Bio-signal Based Rehabilitation Program and Data Management (국내 연하장애 기능적 전기 자극치료기 사용 현황과 장비관리 실태 분석)

  • This study has been conducted to identify the current status and practices of electrical stimulation for patients with dysphagia in Korea. Main goal of the study is to find evidences of developing and distributing a quality indicator and organized performance exam. This study analyzed 50 survey questionnaires given to clinical professionals in upper-scale general hospitals, university hospitals, nursing homes and professional rehabilitation hospitals. The survey sent out and collected by E-mails or interviews from August 21, 2013 to November 10, 2013. Most of the hospitals showed lack of using a quality indicator and organized performance exams in device maintenance, pre-operational tests and post-operational tests. It is strongly recommended to develop a standardized guideline of a higher quality indicator and organized performance exam in using neuromuscular electrical stimulator for patients who are suffering from dysphagia.

흰쥐에서 반회후두신경 손상후 Pulsed electromagnetic fields(PEMFs)가 기능 회복에 미치는 영향

  • 배경 및 목적 : 성대마비환자에서 마비된 성대의 기능회복을 위해 여러 가지 방법으로 신경재지배를 시도하였으나 대부분이 misdirected reinnervation에 의한 후두의 synkinesis로 임상적으로 널리 이용되지 못하고 아직까지는 성대내전술등으로 음성의 질을 개선시키는 정도로만 치료하고 있는 실정이다 최근 복원에서는 반회후두신경의 절단 후 일차봉합에 의한 신경 재지배 후 후윤상피열근에 만성전기자극을 주었으나 자극을 주지 않은 대조군에 비해 후두기능회복이 더 잘된다는 결과를 확인하지 못한바 있다. 1980년대부터 pulsed electromagnetic fields (PEMFs)가 조직내에 ionic currents를 유도하여 cellular functions을 변화시킨다는 것을 보고된 이래로 PEMFs는 골절의 불유합이 있는 환자들에게 임상적으로 이용되고 있으며 최근 동물실험에서 흰쥐의 절단된 sciati nerve, common peroneal nerve, facial nerve의 신경재생을 촉진시켜 신경 재생이 조기에 되고 기능적으로 더 잘 회복되는 것으로 보고 있다. 이에 본 연구에서는 신경재생이 되더라도 기능적 회복이 힘든 것으로 알려진 반회후두신경의 재생 및 후두기능 회복에 대한 PEMFs의 영향을 알아보고자 하였다.   • A patient-specific musculoskeletal model, whose parameters can be identified noninvasively, was developed for the automatic generation of patient-specific stimulation pattern in FES. The musculotendon system was modeled as a torque-generator and all the passive systems of the musculotendon working at the same joint were included in the skeletal model. Through this, it became possible that the whole model to be identified by using the experimental joint torque or the joint angle trajectories. The model parameters were grouped as recruitment of muscle fibers, passive skeletal system, static and dynamic musculotendon systems, which were identified later in sequence. The parameters in each group were successfully estimated and the maximum normalized RMS errors in all the estimation process was 8%. The model predictions with estimated parameter values were in a good agreement with the experimental results for the sinusoidal, triangular and sawlike stimulation, where the normalized RMS error was less than 17%, Above results show that the suggested musculoskeletal model and its parameter estimation method is reliable.

  • This paper deals with an experimental study on the reaction of muscles by functional electrical stimulation(FES) with the design and fabrication of an ankle reaction apparatus. The ankle reaction apparatus consists of a circuit part and a kinematic part, and it monitors reaction time for a particular angle of joint rotation according to FES. The experimental results showed that the change of the ankle rotation angle was linearly proportional to the change of the magnitude of FES. It also showed that the muscle's reaction time was constant no matter how large the magnitude of the stimulus. The results of this paper can be applied to develop an active-type walking-assistance robot.

The Effect of Self Swallowing Exercise Program with Neuromuscular Electrical Stimulation(NMES) on Swallowing Function of Dysphagia (신경근 전기자극치료와 함께 적용된 자가 삼킴 운동 프로그램이 삼킴 장애 환자의 삼킴 기능 향상에 미치는 효과)

  • Objective : The purpose of this study is to investigate the effect of self swallowing exercise program with neuromuscular electrical stimulation(NMES) on swallowing function. Methods : Subjects who were diagnosed in dysphagia were randomly divided into the control group or experimental group. Both group were received NMES during 60 minutes with traditional swallowing therapy during 30 minutes. Additionally the experimental group was received self swallowing exercise during 30 minutes. We invested subject's characteristics through medical chart. We used VDS(Videofluoroscopic Dysphagia Scale) and PAS(Penetration Aspiration Scale) for assessing the swallowing function. Results : There were not significantly different in both group's pre swallowing function. The control group was significantly improved on pyriform sinus residue, aspiration, and VDS total score(p<.05). The experimental group was significantly improved on vallecular residue, pyriform sinus residue, and VDS total score(p<.05). Both group's difference of pre and post swallowing function were not significantly different. Conclusion : Self swallowing exercise and traditional swallowing therapy with NMES and traditional swallowing therapy with NMES are positive effect on swallowing function. The self swallowing exercise is not effective factor.

  • Response to nerve stimulation platform for implementing measures to detect finger movement has been functioning as an important factor. The finger movement of the muscle response to nerve stimulation and sensing Actuator for the H/W development is needed. In addition, a low power embedded CPU based on the top was used. The Effects of Low-Frequency Electrical Stimulation on Abdominal Core Muscle Activity and Thickness (저주파 전기자극이 복부 중심근육 활성도 및 두께에 미치는 영향)

  • Core muscle is the most crucial for rehabilitation of patients with musculoskeletal and nervous problem, there is pain, imbalance and functional movement disorder of upper and lower extremities. In this study, low-frequency electrical stimulator was developed for the purpose increasing the activity of rectus abdominis (RA) and the thickness of transverse abdominis (TrA), internal obliquus abdominis (IO), and external obliquus abdominis (EO). Fifteen female in their 20's was divided into a experiment groups (traditional electrical stimulation group, developed electrical stimulation group) and control group. Low-frequency electrical stimulation was performed in experimental groups for 4 times a week, 4 weeks. The muscle activities (RA) by EMG and muscle thickness (TrA, IO, EO) by Ultrasonogrphy imaging were measured, respectively. In the result, the muscle activity of RA and muscle thickness of TrA and EO were significantly increased at developed electrical stimulation group (p<.05). Therefore, the developed low-frequency electrical stimulation is useful for rehabilitation with CNS and PNS subjects.

  • This paper experimentally deals with the excitation by functional electrical stimulus(FES) and the response of electromyo-potential at the muscles of antebrachial joint in a human body. The excitation of FES, which results in the contraction of the muscles and thus the flection of the joint, shows that the flection angle of the joint is proportional to the magnitude of the stimulus current. The response of electromyo-potential measured according to the FES shows the linearly-proportional relation between the joint torque and the electromyo-potential. The results can be used for active motion of joint rehabilitation.
