Implant-assisted removable partial denture using freely removable abutment in a fully edentulous patient: A case report (완전 무치악 환자에서 자유 착탈가능 임플란트 지대주를 이용한 임플란트 융합 국소의치 수복증례)

  • Implant-Assisted Removable Partial Dentures (IARPDs) treatment is being performed in a fully edentulous patient using implant surveyed prosthesis as an abutment. Implant-supported prosthesis as an abutment of IARPDs is classified into screw-retained and cement-retained type according to the retention type, and each has advantages and disadvantages. The EZ crown system (Samwon DMP, Yangsan, Korea) has a cylinder combined with abutment, and the nickel-titanium spring in this cylinder provides a constant force on the zirconia ball to obtain retention in EZ crown system. In this patient, the natural abutment teeth of the mandibular overdenture was hopeless. We planned implant assisted removable partial denture using anterior implant surveyed prosthesis considering functional and esthetical rehabilitation, cost and patient's needs. When fabricating IARPDs using implant as abutment, we could compensate for the shortcomings of existing implant-supported prosthesis retention type and made the design of removable partial denture easy due to using EZ crown system.

  • They have been recently introduced many aesthetic implant prosthesis using with zirconia and CAD/CAM. However, there are many limitations in their gingival and occlusal region. In this case, submucosal zirconia implant prosthesis were fabricated with CAD/CAM system. The connection of these screw cement retained prosthesis and titanium abutment was designed to 1mm above the fixture. The clinical results were satisfactory on the aesthetics and function.

  • The dental osseointegration implant should be enough to endure occlusion load and it's required to have efficient design and use of implant to disperse the stress into bones properly. Solidworks as a finite element analysis program for modeling and analysis of stress distribution was used for the research. The simple crown model was designed on applying conjoined condition with tightening torque of 20 Ncm of a abutment screw between a cement retained implant abutment and a fixture. A $45^{\circ}$ oblique loading from lingual to buccal side on buccal cusps of crown and performed finite element analysis by 100 N of external load. The results by a analysis for stress distribution of supporting bones of fixture were as below. The von Mises stress was concentrated on the upper side of supporting compact bone regardless of the diameters and lengths of fixture, and the efficiency result of stress reduction was increase of fixture's diameter than it's length. Therefore, it's effective to use wider fixture as possible to the conditions of supporting jaw bone.

  • A passively fitting prosthesis is an essential prerequisite to attain long-lasting success and maintenance of osseointegration. However, true "passive fit" can not be achieved with the present implant-supported prosthesis fabrication protocol. Many clinical situations are suitably treated with cantilevered implant-supported fixed restorations. The purpose of this study was to compare the stress distribution pattern and magnitude in supporting tissues around ITI implants with cantilevered, implant-supported, screw-retained fixed prosthesis according to the fitness of superstructures. Photoelastic model was made with PL-2 resin (Measurements, Raleigh, USA) and three ITI implants (${\phi}4.1{\times}10mm$) were placed in the mandibular posterior edentulous area distal to the canine. Anterior and posterior extended 4-unit cantilevered FPDs were made with different misfit in the superstructures. 4 types of prosthesis were made by placing a $100{\mu}m$ gap between the abutment and the crown on the second premolar and/or the first molar. Photoelastic stress analysis were carried out to measure the fringe order around the implant supporting structure under simulated loading conditions (30 lb).

