Mediating Effect of Innovative Behavior on the Relationship between Awareness of Nursing Manager's Leadership type in General Hospital Nurses and Future Use of Evidence-based Practice (종합병원 간호사의 간호관리자 리더십 유형 인식과 근거기반실무 적용의도와의 관계: 혁신행동의 매개효과)

  • This study was conducted to examine the mediating effect of innovative behavior in the relationship between nurse manager's leadership and nurses' future use of evidence-based practice. Participants were 183 nurses working at general hospitals with over 500 beds in Korea. Nurse manager's transformational leadership was positively correlated with innovative behavior and nurses' future use of evidence-based practice. Innovative behavior showed partial mediating effects in the relationship between nurse manager's leadership and nurses' future use of evidence-based practice. These findings suggest that in order to enhance nurses' future use of evidence-based practice, the educational strategies to promote nurses' innovative behavior should be developed and provided in nursing field.

Reflected Status of Evidence-Based Guideline on Fundamental Nursing Textbooks for Prevention of Catheter-Associated Urinary Tract Infections (기본간호학 교과서의 근거기반 도뇨관련 요로감염 예방 실무지침에 대한 반영 현황)

  • The purpose of this study was to analyze reflected status of evidence-based guideline on nursing textbook for prevention of catheter-associated urinary tract infections (CAUTIs). The subjects were 14 fundamental nursing textbooks consisting of 7 theoretical and 7 practical books published from March 2012 to March 2017. The evidence-based guideline was consisted of ninety-one recommendations for prevention of CAUTIs which were comprehensively collected from five guidelines. Collected data were analyzed using descriptive statistics. Reflected rate on theoretical and practical fundamental nursing textbooks were 23.0% and 15.2% respectively. The most reflected domains in theoretical and practical textbooks was catheter management and catheter insertion respectively. Sixteen out of 91 individual recommendations with more than 50% consistency rate were identified, and eight (50.0%) recommendations on the catheter maintenance area were identified. Lastly nine inconsistent recommendations were identified. And the most frequent discrepancies were indwelling catheterization for management of urinary incontinence, followed by regular changing indwelling catheter, clamping prior to removal and perineal care. In conclusion, we found that recommendations for the CAUTIs guidelines were poorly reflected on text books. Further revising fundamental nursing textbook should correct the discrepancies between textbooks and the guidelines.

  • The purpose of this research is to evaluate patient's satisfaction and clinician' knowledge level about pain, nausea, vomiting, and temperature management of patients after operation by develop an web-based evidence-based practice guideline about pain, nausea, vomiting, and temperature control after operation in order to apply the guideline operation patients. The collected data was analyzed through real number, average, standard deviation, t-test and repeated ANOVA by using SPSS/WIN 17.0 program. The study subjects showed a significant difference in the level of knowledge about pain, nausea/vomiting, and temperature control after operation and patient's satisfaction, before and after applying the web-based evidence-based practice guideline.

Belief in Evidence-Based Practice, Awareness of Importance and Performance of Nursing Practice Guidelines among Novice Nurses and Preceptors in a Tertiary General Hospital (상급종합병원 신규간호사와 프리셉터 간호사의 근거기반실무에 대한 신념, 간호실무지침에 대한 중요도와 수행도)

  • Purpose: This study was to investigate the belief in evidence-based practice, awareness of importance and performance of intravenous infusion and pressure ulcer evidence-based practice guidelines among nurses in a tertiary general hospital. Methods: The subjects of this study were 217 nurses working in a tertiary general hospital. Data collection was performed between February 11 and February 25, 2022. Data analysis was conducted descriptive statistics, t-test, hierarchical regression analysis, and Importance-Performance Analysis. Results: The mean score of belief for evidence-based practice among novice nurses was 3.34 out of 5, while preceptor nurses scored a mean of 3.41 out of 5. There was no significant difference in belief scores between novice nurses and preceptor nurses (t=-1.21, p=.227). The factors influencing the performance of evidence-based practice guidelines for intravenous infusion were belief in evidence-based practice (β=.14, p=.009) and importance of intravenous infusion (β=.51, p<.001), and the factors influencing the performance of evidence-based practice guidelines for pressure ulcer were belief in evidence-based practice (β=.15, p=.002) and importance of pressure ulcer (β=.65, p<.001). Importance-Performance Analysis of the evidence-based practice guidelines of two groups were used to identify common and different items. Conclusion: To improve the performance of evidence-based practice guidelines, it is necessary to enhance the evidence-based practice belief and importance of evidence-based practice guidelines. In particular, evidence-based practice should be provided to improve nursing quality through education on items of low-importance and low-performance and items of high-importance but low-performance guidelines identified through Importance-Performance Analysis.

