Interaction between Dietary Factors and Gut Microbiota in Ulcerative Colitis (궤양성 대장염에서 식이 인자와 장 마이크로비오타의 상호작용)

  • Ulcerative colitis (UC) exhibits chronic intestinal inflammatory conditions with cycles of relapse and remission. The incidence is rapidly growing in Asian countries including South Korea possibly due to changes in lifestyles. Although the etiology of inflammatory bowel disease is inconclusive, gut microbiota composition is considered a critical factor involved in the pathogenesis of UC. The overgrowth of pathogenic bacteria evokes hyper-immune responses in gut epithelium causing tissue inflammation and damage. Also, failure to regulate gut epithelium integrity due to chronic inflammation and mucus depletion accelerates bacterial translocation aggravating immune dysregulation. Gut microbiota composition responds to the diet in a very rapid manner. Epidemiological studies have indicated that the risk of UC is associated with low plant foods/high animal foods consumption. Several bacterial strains consistently found depleted in UC patients use plant food-originated dietary fiber producing short chain fatty acids to maintain epithelial integrity. These bacteria also use mucus layer mucin to keep gut microbiota diversity. These studies partly explain the association between dietary modification of gut microbiota in UC development. Further human intervention trials are required to allow the use of specific bacterial strains in the management of UC.

  • The number of bedsores patients is increasing every year due to the aging society, and the importance of bedsores management is emerging as a burden on nurses due to the pandemic situation of COVID-19. Curses are a disease that causes ulcers due to floating posture, and it is a disease that is burdensome for nurses to periodically change their body position. Therefore, this study proposes a system that can reduce the burden on nurses by detecting the risk of inducing bedsores and monitoring the status of calls in real time. Through this system, it will be possible to increase nursing work by solving the difficulties of nursing in the bedsores of nurses.

  • Many modern people are exposed to chronic inflammatory diseases, such as inflammatory bowel disease (IBD), atopic dermatitis and immune disorder. Among those chronic diseases, the incidence ratio of IBD has been increased. IBD, including Crohn's disease and ulcerative colitis (UC), is known to cause abnormal inflammation in intestinal tissue. UC is accompanied by abdominal pain, bloody stool and diarrhea. Many therapeutic agents, such as sulfasalazine, corticosteroids, immunosuppressive agents, have been used for treating UC. However, those agents have side-effects and temporary effects on UC. The aim of this study was to investigate the effect of herbal medicine on UC and relationship between UC and intestinal bacteria according characteristic of herbal medicine. To determine the effect of herbal medicine on UC, various herbal medicine were chosen within oriental medicine category such as cheongyeol and onyeol medicine. In this study, we found carthami fructus, included in cheongyeol medicine, had stronger effect than onyeol medicine. Also, we determined influence of carthami fructus against lactic acid bacteria. Catthami fructus and lingon berry extracts affected the composition of mice intestinal bacteria in mice fecal. The symptoms of UC could be regulate by using herbal medicine, according to characteristic of herbal medicine. Also, herbal medicine might be change body condition to healthy by controlling intestinal bacteria composition. Herbal medicine characteristic could be a therapeutic agent by revealing relationship between intestinal bacteria and UC.

Effects of Acupuncture on Symptoms in a Patient with Interstitial Cystitis (간질성 방광염에 대한 침치료 효과)

  • Objective : A highly effective treatment for interstitial cystitis (IC) remains elusive. We determined whether acupuncture might be effective in relieving symptoms of IC. Method : A consecutive patient(F/67yrs) with symptoms and cystoscopic findings compatible with IC underwent one month of continuous acupuncture by way of needles inserted into acupuncture points which are known to be effective for the symptoms of cystitis. Patient filled out voiding frequency, pain and urgency score charts during treatment. Results : Acupuncture significantly improved all measured parameters toward normal values. Voiding frequency decreased twofold from 41 to 18 voids daily the end of treatment. Pelvic pain on a scale of 1 to 10 decreased from 10 to 2.8. Urinary urgency on a scale of I to 10 decreased from 10 to 5.2. Conclusions : Acupuncture significantly decreased symptoms in a patient with IC. These results suggest that acupuncture may be beneficial in treating IC.

