병원에서의 산업보건관리

  • 본 연구는 병원에서의 산업보건관리 실태를 파악하기 위하여 전국에 있는 병원을 대상으로 1992년 12월 1일부터 1993년 1월 23일까지 설문조사를 설시하였다. 설문조사에 응당한 병원은 41개병원으로 회수율은 51.9%였다. 조사에 이용된 설문지는 산업안전보건법 내용을 기초로 하여 본 연구자가 작성한 것이며 안전보전에 관한 제반 규정, 안전보건관리체계, 근로자건강관리, 작업환경관리, 보건교육, 보건관련시설, 질병 및 상해에 대한 보상 등에 관한 45개문항으로 구성되어 있다. 분석결과는 다음과 같다. 첫째, 전체 병원 중 68.3%가 안전보건관리 규정이 있다고 답하였으며, 77.5%의 병원이 단체협약 중 근로자의 안전과 보건에 관한 조항을 포함시키고 있는 것으로 나타났다. 단체협약에 포함된 내용 중에는 정기건강진단(23.7%)과 질병 및 상해에 대한 보상(20.9%), 안전보건위원회의 구성(18.1%)등이 많았다. 둘째, 대상 병원중 66.7%가 안전보건관리책임자, 관리감독자, 안전담당자를 두고 있는 것으로 나타났고, 근로자수가 1,000명 이상인 병원은 83.3%가 이들 인력을 선임하고 있다. 그러나, 보건관리자와 안전관리자를 전담으로 두고 있는 경우는 한 곳도 없었으며 50%이내에서 겸직으로 선임하고 있었다. 또한, 안전보건위원회를 운영하고 있거나 노사협의회로 이를 대치하고 있는 병원은 54.2%로 절반 수준에 머물렀고, 그나마 정기적으로 운영을 하고 있다고 답한 병원은 3개 병원 밖에 되지 않았다. 세째, 근로자 건강관리를 위한 건강진단 실시 현황을 보면, 채용시검진 97.6%, 일반건강진단 100.0%, 특수 건강진단 24.4%가 실시하고 있었다. 건강진단에서 주로 실시하고 있는 항목은 흉부 X선검사, 시력, 혈압, 혈청GOT/GPT, 체중, 신장등이었다. 그러나, 병원에서 감염될 수 있는 기회가 높은 간염항원/항체검사의 실시율은 85.7%로 아직도 실시하지 않는 병원이 있음을 알 수 있다. 네째, 작업환경관리를 위하여 정기적으로 환경측정을 실시하는 병원은 21.6%였고 주로 측정하는 항목은 방사선이었다. 특히 소음, 조명, 분진에 대한 측정이 이루어지지 않음은 근로자뿐아니라 환자의 안위에도부정적인 영향을 미칠 수 있다. 다섯째, 안전한 작업과 근로자의 건강을 위하여 정기적인 보건교육을 설시하고 있는 병원은 불과 5.6%였다. 교육내용은 주로 안전에 관한 것이었다. 여섯째, 근로자의 일상적인 건강문제를 해결해주는 의무실을 독립적으로 마련하고 있는 병원은 한 곳도 없었고, 교대근무자나 당직자를 위한 수면시설은 44.7%, 작업 후에 사용할 수 있는 샤워시설은 35.1%의 병원이 마련하고 있었다. 일곱째, 근무중 다치거나 병에 걸렸을 때 치료비에 대한 부담은 대체로 병원이 하고 있으며 치료기간 중 근무는 병가나 휴직으로 처리되고 있다. 이는 병원이 산재보상보험법에 적용을 받지 않은 사업으로 분류되어 있는 현실에 기인된 것으로 보상과 관련된 부담을 병원이나 근로자 모두가 안게 되는 결과를 초래하게 되는 것이다. 이상에서 볼 때 병원근로자를 위한 산업보건관리는 주로 건강진단과 질병 및 상해에 대한 보상제도가 이루어지고 있는 실정이다. 그러므로 질병의 예방이나 악화방지 나아가 건강증진을 도모하기 위한 지속적인 노력을 기울이고 있는 병원은 전무한 상태임을 알 수 있다. 이러한 현실을 병원근로자를 위한 산업보건관리에 대한 인식이 전반적으로 부족함을 드러내는 것으로 이를 개선하기 위한 보다 적극적인 노력이 시급히 요청되고 있다.

