  • It has been reported that there are many cases of chronic shoulder injuries of golf players, however acute injuries at dominant tide are relatively rare. In this paper, we show a case of acute rotator cuff tears at dominant side due to bad swing in middle-aged woman. We suggest that golfers should be cautious with playing at winter to prevent acute rotator cuff tears by starting with a warm-up and correcting faulty swings.

Optical Microscopic Image Analysis for Damaged GFRP Rebar by Alkali and High Temperature Exposures (알칼리와 고온노출에 의한 GFRP 보강근 손상에 대한 현미경분석 연구)

  • In this experimental study, the characteristic of damages on GFRP rebar exposed to high temperature only and immerged in alkaline solution after the exposure to high temperature was analyzed through microscopic image analysis. The found microcrack and pores in resin matrix were quantitatively compared if there was effect of pre-exposure to high temperature. The damages, such as microcrack and pores in resin matrix, by alkali exposure were mainly found in rebar surface. On the other hand, the pores caused by high temperatures were extensively found in a section and had greater width than those caused by the alkali exposure. In results of the quantitative comparison, the accumulated length and widths of microcrack and pores in resin matrix in pre-exposed GFRP rebar to high temperature were respectively 1.5 and 1.4 times of those in the GFRP rebar only immerged in alkali solution. Therefore, the deterioration of resin matrix by the alkali exposure could be accelerated due to the pre-exposure to high temperature.

Evaluating Quadriceps Muscle Damage after Downhill Running of Different Intensities using Ultrasonography (내리막 달리기 후 국소 근손상의 영상학적 비교분석 : 운동 강도의 영향)

  • The current study was performed to investigate the magnitude of exercise-induced muscle damage (EIMD) after downhill running (DR) of different intensities and to examine the availability of muscle echo intensity as biomarkers to detect regional damage within quadriceps muscle group (QG) following DR. Healthy college-age men (n=11) were experienced twice DR sessions [$50%HR_{max}$ DR, LDR; $70%HR_{max}$ DR, HDR] separated by a 2-week wash-out period with the random order. After DR, severity of EIMD according to exercise intensity were determined by serum creatine kinase (CK) activity, muscle tenderness, and neuromuscular function indicators such as a maximal voluntary isometric contraction (MVIC) and range of motion (ROM). Transvaginal B-mode imaging had been employed to evaluate regional muscle echo intensity within QG [rectus femoris, RF; vastus lateralis, VL; vastus medialis, VM; vastus intermedius, VI]. After both DR sessions, changes in serum CK activity and muscle tenderness have tended to more increase in HDR compared to those of LDR. There was a significant interaction effect between exercise intensity during DR and the time course of serum CK activity(p<.05). However, there were no statistical differences between sessions in muscle tenderness. The time course of changes in the neuromuscular functions after DR were similar to those of regional muscle echo intensity regardless exercise intensity. Although neuromuscular function showed to decline in HDR more than those of LDR after DR, no statistical differences between sessions. In contrast, there were significant interaction effects between sessions and time course of changes in RF and VL muscle echo intensity(p<.01), but not shown in those of VI and VM. These results indicated that each muscles within the QG show different response profiles for EIMD during DR, exercise intensity influences on these responses as well. In particular, current findings suggested that muscle echo intensity derived from ultrasound imaging is capable of detecting regional muscle damage in QG following DR.

The Effect of Hand Grip Force on the Activity of Shoulder Muscles in the Patterns of Arm Position (팔자세의 유형에서 손의 쥐기 강도가 어깨근육의 활성도에 미치는 영향)

  • This study is intended to provide a basic clinical data useful for preventing shoulder injuries related to occupation and curing them, by measuring the shoulder muscle activity of normal adults in an arm posture type of a wide shoulder joint angle, according to several differences of hand grip force. In order to examine the shoulder muscle activity during hand grips at a variety of intensity, according to the arm posture type, MVCs of all subjects were measured, and %MVCs of anterior deltoid, supraspinatus, the upper trapezius and infraspinatus at 0 degree, 90 degrees and 160 degrees of shoulder angle were yielded according to the grip force in an arm posture type, changing the maximum grip force into 30%, 50% and 90%, randomly. When measuring the hand grip at 30%, 50% and 70% of the maximum grip force to compare %MVC of each group depending on the arm posture type, there were no significant differences of muscle activity at 0 degree of shoulder angle among anterior deltoid, supraspinatus, the upper trapezius and infraspinatus, but there were significant differences of it among them at 90 degrees and 160 degrees of should angle(p<.001). According to the results of post-hoc test, also, anterior deltoid had the highest muscle activity, whereas the upper trapezius had the lowest muscle activity.

