The Effect of Juglandis Semen Aquacupuncture on Diuresis of Rat in Glycerol-Induced Acute Renal Failure (호도약침(胡桃藥鍼)이 Glycerol로 유발(誘發)된 급성신부전(急性腎不全) 백서(白鼠)의 이뇨(利尿)에 미치는 영향(影響))

  • This study was undertaken to determine if Juglandis Semen Aquacupuncture(Ja) has a protective effect against glycerol-induced acute renal failure in rats. Rats were dehydrated for 24hr and then injected with $5m{\ell}/kg$ of 50% glycerol, one-half of dose in each hindlimb muscle. In experiments for Ja effect, rats received $0.1m{\ell}$ of Ja extraction in both sides of corresponding $Sh{\grave{e}}ns{\bar{u}}$($BL_{23}$) of human body and non-acupuncture points(the root of tail) for 3 days after injection of glycerol. The experimental group were divided into the Normal group, the Control group, the Ja to $Sh{\grave{e}}ns{\bar{u}}$($BL_{23}$) group(Ja-AS), the Ja to non-acupuncture points group(Ja-AN). There were significant decrease of Urine vollume, total protein and phosphate level in Ja-AS as compared with the control group. There were not any significant change of Urine creatinine in Ja-AS as compared with the control group. There were significant decrease of Unine glucose in Ja-AS, Ja-AN as compared with the control group. This suggests that Ja-AS could be used in prevention and treatment of acute renal failure. However, the precise mechanisms of Ja protection remain to be determined.

The Effects of Hyeongbangjihwang-tang and Sibimikwanjung-tang in Glycerol-Induced Renal Failure in Rats (형방지황탕(荊防地黃湯)과 십이미관중탕(十二味寬中湯)이 Glycerol로 유발된 흰쥐의 급성신부전에 미치는 영향)

  • 1. Objective : The present author have examined the comparative effects of Hyeongbangjihwang-tang and Sibimikwanjung-tang in glycerol-induced acute renal failure, a model for myoglobinuric acute renal failure. 2. Methods : Above two prescriptions were applied to rat ad libitum and serum level of creatinine, glucose and blood urea nitrogen(BUN) were measured. And serum level of $Na^+$, $K^+$ and $Cl^-$, too. 50%(vol/vol) glycerol at a dose of 10 ml/kg of rat body weight intramuscularly and 1% concentration of prescriptions were applied to rats for 7 days ad libitum. 3. Results : Serum level of creatinine and $K^+$ significantly increased by glycerol injection, and these showed pre-renal and renal cause of acute renal failure. Hyeongbangjihwang-tang administered group showed decreasing tendency on serum level of creatinine when compared with control groups, but not significant. Sibimikwanjung-tang administered group showed significant increase of serum level of BUN when compared with control groups. 4. Conclusions : Above two prescriptions showed no significant changes on glycerol induced acute renal failure, but Hyeongbangjihwang-tang and Sibimikwanjung-tang showed a little different manner of response.

  • Acute hepatitis A is a generally self-limiting disease of the liver. Acute renal failure is rare in patients with acute non-fulminant hepatitis A Acute tubular necrosis is the most common form of renal injury found in such patients. The 215 years old male patient visited our hospital with complaint of general weakness, fatigue, nausea, vomiting and myalgia. He was diagnosed with acute renal failure associated with acute non-fulminant hepatitis A We report here on a case of acute renal failure associated with non-fulminant hepatitis A, and we include a review of the literature.

Effects of Sojongchobisunki-tang on Cisplatin-induced Rat Acute Renal Failure (소종조비순기탕(消腫調脾順氣湯)이 Cisplatin으로 유발(誘發)된 흰쥐의 급성신부전(急性腎不全)에 미치는 영향)

  • Objectives : The object of this study was to observe the nephroprotective effects of Sojongchobisunki-tang (SCST), which has traditionally been used as Korean medicine for treating various renal diseases, on cisplatin-induced rat acute renal failure. Methods : Three different dosages of SCST were orally administered once a day for 23 days before cisplatin treatment (5 mg/kg, single intraperitoneally administered) and 5 days after cisplatin treatment (once a day for 28 days). 6 groups, of 8 rats per group were used in the present study after 7 days of acclimatization. Changes of the body weight, kidney weight, serum BUN and creatinine levels were observed, as well as changes of the kidney MDA and GSH contents. The results were compared with captopril 100 mg/kg of which the effects on cisplatin-induced acute renal failures are already confirmed. Results : Acute renal failure induced by cisplatin were induced by oxidative stress and related lipid peroxidation in the present study. However, these acute renal failures and inhibition of antioxidant effects induced by cisplatin were dose-dependently reduced by treatment at all three different dosages of SCST extracts. Conclusions : This study suggests that SCST extracts showed favorable effects on the cisplatin-induced rat ARF.

