The effect of acute diaphragmatic breathing exercise using DiP Belt on diaphragm motion and forced vital capacity (딥벨트를 이용한 일회성 가로막 호흡운동이 가로막 움직임과 노력성 폐활량에 미치는 영향)

  • Background: This study was to determine whether the diaphragmatic breathing exercise using a DiP Belt(Diaphragmatic Pressure Belt) is effective in increasing the diaphragmatic motion and forced vital capacity. Design: Pretest-Posttest design. Methods: A total of 44 subjects(15 male, 29 female) participated in this study. All subjects were measured the diaphragmatic motion with a sonography and the Forced Vital Capacity(FVC) was measured with a digital spirometer. After 4 weeks, the subjects were intervened the diaphragmatic breathing exercise using a DiP belt and were remeasured for diaphragm motion and FVC. Results: After exercise intervention, quiet breathing significantly increased with the change in diaphragmatic motion and showed a moderate effect size (p<.01, Cohen's d = -0.53). In addition, it was significantly increased in deep breathing and showed a high effect size (p<.001, Cohen's d = -1.32). The mean diaphragmatic contraction pressure increased, but there was no significant difference and the peak diaphragmatic contraction pressure increased significantly (p<.05). Both diaphragmatic contraction pressure showed small effect sizes (respectively Cohen's d = -0.28, -0.33). In spirometry, FVC, Forced Expiratory Volume in 1 second (FEV1), and FEV1/FVC% all increased, but there was no significant difference. Only peak expiratory flow increased significantly and showed a small effect size (p<.05, Cohen's d = -0.41). Conclusion: The DiP belt diaphragmatic breathing exercise that the principle of visual feedback can correct diaphragm breathing in a short time, so it is a useful breathing exercise device that can help the diaphragm breathing exercise in the right way in clinical practice.

PCA 알고리즘과 개선된 퍼지 신경망을 이용한 여권 인식 및 얼굴 인증

  • 본 논문에서는 여권 영 상에서 PCA 알고리즘을 이용한 얼굴 인증과 개선된 퍼지 신경망을 이용한 여권 코드 인식 방법을 제안한다. 본 논문에서는 여권영상에 대해 소벨 연산자를 이용하여 에지를 추출하고 에지가 추출된 영상을 수평 스미어링하여 여권코드 영역을 추출한다. 추출된 여권 코드 영역의 기울기를 검사하여 기울기 보정을 하고, 여권 코드 영역을 이진화 한다. 이진화된 여권 코드 영역에 대하여 8방향윤곽선 추적 알고리즘을 적용하여 여권 코드를 추출한다. 추출된 여권 코드는 퍼지 신경망을 개선하여 여권 코드 인식에 적용한다. 개선된 퍼지 신경 망은 입력층과 중간층 사이의 학습 구조로는 FCM 클러스터링 알고리즘을 적용하고 중간층과 출력층 사이의 학습은 일반화된 델타학습 방법을 적용한다. 그리고 학습 성능을 개선하기 위하여 중간층과 출력층의 가중치 조정에 적용되는 학습률을 동적으로 조정하기 위해 퍼지 제어 시스템을 적용한다. 제안된 퍼지 신경망은 목표값과 출력값의 차이에 대한 절대값이 ${\epsilon}$ 보다 적거나 같으면 정확으로 분류하고 크면 부정확으로 분류하여 정확의 총 개수를 퍼지 제어 시스템에 적용하여 학습률과 모멘텀을 동적으로 조정한다. 여권의 주어진 규격에 근거하여 사진 영역을 추출하고 추출된 사진 영역에 대하여 YCbCr와 RGB 정보를 이용하여 얼굴영역을 추출한다. 추출된 얼굴 영역을 PCA 알고리즘과 스냅샷(Snap-Shot) 방법을 적용하여 얼굴 영역의 위조를 판별한다. 제안된 방법의 여권 코드 인식과 얼굴 인증의 성능을 평가하기 위하여 실제 여권 영상에 적용한 결과, 기존의 방법보다 여권 코드 인식과 얼굴 인증에 있어서 효율적인 것을 확인하였다.s, whereas AVs provide much better security.크는 기준년도부터 2031년까지 5년 단위로 계획된 장래도로를 반영하여 구축된다. 교통주제도 및 교통분석용 네트워크는 국가교통DB구축사업을 통해 구축된 자료로서 교통체계효율화법 제9조의4에 따라 공공기관이 교통정책 및 계획수립 등에 활용할 수 있도록 제공하고 있다. 건설교통부의 승인절차를 거쳐 제공하며 활용 후에는 갱신자료 및 활용결과를 통보하는 과정을 거치도록 되어있다. 교통주제도는 국가의 교통정책결정과 관련분야의 기초자료로서 다양하게 활용되고 있으며, 특히 ITS 노드/링크 기본지도로 활용되는 등 교통 분야의 중요한 지리정보로서 구축되고 있다..20{\pm}0.37L$, 72시간에 $1.33{\pm}0.33L$로 유의한 차이를 보였으므로(F=6.153, P=0.004), 술 후 폐환기능 회복에 효과가 있다. 4) 실험군과 대조군의 수술 후 노력성 폐활량은 수술 후 72시간에서 실험군이 $1.90{\pm}0.61L$, 대조군이 $1.51{\pm}0.38L$로 유의한 차이를 보였다(t=2.620, P=0.013). 5) 실험군과 대조군의 수술 후 일초 노력성 호기량은 수술 후 24시간에서 $1.33{\pm}0.56L,\;1.00{\ge}0.28L$로 유의한 차이를 보였고(t=2.530, P=0.017), 술 후 72시간에서 $1.72{\pm}0.65L,\;1.33{\pm}0.3L$로 유의한 차이를 보였다(t=2.540, P=0.016). 6) 대상자의 술 후 폐환기능에 영향을 미치는 요인은 성별로 나타났다. 이에 따

