What are Valuable Positive Signs of Supraspinatus Test for Diagnosis of Torn Rotator Cuff? - Comparison of Pain and Weakness in "Empty Can Test" and "Full Can Test" - (회전근 개 파열의 진단을 위한 극상근 검사에서 유용한 양성 징후는 무엇인가? - "Empty can test"와 full can test"에서 통증과 근력 약화의 비교 -)

  • Purpose: To investigate the validity of positive signs of supraspinatus test. Materials and Methods: The empty can test and full can test were performed on 200 shoulders which were diagnosed with magnetic resonance imaging or surgical findings as full thickness tear, partial thickness tear and no tear. Presence of pain, weakness, pain or weakness, and both pain and weakness were recorded as positive signs separately. The two tests with positive signs were compared and analyzed. Results: Pain and weakness were severity-dependent, and the empty-can test had a higher incidence of pain. The sensitivities of the two supraspinatus tests in all positive signs were higher when including partial-thickness tears in the tear group; however, their specificities were higher when excluding partial-thickness tears. The sensitivities of an empty-can test in 'pain', 'pain or weakness', 'both pain and weakness' were higher than those of the full-can test, otherwise, the specificity of the full-can test for 'pain' and 'pain or weakness' were higher. Concordance rate between 'pain or weakness' and 'pain' was the highest in all categorization. Conclusion: Both empty can and full can test were valuable for detecting torn rotator cuff.

The Difference in Diagnostic Performance for Detection of Supraspinatus Tendon Tears by Adding Angled Oblique Sagittal Plane Image to the Routine Shoulder MRI (고식적 견관절 자기공명영상에 추가적인 사각시상면 영상 이용 시 극상건 손상 검출 진단능 차이에 대한 고찰)

  • Purpose : The purpose of this study is to determine whether adding an angled oblique sagittal plane to the routine shoulder MRI improves the diagnostic performance in the evaluation of supraspinatus tendon tears with arthroscopic correlation. Materials and Methods: The study included 121 patients who had a shoulder MRI followed by arthroscopy. Two radiologists separately evaluated the supraspinatus tendon for tears on shoulder MRI either with or without the angled oblique sagittal images. Arthroscopy was used as the reference standard. The sensitivity and specificity for diagnosing supraspinatus tendon tears were calculated and compared by using McNemar test. Interobserver and intertechnique variability in the interpretation of supraspinatus tendon tears were calculated as a kappa value. Results: Adding the angled oblique sagittal images to the standard shoulder MRI showed improvement in the sensitivity for diagnosing full-thickness supraspinatus tendon tears and also in the sensitivity, specificity and accuracy for the detection of partial-thickness tears. However, there was no statistically significant difference in all of them between with and without the angled set. Interobserver agreement was substantial to almost perfect and intertechnique agreement was moderate. Conclusion: Adding an angled oblique sagittal plane image to the routine shoulder MRI showed no significantly different diagnostic performance in detecting the partial- and full-thickness supraspinatus tendon tears, compared to MRI without angled oblique sagittal plane.

Arthroscopic Repair of Full Thickness Tear of The Supraspinatus; Evaluation of the Clinical Outcome and the Postoperative Rotator Cuff Integrity (견관절 극상건 전층 파열의 관절경적 복원술; 임상적 결과 및 술후 회전근개 상태의 평가)

