노년기 인지기능과 일상생활기능장애 변화에 대한 연구 - 치매발생집단과 정상유지집단 간 차이 - (Changes in cognitive function and functional disability in older adults - Comparison of groups converted and not converted to dementia among cognitively normal older adults -)

  • 본 연구는 노년기 인지기능과 일상생활기능장애 변화양상을 분석하고, 이에 대한 인구사회학적 요인의 영향력을 검증하는 것을 목적으로 하였다. 특히 인지손상정도에 따라 변화양상과 인구사회학적 요인의 영향력이 다를 것이라는 가정 하에, 정상인지를 가진 지역사회 거주노인에서 이후 6년여의 추적조사 기간 동안 치매가 발생한 집단(치매발생집단)과 정상인지를 유지하고 있는 집단(정상유지집단)에 따른 차이를 분석하는 것에 초점을 두었다. 분석 자료는 경기도 연천지역에 거주하는 65세 이상 노인 966명을 대상으로 3년여 간격으로 3회 추적 조사된 역학조사 자료를 사용하였다. 자료 분석은 구조방정식모형을 활용한 잠재성장모형(Latent growth model)과 다중집단분석(Multi-group analysis)을 실시하였으며, 통계프로그램은 AMOS 18.0을 사용하였다. 연구 결과, 치매발생집단은 치매로 진단되기 수년 전부터 정상유지집단에 비해 인지기능이 유의하게 낮을 뿐 아니라 이후 인지저하가 약 3.5배가량 빠르게 진행되었다. 일상생활기능장애 궤적의 경우 초기에는 집단 간 유의한 차이가 없다가, 이후 치매발생집단의 일상생활기능장애가 정상유지집단에 비해 약 3배가량 더 급속히 악화되는 것으로 나타났다. 인지기능과 일상생활기능장애에 대한 인구사회학적 요인의 영향력을 살펴본 결과, 인지저하는 여성에서 더 급격한 반면, 일상생활기능저하는 남성에서 더 현저하게 나타났으며, 생애초기에 결정되는 학력이 생애후기의 인지기능에 강력한 영향력을 갖는 것으로 나타났다. 이러한 연구결과에 근거하여 노년기 인지기능과 일상생활기능장애에 대한 개입에서 주목해야 할 고위험군과 그에 대한 실천적 함의를 논의하였다.

경로당 이용 여성 노인의 친구·이웃 집단따돌림 현상 연구 (Bullying Behaviors among Senior Women at Senior Center)

  • 본 연구는 경로당을 이용하는 여성 노인들 사이에 친구·이웃 집단따돌림 현상이 어떠한 모습으로 나타나는지를 살펴보았다. 문화기술지를 연구방법으로 선택하였으며, 집단따돌림 현상이 나타나는 A 경로당을 이용하는 여성 노인 16명을 연구참여자로 선정하였다. 참여관찰과 면접을 통해 자료를 수집하였고 Spradley(1979)가 제시한 문화기술지적 분석방법을 바탕으로 결과를 분석하였다. 분석 결과, 경로당 이용 여성 노인의 친구·이웃 집단따돌림 현상은 외톨이로 만듦, 피해를 줌, 대응함, 벗어나지 못함이라는 네 가지 영역으로 나타났다. 먼저, 호의적이지 않은 분위기 속에서 집단따돌림 피해자는 의지할 곳 없는 외톨이가 되었다. 가해자들의 '공격함', '차별함', '고립시킴', '경로당 이용을 저지함'으로 인해 피해자들은 불리한 상황에 처하였다. 둘째, 피해자는 피해를 입는 동시에 다른 사람들에게 해를 가하였다. 이는 경로당을 함께 이용하는 사람들을 배려하지 않고, 거짓말로 속이는 모습, 집단생활을 하는 데 능숙하지 못한 측면, 기여가 적거나 통제된 생활을 어려워하는 상황으로 드러났다. 셋째, 경로당 여성 노인들은 '회피함', '지지 않음', '개선하려 함', '다른 관계를 통해 따돌림에서 빠져나오고자 함' 네 가지 방법으로 집단따돌림 현상에 대응하고자 했다. 마지막으로, A 경로당은 지속적으로 집단따돌림 현상에 머물러있는 모습을 보였다. 끊임없이 새로운 따돌림 대상이 출현했으며, 따돌림은 경로당 안뿐만 아니라 경로당 밖까지 퍼져나갔다. 이러한 결과는 경로당 여성 노인의 집단따돌림 현상이 아동·청소년, 직장 집단따돌림과 유사한 역동을 보여주면서도, 상이한 개인적, 환경적 요인으로 인해 이들과 다른 독특한 측면이 나타남을 시사하고 있다. 따라서 경로당 이용 여성들의 집단따돌림 현상의 고유한 전개 양상들에 주목할 필요가 있다. 이와 같은 연구 결과는 한국 사회에서 나타나고 있는 집단따돌림 중 경로당 여성 노인의 집단따돌림에 대한 논의를 보다 구체화하고 확대시킬 수 있을 것이다.

