$^{99m}Tc-HMPAO$ Regional Cerebral Blood Flow SPECT in Transient Ischemic Attacks (일과성 뇌허혈 발작 환자에 있어서 $^{99m}Tc-HMPAO$ 국소 뇌혈류 SPECT의 유용성)

  • Transient ischemic attacks (TIAs) is a syndrome resulting from brain ischemia lasting less than 24 hours. The mechanisms of TIAs may be similar to those of cerebral embolism and thrombosis, and thus TIAs may be followed by cerebral infarction. Despite the availability of CT scanning, the diagnosis and management of TIAs continue to be difficult. Recently SPECT has been advocated as a diagnostic imaging modality. We performed Tc-99m-HMPAO regional cerebral blood flow (rCBF) SPECT in 24 patients with the clinical diagnosis of TIAs to assess its ability to detect early changes of rCBF, and determine the diagnostic value. Ten men and fourteen women with an average of 51 years (range; 27-74 years) were included. All but 8 patients had normal brain CT prior to SPECT. The two patients had moderate degree of brain atrophy and the 6 patients nonspecific calcifications. Eighteen of the 24 patients had abnormal Tc-99m-HMPAO rCBF SPECT. Fifteen had unilateral involvement and the other three had bilateral involvements. Seventy-five percents of the defects were found in the left cerebral hemisphere. According to the distribution of the lesions (total number: 34 lesions), fourteen were in the parietal, eight in the temporal, and the remainders were elsewhere. Tc-99m-HMPAO rCBF SPECT is sensitive in detecting rCBF abnormalities in patients with TIAs, and represent the most accurate diagnostic tool available in the diagnosis of TIAs.

The Effects of Perpendicular Needling Laogong ($PC_8$) on the Improvement of Cerebral Hemodynamics in Rats (노궁(勞宮)($PC_8$) 직자(直刺)가 백서(白鼠)의 뇌혈류력학(腦血流力學)에 미치는 영향)

  • Objectives : This study was designed to investigate the effects of acupuncturing $PC_8$ used perpendicular needling method determine the mechanism of action of acupuncturing $PC_8$ by measuring the changes of regional cerebral blood flow (rCBF) and mean arterial blood pressure (MABP) in normal rats. Methods : This study also investigated the effects of acupuncturing $PC_8$ on the change of rCBF in cerebral ischemic rats, and revealed the mechanism of its action. In addition, the effects of acupuncturing $PC_8$ on focal ischemic brain injury was studied in cerebral ischemic rats. Results : 1. Acupuncturing $PC_8$ significantly increase rCBF but decreased MABP in normal rats. 2. Acupuncturing $PC_8$ increased of rCBF was significantly inhibited by pretreatment with indomethacin (1mg/kg, i.p.), an inhibitor of cyclooxygenase in normal rats. 3. Acupuncturing $PC_8$ increased of rCBF was significantly inhibited by pretreatment methylene blue (10 ${\mu}g$/kg, i.p.), an inhibitor of guanylate cyclase in normal rats. 4. Acupuncturing $PC_8$ was significantly improved the rCBF than control group increased unstable in cerebral ischemic rats. 5. Acupuncturing $PC_8$ was not significantly improved the rCBF than control group by pretreatment with indomethacin (1mg/kg, i.p.), an inhibitor of cyclooxygenase in cerebral ischemic rats. 6. Acupuncturing $PC_8$ was significantly increased the rCBF than control group by pretreatment methylene blue ($10{\mu}g$/kg, i.p.), an inhibitor of guanylate cyclase in cerebral ischemic rats. Conclusions : In conclusion, our study suggested that acupuncturing $PC_8$ can increase rCBF in normal state, and improve stability of rCBF in ischemic state. In addition, we suggested that mechanisms related with acupuncturing $PC_8$ was involved in the guanylate cyclase pathway.

  • The study was designed to investigate the effects of Mixture of Yukgunja-tang and Bambusae Caulis in Liquamen (YTBCL) on the change of regional cerebral blood flow (rCBF) and mean arterial blood pressure (MABP) in rats, and further to determine the mechanism of action of YTBCL. The results in rats were as follows ; YTBCL 25 ${\mu}l$ significantly decreased rCBF and MABP compared with basal condition. YTBCL 100 ${\mu}l$ significantly increased rCBF compared with basal condition, but decreased MABP compared with basal condition. YTBCL 50 ${\mu}l$ significantly increased rCBF compared with basal condition, but MABP was somewhat decreased compared with basal condition. The VTBCL 50 ${\mu}l$-induced increase in rCBF was significantly inhibited by pretreatment with methylene blue (10 ${\mu}g$/kg, i.p.), an inhibitor of guanylate cyclase and indomethacin (1 mg/kg, i.p.), an inhibitor of cyclooxygenase. The YTBCL 50 ${\mu}l$-induced decreased MABP significantly increased by pretreatment with methylene blue but was inhibited by indomethacin. This results were suggested that the mechanism of YTBCL was mediated Dy guanylate cyclase.