  • 본 연구에서는 건축공사의 공정별 공정내용을 파악하고 그에 따라 발생하는 각종 유해요인을 분석하여 공정별 유해요인에 따른 작업환경측정대상과 특수건강진단의 정확한 진단항목 대상을 분석한다. 이를 통한 유해 공정에 따른 정확한 유해요인인자의 파악을 통해 작업환경측정 및 특수건강진단을 유해인자별 정기적으로(산업안전보건법 시행규칙 별표23 특수건강진단의 시기 및 주기를 참조)실시한다면 유해 인자에 따라 표적 추적 검사를 통해 보다 유의한 결과를 측정 가능하며 도출된 결과로 관리적, 공학적, 개인적 개선대책을 세워 관리한다면 직업성 질병을 예방하고, 쾌적한 작업환경을 조성하여 근로자의 안전과 보건을 유지증진 할 것으로 판단되었다. 또한 정기적인 특수건강진단으로 직업성 질병의 예방 및 조기발견으로 사후조치를 적절히 하여 근로자 건강 보호와 노동 생산성 향상을 기대할 수 있을 것으로 기대된다.

Influencing Factors on Workers Satisfaction in Periodic Health Examination (근로자 정기 건강진단 만족도에 영향을 미치는 요인)

  • This study was carried out to investigate what factors affect satisfaction of periodic health examination undertaken by employees. The purpose of the study was to improve quality of periodic health examination and to increase the rate of workers participation in the screening test. The content of questionnaire was designed with focus on the satisfaction and attitude on the periodic health examination. Data were analyzed with 212 males, 181 females among 393 samples who took periodic examination at a general hospital in Seoul. Korea. The results were as follows: 1. Respondents were mainly in the thirties(42.7%), married(58.5%), more than 3 years working experiences(66.7%) and employed in Sales & Customers service industry(60.1%). 2. There were significant differences in the scores of satisfaction by general characteristics among the respondents. The highly educated and the single showed higher satisfaction on the periodic health examination than other respondents. 3. The scores of satisfaction in periodic health examination showed highly influenced by level of age, work duration, level of knowledge to the health screening and income. Based on the results, this study concluded that the periodic health examination might be necessary to modify its items according to the clients characteristics and their individual demands for maintenance of healthy life.

  • Purpose: This thesis is to measure construction working environment and to improve health protection of site workers, who are required to take special health check, by analyzing harmful factors that are occurred by each site operation. Method: This thesis explicates the analysis of harmful factors per the operation at construction site, the target of working environment measurement with examined harmful factors, and the variables of special health check. Result: This thesis figures out the target of working environment measurement and the variables of special health check through the analysis of harmful factors per the operation from 12 construction projects. Conclusion: This This thesis is expected to be an instrument for preventing construction site workers from occupational disease, by regular execution of measuring working environment via harmful factors of each operation as well as of special health check.

A Study on the Status of Oral Health & Oral Health Attitude of Workers in Industries (근로자의 구강건강실태와 구강건강관리행동에 관한 조사)

  • The purpose of this study was to obtain baseline data for establishing oral health policy and developing oral health among industrial workers. A question was used to question paper 226 workers of D heavy industrial company in Sacheon-City. The result obtained were as follows: 1.Generally dental patients asked to not prevention treatment but treatment of dental disease. 2.Most of workers respondent that their oral health is so so.(52.2%). 3.Respondents reported 76.1% of dental calculus, 55.8% of sensitive to cold and hot things. 4.Oral health attitude is tooth brushing experience (39.8%), scaling experience (75.7%), when brushed area all teeth, gingiva and tongue(47.3%). 5.81.8% of respondents received no teeth pain when brushing time is over 3 minutes and 83.7% of smokers had calculus.

