Purpose : The purpose of this study was to determine the clinical value of bicarbonate gap (BG) in acute gastroenteritis by comparison of clinical manifestations and laboratory findings. Methods : We retrospectively analyzed 135 patients who had acute gastroenteritis. We classified them into two groups: $BG{\leq}-5$ (group A), BG>-5 (group B). We made a comparative study for clinical manifestations, serum electrolyte, albumin, protein, blood urea nitrogen (BUN), creatinine, anion gap (AG), delta anion gap (${\Delta}AG$) and delta bicarbonate (${\Delta}HCO_3{^-}$) between the two groups. Results : The duration of hospitalization and diarrhea was significantly longer in group A than group B. In laboratory findings, serum sodium, serum total $CO_2$, total protein, $AG_{corrected}$ and ${\Delta}AG$ were lower in group A than group B. ${\Delta}HCO_3{^-}$ and chloride were higher in group A than group B. Conclusion : BG may be a sensitive predictor enough to access the severity of acute gastroenteritis.
Typhoid fever is a systemic infectious disease which affects many organs. In children, few cases have been reported of acute nephritic syndrome in typhoid fever. We report an immunocompetent 9-year old girl with typhoid fever complicated by acute tubulointerstitial nephritis who presented with prolonged fever and acute renal failure.

Herein, two patients of acute toxic hepatitis occurred to persons had eaten Aplysia kurodai and their eggs. The authors report 2 patients of acute toxic hepatitis with reviews of the clinical features. One patient had gathered Aplysia kurodai and it's eggs from Ulleungdo sea shore on May 17, 2003. He ate with boiling the eggs. This patient developed acute toxic hepatitis and symptoms of aplysianin poisoning, such as abdominal pain, nausea, vomiting and watery diarrhea about 10 and half hours after ingestion. The other patient had gathered Aplysia kurodai and its egg from Ulleungdo sea shore on 16 May 2004, which were consumed without boiling the eggs. This patient developed acute toxic hepatitis and symptoms of aplysianin poisoning, such as abdominal pain, nausea, vomiting and watery diarrhea, about 10 and half hours after ingestion. This patient complained of jaundice and pruritus, about 10 days after ingestion. The 2 patients all improved within 15~25 days with conservative treatment. This is the first significant report of acute toxic hepatitis caused by aplysianin poisoning following the consumption of Aplysia kurodai and their eggs in Korea. The clinical features, management and preventive counterplan of acute toxic hepatitis caused by aplysianin poisoning are also presented.

Purpose : Acute gastroenteritis is a frequent childhood disease. The purpose of this study is to determine if certain pathogens are related to more severe clinical disease in children with acute gastroenteritis. Methods : Two hundred seventy one stool samples were obtained from pediatric patients with acute gastroenteritis that was associated with virus, bacteria and protozoa infection between January 2006 and December 2006. The clinical severity was assessed by using the 24 point scale Clark score. We evaluate the Clark score according to the pathogens. Results : One hundred thirty seven children were noted to have virus, bacteria or protozoal infection. Rotavirus was the most common pathogen found in the children with gastroenteritis (20.6%). Other frequent pathogens were norovirus (10.7%) and E. coli (12.9%). Vomiting (P=0.049) and diarrhea (P=0.003) symptoms were more frequent in the rotavirus group than that in the rotavirus negative group. There were no statistical differences in the Clark score between the virus positive group and the virus negative group. Conclusion : We concluded that rotavirus was the most common pathogen in children with gastroenteritis, yet the clinical severity was not dependent on the presence of viral or bacterial pathogens.

Purpose : Rotavirus is an enteric pathogen that affects millions of children globally each year. But no specific therapy is available for the management of rotavirus diarrhea. Due to the clear need to define improved modality for treatment of rotavirus diarrhea, we evaluated the efficacy of antirotavirus IgY in the treatment of infants and children with gastroenteritis. Methods : First, the amount of viral particle in the stools of thirteen patients(seven were given IgY, 6 placebo) infected by rotavirus were evaluated for 3 days with the quantitative RT-PCR method. Second, 36 children with known rotavirus infection identified by ELISA or semi-quantitative RT-PCR were evaluated. We gave 5 g anti-rotavirus egg yolk daily in two equally divided doses for 3 days to two groups(an 18 IgY group and an 18 placebo group), respectively after parenteral consent. Daily vomiting frequency, stool frequency, oral intake and urine output were monitored for 3 days, and electrolyte and blood chemistry were checked at the first and third days. Results : First, in the placebo group, the amount of virus particles increased daily, but in the IgY group it decreased daily. Second, when IgY and placebos were given to children infected with rotavirus, diarrhea on the third day decreased significantly in the IgY group, compared with the placebo group. Conclusion : Treatment with antirotavirus immunoglobulin from immunized chicken's egg resulted in a decrease in the amount of viral particles in stools and diarrhea frequency in children. These results suggest that anti-rotavirus IgY is effective in the treatment of rotavirus gastroenteritis.
Four cases of acute or acute-on-chronic kidney injury were referred for hemodialysis therapy. All cases had a history of uremic syndrome including refractory vomiting, anorexia, diarrhea and depression. They already had been treated with conventional therapies but the intractable clinical signs were remained without response of medications. Intermittent hemodialysis (IHD) sessions were executed in four cases. This study describes the clinical signs and biochemical features of four hemodialysis patients with dialysis adequacy indexed by Kt/V. This report introduced the estimation of hemodialysis adequacy method with clinical cases for establishment of standardization of hemodialysis therapy assess in dogs.
The acute toxicity of xylooligosaccharide(XO) was evaluated in SD rats. Groups of 15 male and 15 female rats were orally administered XO (0, 5000 or 10000 mg/kg). The changes of body weight and clinical signs were investigated for 14 days after treatments. No death and toxic effects were observed for 14 days. Soft stool and diarrhea appeared right after treatment for over dose and non-digestive feature of XO but these clinical signs disappeared on the next day. No significant changes in body weight and abnormal gross findings were observed in relation to XO. According to the results, XO has no special toxic effects and LD50 values of XO are above 10000 mg/kg in male and female rats.