Implant-supported fixed prostheses with high-performance polymer (PEKK) abutments in partial edentulous patients: A case report (부분 무치악 환자에서 고기능성 폴리머(PEKK) 지대주를 이용한 임플란트 고정성 보철물 수복 증례)

  • Implant treatment, which was first attempted in fully edentulous patients, is now widely used in partially edentulous, and a single tooth missing patients. Moreover, implant treatment has become an essential treatment modality in modern dentistry. The material of fabricating implant prostheses has also become more diverse than before, one of which is the use of high-performance polymers. The frequency of using high-performance polymers, which have been used in the medical field, is also increased in the dental field compared to the past. In the first case, a PEKK abutment and a PFG crown (cement-screw-retaining type) were fabricated in the lower left second premolar, and in the second case, a PEKK abutment and a monolithic zirconia crown (cement-screw-retaining type) were fabricated in the missing upper left first molar, and in the third case two PEKK abutments and a splinted PFM crowns (cement-screw-retaining type) were fabricated and connected to the upper right first and second molar implants. Through these procedures the patients obtained esthetically and functionally satisfactory results after 4 years of follow-up.

Influence of bearing surface angle of abutment screw on mechanical stability of joint in the conical seal design implant system (내부 원추형 연결형태 임플란트에서 지대주 나사머리의 좌면각도가 연결부 기계적 안정성에 미치는 영향)

  • This study is to evaluate how different bearing surface angles of abutment screw affect the mechanical stability of the joint in the conical seal design implant system. Materials and Methods: Internal connection type regular implants, two-piece cemented type abutments and tungsten carbide/carbon-coated titanium alloy abutment screws were selected. Titanium alloy screws with conical ($45^{\circ}$) and flat ($90^{\circ}$) head designs which fit on to abutment were fabricated. The abutments were tightened to implants with 30 Ncm by digital torque gauge. The loading was applied once to the central axis of abutment. The mean axial displacement was measured using micrometer before and after the tightening and loading (n = 5). The abutment was tightened to implants with 30 Ncm and T-shape stainless steel crown was cemented. Then the change in the amount of reverse-torque was measured after the repeated loading to the central axis, and the place 5 mm away from the central axis. Compressive bending and fatigue strength were measured at the place 5 mm away from the central axis (n = 5). Results: Both groups showed the largest axial displacement when abutment screw tightening and total displacement was greater in the flat head group compared to conical head group (P < 0.05). However, there were no significant differences in reverse torque value, compressive bending and fatigue strength (P > 0.05). Conclusion: Within the limitations of this study, the abutment screw head design had no effect on two groups regarding the joint stability, however the conical head design affected the settlement of abutment resulting in the reduced total displacement.

Technical complications of cement-retained implant-supported single crowns and splinted crowns with zirconia frameworks (지르코니아 프레임워크를 이용한 시멘트 유지형 임플란트-지지 단일 크라운과 연결 크라운의 보철적 합병증)

  • Purpose: This study was to assess clinically the success rates and technical complications of cement-retained implant-supported single crowns and splinted crowns with zirconia frameworks. Materials and methods: 75 (single crowns: 51, splinted crowns: 24) cement-retained implant-supported single crowns and splinted crowns with zirconia frameworks which were restored in 67 patients were investigated for the evaluation of the success rates and technical complications. All restorations were cemented with temporary cement. Age, gender, restoration position, opposing teeth, restoration type were assessed as possible factors affecting technical complications. Results: During the mean observation period of 22.2 months, cumulative success rates of all restorations were 66.9 (73.2 - 60.6)%. Retention loss was found in 16 restorations (single crowns: 14, splinted crowns: 2), abutment screw loosening and veneer porcelain fracture were found in each 2 single crowns, respectively. According to a Kaplan-Meier survival analysis of single crowns and splinted crowns, the cumulative success rates were 58.9 (66.6 - 51.2)%, 87.5 (96.1 - 78.9)%, respectively. There was a statistically significant difference. The other possible factors did not have a significant effect on the technical complications. Conclusion: Retention loss was the most frequent technical complication. Abutment screw loosening and veneer porcelain fracture were found rarely in single crowns only. Age, gender, restoration position, and antagonist did not have significant effect on the technical complications. Splinted crowns had a higher success rate than single crowns.