  • This experimental studies were done to investigate the effects of Sambutang(SBT) and consitituent herbs on the regional cerebral blood flow(rCBF) and mean arterial blood pressure(BP) in rats. The results are as follows: SBT and consitituent herbs(SBT omitting Zingiberis Rhizoma Recens, Zingiberis Rhizoma Recens. Ginseng Radix and Aconiti iateralis preparata Radix) was increased rCBF significantly in a dose-dependent manner. SBT omitting Zingiberis Rhizoma Recens and Ginseng Radix were decreased significantly BP in rats. But, SBT, Aconiti iateralis preparata Radix and Zingiberis Rhizoma Recens were not affected BP in rats. According to the above findings. it is suggested that SBT causes a diverse response of rCBF, and specially the rCBF by increasing SBT is mediated action of Zingiberis Rhizoma Recens.

  • This study was designed to investigate the effects of Samul-tang Extract and Bambusae caulis in liquamen(SEB) on the changes in regional cerebral blood flow (rCBF) and mean arterial blood pressure (MABP) in normal rats. The results were as follows ; SBE significantly increased rCBF in a dose-dependent manner, and increased MABP in a dose-dependent manner. This result suggests that SBE significantly increased rCBF by dilating pial arterial diameter. Increase of SBE-induced rCBF was significantly inhibited by pretreatment with indomethacin (1 mg/kg, i.p.), an inhibitor of cyclooxygenase and increase of SBE-induced rCBF was inhibited by pretreatment with methylene blue ($10\;{\mu}g/kg$, i.p.), an inhibitor of guanylate cyclase. Increase of SBE-induced MABP was significantly increased by pretreatment with methylene blue but increase of SBE-induced MABP was decreased by pretreatment with indomethacin. These results suggested that the action of SBE was mediated by cyclooxygenase.

  • Objective : The objective of this study is to measure the changes of regional cerebral blood flow(rCBF) and blood pressure(BP) in rats with the following injection. Methods : The measurement was continually monitored by laser-doppler flowmeter-Transonic Instrument, USA- and pressure tranducer(Grass, USA) in anesthetized adult Sprague-Dawley rats about for two hour to two hour and a half hours through the data acquisition system composed of Maclab and Macintosh computer. Result : This experiment with Samso-eum increased the changes of rCBF in rats, significantly, but did not change the blood pressure. The rCBF of Samso-eum decreased by pretreated propranolol, and was not changed by pretreated L-NNA and ODQ. Conclusion : It is considered that the dosage-dependent increase on rCBF is mediated by adrenegic ${\beta}$-receptor.

Effect of Cheonmabanhwa-Tang on the Changes in Cerebral Hemodynamics in Cerebral Ischemia of Rats (천마반하탕이 뇌허혈로 인한 뇌혈류역학적 변동에 미치는 영향)

  • Cheonmabanhwa- Tang (CBT) has been used in the Oriental Medicine for many centuries as a therapeutic agent for dizziness due to Poong-Dam, We reported that CBT had effects on the cerebral hemodynamics [regional cerebral blood flow (rCBF), pial arterial diameter (PAD), and mean arterial blood pressure (MABP)] in normal and cerebral ischemic rats. Therefor we designed to determine the mechanism of action of CBT. The changes of rCBF and MABP were determinated by laser-Doppler flowmetry(LDF), and the change of PAD was determinated by video-microscopy. The results were as follows: The CBT-induced increase in rCBF was significnatly inhibited by pretreatment with indomethacin (IDN, 1 ㎎/㎏, i.p.), an inhibitor of cyclooxygenase, or methylene blue (MTB, 10 ㎍/㎏, i.p.), an inhibitor of guanylate cyclase. The CBT-induced increase in PAD was also significantly inhibited by pretreatment with IDN or MTB. The CBT-induced increase in MABP was also significantly inhibited by pretreatment with IDN or MTB. In conclusion, it is suggested that CBT causes a diverse effect on cerebral hemodynamics via mediation of cyclooxygenase and guanylate cyclase.

