The Effects of Chest Vibration Prior to Endotracheal Suctioning on Oxygenation and the Amount of Lung Secretions in Premature Infants with Respiratory Distress Syndrome (호흡곤란증 미숙아에 있어 기관지 흡인술전의 흉곽진동법이 산소화와 기관지 분비물의 양에 미치는 영향)

  • 미숙아에 있어 주로 표면활성제의 부족으로 인한 호흡곤란증은 미숙아 사망의 주원인이 된다. 호흡곤란증의 치료목적은 적절한 환기를 통해 체내 산소화를 유지시키는 것이다. 인공호흡기 치료를 받는 미숙아에게 필수적인 간호중재인 기관지 흡인술은 그 효과를 극대화하기 위해 흉곽진동법과 같이 사용될 수도 있다. 그러나 미숙아를 대상으로 하는 흉곽진동법은 그 중재의 안전성이나 효과에 대한 과학적 검증 없이 시행되고 있는 실정이다. 이에 본 연구는 호흡곤란증 미숙아에 있어 기관지 흡인술 이전에 행하여지는 흉곽진동법이 산소화와 기관지 분비물에 미치는 영향을 연구하기 위해 실시되었다. 이를 위해 20명의 호흡곤란증 미숙아를 대상으로 대상자내 반복실험연구가 설계되었다. 독립변수는 기관지흡인술이전에 흉곽진동이고 종속 변수는 산소 포화도, 심박동수, 그리고 기관지 분비물의 양이었다. 각 대상자는 무작위 순서에 따라 한번은 흉곽진동없이 흡인을, 나머지 한번은 흉곽진동과 흡인의 두 가지 형태의 흡인을 경험 하였다. 연구 결과, 기관지 흡인술이전에 흉곽진동을 실시하든, 안하든 산소포화도와 심박동수의 변화양상에는 차이가 없었다. 그러나, 융곽진동법을 실시한 경우가 실시안한 경우에 비해 더 많은 양의 기관지 분비물을 흡인하였다. 이는 기관지 흡인술 이전에 실시하는 흉곽진동법이 미숙아의 체내에 부가적 산소소모를 초래하지 않는 반면, 기도로부터 더 많은 분비물을 흡인하는데 효과적임을 시사한다.

  • Foreign body aspiration (FBA) is a common accident in young children. Undiagnosed and retained foreign bodies may result in severe early and late complications such as asphyxia, pneumonia, atelectasis and bronchiectasis. Moreover, because it can mimic bronchiolitis, croup or asthma, an accurate history and a high index of suspicion are of paramount importance for early diagnosis. With our experience on bronchial FBA initially misdiagnosed as acute bronchiolitis, we emphasize that a minute radiological finding should not be neglected and a repeat chest radiograph may be helpful when the initial study shows normal findings. Multidetector computed tomography is a very useful noninvasive diagnostic modality for FBA.

The Role of Bronchoscopy for the Staging in Patient with Peripheral Lung Cancer (말초형 폐암 환자에 있어서 병기판정을 위한 기관지 내시경검사의 필요성)

  • Background : Bronchoscopy has been widely used for a histologic diagnosis through a transbronchial lung biopsy or for staging of patients with peripheral lung cancer. However a transthoracic needle aspiration (TTNA) has been used more widely for a histologic diagnosis in patient with a small size nodule or a nodule located in the outer portion of the lung because of the low diagnostic yield of bronchoscopy in these cases. The role of bronchoscopy for staging is not well established in patients with peripheral lung cancer diagnosed by a TTNA or patients who are undergoing surgery without a histologic diagnosis. Method: To evaluate the role of bronchoscopy for the staging in patients with peripheral lung cancer, who were diagnosed by TTNA, the medical records of 86 patients with peripheral lung cancer who underwent bronchoscopyat Kyungpook National University Hospital between January 1995 and May 1997 were reviewed. Results : While 53 cases had normal bronchoscopic findings, 33 cases had abnormal bronchoscopic findings comprising 9 cases of tumor, 10 cases of infiltration and 14 cases of compression of which there were 25 cases of T1 and 8 T2 endoscopically. The bronchoscopic staging did not influence the changes of the clinical stage of lung cancer. The frequencies of bronchial involvement tended to increase as the sizes of the nodule increased. Among the 42 patients who underwent surgery, 9 patients staged higher after operation because of lymph node involvement in 8 patients and the involvement of the pulmonary artery in 1 patient. No case staged above after operation due to a bronchial invasion. Conclusion : These findings suggests that bronchoscopy is not useful for staging in patients with peripheral lung cancer diagnosed by a TTNA.