The Characteristics of Evidence in Evidence-based Design Research and Their Implications to the Evidence-based Practice of Healthcare Architecture - Focused on Health Environments Research & Design Journal (근거기반설계 연구의 근거 특성과 의료건축의 근거기반실무를 위한 시사점 - HERD의 연구경향 분석을 중심으로)

  • Purpose: To prove the credibility of EBD, it is necessary to define concepts regarding the nature of credible knowledge and the hierarchy of evidence. In addition, the relationship between EBD and EBP needs to be addressed through a practical viewpoint. Methods: The systematic review is applied to define the nature of knowledge and the characteristics of research papers in architectural EBD while analyzing the field of study, knowledge focus and types of keywords of the first author. Results: As a whole, the types of research focus evaluated using the most frequent keywords include function(40.86%), specialized knowledge(30.52%), professional practice(21.37%), and health(5.78%). Empirical research(83.72%) is more likely to be found than theoretical research(13.95%). The EBD research papers ranked as top-tier are only 7(2.03%) among a total of 344, while the research papers ranked in the third to fourth tiers are 276(80.23%). Implications: There is a deficiency of reliable 1-2 tier research papers. From a practical viewpoint, it would be realistic to recommend that healthcare architects appropriately evaluate the quality of EBD research and apply it in practice, rather than to conduct EBD research themselves. This systematic review illustrates the nature of EBD research.

Development and Effectiveness of Practice Application Program of Intravenous Infusion Evidence-Based Nursing Practice Guideline - for Small and Medium Sized Hospitals (근거기반 정맥주입요법 간호실무지침 실무적용 프로그램 개발 및 평가 - 중소병원을 대상으로)

  • Purpose: This study was conducted to develop and test the effects of a program for practice application of intravenous infusion evidence based nursing practice (EBP) guidelines in small and medium-sized hospitals. Methods: A mixed method research design was used, combining non-equivalent control group pre-post test design with qualitative study analysis. The subjects consisted of 55 nurses. The practice application program was developed based on the Advancing Research and Clinical practice through close Collaboration (ARCC) model. Data were collected for analysis in the following areas: nurses' EBP organizational culture and readiness, EBP beliefs, EBP implementation, importance about intravenous infusion, and performance about intravenous infusion, with data assessed using valid and reliable instruments. Patient outcomes were collected from the hospital's medical records. Data were analyzed using t-test, χ2-test, and Shapiro-Wilk test, with qualitative content analysis used for interview data. Results: Following the intervention, nurses' EBP organizational culture and readiness, EBP beliefs, EBP implementation, and performance of intravenous infusion and perceptions of its importance showed significant improvement in the experimental group. Phlebitis rates decreased in the experimental group compared to the control group. Conclusion: This program is effective to improve nurse's perception and practice of evidence based nursing. Therefore we recommend to use this program at same levels of hospitals.

Knowledge Management, Beliefs, and Competence on Evidence-Based Practice, Evidence-Based Decision Making of Nurses in General Hospitals (간호조직의 지식관리, 간호사의 근거기반실무 신념 및 역량이 근거기반 의사결정에 미치는 영향)

