시각적 되먹임 훈련이 편마비 환자의 균형능력에 미치는 영향 (The effect of visual feedback training on balance ability in patients with hemiparetic)

  • Purpose : The purpose of present study was to determine effects of a visual feedback training on balance ability in poststroke hemiparetic subjects. Methods : Fourteen chronic stroke patients participated. Participants were randomly divided into either visual feedback training group(7 experimental group, 7 control group). All of participated were inpatients at local rehabilitation center and had been receiving a traditional rehabilitation program, five days a week. Exprimental group have additionally undergone for four weeks, three days a week, the visual feedback training but control group was not received any additional program except the traditional rehabilitation program. The Berg Balance Scale(BBS), the FICSIT-4, the MTD-balance system to measure a balance ability were carried out twice before and after training. Results : After participation in the program, subjects of visual feedback training demonstrated a significant improvement in the scores of the BBS, the FICSIT-4, the MTD-Balance system. The control group had no change on the any tests. After the training, the result to improve significantly in visual feedback training group compared to post-test of control group were the score of BBS and the FICSIT-4 and the MTD-Balance system. Conclusions : The present study suggests that the visual feedback training program may become a useful strategy for enhancing balance ability in the rehabilitation of stroke patiens.

목표 지향 측방 보행 훈련이 만성 뇌졸중 환자의 균형 및 보행에 미치는 영향 (Effects of Goal-Oriented Side Walking Training on Balance and Gait in Chronic Stroke Patients)

  • Purpose: The aim of this study was to investigate the effect of goal-oriented side walking training on balance and gait in chronic stroke patients. Methods: This study involved 18 chronic stroke patients who were divided into an experimental group (n=9) and a control group (n=9). The experimental group performed goal-oriented side walking training, and the control group performed general side walking training. The walking variables assessed were walking speed, stride length, and 10-m walking time. The balance variables assessed were foot pressure, timed up and go test (TUG), and Berg balance scale (BBS). These variables were measured before and after the exercise. Wilcoxon's signed-rank test was used to compare the participants' performances before and after the intervention in both groups. The Mann-Whitney U test was conducted for between-group comparisons after the intervention. Statistical significance was set at ${\alpha}=0.05$ Results: Walking speed, stride length, 10-m walking time, TUG, and foot pressure were significantly improved in both groups after the exercise (p<0.05). The between-group comparison showed a significant improvement in the goal-oriented side walking group (p<0.05). However, there were no significant between-group differences in 10-m walking times (p>0.05) and BBS (p=0.05). Conclusion: The results revealed that goal-oriented side walking training was effective in improving the balancing and walking ability of chronic stroke patients.

불안정지지면 훈련과 평류전정자극이 균형 및 고유수용성감각에 미치는 영향 (The Effects of Balance and Proprioception by Unstable Surfaces Training and Galvanic Vestibular Stimulation)

  • Purpose : This study was conducted to examine the effects of unstable surface training and galvanic vestibular stimulation on balance and proprioception. Methods : 4-week training was applied to 40 normal adults in their twenties (unstable surface training group 10, galvanic vestibular stimulation group 10, unstable surface training and galvanic vestibular stimulation group 10, control group 10). Balance and proprioception was measured before the experiment, after two weeks, and after four weeks. Repeated ANOVA was used for balance and proprioception analysis. Results : As a result of repeated ANOVA on balance changes in double support, a significant difference was found in open sight in interaction among groups and periods (p<0.05). As a result of repeated ANOVA on balance changes in double support, there was a significant difference in closed sight and interaction among groups and periods (p<0.05). As a result of repeated ANOVA on balance changes in proprioception, there was no statistically significant difference in interaction among groups and periods. Conclusion : In conclusion, both unstable surface training and galvanic vestibular stimulation had positive effect on balance control and proprioception improvement, and their excellence was not greatly different. Thus, it is possible to use unstable surface training and galvanic vestibular stimulation for physical therapy intervention to improve balance and proprioception. Since the results showed that simultaneous application of two methods had greater effects on difference compared with a single application, it is suggested to use this intervention in the clinical field.

