Relations of Stroke Literacy, Health Literacy, Stroke Knowledge, and Self-Management among Middle-Aged and Older Adults (중노년기 성인의 뇌졸중 문해력, 건강정보 문해력, 뇌졸중 지식과 자기관리간의 관계)

  • This study examined the degree and relations of stroke literacy, health literacy, stroke knowledge, and self-management to identify the significant determining factors affecting stroke prevention among middle-aged and older adults with a high incidence of stroke. Data on the sociodemographic characteristics, stroke literacy, health literacy, stroke knowledge, and self-management of 198 participants were collected from December 5, 2019, to March 31, 2020, using a structured questionnaire. The data were analyzed using the SPSS/WIN 23.0 program. The determining factors affecting self-management in middle-aged and older adults were age (β=.190. p=.015), followed in order by smoking (β=-.226, p=.001), self-rated health status (β=.297, p<.001), and stroke knowledge (β=-.168, p=.014); the explanation power was approximately 20.3%. The results suggest that the self-rated health status and stroke knowledge should be considered when developing nursing interventions to improve self-management among middle-aged and older adults. Furthermore, it will be necessary to find the intervening variable between stroke knowledge and self-management for community-dwelling middle-aged and older adults in the community. Overall, these findings underscore the importance of systematic and ongoing education on stroke prevention.

Significance of Dexamethasone Suppression Test in Patients with Stroke (뇌졸중 환자에서 Dexamethasone 억제검사의 의의)

  • The purpose of this study was to evaluate the effect of stroke on hypothalamic-pituitary axis using dexamethasone suppression test. The effects were evaluated according to age, sex, type, size, and lesion site of stroke. These tests were performed in 62 patients with stroke(cerebral infarction, 42 cases : intracerebral hemorrage, 20 cases) and 21 disabled controlled patients without intracranial diseases at Yeungnam University Hospital from June 1992 to June 1993. The results summarized as follows. 1. Cerebral infarction showed significantly higher frequency of DST non-suppression in stroke patients than control(p<0.05). 2. Patients with left hemisphere stroke showed more frequent abnormal neuroendocrine test results(p<0.01) 3. Patients with large infarction revealed strongly non-suppressed DST results(p<0.01). 4. Sinificantly higher basal cortisol level in patients with cerebral infarction was noted(p<0.01). 5. There are no statistical significance between DST results and sex, age, motor impairment, type of cerebral infarction.

Protective Effects of Traditional Korean Medicine Preparations, Herbs, and Active Compounds on the Blood-brain Barrier in Ischemic Stroke Models (허혈성 뇌졸중 모델에서 혈액-뇌 장벽에 보호효과를 나타내는 한약처방, 한약재 및 활성화합물)

  • Stroke is among the leading causes of death and long-term physical and cognitive disabilities worldwide, affecting an estimated 15 million people annually. The pathophysiological process of stroke is complicated by multiple and coordinated events. The breakdown of the blood-brain barrier (BBB) in people with stroke can significantly contribute to the development of ischemic brain injury. Therefore, BBB disruption is recognized as a hallmark of stroke; thus, it is important to develop novel therapeutic strategies that can protect against BBB dysfunction in ischemic stroke. Traditional medicines are composed of natural products, which represent a promising source of new ingredients for the development of conventional medicines. Indeed, several studies have shown the effectiveness of Korean medicine on stroke, highlighting the value of Korean medicinal treatment for ischemic stroke. This review summarizes the current information and underlying mechanisms regarding the ameliorating effects of the formula, decoction, herbs, and active components of traditional Korean medicine on cerebral ischemia-induced BBB disruption. These traditional medicines were shown to have protective effects on the BBB in many cellular and animal ischemia models of stroke, and experiments in various animal species, such as mice and rats. In addition, they showed brain-protective effects by protecting the BBB through the regulation of tight junction proteins and matrix metalloproteinase-9, reducing edema, neuroinflammation, and neuronal cell death. We hope that this review will help promote further investigation into the neuroprotective effects of traditional Korean medicines and stimulate the performance of clinical trials on Korean herbal medicine-derived drugs in patients with stroke.

