A Modified Stabilization of Medial Shoulder Luxation by Biceps Tendon Transposition in a Dog (개에서 상완이두건 변형 전위에 의한 내측 견관절 탈구의 안정화 증례)

  • A 10-year-old intact female Chihuahua weighing 2 kg was presented for complaint of forelimb lameness with non-weight bearing posture. Greater tubercle was palpated medial to its normal location on palpation and crepitation was intermittent with manipulation. On radiographic examination, left medial scapulohumeral joint luxation was found. Bilateral medial patellar luxations also were found and evaluated as grade III. Surgically, the medial scapulohumeral joint luxation was reduced with transposition of the tendon of the biceps brachii muscle. The patient's gait became normal and non-weight bearing posture was resolved at 6 weeks postoperatively.

Treatment of Patella Recurrent Dislocation Through Percutaneous Lateral Release and Arthroscopic Medial Reefing (슬개골 재발성 탈구에 대한 외측부 유리술 및 내측 중첩술을 이용한 치료)

  • Purpose: To evaluate the results of percutaneous lateral release and arthroscopic medial plication for patients with recurrent patella dislocation. Materials and Methods: This study includes the thirty-one cases of patients who have had a surgical operation from March 2001 to March 2008. All the patients had recurrent patella dislocation after trauma history. The average age was 24.2 years old and the average follow up was 47.4 months (minimum 24 months). Results: The preoperative congruence angle, lateral patellofemoral angle, Lysholm score was $24.8^{\circ}$, $-6.2^{\circ}$, 75 points on average respectively. However, the postoperative congruence angle, lateral patellofemoral angle, Lysholm score was improved to normal range: $-2.8^{\circ}$, $10.2^{\circ}$, 95 points on average. Five cases showed the recurrent dislocation after operation. Among them, three cases showed femoral trochlear dysplasia. Conclusion: Percutaneous lateral release and arthroscopic medial plication showed satisfactory results for recurrent patella dislocation with low complication rate. However, in cases with femoral trochlear dysplasia, another surgical treatment may be considered.

Degenerative Joint Disease After Medial Patellar Luxation Repair in Dogs with or Without Trochleoplasty (개의 슬개골 내측 탈구 교정술에서 활차성형술의 유무에 따른 퇴행성 변화)

  • We evaluated outcomes of dogs surgically treated for grade 2 or 3 medial patellar luxation (MPL) with and without trochleoplasty. A retrospective study of 63 dogs with grade 2 or 3 MPL surgically treated with or without trochleoplasty was performed. Results of radiographic evaluation were expressed numerically as degenerative joint disease score and were compared between the groups with and without trochleoplasty. The study included 7 (7/68, 10.3%) cases of reluxation and 1 case of tibial tuberosity transposition implant failure. Reluxation and complications requiring additional surgery were confirmed in 3 cases (3/47, 6.4%) in trochleoplasty group and in 1 case (1/21, 4.8%) in nontrochleoplasty group. The nontrochleoplasty group showed a shorter recovery time than the trochleoplasty group (P < 0.05). There was a significant difference in degenerative joint disease scores over time between the groups, with nontrochleoplasty group having lower scores (P < 0.05). This study suggests that surgical treatment without trochleoplasty results in favorable outcomes compared to treatment with trochleoplasty. It is not mandatory to exclude trochleoplasty when performing surgery for grade 2 or 3 MPL, but we propose that surgical treatment without trochleoplasty is one option when choosing a combination of surgical techniques.

Arthroscopy for the Diagnosis and Treatment of Failed Trochleoplasty in a Dog (활차구 성형술이 실패한 개에서 관절경을 이용한 진단과 치료)