The Effect of Convergent Aquatic Exercise on Shoulder and Pulmonary Function and Quality of Life in Breast Cancer Patients (융복합적 수중운동이 유방암 환자의 어깨 기능, 폐기능과 삶의 질에 미치는 영향)

  • This study applied aquatic and land-based exercises to breast cancer patients for comparative analysis and elucidation of their effects on the rehabilitation of cancer patients. Participants included 40 breast cancer patients who were divided into two groups: 20 participants in the experimental group using aquatic exercise and 20 participants in the control group using land-based exercise. After the addition of aquatic or land-based exercises five times per week for eight weeks, differences in each group's shoulder and pulmonary functions as well as their quality of life were compared and analyzed. The results showed a statistically significant difference (p<.001) for shoulder dysfunction, with the experimental group showing a greater improvement than the comparison group. Statistically significant differences (p<.05 and p<.001, respectively) were also observed for forced vital capacity (FVC) and forced expiratory volume in one second (FEV1), with the experimental group showing greater improvements. Finally, a statistically significant difference (p<.001) was observed for change in the quality of life, with greater improvements in the experimental group than in the control group. The results of further research in this field are expected to indicate the most efficient method for the recovery and management of physical, psychological, and social health impacts of breast cancer on patients. Furthermore, the research aims to provide fundamental resources for breast cancer rehabilitation in South Korea by conducting studies on patients with lung cancer or colorectal cancer and the application of various aquatic exercise methods.

Value of Pulmonary Function Test as a Predicting Factor of Pneumothorax in CT-guided Needle Aspiration of the Lung (전산화단층촬영 유도하 경피적 폐침생검시 기흉발생 예측인자로써의 폐기능검사의 가치)

  • Background: To evaluate the risk factor of pneumothorax (PNX) which is the most common complication of CT-guided needle aspiration of the lung, we have examined the frequency of PNX according to the presence of obstructive ventilatory impairment determined by pulmonary function tests. Methods: A comparative study of analysis of forecd expiratory volume and folw-volume curves, and determinations of diffusing capacity taken before procedure were made between each 16 cases with PNX and controls with no PNX. Each of the control group was matched for sex, age, height, and size and depth of lesion with the former. Results: 1) In comparison of vital capacity and parameters derived from forced expiratory volume curve between two groups, VC and FVC were not significantly different, whereas $FEV_1$, $FEV_1$/FVC%, and FEF25-75% showed a significant decrease in the PNX gorup. Also, in the PNX group, all the observed values of parameters analyzed from flow-volune curve were siginificantly reduced in the PNX group compared with those in the control group. 2) The diffusing capacity tended to decrease along with varying individual differences in the PNX group. 3) Patients who had obstructive ventilatory impairment according to the results of pulmonary function tests experienced a twofold increase in the frequency of PNX and a sixfold increase in the frequency of chest tube drainage for treatment of PNX compared with those whose results were normal. Conclusion: These findings suggest that the exact evaluation of obstructive lung disease determined by pulmonary function test be considered assessing a pastient's risk for PNX in the patients who will take the CT-guided needle aspiration of the lung.