Effect of Nodus Nelumbinis Rhizomatis Extract on the Regulation of Regional Cerebral Blood Flow in Rats (흰쥐에서 우절(藕節) 추출물의 국소 뇌혈류량 조절 효과)

  • Objectives : In brain disorders such as ischemic stroke, the final outcome depends largely on the duration and the degree of the ischemia as well as the susceptibility of various cell types in the affected brain region. In the present study, the effects of Nodus Nelumbinis Rhizomatis Extract(NNRe) were tested for the anti-oxidative action of rCBF. Methods : Regional cerebral blood flow(rCBF) were determined by LDF methods. LDF allows for real time, noninvasive, continuous recordings of local CBF. The LDF method has been widely used to trace hemodynamic changes in the superficial or the deep brain structures in experimental stroke research. Results : NNRe treatment showed no change on rCBF in methylene blue, ODQ and L-NNA pretreated rats. 120 minutes of MCAO and followed reperfusion, 0.1% concentration of NNR treatment improved the altered cerebral hemodynamics of cerebral ischemic by increasing rCBF. Conclusions : The ischemia/reperfusion induced oxidative stress may have contributed to cerebral damage in rats, and the present study provides clear evidences for the beneficial effect of NNR on ischemia/reperfusion induced brain injury.

Effects of Bee Venom Extract Pharmacopuncture on the Regional Cerebral Blood Flow and Mean Blood Pressure in Rats (봉독약침이 흰쥐의 국소뇌혈류량 및 평균혈압에 미치는 영향)

  • Objectives The present study was designed to investigate the effects of bee venom extract pharmacopunctureon (BVP) at 肩井 (GB21) on the changes in regional cerebral blood flow (rCBF) and mean arterial blood pressure (MABP) in normal rats, and further to determine the mechanisms. Methods We purchased rats about 300g and prepared diluted BVP with normal saline. And we injected diluted BVP to rats gradually progressed from 0.001 mg/kg to 0.1 mg/kg at corresponding region to GB21. After injection, we measured the changes in rCBF and MABP of rats. In addition, to determine the mechanisms of this changes, we did the same experiments twice more after pretreatment with indomethacin and metylene blue separately. Results BVP significantly increased rCBF but decreased MABP, suggesting that BVP potently may increase rCBF by dilating pial arterial diameter. Furthermore, the increase of BVP-induced rCBF and the decrease of BVP-induced MABP were significantly blocked by pretreatment with cyclooxygenase inhibitor, indomethacin (1 mg/kg, i.p.). But the increase of BVP-induced rCBF and the decrease of BVP-induced MABP were not blocked by pretreatment with guanylate cyclase inhibitor, methylene blue (0.01 mg/kg, i.p.). Conclusions These findings indicate that the action of BVP is mediated by cyclooxygenase. Furthermore these results suggest that BVP can increase rCBF in normal state, as well as improve the stability of rCBF in ischemic state.

Mechanism of Jaeumgenby-tang on the Regional Cerebral Blood Flow, Mean Arterial Blood Pressure and Cardiac Muscle Contractile Force in Rats (자음건비탕이 국소뇌혈류량, 평균혈압, 심근수축력에 미치는 작용기전)

  • Jaeumgenby-tang(JGT) have been used in oriental medicine for many centries as a a therapeutic agent of vertigo caused by deficiency of qi and blood. The effects of JGT on the regional cerebral blood flow(rCBF), mean arterial blood pressure(MABP) and cardiac muscle contractile force(CMF) is not known. The purpose of this Study was to investigate effects of JGT on the rCBF, MABP, CMF and mechanism of JGT induced changed rCBF, MABP, CMF. The changes of rCBF, MABP and CMF were determinated by Laser-Doppler Flowmetry(LDF). The results were as follows; JGT extract was increased rCBF, MABP and CMF in a dose-dependent, specially JGT extract was significantly increased rCBF and MABP. Pretreatment with propranolol was significantly inhibited JGT induced increase of rCBF but pretreatment with indomethacin and methylene blue were accelerated JGT induced increase of rCBF. Pretreatment with propranolol and indomethacin were inhibited JGT induced increase of MABP, but pretreatment with methylene blue was accelerated JGT induced increase of MABP. Pretreatment with propranolol was significantly inhibited JGT induced increase of CMF but pretreatment with indomethacin and methylene blue were accelerated JGT induced increase of CMF. This results suggest that JGT increased rCBF by increasing MABP and CMF and the action of JGT is mediated by adrenergic β-receptor.