율동적 동작(Dance movement) 훈련이 노년기 여성의 생리, 심리적 변수에 미치는 영향 (Effect of Korean Traditional Dance Movement Training on Psychophysiological Variables in Korean Elderly Women)

  • Regular long term dance movement could be one of ways to induce improvement of psychophysiological variables, resulting in improvement of quality of life. However, there have been few studies to evaluate the effect of dance movement training on both physiological and psychological variables in the elderly. This study was focused to determine the effect of Korean traditional dance movement training on psychophysiological variables-body weight, body fat, lean body mass, muscle strength, muscle endurance, flexibility, agility, resting heart rate and blood pressure, depression and life satisfaction-in Korean elderly women. Thirty four subjects, aged between 65 and 75years who have normal cognition, sensory function, cerebellum function, cardiovascular function, participated in this study. Seventeen experimental group subjects were selected from E-elderly university in Kyung Gi province, and Seventeen control group subjects were selected from N-welfare facility in Seoul City. Seventeen experimental group subjects participated for 12weeks dance movement program. Korean traditional dance movement program was developed on the basis of Korean traditional dance and music by the author. The program consisted of approximately 50minutes of dance, 3times a week for 12weeks. During 50minutes workout, there were 15minutes of warm-up dancing, 25minutes of conditioning dance and 10minutes of cool-down dancing. The intensity for the conditioning phase was at between 60% and 65% of age-adjusted maximum heart rates. The body weight, body fat, lean body mass, muscle strength(grip strength, leg strength), muscle endurance, flexibility, agility, resting heart rate and blood pressure, depression and life satis-faction were measured prior to and following the experimental treatment. The participants in dance movement were interviewed focusing on subjective feeling following 12 week's regular dance movement. Data were analyzed with mean, standard deviation, percentage of change, X²-test, t-test, and ANCOVA test using SPSS PC/sup +/ program. Subjective feeling was categorized into cognitopsy-chological and physiological responses. Results were obtained as follows : 1) The body weight (F=15.52, p=.000), body fat (F=18.33, p=.000) and lean body mass (F=7.28, p=.011) of the experimental group were significantly lower than those of the control group following the dance movement training. 2) The leg strength (F=30.96, p=.000), muscle endurance (F=9.06, p=.005), agility(F=44.92, 000), flexibility(F=6.84, p=.014) of the experimental group were significantly higher than those of the control group following the dance movement training. There was no significant difference of grip strength(F=.43, p=.515) between experimental and control groups. 3) The heart rate(F=26.96, p=.000), systolic (F=10.40, p=.000) and diastolic(F=3.99, p=.005) blood pressure at rest of the experimental group were significantly lower than those of the control group following the dance movement training. 4) No significant difference of score of depression (F=3.49, p=.071) was observed between experimental and control groups. 5) Score of life satisfaction of experimental group was remarkably higher than that of control group following 12weeks of dance movement training (p<0.05). 6) Thematic responses about the dance movement following the training were positive. "I feel good" was the most frequent among cognitopsychological responses and "I feel lightness of body" was the most frequent among physiological responses. The results suggest that Korean traditional dance movement training can improve psychophysiological variables of Korean elderly.