Nephroprotective Effects of Daebuncheong-eum Aqueous Extracts on Cisplatin-induced Acute Renal Failure in Rats (대분청음(大分淸飮)이 Cisplatin으로 유발(誘發) 된 흰쥐의 급성신부전(急性腎不全)에 미치는 영향(影響))

  • Purpose : This study was to observe the nephroprotective effects of the traditional prescription, Daebuncheong-eum (DBCE). DBCE has generally been used for treating various renal diseases, including renal failure. Methods : Three different dosages of DBCE extract were orally administered once a day for 28 days. At the 23rd day after DBCE extract treatment, cisplatin was also treated. Then, 5 days after cisplatin treatment, all rats (6 groups of 8 rats each) were sacrificed. Changes on the body weight, kidney weight, serum BUN and creatinine levels were observed, along with changes to the kidney MDA and GSH contents. The results were compared with captopril 100mg/kg, from which the effects on cisplatin-induced acute renal failures have already been confirmed. Results : Cisplatin induced ARF are induced by oxidative stress and related lipid peroxidation in the present study. However, these ARFs and inhibition of antioxidant effects induced by cisplatin were dose-dependently reduced by treatment of all three different dosages of DBCE extracts. Effect of Acupuncture and Moxibustion at SP6(Sanyinjiao) and BL28(Pangguangshu) on Experimental Acute renal failure in rats (삼음교(三陰交)(SP6), 방광유(膀胱兪)(BL28)의 침자 및 애구자극이 흰쥐의 실험적 급성신부전에 미치는 영향)

  • Objective : The aim of this experiment is to investigate the effect of acupuncture and moxibustion at SP6(Sanyinjiao) and BL28(Pangguanshu) on uranyl acetate-induced acute renal failure in rats. Methods : To induce experimental acute renal failure, uranyl acetate was intraperitoneally injected to rats by 5mg/kg. 3 days later acupuncture and moxibustion treatment was done at SP6(Sanyinjiao) and BL28(Pangguanshu) bilaterally of the rats. And then 4 days later, we measured rats' body weight, kidney weight, serum BUN, creatinine and serum eletrolyte levels(Na, K and Cl). Results : In the moxibustion group, hypertrophy index of kidney and serum BUN level were decreased marginally significantly, and serum creatinine, K levels were decreased significantly. But in the acupuncture group, there was no significance at this experiment. Conclusions : SP6(Sanyinjiao) and BL28(Pangguanshu) moxibustion can decrease serum BUN, creatinine, and serum eletrolyte levels on uranyl acetate-induced acute renal failure in rats. And the study will be done continuously about non-effect in the acupuncture group at this experiment.

Treatment for acute renal failure occurred by ingestion of grape skins in a dog (개에서 포도껍질 섭취 후 발생한 급성신부전 치료 증례)

  • A 2-year-old, female, Maltese dog (3.2 kg of body weight) was referred to the Veterinary Medical Teaching Hospital, Chungnam National University with vomiting, anorexia and depression. Twelve hours prior to the onset of clinical signs, the dog ingested some grape skins (about 60 g). Physical examination at the time of presentation showed peripheral edema and mucous pallor. Blood and blood chemical analysis revealed anemia and moderate azotemia with elevated blood urea nitrogen (107.2 mg/dl), creatinine (6.3 mg/dl) and hyperphosphatemia (11.3 mg/dl). Echogenicity of renal cortex were observred by ultrasonography. The dog was diagnosed as acute renal failure occurred by grape skin toxicosis. The dog was treated with supportive care such as fluid therapy, diuretics (furosemide) and phosphorus binder (almagate). A normal condition of blood and blood chemical findings and clinical signs was observed at five days after treatment, and prognosis is good to date.