FES와 부분적인 체중지지를 결합한 지상보행훈련이 만성 뇌졸중 환자의 보행과 균형에 미치는 영향 (Effect of the Combined Use of FES and Over ground Walking with Partial Body-Weight Support on Walking and Balance Competency in Patients with Chronic Strokes)

  • Purpose : We investigated the effects of the combined use of FES and over ground walking with partial body-weight support (PBWS) on walking function and balance control in people with chronic strokes. Methods : Twenty-seven people who were ambulatory after chronic strokes were evaluated. The exercise's intervention consisted of the combined use of FES and over ground walking with PBWS and general exercise groups. The FES + PBWS group and general exercise group consisted on a-20-minute session per day, 3 times a week during a 4 week period. The evaluation was carried out before, after, and two weeks after the exercise intervention. Outcome measures were a 6 Minute Walk Test, 6-Meter walk Test, Timed Up and Go Test, and a Balance Test, measured before and after the exercise interventions at a-2 week follow up. Results : The endurance was significantly increased in both the FES+PBWS group and general exercise group (p<0.05). Significant increase on the gait velocity was observed in both the FES+PBWS group and general exercise group (p<0.05). The TUG was significantly different in both the FES + PBWS group and general exercise group (p<0.05). However there were no differences in both the between-group & interaction. The stability index was significantly different in both the FES + PBWS group and general exercise group (p<0.05). Conclusion : In conclusion, the combined use of FES and over ground walking with PBWS led to an improvement in walking function and balance control. Thus, it is possible to combine the use of FES and over ground walking with PBWS for physical therapy intervention to improve walking function and balance control. It is suggested to apply this intervention in the clinical field.

불안정한 지지면에서의 균형훈련이 발목관절 근활성도에 미치는 영향 (The Effect of Balance Training on Balance Ability and Ankle Joint Muscle Activity)

  • Purpose : This study investigates the effects on balanced ability and ankle joint muscle following balance pad and trampoline training periods in order to report a clinical directing about this matter. Methods : A total of nine members participated in a balance training program that put a balance pad and trampoline to use. Participants trained three times a week for thirty minutes in groups of six. Resting time was set for two minutes. EMG (median frequency and root mean square) was used to measure muscle activity at 3 weeks and 6 weeks, respectively. The tibialis anterior and gastrocnemius muscles were measured. Participants were measured with eyes open and closed. Results : The median frequency showed a significant difference according to the change in time in both muscles. Also, results showed a significant interaction between eyes open and closed in the gastrocnemius muscle. The root mean square only showed a significant difference according to the change in time in the gastrocnemius muscle. Conclusion : We suggest that the balance pad and trampoline training influence a decrease in the recruitment rate of slow twitch muscles through an improvement in balance ability; however, the training did not show a statistically significant difference with regards to eyes open and closed.

시지각 바이오피드백 훈련이 뇌졸중 환자의 균형 및 자세조절에 미치는 영향 (The Effect of Visual Bio-feedback Training on Balance and Postural Control in Stroke Patients)

  • Objectives : This study was to investigate the visual bio-feedback training for 5 weeks on balance and postural control for patients with stroke. Methods : The 26 subjects were randomly selected from the patients of the E hospital in the S city who met the study conditions. They were divided into a visual bio-feedback training group of 13 patients and a self-resistance exercise group of 13 patients. The visual bio-feedback training group received visual bio-feedback and general physiotherapy for five weeks and the self-resistance exercise group received cycling and general physiotherapy for the same period. The subjects were measured and compared for stability index, weight distribution index, fall down index, functional reach test and timed up and go test before and after the program. Results : The visual bio-feedback training group showed significant changes after the experiment in stability index, weight distribution index, functional reach test and timed up and go test(p<0.05), and the self-resistance exercise group also showed significant differences(p<0.05). The changes between prior to and after the experiment show that the visual bio-feedback training group had more significant effects than the self-resistance exercise group(p<0.05). Conclusions : The visual bio-feedback training for five weeks had effects in the improvement of the balance and posture control of stroke patients. Based on these results, more effective training programs should be developed and propagated.

이중과제 보행훈련이 파킨슨병 환자의 균형, 보행능력 및 일상생활동작에 미치는 효과 -단일사례연구- (The Effect of Dual-task Gait Training on Balance, Gait, and Activities of Daily Living for Patients with Parkinson's Disease -A Single-subject Experimental Design-)

  • Purpose: This study identified the effects of dual-task gait training on balance, gait function, and activity of daily living in patients with Parkinson's disease. Methods: This study used a single-subject design. Two patients with Parkinson's disease participated in this study. Dual-task gait training was performed 1 hour per day 8 times during intervention phase. The subjects were measured 8 times in the baseline phase, 8 times in the intervention phase, and 8 times in the follow-up phase. The outcome measurements included a timed up and go test (TUG), a Berg balance scale (BBS), a 10 meter walk test (10MWT), a 6 minute walk test (6MWT), a dynamic gait index (DGI) and a Korean modified Barthel index (K-MBI). Results: When compared to the average of the baseline process, the data collected during the intervention period showed that the TUG and 10MWT results improved and the tendency line was above the baseline. In addition, BBS, 6MWT, DGI, and K-MBI values for both patients increased remarkably after the training. Conclusion: The results of this study revealed that dual-task gait training may be helpful to improve balance, walking function, and activity of daily living for patients with Parkinson's disease. Further studies need to confirm our findings.

