MRI상 극상근건 부분파열로 진단받은 견비통 환자의 치험 2례 보고 (Report of Two Cases of Shoulder Pain Diagnosed through MRI as Partial Tear of Supraspinatus Tendon Treated by Oriental Medical Treatment)

  • Objectives: The purpose of this study is to report the improvement after oriental medical treatment of partial tear of supraspinatus tendon. Method: We treated 2 patients having shoulder pain due to partial tear of supraspinatus tendon with Oriental medical treatment, including Sa-am acupuncture DaeJang-Jeonggyeok, A-shi point(阿是穴) and herbal medicine. We checked visual analog scale (VAS) score and range of movement (ROM). Results: We treated shoulder pain. Thereafter ROM improved and V AS score dropped to the level of 2 to 4 points from 10 points at onset. Conclusion: This report shows Oriental medical treatment has effectiveness on partial tear of supraspinatus tendon. Follow-up study is needed.

초오 약침시술을 병행한 한의학적 치료로 통증과 가동범위가 호전된 극상근건 전층파열 환자 치험 1예 (Supraspinatus Tendon Full-Thickness Tear Patient's Pain and Range of Motion Improved by Korean Medicine Treatment with Aconitum ciliare Decaisne Pharmacopuncture: A Case Report)

  • The objective of this study is to report the improved case of Korean medicine treatment with Aconitum ciliare decaisne pharmacopuncture for supraspinatus tendon full-thickness tear. We used Aconitum ciliare Decaisne pharmacopuncture with other Korean medicine treatments (acupuncture, electroacupuncture, and cupping) for this patient. This case is measured and assessed by the numerical rating scale (NRS), shoulder range of motion (ROM), shoulder pain and disability index (SPADI) and shoulder physical examination. As the result, we found the improvement of NRS, SPADI, ROM, and shoulder physical examination. This study suggested that Korean medicine treatment with Aconitum ciliare Decaisne pharmacopuncture is proved to be helpful to relieve pain and recover function for supraspinatus tendon full-thickness tear. But further clinical studies are needed to clarify the effect of Aconitum ciliare Decaisne pharmacopuncture therapy for patients with supraspinatus tendon full-thickness tear.

극상근건 파열에 대한 복합 한방치료의 효과 : 증례보고 (Effect of Integrative Korean Medicine Treatment for Tear of Supraspinatus Tendon: A Case Report)

  • A 53-year-old woman with right shoulder pain after falling down presented at our outpatient department after conventional treatment that had little effect. She was treated with integrative Korean Medicine treatment as an outpatient from February 2, 2022 to July 4, 2022. Based on Magnetic Resonance Imaging (MRI) findings, the patient was diagnosed with a right rotator cuff tear. With 39 treatment sessions, Numeric Rating Scale (NRS) score decreased and showed improvement in assessment. On May 6, MRI was retaken to show restoration of the supraspinatus tendon tear. The patient felt almost no pain on July 4, declaring the end of treatment. This successful case suggests that integrative Korean Medicine treatment might be an attractive alternative for severe rotator cuff tears.

초음파 유도하 견관절 주사요법 (Ultrasound-Guided Shoulder Injections)

  • 견관절 주위 통증은 정형외과 의사가 외래에서 접하는 가장 흔한 문제 중 하나이다. 견관절 주위 통증을 치료하기 위한 여러 치료 방법 중 상견갑신경 차단술뿐만 아니라 관절 및 관절 주위 주사 요법은 좋은 임상적 결과를 보이고 있다. 초음파 유도하 주사요법은 안전한 테크닉으로 합병증을 줄이고 시술의 안전성과 정확도를 높이고 있다. 초음파 유도하 견관절 주위 주사요법을 하기 위해서는 표면 해부학을 정확히 숙지하는 것이 중요하다. 이 논문은 표면해부학에 대한 설명 및 견관절과 주위 구조물의 초음파 해부학을 기술하는 데 그 목적이 있다. 또한 여러 주사방법 및 신경 차단술에 대해 설명하고자 한다.

  • $RuO_2$를 하부전극으로 적용한 (Ba,Sr)$TiO_3$[BST] 박막의 Sputtering 가스내 $O_2/Ar$ 비에 따른 특성을 고찰하였다. $O_2/Ar$ 비가 1/9에서 5/5로 증가함에 따라 BST 박막의 유전상수는 135에서 190로 증가한 반면, 누설전류 특성은 $1.9{\times}10^{-7}\; A/{\textrm}{cm}^2$에서 $1.7{\times}10^{-6}; A/{\textrm}{cm}^2$로 저하되었다. $O_2/Ar$ 비 증가에 따른 BST 박막의 결정성의 향상에도 불구하고 BST 박막의 표면거칠기의 증가와 BST/ $RuO_2$계면에서의 산소결핍 지역의 확장 등이 BST 박막의 누설전류 특성의 저하를 초래하였다.

극상근건 부분파열 환자에 매선과 한의치료를 시행한 증례 보고 (Effect of Needle-embedding Therapy on Supraspinatus Tendon Partial Tear Combined with Oriental Medical Treatment : Case Report)

  • The purpose of this study was to investigate the clinical effects of Korean medical treatments combined with Embedding acupuncture on patients with supraspinatus tendon partial tear. We reviewed the medical records of 1 patients with supraspinatus tendon partial tear at Pureunsan oriental medical clinic from Oct, 2017 to November, 2017. The patient received embedding acupuncture therapy and oriental medical therapy. To evaluate the efficacy of the treatments, the patient was asked to complete a Visual analogue scale(VAS) and the Shoulder pain and disability index during treatment period. The VAS of the patient decreased more than the screening period. The SPADI change of the embedding acupuncture group were greater than non treatment period. Embedding therapy combined with Korean medical treatment might be effective in reducing pain and improving the life quality of patients with supraspinatus tendon partial tear. We hope that further studies will be done to produce more clinical data and ensure effective application of these results.