A Survey on the Status of Health Examination among Farmers in a Rural Area (일부 농촌지역 농업종사자들의 건강진단 수검 실태)

  • This study was carried out to reveal the status of health examination among farmers and to attract more attention to the health care system for farmers. Ten pre-trained medical students interviewed the rural residents 18 years of age and older in eight villages which were randomly selected from a county near Taegu city in Korea, in August 1996. Finally 751 persons were interviewed of whom the percentages of male and female were 41.8%, 58.2% respectively. Among the subjects, 361(48.3%) were fully engaged in farming, 184(24.4%) were partly engaged, and the remaining 206(27.3%) were not engaged in farming at all. The overall prevalence of farmer's disease was 23.0% and there was no significant difference between the group of fully engaged in farming(23.3%) and the group of not-fully engaged(22.9%). But the prevalence of farmer's disease in female subjects(27.8%) was significantly higher than that in male(16.2%)(p<0.01). Among the 288 farmer engaged in spraying pesticide, 113(39.2%) had experienced one or more pesticide related symptoms during last one year, but only 18(15.9%) of them had visited medical facilities due to their symptoms. The experience of receiving education about pesticide was significantly correlated with the degree of wearing protectors during pesticide spraying(p<0.001). Among the 736 persons excluding non-respondents, 281(38.2%) received health examination during last one year ; 176(62.6%) of them received free health examination, and 105(37.4%) received charged one. Among the 533 persons 40 years age and older, only 124(23.3%) had received the 'health examination for the elderly' during last one year, which is provided for the 40 years age and older by Korea medical insurance corporation and medical insurance societies. Most of all beneficiaries of self-employed medical insurance thought the imposed contributions as very expensive(77.4%) or moderately expensive(13.2%). The great majority of farmers are exposed to various health risk factors including pesticide, high temperature, overwork etc. comparable to industrial workers. But farmers are excluded from the regular yearly worker's health examination because of not belonging to a company despite they pay relatively more medical insurance contributions compared with the industrial workers and the urban self-employed medical insureds. It is necessary to develop special health management program for farmers such as the special health examination for the industrial workers exposed harmful agents.

Long term noise exposure of steel mill workers, hearing loss and blood pressure (제강소 장기근무자의 소음 노출 및 청력손실과 혈압과의 관계에 관한 연구)

  • A cross-sectional study was conducted to investigate the effect of long term noise exposure on blood pressure among steel mill workers. The workers participated in periodic medical examinations performed from August 27 to September 6 in 1990. Examined were 1,034 workers with high-level noise exposure(average $91.8{\pm}5.2dB(A)$) and 390 workers with low-level noise exposure(average $75.2{\pm}4.6dB(A)$). No significant difference was found in systolic or diastolic flood pressure between the two exposure groups. Prevalence of hypertension (${\geq}160mmHg\;systolic\;or{\geq}100mmHg\;diastolic$) was higher in a younger age group (${\leq}40$ years old) of high-level noise exposure than of low-level noise exposure. However, the difference was not statistically significant. Furthermore, in younger ages, prevalence of hypertension appeared to be higher in the hearing loss group (${\geq}25dB\;at\;1000Hz\;or{\geq}40dB$ at 4000Hz in at least one ear) than in the normal hearing group. From multiple regression analysis, hearing loss, body mass index, age, alcohol and family history of hypertension were proven to be predictors of diastolic blood Pressure (p<0.02). When regression was performed for each age group, hearing loss was a strong predictor of diastolic pressure in the younger age group (p<0.01).

