  • 오늘날의 대부분 의료기관이 성취해야 할 가장 중요한 과제는 의료 관리비용의 절감과 의료 서비스품질의 향상이다. 최근 병원의 대형화 및 병원간의 경쟁 과다로 인하여 환자에 대한 서비스는 호전되었지만, 병원 자체 인력에 대한 효율적 배분 및 인력 운용에 있어서의 형평성 문제는 아직까지 해결되지 못한 부분이 많다. 특히 모든 일을 경제 원리로 풀려고 하는 사회적 풍토 및 현대화, 대형화에 따른 병원 재정의 압박은 병원 운용 인력의 $30{\sim}40\%$를 차지하는 간호 인력에 대한 관심을 소홀히 하고 있다. 이에 본 연구에서는 국내 대형 병원을 사례기관으로 선정하여 기존의 간호사 근무 편성 기준을 분석하고, 이를 토대로 간호사들의 요일별, 시간별 선호도를 고려한 근무 편성표를 작성하여 간호사 근무에 형평성을 기하고자 한다. 또한 기존 휴리스틱에 의거한 근무표와 수리계획법을 활용하여 작성한 근무표 간의 비교를 통하여, 간호사 개인이 특정 근무 기간동안 얻게 되는 만족도를 분석하고자 한다.

  • Crew rostering in railway operations is usually done by arranging a crew diagram in accordance with working standards every month. This study was done to identify the problems related to the creation of crew rosters in railway operations and to suggest an optimum crew rostering method that can be applied in railway operations planning. To do this, the work standards of a railway company were identified, and a genetic algorithm was used to develop an optimal roster with equal working time while considering actual working patterns. The optimization process is composed of analysis of the input data, creation of work patterns, creation of a solution, and optimization steps. To verify the method, the roster derived from the proposed process was compared with a manually created roster. The results of the study could be used to reduce the deviation of business hours when generating a roster because the standard deviation of working time is the objective function.

Analysis of the Status of Nursing Shifts and Nurses' Perceptions of Work Schedules in General Wards Based on the Government Guidelines for Night Shift Work (보건복지부 야간근무 가이드라인에 따른 일반병동 간호사의 교대근무 현황과 근무표에 대한 인식)

  • Purpose: To analyze nurses' shift work according to the government guidelines for night work and their perceptions of their work schedules. Methods: The study sample included 487 nurses who provided information on their schedules, including the normal working hours of each shift, and overtime per shift during September 2020. Nurses' perceptions were measured in terms of satisfaction, appropriateness for work-life balance, and fairness to their work schedule. Results: One-third of the respondents worked more than 40 hours per week. The average overtime hour was 1.14 hours per shift. Unsocial hours (8 pm to 6 am on weekdays, midnight to midnight on weekends and public holidays) accounted for 56.4% of all working hours. During their last night shift, on average, nurses worked 9.62 hours and had a break of 39 minutes, although 20.5% reported no break. Sixty-eight percent of nurses had at least one between-shift break shorter than 48 hours after a consecutive night shifts. Fifty-seven percent were satisfied with their schedule. One-third perceived their schedule as appropriate for work-life balance, and two-thirds perceived that days off on weekends and nights were fairly distributed within the unit. Working and overtime hours had an inverse relationship with all three aspects of nurses' perceptions. A higher proportion of unsocial hours and having no breaks during the night shift were associated with lower perceptions of fairness. Conclusion: Reducing working hours, ensuring breaks during night shifts, and increasing rewards for unsocial hours are required to improve nurses' perceptions and reduce turnover due to shift work.

A Development of Decision Support System for Nurse Scheduling (일 대학병원 간호직원 근무 스케쥴링 전산화 개발 사례)

  • The critical problem of nurse scheduling in the hospital is determining the day-to -day shift assignments for each nurse for the specified period in a way that satisfies the given requirements of the hospital. As nurse scheduling involves many factors and requirements, manual scheduling requires much time and effort to produce an adequate schedule. A PC - based decision support system, developed in Turbo - C/sup ++/, for nurse scheduling was introduced. The system is composed of 4 sub-systems : 1) Entering basic information for each nursing unit : 2) Generation of an appropriate initial schedule and revised schedule for a given period, maximally satisfying each nurse's duty requests : 3) Provision of variety of statistical information, and 4) Help messages for each modular function. Icons and a mouse are used for easier graphic user interface and reducing the need for typing efforts. This system can help nurses develop quick and easy schedule generation and allow more time for the patient care.

A Study on the Nurse Scheduling Optimization Model for Nurse Needs-Type Scheduling Automation System

  • Today, with the development of information technology, hospitals are actively researching hospital information systems that are not limited by time and space to integrate mobile computing technology into the medical field to manage the bulk data of medical information. Nevertheless, most hospitals still spend a lot of time and effort creating manual schedules. In this paper, we studied an optimization model for organizing nurses' shift work and constructed an automated nurse-type job organization system. For nurses working in S hospital, information data, requirements and constraints of nurses were constructed. By applying this, we proposed an optimized scheduling method and built a web-based platform used by head nurses and a mobile app platform used by general nurses to enable real-time interchange and sharing around web servers. Therefore, through the developed nurse needs type automated system, the head nurses will increase the convenience of the nurses to organize the work every month, and general nurses will help them to work more accurately through personal schedule management. It is also expected to increase work efficiency by sharing work schedules among nurses.