  • A 3-year-old castrated male Spitz dog (8.4 kg) was referred for hindlimb lameness. The dog had undergone corrective surgery for bilateral medial patellar luxation 6 weeks prior to presentation. Intermittent lameness of both hind limbs was evident on general inspection. Physical examination revealed moderate pain and crepitus upon flexion and extension of the stifle joints. Radiography revealed features consistent with distal deviation of the cartilage flaps and effusion in both stifle joints. Detachment and deviation of both osteochondral blocks were suspected based on the physical and radiographic findings. Arthroscopy confirmed failed osteochondral blocks (right $1.5cm{\times}0.9cm{\times}0.2cm$, left $1.3cm{\times}0.9cm{\times}0.1cm$), which were removed. After removal of the osteochondral blocks, the stepped trochlear grooves were located and debrided using an arthroscopic burr, to prevent patellar tracking disorder. The dog resumed normal ambulation without medial patellar luxation 2 weeks postoperatively, and there were no complications. Arthroscopy is an effective surgical modality for the diagnosis and treatment of canine joint disorders. It offers a magnified view of the joint, which allows accurate evaluation of intra-articular structures. In addition, arthroscopic surgery can reduce the postoperative complications associated with surgical wound.

Arthroscopic Reduction of Subluxed Medial Meniscus using Suture Anchor for Restoration of Hoop Stress - Technical Note - (버팀테응력 회복을 위한 아탈구된 내측 반월상 연골의 Suture anchor를 이용한 정복술 - 술기 보고 -)

  • Purpose: The authors introduce a new technique of arthroscopic reduction of subluxed medial meniscus using suture anchor for the restoration of hoop stress. Operative Technique: Anterolateral, anteromedial, and medial midpatellar arthroscopic portal are used. Arthroscope was inserted through anterolateral portal. Through the scope, we confirmed subluxation of medial meniscus. Transection of menisci including radial and root tear were excluded. We released the anterior horn of medial meniscus through anteromedial and burred the future insertion site of suture anchor. After inserting suture anchor through medial midpatellar portal, we used 90 degree suture hook and no.2 Nylon to retrieve the suture of inserted anchor. We tied the suture by sliding knot-tying method. Weight bearing was limited for 6 weeks postoperatively. Conclusion: Arthroscopic retightening of medial meniscus is less invasive, conserving and progressed method for subluxed meniscus.

Long-term Follow-up after Implantation of Autologous Adipose Tissue Derived Mesenchymal Stem Cells to Treat a Dog with Stifle Joint Osteoarthrosis (골관절증을 보이는 개에서 자가지방 유래 줄기세포 치료 증례)

  • A 5-year-old castrated male Chihuahua weighing 1.54 kg was examined because of a several month history of progressive right hind limb lameness. Physical examination of the stifle joints revealed pain and a grade IV medial patellar luxation on the right stifle joint. The right and left stifle joints were associated with a lameness of grade 2 and grade 0, respectively. Radiography revealed osteophytes or subchondral cystic lesions on the right and left stifle joints. Osteoarthrosis (OA) scores for the right and left stifle joints were 20 and 12 respectively. Combination of surgery and implantation of autologous adipose tissue derived mesenchymal stem cells (aAT-MSCs) was determined with informed consent. $1{\times}10^6$ aAT-MSCs suspended in PBS and 0.6 mL of hyaluronic acid were injected in the right stifle joint postoperatively. Osteoarthrosis scores and the lameness grade for the right and left stifle joints were 19 and 13, and 0 and 0 19 months after treatment, respectively, and 14 and 15, and 0 and 0 five years after treatment, respectively. This case report shows radiographical evidence of a decrease in osteophytes and subchondral cystic lesions on the stifle joint with OA after aAT-MSCs injection.

Arthroscopically Assisted Lateral Release and Medial Imbrication for Recurrent Patella Dislocation (재발성 슬개골 탈구에서 관절경적 외측 유리술 및 내측부 중첩술)