Correlation Between NT-proBNP and Pulmonary Arterial Pressure in COPD Patients (만성 폐쇄성 폐질환 환자에서 뇌나트륨이뇨펩티드와 폐동맥압과의 상관관계에 관한 연구)

  • Background: Pulmonary hypertension is one of the cardiovascular complications of in COPD. However, a diagnosis of pulmonary hypertension requires an invasive test, such as right heart catheterization. NT-proBNP is a cardiac hormone that is elevated when a cardiac volume or pressure overload is present. It was hypothesized that NT-proBNP might play a role in detecting of pulmonary hypertension in COPD patients. Method: The 31 COPD patients, who underwent all of NT-proBNP, echocardiography, and spirometry in Seoul National University Bundang Hospital during the period from November 2003 to July 2005, were retrospectively analyzed. Result: Of the 31 COPD patients, 9 patients had pulmonary hypertension. A significant positive correlation was observed between the NT-proBNP and pulmonary arterial pressure (r=0.589, p=0.002). However, there was no significant correlation observed between the $FEV_1$ and NT-proBNP and $FEV_1$ and pulmonary arterial pressure. Conclusion: NT-proBNP might indicate the presence of pulmonary hypertension in COPD patients.

  • Background : Unexplained weight loss, which commonly occurs in patients with chronic obstructive pulmonary disease(COPD), is important because weight loss is an independent risk factor of mortality and morbidity in these patients. Leptin is known to play an important role in regulating body weight. In addition, the tumor necrosis factor($TNF-{\alpha}$) might also play a potential role in the weight loss experienced in chronic wasting disease. The aim of this study was to determine the influence of plasma leptin and the circulating $TNF-{\alpha}$ system to the difference in the body compositions in patients with COPD. Methods : Spirometry, body composition analysis and the plasma concentrations of leptin, $TNF-{\alpha}$ and a soluble TNF receptor (STNF-R55, -R75) were measured in 31 patients with chronic bronchitis and 10 patients with emphysema. The COPD subtype was classified by the transfer coefficient of carbon monoxide, DLco/VA. Results : The circulating levels of leptin were significantly lower in those patients with emphysema($108.5{\pm}39.37pg/ml$) than those with chronic bronchitis($180.9{\pm}57.7pg/ml$). The circulating levels of sTNF-R55 were significantly higher in the emphysema patients($920.4{\pm}116.4pg/ml$) than in those with chronic bronchitis($803.2{\pm}80.8pg/ml$). There was no relationship between the circulating leptin levels and the activated TNF system in patients with chronic bronchitis and emphysema. However, the circulating leptin levels correlated well with the BMI and fat mass in both patient groups. Conclusion : These results suggest that the weight loss noted in emphysema patients may be associated with the activation of the $TNF-{\alpha}$ system rather than the plasma leptin level.

  • From August 1979 to August 1995, 73 consecutive patients with various pulmonary diseases underwent pneumonectomy Underlying diseases were lung cancer(53 cases), pulmonary tuberculosis(10 cases), bronchiectasis(4 cases) and others(6 cases). Operative mortality and complication rate for 73 patients and respiratory capacity for 53 patients at postoperative 6 months were measured, and statistically analysed for the influencing factors. The influencing factors on prognosis included age, sex, pathologic finding (benign or malignant), associated diseases, preoperative pulmonary function test and operation time. The statistically significant factors for operative mortality were preoperative MW(% prep)(P=0.013) and operation time(P=0.009). The factors influencing operative complication was infectious disease (P=0. 015), and for respiratory capacity a postoperative 6 months, preoperative FVC(%. prod) (PED.0018), FEVI(%. prod)(P=0.0024), and MW(%. prod) (P=0.004)) were statistically significant factors. The preoperative FVC(%. tyred), FEVI(% . prod) and MW(%. prod) should be measured exactly. We conclude that preoperative lung function, cardiovascular and nutritional status, postoperative care and infection prevention were important factors to decrease the operative mortality and complication as well as to increase respiratory capacity.

