The Effect of Vision and Hearing Limitation on the Onset of Disability among Korean Elderly with the Consideration of Social Participation and Depression (시력·청력제한이 노년기 장애 발생에 미치는 영향 연구: 사회참여와 우울을 중심으로)

  • The purpose of this study was to examine the effect of vision and hearing limitation on the onset of disability among Korean elderly with the consideration of social participation and depression. Study population consisted of 2,670 people aged 65 and above who participated in both the 1st(2006) and 2nd(2008) wave of the Korean Longitudinal Study of Aging(KLoSA), and also reported no ADL or IADL disability in 2006. As the results, first, it found that vision and hearing limitation, social participation, and depression were significant risk factors for the onset of IADL disability in unadjusted analysis. After multivariable adjustment, however, only vision limitation showed significant association with the onset of IADL disability. Second, in the logistic analysis with four categorized variables(vision/hearing limitation + social participation, vision/hearing limitation + depression), the elderly who had vision or hearing limitation had a significantly 1.7 times higher odds ratio for disability onset if they also had low social participation compared with those who had no vision or hearing limitation, and high social participation. Whereas, depression was not significantly associated with the relationship between vision or hearing limitation, and disability onset. The study suggests that the intervention program developed to prevent social isolation of older people with vision or hearing limitation, may be helpful to reduce and postpone the risk of disability onset as well as early detection and treatment of vision or hearing problems.

Comparison of quality of life and related factors according to hearing impairment in elders using the Korea National Health and Nutrition Examination Survey (2016-2018) (청력저하 여부에 따른 노인의 삶의 질 관련 요인: 국민건강영양조사 자료(2016-2018년) 분석)

  • The Purpose of this paper was to explore the effect of hearing impairment on HRQOL in Korean elders. We carry out a cross-sectional analysis using nationally representative data from the KNHANES, 2016-2018. The survey was conducted on 4,754 elders who responded to questions about hearing impairment. Quality of life was compared between hearing impaired elders and elders with no hearing impairment using the t-test and chi-square test, and factors related to quality of life were analyzed by multivariate logistic regression using SPSS version 22.0. There was significant difference in quality of life between hearing impaired elders and elders with no hearing impairment. Walking exercise were identified as factors related to quality of life in elders with hearing impairment, while marriage status, walking exercise and limited movement were found to be related to quality of life among elders with no hearing impairment. In order to improve the HRQOL of elders with hearing impairment, multidisciplinary efforts and development of educational programs are required.

Relationship between Naming Abilities, Hearing Handicap, and Depression in Community-Dwelling Elderly (지역사회 노인의 연령에 따른 이름대기 능력, 청력장애, 우울 수준의 관계)

  • The purpose of the study was to investigate whether there are differences in confrontation naming ability, generative naming ability, hearing handicap, and depression in community-dwelling elderly by age group, and whether there is any correlation among them, and to find out what factors explain the two naming abilities (confront and generative) of the elderly. Sixty-five community-dwelling elderly people participated: 65~74 years were classified into young-old; 75~84 years into old-old; and 85 years and over into oldest-old. K-BNT-15 for the confrontation naming ability, animal naming test for the generative naming ability, K-HHIE for the subjective hearing handicap, and GDS for the subjective depression were administered. First, there was a statistically significant difference in confrontation naming and depression levels according to age groups. Second, there was a positive correlation between chronological age and subjective depression level and between subjective hearing handicap level and subjective depression level, and a negative correlation between education level and subjective depression level. Third, the confrontation naming score was explained by age, and the generative naming score was explained by subjective depression level. The results show that naming abilities, subjective hearing handicap, and subjective depression level are not simply the result of normal aging and that a convergence approach is needed in various fields to solve the issue.

  • Although hearing loss in the elderly is one of the common symptoms of aging, as the aging population continues to grow, policies such as home care and welfare housing for the elderly are implemented with a focus on mobility, often overlooking the issue of hearing loss in the elderly. In this study, our aim is to enhance the quality of life for the elderly by improving the auditory environment within residential spaces, which plays a pivotal role in determining their overall well-being. We have proposed a technique that focuses on reducing reverberation, minimizing noise levels, and enhancing sound quality to improve the listening environment for the elderly, and we have verified its effectiveness. Building upon this, we have developed an acoustic design model for residential facilities catering to elderly home care.

