Evaluation of the Usefulness of Ancillary Devices for Patients with Acute Lumbar Pain During Magnetic Resonance Imaging (자기공명영상검사 시 급성 요추 통증 환자를 위한 보조기구의 유용성 평가)

  • The purpose of this study was to evaluate the convenience and image quality of patients with acute lumbar pain patients at a general hospital in Daejeon using ancillary devices for postural changes and correction. The results of the study are summarized as follows. First, the Turbo S pin Echo technique(TS E) using ancillary equipment has the highest image evaluation rating with an average score of 4.440, which is highly valuable on a diagnosis. Second, the average score for patient the questionnaire 'When using ancillary equipment, I feel that my body is calibrated to side without bias.' was shown as 4.440, which is very useful for the correction of the patient's body when using ancillary equipment. Finally, Breath Hold technique(BH) is very effective in shortening test time of acute lumbar pain patients, because it can reduce test time 86.4% faster than Turbo Spin Echo technique(TSE). The results of the study showed that the use of ancillary equipment to perform the test through the side lying postures helped to reduce the pain and control the patient's breathing, and the diagnostic value of the image was high.

Clinical Study on 16 Conservatively Treated Acute Compression Fracture Patients with Focus on Compression Ratio (급성 흉요추부 압박골절 환자 16례의 보존 치료에 따른 Compression Ratio에 중점을 둔 임상 고찰)

  • 목적 : 한양방 협진을 통한 보존적 입원 치료를 받은 급성 흉요추 압박골절 환자군에서의 압박골절 compression ratio의 급성기 내의 변화를 기록 분석하여 차후 환자군의 예후를 예측하고자 하였다. 방법 : 한양방 협진병원 척추센터에 입원하여 보존치료를 시행한 발병 2주 내의 압박골절 환자 중 2회 이상의 시점에서 압박골절 부위의 측면 X-ray 영상이 있는 환자 16명의 기록을 수집하였다. X-ray상 급성 압박골절이 발생한 추체(n=19)의 anterior wall과 posterior wall의 높이를 입원 1주차, 입원 2주차, 입원 3주차와 최종 영상 확보 시점의 X-ray에서 측정해 wedge fracture에 해당하는 anterior column fracture가 있는 추체(n=14)에 한해 anterior height/posterior height의 공식을 통해 compression ratio를 산출하였다. 2차적 지표로 입원 1주차에서 입원 3주차까지 환자의 VAS(visual analog scale) 통증 척도 점수를 분석하였다. 그 외 연령, 성별, 입원 기간, 총 치료기간, 당뇨, 골다공증, 골밀도, 압박골절 기왕력, 압박골절의 형태의 빈도와 비율을 분석하였다. 결과 : 1. 전방 골주 단독 골절에서의 compression ratio는 입원 1주차에서 3주차, 그리고 최종 영상 확보시점까지 각각 20.92${\pm}$10.42, 25.22${\pm}$10.90, 25.57${\pm}$11.04, 25.07${\pm}$11.94로 기록되었다. 시점간에는 통계적으로 유의한 compression ratio의 차이가 나타나지 않았다. 2. 통증 VAS 점수 평균은 입원 1주차부터 3주차까지 각각 7.44${\pm}$2.07, 4.67${\pm}$1.63, 3.00${\pm}$1.80으로 기록되었다. 시점간 모두 통계적으로 유의한 감소가 있었다(1주차-2주차 p=.003, 1주차-3주차 p<.000 2주차-3주차 p=0.021). 결론 : 본 연구의 협진 입원치료 압박골절의 compression ratio는 입원 1주차 시점에서 최종 follow-up 시점까지 유의한 차이를 보일 만큼의 추가적인 추체 높이 감소를 보이지 않아 향후 좋은 예후를 예상할 수 있다. 통증 VAS 점수는 모든 주별 시점간에서 유의하게 감소되었다.

Clinical Study of the Treatment of Spondylolisthesis (척추전방전위증의 한방 치료에 관한 임상적 고찰)

  • Objectives : This study was performed to evaluate the clinical results of the spodylolisthesis patients treated with oriental medical methods. Methods : We diagnosed spondylolisthesis by L-spine simple x-ray. 28 patients with spondylolisthesis were hospitalized at WonKwang University KwangJu Oriental Medical Hospital from April 1997 to January 2000 and treated by acupuncture, moxibustion, bed rest, herb-medicine and physical therapy. Results : 1. Distribution showed female predominence(82.1%) in general. 2. In the duration of symptoms, the largest group was the group of "acutest" (32.1%). 3. In the distribution of causes, the largest group was "overwork" (35.7%). 4. Isthmic type is the most common(75%) in etiology. 5. Isthmic type involved the 5th lumbar vertebra in 57.1% and degenerative type involved the 4th lumbar vertebra in 57.1%. 6. Most of cases were grade I(96.4%) in degree of slipping. 7. The common symptoms were radiating pain(92.9%) and low back pain(78.6%). 8. Therapeutic efforts above "good" by Kim's criteria was 20 cases(71.4%) Conclusions : The result of treatment by oriental medical methods is satisfactory for the treatment of spondylolisthsis.

