Effects of Methylprednisolone on TGF-${\beta}1$, Apoptosis and Renal Scarring in Experimental Acute Pyelonephritic Weaning Rats (실험적 급성 신우신염이 유발된 이유기 백서에서 methylprednisolone이 TGF-${\beta}1$, 세포고사 및 신반흔에 미치는 영향)

  • Purpose Acute pyelonephritis of growing kidneys may result in renal scarring. TGF-${\beta}1$, inflammatory cytokine, has been suggested to play an important role in promoting renal scarring through apoptosis, suppression of cellular proliferation and fibrosis. We observed the effects of a potent anti-inflammatory agent, methylprednisolone on apoptosis and renal scarring in experimentally induced acute pyelonephritic weaning rats. Materials and Methods: To induce ascending pyelonephritis a saline solution containing Escherichia coli type ATCC No. 25922, pili- form (107 bacteria/mL) was infused into the bladder through the 16-guage silicone cannula for 48 hours to 102 three-week-old Sprague-Dawley rats (50-60g). Experimental groups were divided into three groups according to the treatment protocols, group I (ceftriaxone only, n=3l), group II (methylprednisolone+ceftriaxone n=28), control group (n=43) was not treated. Histopathologic scores of inflammatory changes, fibrosis and tubular atrophy, the apoptosis index and TGF-${\beta}1$ expression score were observed at post-infection 1 and 3 week. Datas were analysed using ANOVA test and P value below 0.05 was interpreted as significant. Results: The mortality rate ($21.4\%$) of group II was not different to the control group ($41.9\%$) and group I ($32.3\%$). The inflammatory score of group II ($0.8{\pm}0.87$) at week 1 was significantly lower than those of the control group ($2.3{\pm}0.87$) and Group I ($1.7{\pm}0.79$) (P<0.05). Apoptosis index of group II ($2.9{\pm}2.15$) at week 1 was significantly lower than those of the control group ($10.0{\pm}1.95$) and group 1 ($8.3{\pm}2.53$) (P<0.05). TCF-${\beta}1$ expression score of group II ($0.8{\pm}0.72$) at week 1 was significantly lower than those of the control group ($1.9{\pm}0.68$) and group I ($1.8{\pm}0.60$) (P<0.05). The fibrosis score of group II ($1.1{\pm}1.10$) at week 3 was significantly lower than that of the group I ($1.8{\pm}0.83$) (P<0.05) Conclusion: Conclusion Combined treatment with methylprednisolone and ceftriaxone reduced inflammation, fibrosis, apoptosis and TGF-${\beta}1$ expression in acute pyelonephritic weaning rats, compared to ceftriaxone alone. Anti-inflammatory agent supplemented to antibiotics could prevent renal scarring more effectively. (J Korean Soc Pediatr Nephrol 2002 ; 6 : 75-84)

  • Objectives : The Purpose of this report is to examine the effect of Remote Acupuncture Point Needling in the Low Back Pain patient of Acute Stage. Methods : From October 1st 2005 to May 31th 2006, 30 cases of low back pain patients at acute stage were divided into 2 groups; one group(control group) took near acupuncture point needling, and the other group(test group) took near acupuncture point needling with remote acupuncture point needling. For evaluating change of pain, Visual Analog Scale(VAS) was checked before and after Treatment. Results : Both acupuncture therapy showed good effect on low back pain of acute stage. And test group showed better effect on decreasing pain than control group. It was proved by the difference between VAS checked before treatment and what checked after treatment. But it was not significant statistically. Conclusion : Near acupuncture point needling and remote acupuncture point needling can be recommended as a useful therapy to treat low back pain of acute stage.

Validity and Reliability of Korean Version of Older Patients in Acute Care Survey (한국어판 급성기 노인 환자 돌봄 측정도구의 타당도 및 신뢰도)

  • Objectives : This study was a methodological study to verify the validity and reliability of the translated and modified Older Patients in Acute Care Survey-US (OPACS) tool, which was originally developed by Dikken and his colleagues. Methods : The OPACS was translated into Korean, and the content validity, composition validity, and reliability were verified using 348 nurses working for hospitals with over 200 beds and located in C city, B city, and K province. Results : The analysis showed that the Korean version of the OPACS had of eleven questions in the practice experience section and twelve in the general opinion section for older patients in the acute phase. Cronbach's ${\alpha}s$ were 0.84 for practice experience and 0.81 for general opinion. Conclusions : The study showed that the OPACS could measure the practice and opinion of Korean nurses who cared for elderly patients in the acute phase. The significance of the study was that it provided baseline data information regarding the performance and opinion of these nurses for nursing managers to improve the nursing environment.

ICF Core Sets for Measuring Functional Status of Acute Stroke Patients (급성기 뇌졸중 환자의 기능상태 측정을 위한 ICF 핵심지표)

  • Purpose: This study was conducted to extract international classification of functioning, disability, and health (ICF) core sets for measuring functional status in acute stroke patients, and to evaluate clinical applicability of the core sets. Methods: A set of 22 ICF items on functional status in acute stroke patients were extracted from the Korean general ICF core sets and ICF core sets for stroke patients. The extracted ICF functional items were assessed at the time of admission and discharge among 100 stroke patients who were admitted in a university hospital. Results: Comparing to functional status at admission of acute stroke patients, the overall functional status at discharge was improved. However, functions on defecation, skin protection, and relationship with immediate family at the time of discharge were not significantly changed. Conclusion: The set of ICF functional items identified in this study may be reliable and valid to assess acute stoke patients' body functions, activities and participation and environmental factors in the holistic and comprehensive nursing context. Nursing interventions on bowel elimination and skin protection for acute stroke patients need to be developed.