TheraBand를 이용한 견관절 외전시 삼각근과 극상근의 근전도 변화 (A Changes of Electromyogram Activities in Deltoideus and Supraspinatus During Shoulder Abduction Using TheraBand)

  • The purpose of this study was to compare the changes of muscle activities in deltoid with those in supraspinatus using electromyography(EMG) while subjects abducted their shoulder to different angles with different resistance. Methods : 20 volunteers who were comprised of 10 males(mean age: $21.2{\pm}2.0$) and 10 females(mean age: $20.2{\pm}1.6$) were collected. Surface electrodes were attached on deltoid and supraspinatus muscle for recording. Each reference electrode was located 3 cm to the each recording electrode. Muscle action potentials were recorded with changing the angle of shoulder abduction, $30^{\circ}$, $60^{\circ}$, $90^{\circ}$. This procedure was repeated with different resistance 0 pound, 2 pounds, 4.5pounds. The angle of shoulder abduction was determined by clinical goniometer. SPSS(Statistical Program for Social Science)/WIN 10.0 was used for statistics. Analysis included ANOVA, T-test. Results : The following results were obtained in this study. 1. There was significant differences during isometric shoulder abduction $30^{\circ}$, $60^{\circ}$, $90^{\circ}$ comparing muscle activity in deltoid and supraspinatus muscles at 0 pound, 2pound, 4.5pound resistance(p<0.05). 2. There was no useful significant in the deltoid and supraspinatus muscles activity EMG compared by physical condition. 3. Their was significant in the deltoid EMG compared by gender t=-5.41, P<0.01, but no difference in the supraspinatus EMG(P=0.333 >a=0.05). Conclusions: There are many previous studies on influence of shoulder angles and speeds of muscle activity. However most of them placed the focus on isotonic or isokinetic exercise, or shoulder function. But this study was done during isometric exercise, which is better for early assessment and treatment for injured patients. There is strong relationship between the shoulder angle with resistance in deltoid supraspinatus muscle activity. We conclude that it is important to apply isometric shoulder abduction exercise with correct angle and resistance especially in early stage.

예인수조용 스테레오스코픽 입자영상유속계 시스템의 불확실성 해석 (Uncertainty Assessment of a Towed Underwater Stereoscopic PIV System)

  • Test uncertainty of a towed underwater Stereoscopic Particle Image Velocimetry (SPIV) system was assessed in a towing tank. To estimate the systematic error and random error of mean velocity and turbulence properties measurement, velocity field of uniform flow was measured. Total uncertainty of the axial component of mean velocity was 1.45% of the uniform flow speed and total uncertainty of turbulence properties was 3.03%. Besides, variation of particle displacement was applied to identify the change of error distribution. In results for variation of particle displacement, the error rapidly increases with particle movement under one pixel. In addition, a nominal wake of a model ship was measured and compared with existing experimental data by five-hole Pitot tubes, Pitot-static tube, and hot wire anemometer. For mean velocity, small local vortex was identified with high spatial resolution of SPIV, but has serious disagreement in local maxima of turbulence properties due to limited sampling rate.

  • MeOH-1,1,2,2-tetrachloroethane 혼합용매에서 할로겐화삼차부틸(t-BuCl, t-BuBr, t-BuI)의 가메탄올 분해반응을 속도론적으로 연구하였으며, 이온화에 미치는 용매효과를 고찰하기 위하여 6가지 indicater를 이용한 분광용매화비교법을 적용하였다. 이결과 할로겐화삼차부틸의 가메탄올 분해반응에 미치는 용매화의 작용은 용매의 극상-편극성에 기인되는 상호작용이 주된 것이긴 하지만 halide leaving group의 living ability에 미치는 electrophilic assistance와 t-Butyl 양이온쪽에 대한 nucleophilic assistance도 상당히 작용함을 알 수 있다. 특히 hydrogen bonding에 의한 electrophilic assistance는 basicity가 큰 leaving group일수록 커지며 ($I^-), 탄소 중심일수록 커짐을 밝힐 수 있었다. 주왕산국립공원 삼림군집구조분석 - 이전동 절터지역을 중심으로 - (The Analysis of the Forest Community Structure of Chuwangsan National Park - Case Study of Ijontong Cholto -)

  • 주왕산국립공원 이전동 절터지역을 중심으로 삼림군집구조 분석을 실시하기 위해 24개소에 조사구(각 l0$\times$10m)를 설치하고 식생조사를 실시하여 TWINSPAN과 ordination기법의 일종인 DCA를 적용하여 분석하였다. 전체 24개 조사구는 DCA에 의해 졸참나무-굴참나무군집 졸참나무-소나무군집, 신갈나무-생강나무군집, 까치 박달-당단풍군집, 그리고 고로쇠나무군집의 5개 군집으로 분리되었다. 삼림군집구조분석결과 추정되는 천이계열은 교목충에서 소나무$\longrightarrow$졸참나무, 굴참나무, 신갈나무$\longrightarrow$까치박달, 서어나무, 고로쇠나무로, 아교목층에서는 진달래, 철쭉$\longrightarrow$당단풍, 생강나무 순으로 예측되었다. 그러나 일부지역에서는 신갈나무가 토지극상으로 발전될 가능성을 보였다.