손과 손가락 근관절운동이 노년기 여성의 악력과 잡기력에 미치는 영향 (The Effect of Hand and Finger Exercise on Grip Strength and Pinch Pressure in Elderly Women)

  • Introduction : Exercise has been suggested as an important nursing strategy in which to help elderly maintain functional performance and to enhanced quality of life. Most of exercise study has been reported on fitness exercise such as walking, swimming, dance etc for health of elderly. There have been few reports about exercise on the promotion of small and fine movement of elderly. The purpose of this pilot study was to determine an effect of 6 weeks hand and finger exercise in home to improve hand muscle strength such as grip strength and finger pinch pressure. Materials and Methods Design: This pilot study was used one group pre and post-test design. Sample: Twelve elderly women above 60 years of age or older living in community were selected by convenient sampling. Procedure: Signed informed consent was obtained prior to participate in this study. The authors met elderly and taught hand and finger exercise, daily a week for 6 weeks, within 30 minutes per session. But exercise frequency and strength were not same. Instruments: Left and right grip strength were measured by Bulb Dynamometer(made in USA) and left and right pinch pressure were measured by Baseline Hydraulic Pinch Gauge(made in USA). Data analysis: Discriptive data analyses were performed on all variables. Wilcoxon matched-pairs Signed-Ranks test were used to find difference of grip strength and pinch pressure between pre and post exercise using SPSS 10.0 for Window. Results: Samples age ranged from 60 to 73, Mean age was 65.3. All were women. Ten elderly were diagnosed osteoarthritis and one had DM. After six weeks hand and finger exercise, Left and right grip strength were higher than those of before exercise(Z=-2.667, P<0.01 ; Z=-3.065, p<0.01). And left pinch pressure after hand and linger exercise were higher than that of before pinch pressure (Z=-2.315, p<0.05). But Right pinch pressure was not shown significant change(Z=-1.099, p>0.05). Conclusions: Although this study was limited by the sample size and design, the findings provide some important implications for community based exercise nursing intervention. Short term (six weeks) exercise of hand and linger is shown to be useful as nursing intervention to maintain routine daily activities such as eating, writing, grip something for elderly.

노년기 여성의 하반신 체형분석에 따른 스커트 원형 제작에 관한 연구 (A research on the pattern fabrication of skirt due to the lower body type of the old aged woman)

  • Due to the population growth of the old aged, Korea is also entering into an aging society. But a research on the pattern design to the old aged is much to be desired. The purpose of this research is to design skirt pattern fitted for each body type by categorizing the lower body type of the old aged woman. For the way of research, categorized the body type by using the female measuring size from the 65 years old to 99 years old which has been measured by the Korean body size research as an assay data, and base on this data, a suitable skirt pattern for each body type of the old aged woman was designed by practicing the exterior evaluation. The data analysis was done with the statistical treatment in SPSS 12.0, and the results are as follows. The lower body type of the old aged woman were divided into the type 1. The circumference article is the biggest compared to the height article regarding to the lower body type of the old aged woman. The type 2. The height article and the circumference and thickness articles are the poorest. The type 3. The height article is the biggest and the hip width and calf circumference are thicker than the waist width. The type 4. The height article is the lowest and the waist width is thicker instead the hip and leg are thinner. In type 1, The pattern was modified by increasing 1cm a length of the dart and raising up 1.2cm at the cross point between side line and waist line. In type 2, the basic pattern was used, and in type 3, using the outline of the basic pattern however, increased 1cm a length of the dart and modified the amounts of dart by handling the art in curve, and modified the waist line to be more three-dimensional curves by raising up 1 cm at the cross point between waist line and side line and falling down 0.5cm at the cross point between the back center line and the waist line. In type 4, modified the amounts of front and back dart to 1.25cm, and raised up 0.8cm at the cross point between side line and waist line, and modified the waist line and the side seam line in a gentle curve by reducing circumference of the skirt tail to 1cm from the side seam line.