  • Objectives : The purpose of this study was to evaluate the effectiveness of Calculus Bovis Fel Ursi Moschus(BUM) pharmacopuncture on the treatment of acute low back pain patients. Methods : The subjects in this study were 30 patients with acute low back pain who were treated in Dept. of Acupuncture and Moxibustion, Jein Oriental Medicine Hospital from November 1, 2010 to June 30, 2011. These patients were randomly divided into two groups. In one group(control group), patients were treated with acupuncture. And in the other group(experimental group), patients were treated with both acupuncture and BUM pharmacopuncture. For evaluating change of pain, visual analog scale(VAS) and Oswestry disability index(ODI) were checked before and after. Results : Each group had significant decrease in VAS and ODI after each treatment. Experimental group had more significant decrease in VAS after each treatment than control group. Conclusions : We suggest that cotreatment of acupuncture and BUM pharmacopuncture on acute low back pain patients can be recommended as a useful therapy.

The Efficacy of Epidural Blockade on Acute Herpes Zoster (대상포진 급성기에 시행한 경막외 차단술의 효과)

  • Background: The efficacy of nerve block on the progression of herpes zoster still remains controversy. The purpose of this study was to evaluate the efficacy of epidural blockade on the pain course of acute herpes zoster. Methods: Among 75 patients admitted with acute herpes zoster, 40 patients were treated with acyclovir and epidural block (epidural group), 35 patients were treated with acyclovir only (no block group). Follow up with patients was done for up to 3 months, either at the outpatient department or by telephone. The days required for pain relief were compared. We also investigated the factors associated with outcome. Results: Though statistically insignificant, patients in the epidural group tended to have faster pain relief. Patients with shorter symptom duration experienced significantly faster pain relief. But other factors such as sex, age, involved dermatome and other coexisting diseases (cancer, diabetes mellitus) were not associated with the outcome in this study. Conclusions: Though patients with epidural block leaded to have taster pair relief, more studies with larger patient population may be needed to confirm the efficacy of epidural block on herpes zoster.

  • Background : Recent studies have demonstrated relations between inflammation and stroke. The aim of this study was to investigate CRP level in independent risk factors of stroke patients. Method : Thirty-five ischemic stroke patients were included in this study from Febrary to September 2003. Plasma concentration of CRP was measured over 72 hours after stroke. We examined a average CRP level and associations between CRP and other variables. Result : This study didn't show high CRP level in stroke patients comparing with recent reported studies. Associations between CRP level and other variables didn't show any significant change. Conclusion: In this study, CRP level was not associated with acute stroke significantly.

  • Many people in these days participate in running as leisure due to urbanization and socio-economic development. Running is a simple exercise but it can induce its own specific injury pattern because of its repetitive motion. Most runners' injury is caused by chronic overuse syndrome rather than acute trauma. And common accompanying injury in running are anterior knee pain syndrome, Iliotibial band syndrome, stress fracture, plantar fasciitis, Achilles tendinitis, posterior tibial tendon syndrome. Most common area of runners' injury is knee joint. Therefore the authors reviewed the recent literatures and described the classification, etiology, prevention, rehabilitation in this article.

May Low Level Laser Therapy be the Candidate of First Choice for the Acute Stroke? (중풍 급성기에 있어서 레이저치료에 대한 최신지견 고찰 : 임상 논문을 중심으로)

  • Background : Low level laser therapy may be an effective method to protect tissue damage in acute stroke. Recently, series of clinical studies on the basis of animal experiments report efficacy and safety of laser therapy at early stages of acute stroke. Laser promotes mitochondrial ATP synthesis to reduce cell death by ischemic infarction. Objectives : To report possibility of non-invasive laser therapy for acute stroke by reviewing literature about its effectiveness, safety and mechanism. Methods : We searched papers using PubMed and 'Web of Knowledge' of Thomson ISI, using the keywords "Laser Therapy, Low-Level" and "Stroke". Limitations were last 10 years of publications and only in English. Search range includes RCTs, clinical reports, reviews and animal experiments. Papers not matched with inclusion criteria were excluded. Results : A total 223 studies were found, 203 excluded during title and extract screening. After scanning 20 papers the final 2 serial RCTs were selected and analyzed. They reported that transcranial laser therapy led in neuroprotective effect for acute stroke patents. Clinical evaluation factors showed favorable trend and initial safety. Conclusions : Non-invasive laser secured safety of clinical application. It may be a favorable choice for the acute stage of stroke.

A Comparison of Interventions Recorded in Nursing Notes between Actue and Subacute Stage after a Cerebrovascular Accident (신경과 병동에 입원한 노졸중환자의 간호일지에 나타난 급성기와 아급성기의 간호중재 비교)

  • Purpose: This study aimed to: 1) determine the core nursing interventions, and 2) compare acute interventions with subacute interventions recorded in the nursing notes of patients with cerebrovascular accidents (CVA). Methods: The nursing records covering the first 10 days of 30 patients with a CVA who were admitted from January to December 2004 at C University Hospital in Korea were examined. Data was collected using the nursing interventions classification (NIC) from January to April 2005. Finally, data analysis was carried out using mean, SD, and paired t-test according to domains, classes, and interventions. Results: The most frequent nursing intervention at both stage was 'Neurologic monitoring'. There were differences in interventions belonging to the 'Physiological: complex,' 'Behavioral,' 'Safety,' and 'Health system' domains between the acute and subacute stages. The frequency of interventions belonging to the 'Immobility management,' 'Neurological management,' 'Tissue perfusion management,' 'Patient education,' 'Risk management,' 'Health system mediation,' and 'Information management' classes at the acute stage was higher compared to the subacute stage. Conclusions: This study found out that nurses relatively recorded more nursing interventions during the acute stage hence the unsuccessful documentation of the subacute stage particularly in describing the specific nursing interventions at this stage.