  • Purpose: The purpose of this study was to explore how knowledge management of hospital and nurses' beliefs and competences on evidence-based practice can affect evidence-based decision making. Methods: In this descriptive study, a total of 184 nurses who were working in the five general hospitals participated. The data were collected through a self-administered questionnaire in September, 2014. Data were analyzed using descriptive statistics, independent t-test, one-way ANOVA, Pearson correlation coefficient, and step-wise multiple regression with SPSS/WIN Statistics 21.0 program. Results: Evidence-based decision making was correlated with EBP beliefs (r=.55, p<.001), EBP competence (r=.57, p<.001), and knowledge management (r=.50, p<.001). Hierarchical regression analysis showed that EBP beliefs (${\beta}=.18$, p=.005), EBP competence (${\beta}=.37$, p<.001), organizational knowledge management (${\beta}=.27$, p<.001) explained 48.6% of evidence based decision making (p<.001). Conclusion: The study results indicated that evidence-based practice competences, organizational knowledge management, and evidence-based practice beliefs were important factors on evidence-based decision making. In order to improve evidence-based practice among nurses through organizational knowledge management, EBP beliefs and competence at individual level need to be considered and incorporated into any systemic training of EBP.

Evidence-Based Nursing Practice Guideline: Ostomy Care (근거기반 임상실무지침: 장루간호)

  • Purpose: The aim of this study was to develop an evidence-based guideline for stoma management providing institutional policy, assessment, complications and follow-up care. Methods: The guideline adaptation manual consisting of 23 steps developed by the National Evidence-Based Healthcare Collaborating Agency was used for this study. It presents an overview of the process used to develop the guideline and lists specific recommendations from the guideline. Results: It provides 55 recommendations that include the following 8 topics: 1) Organization and policy recommendations, 2) Preoperative nursing; Ostomy education, stoma site marking, 3) Ostomy formation, 4) Postpoperative nursing; education, assessment, high output stoma management, 5) Selection of ostomy products, 6) Colostomy irrigation, 7) Stomal and peristomal complications, 8) Follow-up care after discharge. Conclusion: The guideline can be used to address stoma management in hospital settings. The intent of the guideline is to provide information that will assist healthcare providers to manage adult patients with ostomies, prevent or decrease complications, and improve patients' outcomes.

Development of Guidelines for Raid Response to Acute Deteriorating of Hospitalized Patients (입원환자의 급성 악화 신속대응 가이드라인 개발)

  • The purpose of this study was to develop evidence-based practice guideline for rapid response to acute deterioration of hospitalized patients. The guideline was developed according to the Clinical Practice Guideline Adaptation Manual. and evaluated using AGREE II tool. The guidelines were checked content validation by an expert group, final 130 recommendations in 5 sections including management, recognition, activation, response, and evaluation. The average of the overall practical application to the guidelines was 4.41±0.78 out of 5, which showed high applicability in clinical work.

Factors Influencing of Evidence based Practice Competency and Evidence based Practice Readiness in General Hospital Nurses (종합병원 간호사의 근거기반실무 준비도와 근거기반실무 역량의 영향 요인)

  • Purpose: The study was done to verify evidence-based practice (EBP) readiness and factors contributing to EBP competency in general hospital nurses. Methods: Participants were 219 nurses working in 7 general hospitals in G-city and J Province. Data were collected from May 12 to May 28, 2015 and analyzed using one-way ANOVA, Pearson's correlation and stepwise multiple regression. Results: Mean score for EBP competency was $3.89{\pm}0.83$, for critical thinking disposition, $3.35{\pm}0.36$, for EBP attitude, $3.53{\pm}0.66$, for beliefs about value of EBP, $3.42{\pm}0.56$, for ability in research performance, $2.86{\pm}0.57$, for time to devote to EBP, $2.57{\pm}0.69$ and for organization support for EBP, $3.36{\pm}0.46$. EBP competency was significantly correlated with critical thinking disposition (r=.37, p<.001), beliefs about value of EBP (r=.43, p<.001), ability in research performance (r=.44, p<.001), and time to devote to EBP(r=.33, p<.001). Factors influencing EBP competency in general hospital nurses were research performance (${\beta}=.29$, p<.001), beliefs about value of EBP (${\beta}=.24$, p<.001), critical thinking disposition (${\beta}=.15$, p=.014), recognition of EBP (${\beta}=-.18$, p=.002), which together explained 35% of total variance. Conclusion: Findings indicate that introduction and activation of EBP in general hospitals is essential and development of systematic training programs to strengthen critical thinking disposition and EBP competency are needed.