Knowledge and Attitude of the Workers and the Health Personnel on the Health Management in Kyung-In Area (경인지역 일부 근로자와 보건담당자의 보건관리에 대한 인식 및 태도)

  • This study was performed to investigate the knowledge and attitude of workers in small scale industries on health management, and to provide the basic data for more effective service by the group occupational health service system. The knowledge and attitude of 247 workers and 46 health personnel in the industries scattered around Incheon were investigated from December 1992 to February 1993. The results were summerized as follows ; 1. There were significant differences between the workers and the health personnel by age, sex, marriage status, job-position and education level. 2. The recognition level of the workers to contract work related disease was significantly higher than that of health personnel, and recognition level of the workers on environmental hazards and on the utility of measuring hazards were lower than that of health personnel. 3. The recognition level on the content of the group occupational health service system was significantly different between workers and health personnel, 72.6% of the responses from the workers answered that they did not know what the group occupational healthe service system was, but 82.2% of the responses from the health personnel answered that they knew well what it was. And 79.0% of all respondents thought it was necessary for worker's health. 4. Seventy three percent of the respondents from the workers indicated that they had never taken health education. However, 93.0% of all respondents answered positively for the need of health education to promote their health. 5. Current health service system was judged to be insufficient for the demand of workers for better health. Most of the respondents prefered a formal but flexible health service system and they wanted the periodic health examination to be followed up. It was revealed that despite of poor knowledge, the demand of workers for health service was higher than the current supply. Therefore, this study suggests that educating both health personnel and workers to obtain correct knowledge on the hazards to work enviroment and health management is needed for effective occupational health service.

Trends of Study and Classification of Reference on Occupational Health Management in Korea after Liberation (해방 이후 우리나라 산업보건관리에 관한 문헌분류 및 연구동향)

  • The purposes of this study are to define the scope of occupational health management and to classify occupational management by review of related journals from 1945 to 1994 in Korea. The steps of this study were as follows: (1) Search of secondary reference; (2) Collection and review of primary reference; (3) Survey; and (4) Analysis and discussion. The results were as follows ; 1. Most of the respondents majored in occupational health(71.6%), and were working in university (68.3%), males and over the age 40. Seventy percent of the respondents agreed with the idea that classification of occupational health management is necessary, and 10% disagreed. 2. After integration of the idea of respondents, we reclassified the scope of occupational health management. It was defined 3 parts, that is , occupational health system, occupational health service and others (such as assessment, epidemiology, cost-effectiveness analysis and so on). 3. The number of journals on occupational health management was 510. It was sightly increased from 1986 and abruptly increased after 1991. The kinds of journals related to occupational health management were The Korean Journal of Occupational Medicine(18.2%), Several Kinds of Medical Colloge Journal(17.0%), The Korean Journal Occupational Health(15.1%), The Korean Journal of Preventive Medicine(15.1%) and others(34.6%). As for the contents, the number of journals on occupational health management systems was 33(6.5%) and occupational health services 477(93.5%). Of the journals on occupational health management systems, the number of journals on the occupational health resource system was 15(45.5%), occupational finance system 8(24.2%), occupational health management system 6(18.2%), occupational organization 3(9.1%) and occupational health delivery system 1 (3.0%). Of the journals on occupational health services, the number of journals on disease management was 269(57.2%), health management 116(24.7%), working environmental management 85(18.1%). As for the subjects, the number of journals on general workers was 185(71.1%), followed by women worker, white coiler workers and so on. 4. Respondents made occupational health service(such as health management, working environmental management and health education) the first priority of occupational health management. Tied for the second are quality analysis(such as education, training and job contents of occupational health manager) and occupational health systems(such as the recommendation of systems of occupational and general disease and occupational health organization). 5. Thirty seven respondents suggested 48 ideas about the future research of occupational health management. The results were as follows: (1) Study of occupational health service 40.5%; (2) Study of organization system 27.1%; (3) Study of occupational health system (e.g. information network) 8.3%; (4) Study of working condition 6.2%; and (5) Study of occupational health service analysis 4.2%.