  • Purpose: We reported the results of arthroscopically assisted lateral release and medial imbrication for the recurrent patella dislocation. Materials and Methods: Twenty patients (20 knees) underwent arthroscopically assisted surgery for the recurrent patella dislocation. There were 4 males and 16 female. The average age was 20.2 years. All patients had definite trauma history and average follow-up period was 19 months. The surgical results were evaluated according to the Lysholm knee score and the Kujala score. The congruence angle and lateral patellofemoral angle were measured on plain radiograph and the tibial tubercle-trochlear groove distance was calculated on computerized tomography. Results: The median value of preoperative congruence angle was $16.5^{\circ}$ (range, $0.0{\sim}+34^{\circ}$) and the average final follow-up was $-6.4^{\circ}$ (range, $-19{\sim}10^{\circ}$) with statistically significant improvement (p=0.025). The median value of preoperative Lysholm knee score was 70 (range, 63~81) and the final follow-up score had changed to 88 (range, 80~95) with statistically significant improvement (p=0.0341). The median value of preoperative Kujala score was 72 (range, 65~80) and the average final follow-up score showed 87 (range, 80~92) with statistically significant improvement (p=0.024). Recurrent dislocations after surgery occurred in 2 cases, one case which showed positive "thumb to forearm test" had been treated with medial patellofemoral ligament reconstruction. Conclusion: Arthroscopically assisted lateral release and medial imbrication for recurrent patella dislocation without bony malaligmenent showed the effective treatment, but would be inappropriate for the patients with the generalized joint laxity.

  • Acute patellar dislocation is a painful, terrifying experience that always occurs suddenly. The recurrence is unfortunately common and each experience is just as painful to the patient. With each acute dislocation, additional intraarticular derangement occurs. Therefore the goals of treatments are not only to relieve acute pain by prompt reduction of the dislocation but also to remove any chondral or osteochondral fragments that may be present within the knee joint and to restore normal patellofemoral anatomy, thereby preventing recurrent dislocation and avoiding future patellar subluxation. To present the details of arthroscopic medial retinacular repair in acute patellar dislocation. we report 2 patients with treatment of acute patellar dislocation with osteochondral loose body.

Postoperative Electroacupuncture in 2 Dogs showed Continuous Lameness after the Reduction of Medial Patellar Luxation (슬개골 내측탈구 교정술 후 지속적 파행을 보이는 개에서의 전침치료 2례)

  • Canine medial patellar luxation can cause hindlimb lameness in toy breeds and surgical operation needs when the lameness does not improve or worsen. In this study, the 2 dogs that showed postoperative non-weight bearing lameness were treated with electroacupuncture (EA) and the effects of postoperative EA were examined. Xue Hai (Sea of Blood), Du Bi (calf's nose, 2 acupoint), Zu San Li (leg 3 miles) acupoints were used in acupuncture. Xue Hai acupoint was connected to positive pole and Zu San Li was negative for 20 min with 2~5 volt, 5~30 Hz in EA. In case 1, EA were performed eight times for 26 days after 29th postoperative day and walking was improved from 6th EA. In case 2, EA were conducted five times for 17 day after 26th postoperative day and walking was improved from 4th EA. It may be possible to improve walking by EA treatment in the dogs that show post-operative non-weight bearing lameness, but the relationship between the effects of EA and the injury of stifle joint should be inspected.

Compensatory Change of a Dog with Glenoid Dysplasia in Kinetic Gait Analysis before and after Reconstruction of Medial Patellar Luxation (Case report) (관절오목 이형성이 있는 개의 내측 슬개골 탈구 수술 전, 후의 운동역학적 보상적 보행 변화 (증례보고))

  • Background: This study aims to investigate compensatory strategy in a dog with glenoid dysplasia using kinetic gait analysis before and after reconstruction of medial patellar luxation. Design: Case report Method: On the platform, gait analysis was evaluated for a dog with congenital luxation and bilateral medial patellar luxation (MPL). A dog was evaluated for maximal vertical force (MVF), body load distribution (BLD), and symmetry index (SI), including the left forelimb with congenital luxation before MPL surgery, 15 days of surgery, and 40 days of surgery. Result:: In the comparison between the preoperative and the 15 days of surgery, the MVF of the bilateral forelimbs, especially in the non-affected forelimbs, increased, and the SI also increased. For BLD, the maximum load distribution increased, but the total load distribution decreased. In the comparison of 15 days and 40 days of surgery, MVF and BLD increased, and SI decreased on 40days of surgery. Conclusions: In a dog, the shift in weight load to the non-affected side occurs all the limbs, affecting the peak vertical force, weight load distribution, and symmetry index.
