemic time was 30, 36 and 56 minutes respectively for TCA group and selective perfusion time was 41, 56 and 92 minutes respectively for selective perfusion group. The rectal temperatures for flat EEG were between 16.1 and 22. $1^{\circ}C$ (mean: 18.4 $\pm$ 2.0): the esophageal temperatures between 12.7 and $16.4^{\circ}C$ (mean $14.7\pm1.6).$ The temperatures at which EEG reappeared $5~15.4^{\circ}C$ for esophagus. There was no neurological defic t and no surgical mortality in this series. In summary, the electrical cerebral activity Teappeared within 23 minutes at the temperature less than $16^{\circ}C$ for rectum. It seemed that $15^{\circ}C$ of esophageal temperature was not safe for 20 minutes of TCA and continuous monitoring the EEG with CSA to identify the electrocerebral silence was useful.
마취과 의사는 신경외과수술시 뇌압이 상승되어 있는 환자의 마취를 자주 접하게 된다. 따라서 이러한 환자에서 뇌압의 상승을 억제시키고 뇌압상승 요인들을 제거하며 적당한 뇌혈류량을 유지시킴으로서 이미 손상된 뇌조직을 보호하고 더 이상의 뇌손상을 가하지 않도록 노력해야 한다. 이를 위하여 마취과적 입장에서 고려해 보아야 할 여러 문제점들, 특히 두개강내 역학 즉 뇌용적, 뇌압, 뇌혈류, 뇌산소대사율의 유기적 관계와 마취과에서 쓰여지는 여러약제 및 기타 뇌압과 관계되는 약제 및 요인들에 관하여 기술해 보았다.

Background: Surgery for Stanford type A aortic dissection shows a high operative mortality rate and frequent postoperative brain injury. This study was designed to find out the risk factors leading to operative mortality and brain injury after surgical repair in patients with type A aortic dissection. Material and Method: One hundred and eleven patients with type A aortic dissection who underwent surgical repair between February, 1995 and January 2005 were reviewed retrospectively. There were 99 acute dissections and 12 chronic dissections. Univariate and multivariate analysis were performed to identify risk factors of operative mortality and brain injury. Resuit: Hospital mortality occurred in 6 patients (5.4%). Permanent neurologic deficit occurred in 8 patients (7.2%) and transient neurologic deficit in 4 (3.6%). Overall 1, 5, 7 year survival rate was 94.4, 86.3, and 81.5%, respectively. Univariate analysis revealed 4 risk factors to be statistically significant as predictors of mortality: previous chronic type III dissection, emergency operation, intimal tear in aortic arch, and deep hypothemic circulatory arrest (DHCA) for more than 45 minutes. Multivariate analysis revealed previous chronic type III aortic dissection (odds ratio (OR) 52.2), and DHCA for more than 45 minutes (OR 12.0) as risk factors of operative mortality. Pathological obesity (OR 12.9) and total arch replacement (OR 8.5) were statistically significant risk factors of brain injury in multivariate analysis. Conclusion: The result of surgical repair for Stanford type A aortic dissection was good when we took into account the mortality rate, the incidence of neurologic injury, and the long-term survival rate. Surgery of type A aortic dissection in patients with a history of chronic type III dissection may increase the risk of operative mortality. Special care should be taken and efforts to reduce the hypothermic circulatory arrest time should alway: be kept in mind. Surgeons who are planning to operate on patients with pathological obesity, or total arch replacement should be seriously consider for there is a higher risk of brain injury.
Substantial alterations in cerebral blood flow(CBF) are known to occur during cardiopulmonary bypass(CPB). Many investigators have speculated that these changes may be responsible for both minor and major cerebral damages after CPB. More recently, these changes in CBF have been observed to be intimately related to the arterial carbon dioxide tension(Pa$CO_2$) maintained during CPB. The present study was prospectively designed to investigate the clinical effects of normocapnic and hypercapnic CPB on the cerebral oxygen metabolism in cardiac surgery Material and Method: Thirty-six adult patients scheduled for elective cardiac surgery were randomized to either normocapnic group (Pa$CO_2$35~40 mmHg, n=18) or hypercapnic group(Pa$CO_2$, 45~55 mmHg, n=18) with moderately hypothermic nonpulsatile CPB(nasopharyngeal temperature of 29~3$0^{\circ}C$). In each patient, middle cerebral artery blood flow velocity( $V_{MCA}$), cerebral arteriovenous oxygen content difference (C(a-v) $O_2$), cerebral oxygen extraction(COE), cerebral metabolic rate for oxygen(CMR $O_2$), cerebral oxygen transport( $T_{E}$$O_2$), $T_{E}$$O_2$/CMR $O_2$ ratio, cerebral desaturation(internal jugular bulb blood oxygen saturation $\leq$ 50%), and arterial and jugular bulb blood gas were evaluated throughout the operation. Postoperative neuropsychologic complications were assessed in all patients. All variables were compared between the two groups. Result: VMCA(169.13 $\pm$ 8.32 vs 153.11 $\pm$8.98%), TE $O_2$(1,911.17$\pm$250.14 vs 1,757.40$\pm$249.56), $T_{E}$$O_2$,/CMR $O_2$ ratio(287.38$\pm$28.051 vs 246.77$\pm$25.84), $O_2$ tension in internal jugular bulb (41.66$\pm$9.19 vs 31.50$\pm$6.09 mmHg), and $O_2$saturation in internal jugular bulb(68.97$\pm$10.96 vs 58.12$\pm$12.11%) during CPB were significantly lower in normocapnic group(p=0.03), whereas hypercapnic group had lower C(a-v) $O_2$(3.9$\pm$0.3 vs 4.9$\pm$0.3 mL/dL), COE(0.3$\pm$0.03 vs 0.4$\pm$0.03), CMR $O_2$(5.8 $\pm$0.5 vs 6.8$\pm$0.6), and arterial blood pH(7.36$\pm$0.09 vs 7.46$\pm$0.07, p=0.04) during CPB. Hypercapnic group had lower incidence of cerebral desaturation than normocapnic group(3 vs 9 patients, p=0.03). Duration of the neuropsychologic complication(delirium) were shorter in hypercapnic group than in normocapnic group(36 vs 60 hrs, p=0.009). Conclusion: These findings suggest that hypercapnic CPB may have salutary effects on the cerebral oxygen metabolism and postoperative neurologic outcomes in cardiac surgery.surgery.