  • Occult bronchial foreign bodies are rare in adults, whereas tracheobronchial aspiration of foreign bodies occurs commonly in children. Treatment guidelines, according to the severity of the bronchial or lung parenchymal damage and the duration of foreign body retention, have not been established. A 40-year-old man with chronic cough, sputum production, and fever was transferred for treatment of right middle and lower lobe collapse and obstructive pneumonitis as evidenced by imaging studies. He had aspirated the cap of a felt-tipped pen 30 years before presentation, which was unrevealed until his medical history was carefully reviewed during this episode. The patient was treated with right middle-lower bilobectomy because fiberoptic bronchoscopic removal of the foreign body failed. This case added important information to our body of knowledge concerning the various clinical features of occult bronchial foreign bodies.

  • We applied our technique of selective bronchial suctioning (SBS) for the treatment of atelectasis after resection surgery of lung in four patients with refractory atelectasis who were treated successfully. We considered that SBS using hydro-catheter insertion under local anesthesia above fourth tracheal ring is the effective technique for the treatment of refractory atelectasis when conventional respiratory therapy is not effective and a bronchoscopist is not available.

National-Wide Survey on Endotracheal Suctioning in High-Risk Infants (고위험신생아를 위한 기관지흡인에 대한 실태조사)

  • The goal of respiratory management in high risk infants is to maintain proper oxygenation by supporting respiration, therefore to minimize the secondary complications and to promote the maximum growth and development. While on artificial ventilator to achieve this goal, the infants require endotracheal suctioning(ETS) to remove lung secretions. However, the negative effects of ETS in neoates have been documented and include hypoxia, bradycardia, mucosal damage, increased intracranial pressure, and death result. The purpose of the study was to investigate how ETS is currently performed in NICU, which would be beneficial to develop the standardized ETS protocol and to apply it to these population. A national-wide survey on clinical protocol of ETS was performed to 149 neonatal nurses with the average of 3 years and 6 months experience in neonatal nursing, 34.2% of whom was bachelor in nursing. The results showed that about 89% of the nurses initiate En primarily based on the need of the subjects. The aseptic regulation on ETS was used in 83.9% of the subjects. There was no regulation on the length of catheter in 32.9% and on ID/OD ratio in 17.4%. Many nurses administered hyperoygenation/hyperinflation/hyperventilation based on personal knowhow, rather than scientific rationals (77.2%, 40.9%, 75.2%, retrospectively). About 41% of the nurse regulate subjectively the suction power, while 73.8% of them rotate the sub ject's head during suctioning and the half of the nurses was favorable in adapting the closed-suctioning protocol. With the findings of the study, the current clinical application of E% in neonates appears to be based on adult care practices, or personal preference, rather than scientific validation of the safety and effectiveness of the procedure. This study support the needs for developing and applying the standardized ETS protocolin conjunction with the consideration given to the physiologic characteristics of the neonates in respiratory distress.

  • Recently, The non-penetrating injury of bronchus has been increased, especially by traffic accident. Early diagnosis and primary repair of bronchial injury not only restore normal lung function but also avoid the difficulties and complications associated with delayed diagnosis and repair. This report describes about a case of total collapse and consolidation of left lung with the complete transection of nearly bifurcated portion of left main bronchus , lasted for 2weeks after traffic accident. This was diagnosed by fiberbronchoscopy and 3-D chest computed tomography(CT). She underwent the sleeve resection and end to end anastomosis, and postoperative PEEP for 2 days, suctioning twice by fiberbronchoscopy, continue postural drainge and physiotherapy were applied. She had almost full expansion of the left lung at discharge.