  • In this study we investigated the effects of physical therapeutic intervention through electromyography, ultrasonographic imaging and changes of the IL-$1{\beta}$, IGF-1 and IGF-2 in skeletal muscle of rats injured experimentally. The twenty Sprague-Dawley male rats were randomly divided into the 4 groups: a normal, a control, a low power laser and a neuromuscular electrical stimulation group. Abnormal spontaneous activities had not been shown, both in normal and skeletal muscle injured rats. The maximum diameter of the calf muscle was significantly increased in the low power laser and neuromuscular electrical stimulation groups compared with control group. The level of the serum IL-$1{\beta}$ was more decreased in the low power laser and neuromuscular electrical stimulation groups than that of control group. The activation level of the IGF-1 and the IGF-2 were significantly higher in the control, low power laser and neuromuscular electrical stimulation groups than that of normal group. However, there was no statistically significant difference among the control, low power laser and neuromuscular electrical stimulation groups.

Electromyographic features of upper body during wheelchair cycle ramps ascent for disabled with spinal cord injury (휠체어 사이클 경사로 주행 시 척수손상 장애인의 상체 근전도 특성 분석)

  • The aim of this study was to investigate difference of the muscle activation patterns of the upper body during wheelchair cycle ramps ascent of different slopes for disabled with spinal cord injury. Three subjects who is disabled with spinal cord injury participated in this study. Surface electromyography (EMG) data (reaction time [RT], onset-offset time, and peak value of muscle activation) were collected biceps, triceps, upper trapezius, anterior deltoid, latissimus dorsi, and upper rectus abdominal muscles during wheelchair cycle ramps ascent ($0^{\circ}$, $3^{\circ}$, and $6^{\circ}$). For latissimus dorsi muscle, RT and peak value of muscle activation was were increased and offset time was delayed as the slope increased (p < 0.05). These results indicate that wheelchair cycle ramps ascent might cause excessive overuse of latissimus dorsi muscle.

  • 이두 근 활차의 역할은 견관절 운동 시 이두 근 장두에 가해지는 전상방 전단응력을 막아주며, 관절 내에서 이두 근의 운동을 인도해줌으로써, 이두 근 장두의 관절 내 유효거리를 유지해 주게 된다. 견관절의 외전 및 외회전 운동시 이두 근 장두의 내측으로 가해지는 응력은 이두 근 활차 및 견갑하건의 손상과 함께 이두 근 장두의 내측 탈구를 유발할 수 있으며. 회전근 개 파열과 동반될 경우 증상을 더욱 악화시킬 수 있으므로 진단 및 치료에 주의를 요한다.

  • The aims of free tissue transplantation to the injured foot are to cover the exposed blood vessels, nerves, muscles, tendons and bones, to clear up infection, to lessen the morbidity, to shorten the hospitalization, and to prepare for further surgical procedures when no local or transpositioning flaps are available. Authors have carried out free flap transplantation in 13 cases of crushing injury, osteomyelitis and electrical burn in the foot at Chonbuk National University Hospital from June 1992 through May 1996. The results were as follows : 1. 9 cases of 13 (69%) were sustained from the traffic accident. 2. The dorsalis pedis free flap transplantation has been performed most frequently in 5 cases (38.5%), followed gracilis muscle flap in 4 cases(30.7%), rectus abdominis muscle flap in 2 cases(15.4%), latissimus dorsi muscle flap and upper arm flap in 1 each. 3. 6 muscle flaps were covered with split thickness skin graft 20 days after microsurgical anastomosis. 4. All of 13 cases were survived after microsurgical procedure and showed excellent coverage in the foot.

The Effects of Treadmill Exercise on Inhibition of Soleus Muscle Atrophy and Improvement Functional Recovery after Spinal Cord Contusion in the Rats (척수타박손상 후 트레드밀운동이 근 위축지연과 기능적 회복에 미치는 효과)

  • Physical activity and exercise can promote sensorimotor recovery from central nerve injury. It has been suggested that the functional recovery promoted by exercise training after spinal cord injury might be associated with insulin-like growth factor-I in the inflicted muscle. To investigate morphological and biochemical change of the soleus muscle after spinal cord injury, all tissues were used for H&E, immunofluorescence staining and Western blot. Also, BBB-test was used to evaluate behavioral improvement after spinal cord contusion. Thirty male Sprague-Dawley rats ($230{\pm}10\;g$; 7week in age) were assigned equally to three different groups; Normal (n=10), SCI (n=10), SCI+TMT (n=10). Every rat in SCI and SCI+TMT groups underwent laminectomy at T9 level and then contusion on the exposed spinal cord site in anesthetized condition. After one week-recovery from contusion, every rat in the SCI+TMT group exercised on a motorized treadmill for 30min/d, 5d/wk for 7wks. TMT followed by injury increased IGF-I induction levels in the soleus muscle and inhibited muscle atrophy. Behavioral scales for 4 and 8 weeks after spinal cord injury were improved in the SCI+TMT group compared to the SCI group. These results suggest that treadmill exercise after spinal cord injury might promote functional recovery along with muscle regrowth through the up-regulation of IGF-1 in muscle tissue.