  • 기존의 코드북 설계방법의 많은 계산량과 초기 코드북 문제를 해결하기 위해 영상에 대한 DCT-기반 벡터양자화 코드북 설계방식이 제안되었다. 이 방법은 훈련벡터들의 DCT 계수들에 대한 분산을 각각 구하여 그 중 최대값을 가지는 계수를 분할키로 하고 그 평균값을 분할경계값으로 하여 균형 이진트리를 만들어 종단노드들을 최종코드북으로 하는 방법인데, 에지부분의 열화가 생기는 단점이 있다. 본 논문에서는 중복된 벡터가 많은 평탄영역 벡터들의 노드 분할을 억제하고 다양한 에지영역의 코드벡터들을 포함하는 코드북을 만들기 위해, 각 노드들의 분할키들을 비교하여 값이 가장 큰 노드부터 분할하는 불균형 이진트리를 이용한 벡터양자화 코드북 설계 방법을 제안하였다. 모의실험 결과 에지부분의 복원성과 PSNR 측면에서 기존의 방법에 비해 좋은 결과를 얻을 수 있었다.

앉고 일어서기 동작에서 점진적 체중 이동을 이용한 시각적 되먹임 훈련이 만성 편마비 환자의 균형 및 보행능력에 미치는 영향 (Effect of Visual Feedback Training for Gradual Weight Shift in the Sit-to-stand Training on the Balance and Walking Abilities of Chronic Hemiplegia Patients)

  • Purpose: This study aimed to investigate the effect of visual feedback training-for gradual weight shift in sit-to-stand training-on the balance and walking abilities of chronic hemiplegia patients. Methods: Twenty patients with chronic hemiplegia volunteered to participate in this study. The experimental group received visual feedback for gradual weight shift in the sit-to-stand training, while the contrast group followed the standard process for the sit-to-stand training. The evaluation of the balance and walking ability was conducted with the functional reach test (FRT), Berg balance scale (BBS), five time sit-to-stand (FTSTS) test, timed up and go (TUG) test, 10 m walk test (10MWT), balancia, activities-specific balance confidence (ABC) scale, and falls efficacy scale (FES). Results: In the results before and after intervention, there was a significant difference in TUG, 10MWT, ABC, and FES in the visual feedback training group (p < 0.05). In the control group, there was a significant difference in the 10MWT and ABC (p < 0.05). Also, in the evaluation of the postural fluctuations, the control group data showed a significant increase in Covar. The visual feedback group showed a significant difference in the W average. Conclusion: The visual feedback training group showed some improvement in terms balance and walking ability and on the ABC scale and FES. Therefore, if the diagonal progressive weight bearing exercise is combined with the various patterns and basic principles of PNF, it may be a more efficient intervention method.

고유수용성신경근촉진법을 이용한 저항보행훈련이 만성뇌졸중환자의 보행과 균형능력에 미치는 영향 (The Effects of Resistant Gait Training with Proprioceptive Neuromuscular Facilitation on the Walking and Balancing Abilities of Chronic Stroke Patients)

  • Purpose: The purpose of this study was to examine the effects of resistant gait training with proprioceptive neuromuscular facilitation (PNF) on the walking and balancing abilities of chronic stroke patients. Methods: Twelve chronic stroke patients were randomly assigned to either a control group (n = 6) that underwent treadmill gait training or an experimental group (n = 6) that underwent resistant gait training using PNF. The interventions were performed five times per week for four weeks. Gait variables were measured using a GAITRite system (CIR System Inc, Clifton, NJ, USA) to examine changes in walking ability; the Berg balance scale (BBS) was used to measure changes in balance; and the activity-specific balance confidence scale (ABC) was measured to examine changes in confidence about balance. A Wilcoxon signed-rank test was used to examine intragroup differences before and after the interventions, and a Mann-Whitney U-test was used for intergroup comparisons of the effects of the interventions. All statistical significance levels were set to ${\alpha}=0.05$. Results: Both the experimental group and the control group showed significant intragroup improvement in walking speed, the number of steps per minute, stride length, double support time, balance, and confidence about balance after the interventions (p < 0.05). In intergroup comparisons after the interventions, the experimental group showed significant improvements over the control group in walking speed, the number of steps per minute, stride length, balance, and confidence about balance (p < 0.05). No significant difference in double support time was seen in the intergroup comparison after training (p > 0.05). Conclusion: This study applied resistant gait training using PNF to chronic stroke patients, and the results showed significant improvements in the patients' walking and balancing abilities. Therefore, resistant gait training using PNF is thought to be applicable as an intervention method for chronic stroke patients.