대구 시내 및 근교 농촌지역 노년기 여성의 리보플라빈 배설량 연구 (A Study on Urinary Riboflavin Excretion of Elderly Women in Taegu and Rural Area in the Suburbs of Taegu)

  • 노년기 여성들의 일상적인 활동상황과 그와 관련된 리보플라빈 생화학적 영양상태에 관한 연구하기 위해 일상적인 생활양식이 다른 대구 시내와 근교 농촌지역의 여자노인들의 활동상황 및 영양소 섭취량, 소변중 리보플라빈 배설량을 조사하여 분석한 결과는 다음과 같다. 전체 조사대상노인들의 일일 리보플라빈 평균 섭취량은 0.92mg으로 권장량에 매우 미달되었다. 반면에 소변중 리보플라빈 평균 배설량은 $175.6\mu\textrm{g}/24hr$으로 영양불량 판정 기준치보다 높아 리보플라빈 생화학적 영양상태는 평균적으로 양호한 것으로 나타났다. 전체 조사대상노인들의 연령이 증가할수록 활동강도(p<0.005), 에너지 소비량(p<0.005), 크레아티닌 배설량(p<0.005)은 상당히 유의하게 감소하였으며, 연령증가와 소변중 리보플라빈 배설량간에는 유의한 상관관계가 없었다. 소변중 리보플라빈 배설량은 에너지 소비량 및 활동강도와 유의한 상관관계가 없었으며, 리보플라빈 섭취량과도 유의한 상관관계가 없었다. 도시와 농촌여자노인들간에 리보플라빈 섭취량, 에너지 섭취량, 단백질 섭취량 등 식이섭취량은 유의한 차이가 없었으나, 에너지 소비량(p<0.005), 활동강도(p<0.005), 소변중 리보플라빈 배설량(p=0.053) 등은 유의한 차이가 있었다. 즉 농촌 여자노인들의 경우 도시 여자노인들보다 에너지 소비량, 활동강도가 유의하게 많았던 반면 소변중 리보플라빈 배설량은 유의하게 낮았다. 따라서 리보플라빈 생화학적 영양상태는 활동량의 영향을 받았음을 알 수 있으며, 이는 활동량이 많을 경우 리보플라빈 섭취량을 증가시켜야함을 시사한다.

중년기, 노년기 여성에서 무지방조직과 악력, 식이섭취 및 운동습관과의 관계 (Relationship between Fat-Free Mass and Grip Strength, Nutrient Intakes, Exercise Behavior in Middle- and Old-Aged Women)

  • This study was performed to show change of fat-free mass(FFM), representing mostly the muscle mass change, and muscle strength with increasing age, and relationship between dietary, exercise behaviors and FFM in healthy middle-and old aged women who are of age over 55 years. The FFM and correspondingly hand grip strength showed significant positive correlation with age. But concentration of serum albumin showed no significant relationship with age. The subjects were categorized into groups according to FFM tertile. The anthropometry such as weight, BMI, fat mass, circumferences of waist and hip, WHR, and hand grip strength decreased significantly in the lowest FFM group. But the albumin level showed no change according to FFM level. The FFM showed significant correlation with nutrient intakes such as energy, carbohydrate, protein, Fe, P, Ca. No association, however, was shown with exercise behavior probably because of no case with resistance exercise habits. The variance of FFM was explained 55.2% by height and carbohydrate intake. The variance of height-adjusted FFM could be explained only 16.2% by intake amount of carbohydrate. In conclusion, the decrease of FFM may cause to reduce muscle strength in female elderly. The increasing nutrient intakes were associated with the increased FFM and may protect from risk of sarcopenia. However, only the carbohydrate intake could influence independently the FFM in middle- and old-aged women. The FFM has no association with endurance exercise habits. 중년기와 노년기 폐경 여성의 신체활동이 정신건강에 미치는 영향 - 2010년 국민건강영양조사 자료 이용 - (Influences of Physical Activities on Mental Health of Middle and Old Age Menopause Women : Using the 2010 Korea National Health and Nutritional Examination Survey Data)