Full mouth rehabilitation of edentulous patient with fixed implant prosthesis (고정성 임플란트 보철물을 이용한 완전 무치악 환자의 구강회복 증례)

  • There are various treatment options such as conventional complete denture, hybrid prosthesis and implant-supported fixed prosthesis for fully edentulous patients. In case of implant-supported fixed prosthesis, compared to removable prosthesis, it is difficult to place the implant in the correct position considering the anatomical contours of the final prosthesis. In this case, a full mouth rehabilitation with implant-supported fixed prosthesis was performed for a patient who required extraction of all remaining teeth due to dental caries and chronic periodontitis. In the implant placement stage, the implant was placed in the desired position using a surgical guide fabricated considering the anatomical contours of the final prosthesis, and the function and esthetics were evaluated through correction and re-fabrication of the fixed provisional restoration. A final restoration of porcelain fused to gold prosthesis was delivered to the patient based on the provisional restoration. To cope with complications such as loosening of screws and fracture of porcelain, a screw-retained type prosthesis was fabricated for the posterior part and a screw-cement-retained type prosthesis for the anterior part. As a result, the patient showed an improved prognosis in terms of functional and esthetics after the final prosthesis was delivered.

  • All the faculties at the prosthodontic department in Kyung Hee University Dental Hospital at Gangdong, have been implementing the Top-Down concept of treatment approach since 2006 in which the outcomes of the last prosthetic treatment are predicted in advance during the treatment planning stage of patients with dental tissue defects. Based on the Top-Down concept, this report is also an example of how the final prosthetic treatment was performed in advance before going into implant surgery for the missing teeth. Among the various methods of connecting implant fixture and restoration, the cement-retained method is relatively simple to manufacture restoration without being constrained by the angle of the implants placed, but difficult to remove remaining subgingival excess cement completely, and to detach it being when necessary. In the report, SB-locking method will be introduced which enables an aesthetic implant restoration without either a screw hole or residual excess cement.

원지점 차넣기 모타 적용 잠입노즐 기초 기술 개발

  • 잠입노즐은 로케트 추진기관의 길이 및 중량을 감소시켜 체계설계의 관점에서 볼 때 많은 이점을 제공한다. 본 연구에서는 3단형 과학로케트 원지점 차넣기 모타(apogee kick motor)에 적용하기위한 잠입노즐의 기초기술 개발에 주안점을 두었다. 고고도에서 저속으로 회전하며 비행하는 원지점 차넣기 모타를 제작하기위해서 체계 요구성능에 의해 예상된 실물형의 50% 크기에 해당하는 축소형 잠입노즐을 제작하였다. 잠입노즐은 잠입부의 내외부가 고온의 추진제 연소가스에 노출된 상태에서 노즐 내부 압력 외에 연소실압에 의한 외부압력이 작용하므로 이를 고려한 열 및 구조설계가 중요하다. 본 연구에서는 노즐 수렴부와 목부에 일체형 그라파이트 소재를 적용하고 확장부 내열재 및 잠입부 배면내열재에 탄소/페놀 복합재를 노즐 내열재로 사용하였다. 그리고 이들의 구조적 지지를 위해 스틸구조물을 적용하였다. 적용된 스틸구조물에는 K형 열전쌍을 이용해 내열재와 구조물 온도를 측정할 수 있는 관통구멍 및 나사부를 구조물 외변에 가공하였다. 열전쌍은 노즐 목직경의 2, 4배 되는 확장부 내열재 단면위치의 2mm와 4mm 깊이와 구조물 내면 및 외면의 4개소에 열전쌍을 부착하여 지상연소시험시 노즐 내열재와 구조물의 온도분포를 관찰한다. 그리고 노즐 조립시 확장부 내열재와 구조물에 각 각 반원형 홈을 내어 여분의 접착제가 원형 홈에 밀려들어가 경화되어 노즐 기밀유지와 체결력을 향상시킬 수 있는 원형공간 접착제 충전 공법을 적용하여 실제모타에 대한 적용가능성을 지상연소시험을 통해 확인한다.