Validity of Peak Expiratory Flow for Assessing Reversible Airflow Obstruction (기류 가역성 평가에 있어서 최대호기유속 측정의 유용성)

  • Backgrounds : Assessment of the presence and degree of reversibility of airflow obstruction is clinically important in patients with asthma or chronic obstructive pulmonary disease. The measurement of peak expiratory flow(PEF) is a simple, fast, and cheap method to assess the severity of obstruction and its degree of reversibility. Assessing the reversibility of airflow obstruction by peak expiratory flow(PEF) measurements is practicable in general practice, but its usefulness has not been well investigated. We compared PEF and $FEV_1$ in assessing reversibility of airflow obstruction in patients with chronic obstructive pulmonary disease or asthma and developed a practical criterion for assessing the presence of reversibility in general practice. Methods : PEF measurements were performed (Spirometry) in 80 patients(aged 24-78) with a history of asthma or chronic obstructive lung disease before and after the inhalation of 200 g salbutamol. The change in PEF was compared with the change in forced expiratory volume in one second($FEV_1$). Reversible airflow obstruction was analyzed according to American Thoracic Society(ATS) criteria. Results : A 12% increase above the prebronchodilator value and a 200ml increase in either FVC or $FEV_1$ reversibility were observed in 45%(36) of the patients. Relative operating characteristic(ROC) analysis showed that an absolute improvement in PEF of 30 l/min gave optimal discrimination between patients with reversible and irreversible airflow obstruction(the sensitivity and specificity of an increase of 30 l/min in detecting a 12% increase above the prebronchodilator value and a 200ml increase in either FVC or $FEV_1$ were 72.2% and 72.7% respectively, with a positive predictive value of 68.4%). Conclusions : Absolute changes in PEF can be used to diagnose reversible airflow obstruction.

Effects of Abdominal Functional Electrical Stimulation on Peak Cough Flow and Forced Vital Capacity in Patients with Cervical Spinal Cord Injury (복부 기능적전기자극이 목척수손상환자의 최대기침유량과 노력성폐활량에 미치는 효과)

  • Purpose : The purpose of this study was to investigate the effects of abdominal functional electrical stimulation on peak cough flow and forced vital capacity in patients with cervical spinal cord injury. Methods : The study examined 20 patients with cervical spinal cord injury. The subjects were randomly divided into two groups. All subjects performed conservative physical therapy for 30 minutes. The experimental group also underwent abdominal functional electrical stimulation for at least 20 minutes per day. Abdominal functional electrical stimulation was applied to the rectus abdominis muscle twice each day, three times a week, for four weeks. In all subjects, the peak cough flow was measured using a peak flow meter and forced vital capacity was assessed using a spirometer. Results : The experimental group showed a significant increase in peak cough flow and forced vital capacity in pre-post measurements (p<.05), while the control group showed a significant increase only in peak cough flow. Conclusion : These findings suggest that conservative physical therapy in combination with the abdominal functional electrical stimulation can improve peak cough flow and forced vital capacity in patients with cervical spinal cord injury.

The Influence of Video-Assisted Thoracic Surgery on Hospital Course of Spontaneous Pneumothorax (비디오 흉강경 수술법이 자연기흉의 치료과정에 미치는 영향)

  • Video-assisted thoracic surgery(VATS) is emerging as a viable alternative to thoracotomy when surgical treatment of spontaneous pneumothorax is required. 20 patients with spontaneous pneumothorax underwent bullectomy between July 1995 and May 1996. The patients were divided into two groups : Control group ; the patients who received with mid-axillary approach(n=10), Experimental group ; the patients who received with VATS (n=10). The results were as follows ; 1. The total sex distribution was male predominance (male:female=17:3). Mean age of control group was 29.6$\pm$9.8 years and experimental group was 27.2$\pm$11.9 years. 2. The mean period of postoperative chest tube indwelling duration and hospital stay were 3.3$\pm$0.8 days and 7.9$\pm$1.2 days in control group and 2.1$\pm$0.9 days and 5.2$\pm$3.1 days in experimental group(p=0.005 and p=0.02). 3. The mean time of operation, vital signs and arterial blood gas analysis did not showed any statistical differences between the groups. 4. Percent recovery of tidal volume and forced vital capacity were significantly improved in experimental group comparing with control group (p<0.05). 5. The patients undergoing VATS experienced significantly less postoperative pain and limitation of motion. In conclusion, VATS is safe and offers the potential benefits of shorter postoperative hospital stays and less pain with cosmetic benefits.