  • Objectives Clinical pathway (CP), is management plans that display goals for patients and have led to improve outcomes for many diseases. In Korea, Interest in Korean medicine's stabilization (Clinical pathway, Clinical Practice Guideline) is increasing, But the number of studies is scare. Method and Analysis This trials composes nonequivalent control group pretest-posttest design to conduct clincal pathway trial for the acute pain after back surgery. The subjects were 10 control patients with back surgery, and 10 experimental patients with application of integrative CP. Each group patient will observed 6 weeks. We check validation of CP. Also we compared the patient's status using the pain, function, Quality of life index between the two groups. Discussion This trial is the first CP for the acute pain after back surgery using integrative medicine concepts. Aim of this trial is to find the effectiveness and validity of clincal pathway for acute pain after back surgery.

Effects of Acupotomy on Pain and Functional Improvement in Acute Low Back Pain Patients: A Retrospective Study (도침치료가 급성 요추 염좌 환자의 통증 및 기능개선에 미치는 영향: 후향적 연구)

  • Objectives The aim of this study is to investigate the effect of Acupotomy on the pain reduction and functional improvement of patients with Acute low back pain. Methods We studied 60 patients with Acute low back pain with Numeric Rating Scale (NRS) 5 or higher in admission and had admitted to Department of Korean Medicine Rehabilitation of Daejeon University from March 1, 2017 to October 31, 2017. 30 patients had received Acupotomy combined Korean Medicine treatment(such as acupuncture, moxabustion, herbal medicine, etc) and the other 30 had received Korean Medicine treatment only. The analysis was conducted as a retrospective study which analyzes the patient's medical records. Statistical analysis was performed using the IBM SPSS Statistics 24 program. We used NRS to evaluate pain reduction, used Range of Motion (ROM) and Roland Morris Disability Questionnaire (RMDQ) to evaluate function improvement, and used EuroQol-5 Dimension (EQ-5D) and EuroQoL-Visual Analogue Scale (EQ-VAS) to assess quality of life. Finally, a five-point Likert scale was used to assess treatment satisfaction. Results The analysis revealed that patients who were treated with Acupotomy showed statistically significant NRS reduction, improvement of ROM and RMDQ, improvement of EQ-5D and EQ-VAS and satisfaction compared to those who just recevied Korean Medicine treatment. Conclusions In conclusion, we found that the Acupotomy showed a positive effect on pain resolving, functional rehabilitation and quality of life in patients with Acute low back pain.

The Survey on Contents Validity of 'Preliminary Critical Pathway for Acute Postoperative Pain after Back Surgery' ('요추 수술 후 급성기 통증 환자의 예비 표준임상경로지 내용타당도 설문' 조사 보고)

  • Objectives The aim of this study is to investigate the possibility of making Korean Medicine-Western Medicine collaboration Critical Pathway for Acute Postoperative Pain after Back Surgery. Methods This preliminary Critical Pathway (CP) was composed of 9 Index of vertical axis (assesment, treatment, patient movements, consultation, diet, other, administration, examination, education) and 5 hospitalization periods of horizontal axis through a review of the literature and reference data of medical records. This preliminary CP's Content Validity Index (CVI) survey was carried out by 20 personnel of two different medical institutions from 06, June, 2016 to 15, June, 2016. All data were double-cross checked and analyzed. Results Of those questioned, there were 13 males and 7 females in age, 6 professors, 7 medical residents, 7 nurses in position of 20 personnel. According to the survey, among the 62 contents, 32 contents are above 80 percent agreements, 12 contents were between 70 to 80 percent and 18 contents below 70 percent agreements. Especially, Most contents in treatment index were below fixed validity. Overall Contents Validity in Index, Treatments (72.7%), Diet (100.0%), Other (100.0%), Administration (75.0%) and Examination (100.0%) were in high validity. On the other hands, Assessments (29.4%), Patient movements (0.0%), Consultation (30.0%) and Education (33.3%) were in low position. Conclusions This survey results can be evidence of possibility to develop New Korean Medicine-Western Medicine Collaboration Critical Pathway for Acute Postoperative Pain after Back Surgery.