  • Purpose: The purpose of this study was to evaluate the tendon healing of arthroscopic repair in full-thickness supraspinatus tears. We evaluate the effectiveness of the arthroscopic repair of full-thickness supraspinatus tears by assessing functional improvement. Materials and Methods: Thirty consecutive full-thickness supraspinatus tears were repaired arthroscopically in 19 patients with a one row of anchor and 11 patients with two rows of anchors. Patients ranged in age from 51 to 79 years (average 63 years). Average follow-up was 16 month (range, 12 to 28 months). To evaluate the effectiveness of the arthroscopic repair of full-thickness supraspinatus tears by assessing functional improvement, we calculate the Constant, ASES, UCLA scores. The 30 patients had either an MR Arthrogram (25 cases) or an MRI (5 cases), performed between 5 months and 20 months (mean 10 months) after surgery. Results: The cuff was healed in 21/30 cases (70%) and partially torn in 3 cases (10%) after the arthroscopic repair of full-thickness supraspinatus tear. Although the supraspinatus tendon was totally torn to the tuberosity in 6 cases(20%) after the arthroscopic repair, the size of the tear was smaller than the initial in 5 cases. The Constant score improved from an average of $55.7{\pm}7.1$ points preoperatively to $77.7{\pm}9.7$ points at the last follow-up (p<0.001), and the average ASES score improved from $39.2{\pm}7.4\;to\;72.4{\pm}12.6$ (p<0.001), and the average UCLA score improved from $17.9{\pm}2.2\;to\;26.8{\pm}5.0$ (p<0.001). Strength of elevation was significantly better $(7.1kgs{\pm}2.4)$ in the shoulders with a healed tendon that in those with an total or partial re-tear tendon $(4.5kgs{\pm}1.0)$ (p<0.05). Factors adversely affecting tendon healing were increasing age, Only 41.7% of the repairs completely healed in patients over 65 years (p<0.05). Conclusion: Arthroscopic repair of isolated full-thickness tear of the supraspinatus leads to completely healing in 70% of the cases. Total or partial re-tear of the repaired rotator cuff is associated with a decreased strength. Older patients had significantly lower healing rates.

Pathology of the Rotator Cuff in Adhesive Capsulitis Patients (견관절 유착성 관절 낭염 환자에 동반된 회전근 개 병변)

  • Purpose: To evaluate any combined rotator cuff pathologies in adhesive capsulitis patients with magnetic resonance arthrography (MRA) or ultrasonography (USG), and to see any differences in findings between MRA and USG. Materials and Methods: From June to December 2005, 80 consecutive patients with adhesive capsulitis were prospectively evaluated with either MRA or USG. Two groups were randomly assigned for examination. Evaluation were focused on any combined rotator cuff pathologies especially supraspinatus tendon. Results: Small (less than 1 cm) full-thickness SSP tendon tear were seen in 6 patients (MRA 4, USG 2, 8%) and partial-thickness SSP tendon tears in 21 (MRA 12, USG 9, 26%). In addition, supraspinatus tendinopathy were seen in 15 patients (MRA 7, USG 8, 19%). Overall, various SSP pathologies were reported in 42 patients (53%) of the study objects (MRA 23, 68% and USG 19, 41%). Subscapularis tendon partial tears were reported in 9 patients (MRA 6, USG 3, 11%). There were no statistical differences of the findings between MRA and USG in detecting rotator cuff pathologies (p>0.5). Conclusion: Nearly one half of the adhesive capsulitis patients showed various supraspinatus tendon pathology in MRA or USG. Although MRA group showed slight higher percentage of associated rotator cuff pathology than USG group (without statistical significance), this could be attributed to better resolution capacity of MRA than USG.

The Usefulness of Multidetector CT Arthrography in the Diagnosis of Shoulder Pathology -Comparison with MR Arthrography and Arthroscopic Findings in the Same Patient- (견관절 병변의 진단에서 다중 검출 전산화 단층촬영을 이용한 관절조영술의 유용성 - 동일 환자에서 시행한 자기공명 관절조영술 및 관절경 소견과의 비교 -)