신경외과는 뇌수술과 척추수술로 나눈다. 이중뇌수술 분야에서는 두개강내압 항진시 감압술이라고 '머리의 뚜껑을 연다'라는 표현과 마찬가지로 두개골의 일부분(손바닥크기)을 절제하는 수술이 있다. 이때 과거에는 떼어낸 뼈를 복부의 피하지방밑에 심어서 보관을 하다가 3-6개월 후 환자의 뇌상태가 안정이 되면 다시 꺼내어 제자리에 놓았는데, 골편이 피하지방에서 녹는 경우가 다소 있고, 복부에 이식한 장소가염증이 생기는 경우도 있어서 요즘은 냉동고에서 -70도를 유지하여 보관 후 나중이 복원 수술을 할 때녹여서 사용한다. 이를 '자가골 두개성형술' 이라 한다. 하지만골편의 오염이나 소실 혹은 1차 두개성형술 후 감염 등 어쩔 수 없이 자가골을 사용 못하는 경우에는인공으로 두개골편 모형을 제작하여 '뚜껑'으로 사용해야 한다. 현재PMMA를 이용하여 수술 시 모형을 제작하는 방법이 많이 사용되며 최근 단단한 스펀지 형태의 인공제품이 사용되고 있으나 가격이 매우비싸지만 스펀지 기공내로 자가골이 자란다는 장점이 있다. 척추수술 분야에서는 뼈 대치품을 비교적 많이필요로 한다. 즉 척추 후방고정을 하는 경우 원래 수술의 목적인 감압술이나 교정술 등을 한 후 척추분절간 쇠(티타늄사용)고정을 한 뒤에 뼈조직(자가골이나 동종이식골, DBM, HA stick) 등을 충분히 사용하여덮어줌으로써 분절간 골유합을 유도할 때 많은 양이 필요하며, 척추 전방으로 수술을 하는 경우에도 디스크공간이 빈 상태에서 Cage(추체와 추체를 지지하는 작은 상자모양) 내에뼈조직을 넣어서 척추분절간 골유합이 일어나도록 한다. 최근 HA를 cage 대용품으로 사용한 경우에서 HA가 부서지는 경우가 기사화된바 있다. 또한 경추의 추체 내에 작은 구멍만 뚫고 디스크 수술을 한 후에 그 구멍에 HA+B-TCP 소재를 넣어 천연뼈의 성장 유도를 위해 사용되는 등 신경외과의 다양한 분야에서 생체소재들이 사용되고 있다.
Nonsurgical management of four cases of the parasagittal epidural hematoma were experienced. Patients were mildly symptomatic or minimal neurological disturbances on admission. Patients were treated conservatively because of stable neurologic sign. All patients had who diastatic fracture and/or suture have become a complete neurological recovery with satisfactory absorption of EDH over a period of 5 to 12 weeks.

목적 : 뇌정위 방사선수술은 AVM(ateriovenous malformation)이나 작은 크기의 종양에 1회에 고선량의 방사선을 조사하는 기술이다. 선형가속기를 이용한 방사선 수술을 하기 위하여 최근 본원에 설치한 Philips SL 75-5 선형가속기와 isocentric sub system(ISS)에 의한 뇌정위 방사선 수술에 있어서 표적의 위치선정과, gantry와 couch의 회전시 기하학적 오차가 중요시 되는데 isocentric sub system의 오차를 분석 하였다. 대상 및 방법 : 방사선원으로는 Philips SL 75-5 선형가속기의 5MV 광자선을 사용하였고, 원형의 작은 광자선속을 위하여 isocenter에서의 직경이 26mm인 secondary cone을 gimbal baaring에 삽입하여 사용하였다. 표적의 크기와 좌표를 정하기 위하여 CT나 angio localizer를 이용하고, 표적좌표 선정을 위하여 BRW phantom base와 target pointer를 이용하여 임의의 BRW-coordinator를 바꾸어 가면서 gantry angle와 ISS head 각도를 임의로 바꾸어 가면서 film에 방사선을 조사하였다. 흑화된 film을 view box 위에 놓고 광학판독기구로 film 가장자리의 오차를 scale 확대경으로 측정하여 오차를 분석하였다. 결과 : 표적좌표 선정의 정확도를 확인하기 위하여 임의의 표적좌표에 gantry의 10개각도 ISShead의 10개각도에서 각각 광자선을 조사시켜 film을 이용하여 오차를 측정한 결과 collimator cone의 직경이 26mm일때 전체 평균오차가 0.219+-0.03mm이었다. 결론 : Isocentric sub system은 gantry head와 ISS arm 사이에 gimbal bearing이 있어서 이 부위를 flexible하게 연결함으로 gantry의 회전에 무관하게 정확한 isocenter를 유지시켜 주고 ISS head는 couch와 독립되어 움직이므로 isocentric sub system isocenter의 오차를 최대한 줄일수 있음을 알았다.