Purpose : This study was conducted to evaluate epidemiological data of the viral pathogens obtained from stool exams and provide information on the regional prevalence of infectious diarrheal disease west in Gyeonggi Province, Korea. Methods : We enrolled a cohort of children <10 years of age admitted for treatment of acute diarrhea at Bucheon St. Mary's Hospital, College of Medicine, The Catholic University of Korea. In total, 310 fecal specimens, documented to be free of common bacterial pathogens, were collected from pediatric patients during a 12-month period from January to December 2009 and were tested for the presence of rotavirus, parechovirus, adenovirus, astrovirus, enterovirus, and norovirus using polymerase chain reaction (PCR) and reverse transcription polymerase chain reaction (RT-PCR) assay. Results : The most common virus was parechovirus (16%), followed by adenovirus (15%), astrovirus (14%), rotavirus (13%), and enterovirus (5%). Interestingly, only one of the specimens was positive for norovirus. Single infection cases were detected in 173 (55.8%) of the 310 children, whereas mixed viral infections were detected in 10 (3.2%) of the same children. Viral gastroenteritis generally showed a double peak of incidence. Parechovirus, rotavirus, and adenovirus shared a similar pattern of peak incidence with overall viruses; however, astrovirus infections occurred more frequently in the spring. Eighty-five percent of the confirmed viral gastroenteritis cases developed in under 24 months. Conclusion : The results support the importance of parechovirus, adenovirus, astrovirus, and enterovirus as causative agents of diarrhea in children, which may be underestimated by current routine diagnostic testing.
Purpose: To identify factors that influence serum zinc concentrations in children with acute gastroenteritis. Methods: Thirty-two children under 5 years of age (15 boys and 17 girls) were selected randomly among those who visited to an pediatric emergency room of Ehwa Womans University Mokdong Hospital with acute gastroenteritis from May to August 2005. This study estimated the association between serum zinc concentrations and clinical, biochemical variables in patients with acute gastroenteritis. Results: Serum zinc concentration was lower in febrile patients than afebrile patients with acute gastroenteritis ($67.5{\pm}25.3$ vs $85.5{\pm}14.2$, p<0.05). It also was lower in patients with positive C-reactive protein (CRP) than those with negative CRP ($63.9{\pm}25.4$vs $86.7{\pm}13.8$, p<0.05). Serum zinc concentration was negatively correlated (r=-0.494, p<0.05) with CRP concentration, whereas positively correlated with hematocrit (r=0.370, p<0.05), total protein (r=0.474, p<0.05), and albumin (r=0.636, p<0.05). Twelve patients (37.5%) showed very low serum zinc concentration (< $70{\mu}g/dL$) without clinical symptoms of deficiency or growth retardation. Frequency of febrile illness or positive CRP is significantly greater in group with zinc < $70{\mu}g/dL$ than the group with zinc ${\geq}70{\mu}g/dL$ (91.7% vs 55%, p<0.05; 91.7% vs 40%, p<0.05, respectively). Conclusion: In patients with acute gastroenteritis, serum zinc concentration was influenced by various factors such as fever, CRP, and biochemical factors. Bicyclomycin(BCM)은 1972년에 일본 Hokkaido, Sapporo지방의 토양균중의 하나인 Streptomyces Sapporoneosis의 배양액으로부터 최초로 분리된 항생물질로서 특히 Gram(-) 박테리아인 Escherichia coli. Klebsiella, Shigella, Salmonella, Citrobacter, Enterobacter cloacae, Neisseria gonorrhoeae등에 대한 선택적인 항균효과를 가지고 있고 Streptomycin, Kanamycin, Chloramphenicol, Tetracyclin, Aminobenzylpenicillin 및 naldixic acid 등에 대해 교차내성을 가지고 있지 않다. BCM은 구조적인 면에서 [4.2.2]bicyclic구조를 가지고있는 특이한 항생물질로서 C(1)-triol부분, diketopiperazine부분 그리고 C(5)-C(5a)exomethylene의 3 부분으로 크게 나눌수 있다(Fig. 1). BGM은 E. coli에 대한 항균력으로서 250-500 $\mu\textrm{g}$/mL의 MIC value를 가지고 있고 급성독성으로서 LD$_{50}$가 4g/kg 이상으로 거의 독성이 없는 것이 특징이다. 현재 Fujisawa pharmaceutical Ltd.로부터 Bicozamycin이라는 상품명으로 설사치료제 용도로 시판되고 있다.