The Effects of Nursing Work Environment and Emotional Labor on Job Satisfaction of Nurses in Provincial Medical Centers (공공병원 간호사의 간호근무환경과 감정노동이 직무만족에 미치는 영향)

  • Identifying the factors between nursing work environment and emotional labor to job satisfaction for nurses in public hospitals, 176 nurses who worked at six medical centers in Gyeonggi-do as study subjects were selected. The data was analyzed using descriptive statistics, ANOVA, t-test, post-test Scheffe's test, and multiple regression analysis with SPSS/WIN program(23.0). In the results to identify the factors that influence the job satisfaction, the better the nursing work environment(β=.797, p=.000) and the lower the degree of emotional labor(β=-.089, p=.045), the higher the job satisfaction. Additionally, job satisfaction increased when nurses cared the fewer number of patients. To increase job satisfaction for nurses, the policy should be considered to care fewer patients per a nurse, improve work environment and decrease emotional labor.

Rotating Shift and Daytime Fixed Work Schedules as a Risk Factor for Depression in Korean Police Officers (순환교대근무와 주간고정근무 경찰공무원의 우울감에 미치는 요인에 대한 연구)

  • Objectives: Working at other times than the regular day shift has been reported to be a stressor associated with health consequences and mental disorders as well as disturbance of sleep. In current study, we aimed at investigating the relationship between work schedule, sleep quality and depression among police officers. Methods: Eleven hundreds and forty five police officers (male: 1040, female: 105) completed questionnaires of basic socio-demographic data, Pittsburgh Sleep Quality Index (PSQI), Korean Scale of Occupational Stress (KOSS), Impact of Event Scale - Revised (IES-R) and Center for Epidemiologic Study-Depression (CES-D). Results: After controlling for age, sex and educational level, score of CES-D was correlated positively with the score of KOSS, PSQI and IES-R on partial correlation analysis (r=-0.077; p=0.009, r=0.262; p<0.000 and r=0.421, p<0.000, respectively). Logistic regression analysis revealed that female sex, age, the score of KOSS and IES-R and schedule of rotating shift work predicted higher score than 16 score of CES-D significantly in total subjects (p=0.023, p=0.015, p=0.000, p=0.000 and p=0.022, respectively). Conclusion: Current result suggested that not only female sex, age, higher occupational stress and impact of event scale but also rotating shift work schedule might be related to depression among police officers.

  • Crew scheduling is the problem of pairing crews with each of the vehicles in operation during a certain period of time. A typical procedure of crew schedule optimization consists of enumerating all possible pairings and then selecting the subset which can cover all the operating vehicles, with the goal of minimizing the number of pairings in the subset. The linear programming approach popularly adopted for optimal selection of pairings, however, is not applicable when the objective function cannot be expressed in a linear form. This paper proposes a method of integrating integer programming and heuristic search to solve difficult crew scheduling problems in which the objective function cannot be expressed in linear form and at the same time the number of crews available is limited. The role of heuristic search is to improve the incomplete solution generated by integer programming through iterative repair. Experimental results show that our method outperforms human experts in terms of both solution quality and execution time when applied to real world crew scheduling Problems which can hardly be solved by traditional methods.

A Study on the Status of Occupational Environment and Health Examination Data of Aged Workers in Korea (고령취업자의 작업환경과 건강진단 결과에 관한 실태조사)

  • 고령 근로자의 작업환경 실태와 건강검진 자료를 분석하여 그들의 건강한 생활과 삶의 질을 향상시킬 수 있는 정책의 기초자료를 마련하고자 시도되었다. 대상자는 부산, 경남지역에 소재하는 41개 사업장에서 유해인자에 노출되어 작업하는 공정에 있는 50세 이상 근로자 112명(남성 91명, 여성 21명)을 대상으로 소음, 분진, 유기용제, 중금속, 작업형태 및 작업강도 등을 측정, 분석하였으며, 건강검진 자료는 일반건강진단표를 이용하여 다음과 같은 결과를 얻었다. 1. 대상자의 평균연령은 54.9(남성 55세, 여성 54.7세) 세였으며, 평균근무기간은 8.7년이 였으며, 조사대상 사업장중 71.5%가 50인 이하의 근로자를 고용하고 있는 사업장이 였다. 2. 소음에 폭로 대상자 중 31.3%가 90dB(A)인 노출기준치을 초과하였으며, 그 외의 분진, 유기용제, 중금속 등의 노출기준치를 초과한 대상자는 없었다. 3. 조사대상자 중 9%는 청력이 비정상이였으며, 78.6%의 대상자는 경작업과 중등작업을 하였으며, 52.7%는 전신을 이용하여 작업하는 형태였다.

A Mathematical Model for Airline Ground Crew Scheduling Problem (항공사 지상직 승무원 근무 당번표 작성문제)

  • For the past several decades, personnel scheduling and rostering problem has been one of the most popular research topics in optimization area. Among the numerous applications, airline (aviation) industry has been given most attention due to the economic scale and impact. Most of the literatures about the staff scheduling problem in airline industry are dealing with the air crew, pilots and flight attendances, and the rest of the literatures are about the ground staff, by whom cleaning, maintenance, fueling of aircraft and handling luggage are done from landing to taking off. None of the literatures found by the authors are dealing with the airline ground crew. In this paper roster of airline ground crew, who is responsible for issuing boarding pass, checking baggage, etc, is introduced, formulated and solved using CPLEX. Some expressions of the mathematical formulations, which are not suitable input format of the CPLEX, were transformed. Numerical examples are presented for the validation of proposed scheduling system.