A case of Diagnosing Paraquat Intoxication on Transferred Patient with Acute Renal Failure (원인 불명의 급성 신부전으로 발현된 급성 파라콰트중독 1례)

  • Paraquat, a widely used herbicide, is extremely toxic, causing multiple organ failure in human. Many treatment modality has been used, but now paraquat is very fatal drug. Elimination rate of plasma paraquat seems to be a factor for the survival rate. So early diagnosis and early treatment are very important. Plasma paraquat concentration could be measured by radioimmunoassay. But it is impossible that the test was done at Emergency room and the result was checked immediately. There was relation between plasma paraquat concentrations and urine paraquat concentration. Because of its simplicity and low cost, urine paraquat concentration test is complementary to the plasma concentration measurement. If the patient has psychotic problem or unconscious mental state, and is observed unexplained dyspnea and oral ulcer, urine paraquat test is very important to rule out acute paraquat intoxication. We experienced a patient who was presented as unexplained acute renal failure initially and was diagnosed as paraquat intoxication later.

Clinical Study on Acute Renal Failure after Valve Replacement Surgery (인공판막치환술후 발생한 급성신부전에 대한 임상적 고찰)

  • A retrospective study of 737 consecutive patients surviving the first 24 hours who underwent valve replacement surgery from July 1980 to June 1993 was undertaken to determine the prevalence, variables that could be used to predict outcome and results of therapy for postoperative acute renal failure[ARF]. Twenty-one patients[2.8 %] developed acute renal failure. Positive risk factors noted in the development of postoperative renal failure included age, New York Heart Association class III & IV, endocarditis and elevated preoperative concentration of serum creatinine. The duration of cardiopulmonary bypass, aortic cross-clamping and the total duration of the operation also closely correlated with the incidence of ARF. The mortality rate for established ARF was 38.1% and ARF was associated with a significant increase in the length of hospitalization, ventilator support and intensive care unit stay. The incidence and mortality rate of oliguric renal failure was 38.1% and 85.7%. The highest mortality rate was associated with two or more postoperative complications and serum creatinine value exceeded 5 mg/dl. We concluded that therapy should be aimed at prevention of oliguric renal failure, or at least its conversion to nonoliguric renal failure, and early institution of renal replacement therapy with intensive support probably gives the best chance for survival.

Outcome of Continuous Renal Replacement Therapy in Children (소아에서 지속적 신대체요법의 치료 결과)

  • Purpose : There is growing use of continuous renal replacement therapy(CRRT) for pediatric patients, but reports about the use and outcome of CRRT in children is rare in Korea. We report our experiences of CRRT in critically ill pediatric patients. Methods : We reviewed the medical records of 23 pediatric patients who underwent CRRT at Asan Medical Center between May 2001 and May 2004. We evaluated underlying diseases, clinical features, treatment courses, CRRT modalities and outcomes. Results : Ages ranged from three days to 16 years with a median of five years. Patients weighed 2.4 to 63.9 kg(median 23.0 kg; 10 patients ${\leq}20kg$). The underlying diseases were malignancy(nine cases), multiple organ dysfunction syndrome(five cases), hyperammonemia(four cases), acute renal failure associated with liver failure(three cases), dilated cardiomyopathy(one case) and congenital nephrotic syndrome(one case). Pediatric Risk of Mortality(PRISM) III score was $17.6{\pm}7.6$ and the mean number of failing organs was $3.0{\pm}1.7$. Duration of CRRT was one to 27 days(median : nine days). Eleven patients(47.8%) survived. Chronic renal failure developed in two cases, intracranial hemorrhage in one case, and chylothorax in one case among the survivors. PRISM III score and the number of vasopressor before the start of CRRT was significantly lower in the survivors($12.7{\pm}4.2$ and $0.9{\pm}1.1$) compared with nonsurvivors($22.1{\pm}7.8$ and $2.4{\pm}1.4$)(P<0.05). Conclusion : CRRT driven in venovenous mode is an effective and safe method of renal support for critically-ill infants and children to control fluid balance and metabolic derangement. Survival is affected by PRISM III score and the number of vasopressors at the initiation of CRRT.