  • 최근에는 산업장 근로자나 노인에게 주로 발생되던 청력손실이 청소년에까지 점차 확대되면서 청소년의 청력뿐 아니라 청소년의 청력에 영향을 미치는 여러 환경 및 요인에 대한 관심이 고조되고 있다. 그 중 외국어 공부나 음악감상용으로 청소년들 사이에 생활필수품처럼 널리 이용되고 있는 휴대용 카세트가 청소년의 소음성 난청원으로 매우 위험하다는 연구결과가 제시되어 학부모, 교사, 전문가에 의해 청소년 청력상태에 대한 조사와 청력손실을 유발하는 위험요인에 대해 많은 연구가 실시되었다. 청소년기에 취미/오락활동, 가정내 기계 기구, 음악, 교통 등의 소음에 상시적으로 노출될 수 있다. 직업적으로 소음에 노출된 적이 없는 청소년에게서 소음성 난청이 발생할 수 있다는 연구결과가 나오고 있는데, 청소년의 소음성 난청은 큰 소리를 내는 장난감과 개인용 음향기기, 콘서트장, 밴드활동 등의 음악과 관련이 있으며 이외에 모터사이클, 모터 스포츠 등의 레저 활동과 관련이 있다. 이러한 소음은 작업장 소음에 의한 초기 청력손실처럼 3~6 kHz 역의 청력손실을 특징적으로 보여준다. 이 글에서는 주요 취미활동과 관련하여 MP3 등 개인용 음향기기와 헤드셋 착용 근로자의 소음 노출 수준과 이에 따른 청력영향을 살펴보고, 소음저감방법에 대해 알아보고자 한다.

The Association between Self-Reported Sensory Impairment and Cognitive Function among Korean Older Adults: Results from the 2017 National Survey of the Living Conditions and Welfare Needs of Older Koreans (한국 노인의 주관적 감각장애와 인지기능 간 관계: 2017 노인실태조사 자료를 중심으로)

  • The present study aims to examine the relationship between self-reported sensory impairment and cognitive function among older adults in Korea. Drawn from the 2017 Survey of Living Conditions and Welfare Needs Korean Older Persons, a total of 10,082 adults aged 65 and over were included in analyses. Results from hierarchial multiple regression analysis show that those who reported greater subjective discomfort about vision, hearing, and oral function were more likely to have poor cognitive function. Results from hierarchial logistic regression analysis show. Results from logistic regression analysis show that having hearing and oral discomfort increased the a risk for dementia. Findings suggest that it is necessary to manage sensory function of the elderly at the government level and to develop self-evaluation tools to monitor changes in sensory function for older adults. Future research should target finding effective ways to improve health of older adults.

Effects of Hearing Handicap on Depression and Self-efficacy in the Elderly at Home (재가노인의 청력장애가 우울과 자기효능감에 미치는 영향)

  • The purpose of this study was to identify levels of hearing handicap, depression, and self-efficacy, and to determine the effect of hearing handicap on depression and self-efficacy in home-based elderly. Data were collected from August to September 2017. Two hundred and seventy eight elderly living at home in three cities participated in this study. Data were analyzed using the independent t-test, one-way ANOVA, Scheffe's test, Pearson's correlation coefficients, and by multiple regression analysis using SPSS Ver. 20.0 for Windows. Mean levels of hearing handicap, depression, and self-efficacy were 10.91 (${\pm}19.83$), 4.75 (${\pm}3.90$), and 46.87 (${\pm}6.75$) respectively. A statistically significant positive correlation was observed between hearing handicap and depression (r=.228, p<.001), and significant negative correlations were found between hearing handicap and self-efficacy (r=-.284, p<.001) and between depression and self-efficacy (r=-.526, p<.001). Hearing handicap had a negative effect on self-efficacy (${\beta}=-.161$, p=.008) and its explanatory power was 27.2%. The study results suggest that the developments of multidisciplinary interventions and education programs are important to prevent or minimize hearing handicap in the elderly.

  • In this paper, the analysis of the relation between noise induced hearing loss and noise exposure is described for the A-weighted noise levels and exposure duration. The hearing loss and sensitivity threshold shift is investigated by changing the various parameters such as the effects of aging and noise exposure. Total hearing loss is proportional to a function of exposure level based upon the integral of pressure with time. If the noise exposure term is large so that the presbycusis may be neglected, the hearing loss due to aging and noise exposure becomes the noise induced hearing loss. It is shown that exposure to excessive noise can cause temporary loss of hearing that may become permanent if the exposure is prolonged or intense. An audiogram taken from a person suffering from noise induced hearing loss will usually show the greatest loss of hearing sensitivity in the 4kHz region, which is typically the region most sensitive to damage resulting from many types of industrial noise.

  • 노인성 난청(presbycusis)은 나이가 들어감에 따라 청력이 점진적으로 악화되는 것으로 성인 난청의 가장 흔한 원인중 하나이다. 미국과 마찬가지로 국내의 여러 연구에서 보면, 65세 이상의 노인 중 보청기를 착용해야 할 정도의 난청이 있는 비율이 전체의 30~35% 정도를 차지하고 있다. 이렇기 때문에 노인성 난청은 노인성 만성 질환 중 퇴행성 관절염, 고혈압에 이어 3대 만성 질환으로 분류되고 있다.