  • Purpose: The aim of this study is to examine the effects of physical activities on mental health of menopausal women. Method: The data was collected from 5th National Health and Nutrition Examination Survey. 810 samples of 40 to 64 year age group and 660 samples of 65 and over age group were used. To achieve the results estimated without bias stratification variables and colonies, descriptive statistics, Chi-square test, multiple logistic regression were used. Result: Flexibility exercises showed that it had an effect on stress and suicide intension of menopausal women. On the other hand, vigorous physical activity might be a stress factor of menopausal women. Conclusion: Menopausal women in 65 year and over appeared that they had more beneficial to low-intensity exercise to manage stress or suicide intension. This result will be used to develop the mental health promotion programs for menopausal women.

노년기 여성과 청년기 여성의 아연 영양상태와 미각 기능 (Zinc Status and Taste Acuity of Old and Young Women)

  • In an attempt to figure out the relationship between zinc status and taste acuity of old and young women, dietary zinc intake, urinary zinc excretion, and taste acuity were determined for 118 women. Zinc intake was measured by 2-day food records and food frequency method. Urinary zinc excretion was measured from urine samples collected for twenty four hours. Body fat, lean body mass (LBM), and total body water were measured by bio-impedence. Average dietary zinc intake by food record was 4.15$\pm$1.33mg (=35% if Korean RDA) for the old women and 5.41$\pm$2.76mg (=25% of RDA) for young women. When zinc intake was measured by a frequency method, the average intakes of the old and young women were 3.5$\pm$1.7mg 4.5$\pm$1.9mg, respectively. It appears that dietary zinc intake of young women was significantly higher than that of the old women. Average urinary zinc excretion of the subjects was 0.27$\pm$0.16mg in the elderly and 0.24$\pm$0.13mg in young women, which indicated a marginal zinc status. However, zinc status was not significantly different between old and young women. Correlation analysis indicated that zinc intake and urinary zinc excretion were positively related to BMI and LBM in young women. The old women (m=49) showed significantly higher taste detection thresholds than young subjects (n=47) for both sweet and salty tastes. Recognition thresholds for sodium chloride and sucrose were not significantly different between old and young women. The lower the taste thresholds for salty taste, the higher the average dietary zinc intake. However, taste perception concentration was not related to the urinary zinc excretion level.

노인의 성별에 따른 행복감에 영향을 주는 요인 (Factors Affecting Happiness in the Elderly by Gender)

  • 본 연구는 2017년도 지역사회건강조사 자료를 이용하여 65세 이상 남성노인과 여성노인의 인구사회학적 특성, 건강행태 및 주관적 건강수준, 의료이용 등에 따른 행복감 수준과 행복감에 영향을 미치는 요인을 평가해 보고자 하였다. 연구대상은 남성노인 27,979명, 여성노인 39,856명으로 총 67,835명으로 선정하였다. 연구결과 여성노인은 남성노인보다 교육수준과 가구 총 소득이 행복감에 영향을 주었고 남성노인은 배우자 유무가 행복감에 영향을 주어 배우자가 있는 경우 행복감 수준이 높았다. 그리고 남성노인과 여성노인 모두 건강행태 및 주관적 건강수준이 좋을 때 의료이용에 제한이 없을 때 행복감을 크게 느꼈다. 이는 노년기에는 신체 생리적인 기능의 감퇴로 인하여 다른 연령집단에 비해 신체적 건강에 대한 취약성이 증가하는 시기이므로 건강행태 및 주관적 건강수준, 의료이용이 큰 영향을 미치는 것으로 생각된다. 이러한 결과를 토대로 국가적 차원에서 노인복지를 위한 제정을 확대하여 노인의 성별 특성에 맞는 평생교육의 기회를 제공해야 하며 현재 운영되고 있는 노인교육, 건강관련기관의 프로그램을 노인의 특성에 맞게 운영되어야 할 것이다. 또한 노인과 관련된 사회서비스 체계 및 프로그램의 재평가 해 노년의 행복감을 좀 더 효과적으로 증진시킬 수 있는 서비스와 프로그램 개발을 위한 노력이 필요하다.