  • Purpose: The purpose of the study is to evaluate the usefulness of CT arthrography (CTA) by conducting a comparative study of CTA and MR arthrography (MRA) with the arthroscopic findings in the same patients. Materials and methods: Forty nine patients who suffered from shoulder disease underwent MRA and CTA concurrently. The sensitivity and specificity of CTA was evaluated, as compared to MRA, in the case of four types of shoulder pathological lesions. The accuracy of CTA was evaluated by analyzing the arthroscopic findings of thirty four patients. Results: Compared to MRA, CTA showed a sensitivity of 90% and a specificity of 100% for diagnosing supraspinatus tendon (SST) full thickness tear, and CTA showed a sensitivity of 100% and a specificity of 97.5% for making the diagnosis of superior labrum anterior to posterior (SLAP) lesion. For diagnosing partial articular side supraspinatus avulsion (PASTA) lesion, some studies have shown the usefulness of CTA with a sensitivity of 71.4% and a specificity of 97.7%. However, for diagnosing SST bursal side partial tear, the sensitivity of CTA was as low as 10%. CTA has been shown to be relatively accurate when the diagnoses were verified with the arthroscopic findings; diagnosing SST full thickness tear and SLAP lesion with CTA has shown an accuracy of 100% and87.5% respectively, and CTA showed 71.4% diagnostic accuracy for PASTA lesion. Conclusion: CTA was a useful tool and it was equivalent to MRA for the assessment of SST full thickness tear, SLAP lesion and PASTA lesion, but not bursal side partial tear. Thus, it may be used preferably to diagnose shoulder pathology and for follow up as an inexpensive tool after operation.

Phytosociological Study on Vegetation of Mt. Myongji for Management of Natural Ecosystem (명지산 생태계 관리를 위한 식물사회학적 식생 연구)

  • 본 연구는 명지산 식생의 보전방안의 수립과 생태계의 건전한 이용을 위해 수행되었다. 인위적인 훼손이 지속되는 등산로 주변은 질경이군란과 쑥-미국쑥부쟁이군락 등 노변식물 군락으로 나타났으며, 등산로 주변과 벌목지는 중부지방에서 잡목림이라 할 수 있는 고추나무-산뽕나무군락으로 나타났다. 그러나 중부지방에 위치한 명지산의 기후적 극상림인 낙엽활렵수림의 군락은 표고 300m 이상의 깊은 계곡과 정상부근에 극상림에 가까운 신갈나무-당단풍군락으로 잔존하는 것으로 나타났다. 또한 신갈나무-당단풍군락 주변에는 비교적 인위적인 영향을 크게 받은 병꽃나무-조록싸리 군락으로 구분되었다. 또한 이들 군락 내에서는 금강초롱꽃, 백작약, 도깨비부채, 개족도리 등의 희귀 및 멸종위기 식물들이 다수 관찰되었다.

Plant Community Structure of Nojasan in Koje Island (거제도 노자산지역의 식물군집구조)

  • 거제도 노자산지역의 식물군집구조를 파악하기 위해 43개 조사구(각 조사구당 10m$\times$10m)를 거제휴양림 임도 주변과 노자산 북사면에 설정하고 식생조사를 실시하였다. 식생조사자료를 이용 TWINSPAN에 의한 classification과 DCA에 의한 ordinationqnstjr을 실시하였다. TWINSPAN분석과 DCA분석 결과 소사나무군집(군집I), 소나무군집(군집II), 졸참나무군집(군집(III), 고로쇠나무, 느티나무, 비목나무, 까치박달 등을 우점종으로 하는 낙엽활엽수군집(군집IV)의 4개 군집으로 분리되었다. 식물군집구조 분석 결과 노자산지역의 식생은 소나무$\longrightarrow$졸참나무군집$\longrightarrow$낙엽활엽수군집으로 생태적 천이가 이루어질 것으로 판단되었으며, 소사나무군집은 해발고도가 높고 건조한 지역에 나타나는 토지극상으로 판단되었다.

Estimated Action Potentials During Repolarization Phase form the Body Surface Electrocardiogram (심전도의 재분극상에서의 활동전임의 추정)

  • The body surface ECG(electrocardiogram) is produced by the electric fields caused by the propagation of action potentials within the myocardial cells. The repolarization phase of the action potential is very sensitive to factors of clinical importance. Therefore, in this paper of the inverse electrocardiography, we studied a method of estimating the uniform action potentials during repolarization phase from the body surface ECG using digital signal identification techniques. The estimated action potential of a normal was similar to that of clinical data in the repolarization phase.
