An App-based Evolving Medical Nomogram Service System (앱기반 진화 의료 노모그램 서비스 시스템)

  • Clinical nomogram is a graphical representation of numeric formula, constructed from clinical cases database of followed patients' treatment, which is used for medical predication. For a clinical nomogram to contribute patient care, it is required to accumulate as many as clinical cases and to extract medical prediction knowledge. It needs to be equipped with an effective method to build medical nomogram with high predication accuracy. It is desirable for medical nomogram to be accessible at patient care point. This paper proposes a medical nomogram service system architecture which takes into account the above-mentioned issues. The proposed system architecture includes a web-based database subsystem to maintain and keep track of clinical cases. On the periodic basis, a new clinical nomogram is reconstructed for the updated clinical database. For the convenient use of patient care practice environment, an app-based program is provided which makes prediction based on the most recent clinical nomogram constructed in the service system. The proposed method has been applied to a clinical nomogram service system development for recurrence and survival prediction in bladder cancer patients.

  • Nomogram is a statistical tool that visualizes the risk factors of the disease and then helps to understand the untrained people. This study used risk factors of chronic obstructive pulmonary disease (COPD) and compared with logistic regression model and naïve Bayesian classifier model. Data were analyzed using the Korean National Health and Nutrition Examination Survey 6th (2013-2015). First, we used 6 risk factors about COPD. We constructed nomogram using logistic regression model and naïve Bayesian classifier model. We also compared the nomograms constructed using the two methods to find out which method is more appropriate. The receiver operating characteristic curve and the calibration plot were used to verify each nomograms.

  • In this study, we fit the logistic regression model and naïve Bayesian classifier model using 11 risk factors to predict the incidence rate probability for type 2 diabetes mellitus. We then introduce how to construct a nomogram that can help people visually understand it. We use data from the 2013-2015 Korean National Health and Nutrition Examination Survey (KNHANES). We take 3 interactions in the logistic regression model to improve the quality of the analysis and facilitate the application of the left-aligned method to the Bayesian nomogram. Finally, we compare the two nomograms and examine their utility. Then we verify the nomogram using the ROC curve.

  • Hypertension has a steadily increasing incidence rate as well as represents a risk factors for secondary diseases such as cardiovascular disease. Therefore, it is important to predict the incidence rate of the disease. In this study, we constructed nomograms that can predict the incidence rate of hypertension. We use data from the Korean National Health and Nutrition Examination Survey (KNHANES) for 2013-2016. The complex sampling data required the use of a Rao-Scott chi-squared test to identify 10 risk factors for hypertension. Smoking and exercise variables were not statistically significant in the Logistic regression; therefore, eight effects were selected as risk factors for hypertension. Logistic and Bayesian nomograms constructed from the selected risk factors were proposed and compared. The constructed nomograms were then verified using a receiver operating characteristics curve and calibration plot.

  • In medical practice, the diagnosis or prediction models requiring complicated computations are not widely recognized due to difficulty in interpreting the course of reasoning and the complexity of computations. Medical personnel have used the nomograms which are a graphical representation for numerical relationships that enables to easily compute a complicated function without help of computation machines. It has been widely paid attention in diagnosing diseases or predicting the progress of diseases. A nomogram is constructed from a set of clinical data which contain various attributes such as symptoms, lab experiment results, therapy history, progress of diseases or identification of diseases. It is of importance to select effective ones from available attributes, sometimes along with parameters accompanying the attributes. This paper introduces a nomogram construction method that uses a naive Bayesian technique to construct a nomogram as well as a genetic algorithm to select effective attributes and parameters. The proposed method has been applied to the construction of a nomogram for a real clinical data set.

  • Dyslipidemia is a representative chronic disease affecting Koreans that requires continuous management. It is also a known risk factor for cardiovascular disease such as hypertension and diabetes. However, it is difficult to diagnose vascular disease without a medical examination. This study identifies risk factors for the recognition and prevention of dyslipidemia. By integrating them, we construct a statistical instrumental nomogram that can predict the incidence rate while visualizing. Data were from the Korean National Health and Nutrition Examination Survey (KNHANES) for 2013-2016. First, a chi-squared test identified twelve risk factors of dyslipidemia. We used a naïve Bayesian classifier model to construct a nomogram for the dyslipidemia. The constructed nomogram was verified using a receiver operating characteristics curve and calibration plot. Finally, we compared the logistic nomogram previously presented with the Bayesian nomogram proposed in this study.

  • The concentration of fine dust has increased in Korea and people have become more concerned with respiratory diseases. This study selected risk factors for chronic obstructive pulmonary disease (COPD) through demographic and clinical features and constructed a nomogram. First, logistic regression analysis was performed using demographic and clinical feature and the pulmonary function test results of the Korean National Health and Nutrition Examination Survey (KNHANES) $6^{th}$ (2013-2015) and the nomogram was constructed to visualize the risk factors of chronic obstructive pulmonary disease in order to facilitate the interpretation of the analysis results. The ROC curve and calibration plot were also used to verify the nomogram of chronic obstructive pulmonary disease.

  • Recently, most of limestone quarries have been not mined by open-pit mining but by underground excavation to reduce environmental pollution. As a result, the size of underground galleries became bigger to maintain mass-production close to open-pit mining. However, the scale of pillars and galleries as well as the excavation methods may induce a few adverse problems for the stability of a mined gallery. In this study, the nomogram analysis and the prediction of rock damage zone induced by blasting were carried out. The testing conditions include concurrent blasting of two adjacent galleries, concurrent blasting of a transport drift and a inclined shaft, sequential blasting of two galleries, and separate blasting for each gallery. For each testing condition, blast vibration velocity was measured and analyzed. From the prediction formulas for blast vibration velocity derived in this study, the maximum depth of rock damage zone induced by blasting were also predicted.

  • The appropriate sample size calculation in dental research is important to achieve the study purpose at the first step in study design. However, it cannot be easy to calculate sample size using standard formulas, because the several factors must be considered for calculation. This study introduced the graphic method for sample size calculation, which is called nomogram. The purpose of this study is to show the effectiveness of the nomogram using examples, expecting the researchers can easily use nomogram for sample size determination.