  • Foreign body in the air passage is not uncommon in the field of otolaryngology. The majority of bronchial foreign bodies can be removed by bronchoscopy, but some cases may require surgical procedure. Recently, we experienced an impacted foreign body (ball-pointed pen cap) in the left main bronchus with pneumothorax on the right side. Authors tried bronchoscopic removal under the general anesthesia but failed. Lateral thoracotomy was performed by chest surgeon and foreign body was successfully removed.

Lipoid Pneumonia After Aspiration of Squalene$^{(R)}$ in Rabbit (토끼에서 Squalene$^{(R)}$ 흡인에 의한 지방성 폐렴)

  • Background : Recently, there are many patients with lipoid pneumonia by ingestion of shark liver oil in Korea, but only a few animal experimentations have been carried out. The purpose of this study is to evaluate sequential change of the lung after aspiration of shark liver oil and to provide the radiologic-pathologic correlation. Methods: A single intratracheal administration of shark liver oil was given to 14 white rabbits. They were then sacrificed sequentially from 1 week to 6 weeks after injection. We investigated the HRCT and pathologic findings Results: One was sudden death immediately after injection. Six of the 13 rabbits showed pneumonic infiltrations on the HRCT. There were air space consolidation with air-bronchogram on the HRCT of the first week. They were associated with the volume loss in the 4th week, and the traction bronchiectasis in the 6th week. The important pathologic findings were peribronchial alveolar inflammation with septal widening and cuboidal metaplasia of the alveolar wall. The number of macrophages in an alveoli was peaked in the second week and then gradually decreased. On the 6th week, we could find the proliferation of fibroblasts. Conclusion: We can prove the development of lipoid pneumonia after aspiration of squalene by animal experimentation, and the understanding of HRCT and pathologic findings may be helpful in proper evaluation of pneumonia due to aspiration of fish-extracted lipid.

Clinical Features of Tracheobronchial Foreign Bodies in Adults according to the Risk of Aspiration (기도 흡인 위험도에 따른 성인의 기도 내 이물의 임상적 특징)

  • Background: We wanted to examine the clinical characteristics of adult patients with tracheobronchial foreign bodies (FBs) according to the risk of aspiration and the outcomes of intervention with using a fiberoptic bronchoscope. Methods: From December 1994 through December 2004 at Kyung Hee Medical Center, we retrospectively analyzed the medical records of 29 adult patients with FBs that were identified by using a fiberoptic bronchoscope. Results: 14 patients were not at risk of aspiration, whereas 15 had cerebrovascular diseases and they were at a high risk of aspiration. No history suggestive of FB aspiration was noted in 7 (24.1%) patients. Respiratory symptom(s) were noted in 22 patients, and these symptoms were cough (62.0%), dyspnea (44.8%), fever (20.7%), wheezing (13.8%), chest pain (10.3%) and hemoptysis (0.4%). Only 60% of those patients at a high risk of aspiration had symptom(s) (92.8% of those patients without a risk of aspiration had symptoms, p=0.005). Those patients at risk for aspiration had a longer duration of symptoms (median: 4 days vs. 2 days for those patients not at risk for aspiration, p=0.007) before diagnosis. Acute respiratory symptom(s) within 3 days after aspiration were more frequent in the patients without a risk of aspiration (9 vs. 4, respectively p=0.048). Chest x-ray revealed radiological abnormalities in 23 patients, and these were opacities suspicious of FB (n=11), pneumonia (n=8), air trapping (n=5) and atelectasis (n=3). There were no differences in radiological findings according to the risk of aspiration. FB aspiration developed most commonly during medical procedures (57.1% for the patients at risk) and during eating (35.7% for the patients without risk). The most common FB materials were teeth (n = 11). Alligator jaw biopsy forceps (n = 23) was the most commonly used equipment. All of the FBs were removed without significant complications. Conclusion: This study underlines that a tracheobronchial FB in the patients who are at a high risk of aspiration are more likely to overlooked because of the more gradual onset of symptoms and the symptoms develop iatrogenically in many cases.