A survey of leisure participation and leisure exploration of community dwelling stroke patients (지역사회 거주 뇌졸중 환자의 여가 참여 및 여가 탐색 조사)

  • The purpose of this study was to provide basic information on leisure participation and leisure exploration of community-dwelling stroke patients through a multi-dimensional leisure participation evaluation tool. The subjects in this study were 35 people living with stroke in the community. As a result of study, the most involved leisure activities were in order of watching TV, communicating, and exercise alone. Most of the satisfaction level of leisure participation was 7-10 points, but exercise alone was 5.2 points due to the purpose of health management. Leisure exploration was the most frequent exercise alone and sports, and interest was 8-10 points. The most important obstacles for leisure participation were physical problems depending on the nature of the stroke disease. Through the results of this study, it is necessary to activate a stroke patient's discharge plan and activate a community stroke patient's leisure intervention program so that they can participate in leisure activities of their choice through voluntary leisure exploration.

The Effects of Task Oriented Activity and Modified Constraint Induced Movement Therapy on Quality of Life for Patients With Stroke (과제 지향적 훈련과 수정된 강제유도 운동치료가 뇌졸중 환자의 삶의 질에 미치는 영향)

  • Objective : The purpose of this study was to investigate the effect of task oriented activity and modified constraint induced movement therapy (mCIMT) on Quality of Life (QOL) for patients with stroke. Methods : Thirty stroke patients were participated voluntarily and were assigned randomly into task oriented activity and mCIMT groups. The QOL of both groups were assessed using Stroke Specific Quality of Life (SS-QOL). Results : The QOL showed a statistically meaningful difference for both groups (p<.05), but after the intervention, the both groups showed no statistically meaningful difference in terms of the QOL (p>.05). Conclusion : We found that task oriented activity and mCIMT improve the QOL of patients with stroke through increasing their affected upper extremity function and movement. It is expected that task oriented activity and mCIMT will have a positive effect on the QOL of stroke patients by applying them to clinic with occupational therapy.

The Effect of Mental Practice on Gait and Quality of Life in Stroke Patients (상상훈련이 뇌졸중 환자의 보행 및 삶의 질에 미치는 영향)

  • The aim of this study was to evaluate the effects of mental practice to improve on gait and quality of life in stroke patients. Participants were randomly allocated to two groups: experimental(n=10) and control (n=10). Both groups received neural development treatment training for 6 sessions 1 hour per week during 6 weeks. Experimental group practiced additional mental practice for 3 session 10 minutes per week 6 weeks. Results were evaluated by spatial-temporal parameter, 10m walk test, and quality of life. There were significantly increased by mental practice in outcomes of the gait performance from the gait velocity, cadence, double support, single support, 10m walk test. There were significantly increased by mental practice in outcomes of stroke impact scale from the strength, social participants, and stroke recovery. In conclusion, the mental practice improves gait performance and quality of life in stroke patients. The results suggest that mental practice training is feasible and suitable to stroke patients.

  • Stroke disease is one of the leading causes of death worldwide, and in particular, it is the most important causative disease that causes disability in the elderly. Since stroke disease often causes death or serious disability, active primary prevention and early detection of prognostic symptoms are very important. In particular, it is necessary to detect and accurately predict stroke prognostic symptoms in daily life and prompt diagnosis and treatment by medical staff. In recent studies, image analysis such as computed tomography (CT) or magnetic resonance imaging (MRI) is mostly used as a methodology for predicting prognostic symptoms in stroke patients. However, this approach has limitations in terms of long test time and high cost. In this paper, we experimented with clinical data on how stroke disease affects foot pressure in elderly in walking. Experiments have shown that there is a significant difference in * p < .05 in 12 cells between the stroke elderly and the normal elderly during walking. As a result, it is significant that we found a significant difference in the gait patterns in daily life of the stroke elderly and the normal elderly.