The Effects of Soyumjungjang-tang on DSS-Induced Ulcerative Colitis in Mouse (소염정장탕(消炎整腸湯)이 DSS로 유발(誘發)된 생쥐의 궤양성 대장염에 미치는 영향(影響))

  • Objectives : This study was carried out to investigate the effects of Soyumjungjang-tang(SJT) on the experimental ulcerative colitis induced by dextran sulfate sodium(DSS) in mice. Methods : Ulcerative colitis was induced through supplying 4% DSS solution as the drinking water for 7 days in 6-week-old male ICR mice. The colitic mice were divided into three groups: the sample groups were orally administered SJT in doses of 25mg/kg(S25 group) or 100mg/kg(S100 group) once a day for 10 days, from 3 days before starting drinking the DSS solution, and the control(C) group was administered normal saline instead of SJT. The DSS solution or SJT was not administered to the normal(N) group. The length of colon, histologic finding, the activities of myeloperoxidase(MPO) and alkaline phosphatase(AP), and the expressions of $IL-1{\beta}$, IL-6, COX-2, $NF-{\kappa}B$, and $I{\kappa}B$ in colonic mucosa was checked using immunoblot, ELISA, etc. The activities of chondroitinase, tryptophanase, ${\beta}-glucuronidase$ and ${\beta}-glucosidase$ in stool were also measured. Results : The length of colon shortened, histologic finding deteriorated, the activities of MPO, AP, chondroitinase, tryptophanase, ${\beta}-glucuronidase$ and ${\beta}-glucosidase$, and the expressions of $IL-1{\beta}$, IL-6, COX-2, $NF-{\kappa}B$ increased, and the expression of $I{\kappa}B$ decreased in the C group. All measures, except $NF-{\kappa}B$, were restored in S25 group, but some measures deteriorated more in the S100 group than in the C group. Conclusions : According to the above results, it is supposed that SJT has a potential therapeutic effect on ulcerative colitis.

  • Objective : this study is designed to evaluate the effects of an oriental medicine therapy, namely gagam-sukhongjeon, on ulcerative colitis Methods : The Clinical data was analyzed on a patient with ulcerative colitis due to hanyeolchakjab(寒熱錯雜), whose symptoms were combined chillness and fever. The patient visited at the internal medicine department of Dong-Shin University Suncheon Oriental Hospital on February 25, 2006, and go into hospital from February 25, 2006 to March 9, 2006. and revisited from March 18, 2006 to April 5, 2006. The patient was treated with Herbal medicine(gagam-sukhongjeon) Result : After treatment, bloody stool and abdominal pain disappeared in visual analogue scale(VAS), pain disability index(PDI) and verbal rating scale(VRS). Conclusions : This study suggests that gagam-Sukhongjeon is significantly effective in treatment of ulcerative colitis.

The Effects of Coicis Semen Extract (CSE) on Dextran Sulfate Sodium - Induced Colitis in Mice (의이인(薏苡仁) 추출물이 DSS(dextran sulfate sodium)로 유발된 생쥐의 궤양성 대장염에 미치는 영향)

  • Objectives : This study was carried out to investigate the effects of Coicis Semen Extract (CSE) on the experimental colitis induced by dextran sulfate sodium (DSS) in mice. Methods : Experimental colitis was induced by daily treatment with 5% DSS in the drinking water for 7 days in 6-week-old male ICR mice. The colitic mice were divided into three groups: the normal (N) group consisted of mice that were not inflammation-induced. The control (C) group was composed of untreated colitis elicited mice. The sample (S) group was administered CSE after colitis elicitation. The effects on colonic mucosal ulcers were evaluated by the morphological, histological and immunohistochemical change of the large intestine. Results : Inhibition of LPS-induced NO decreased in the S group. Inhibition of LPS-induced iNOS and COX-2 mRNA noticeably decreased in the S group from 0.25 mg/ml. In the common morphological and histochemical change, the erosion and the infiltration of inflammatory cells increased in the C group, while they noticeably decreased in the S group. The length of colon was shortened more in the C group than in the S group. The distributions of MUC2 and Hsp70 treated with CSE increased noticeably more in the S group than in the C group (p<0.05). It was confirmed histochemically and immunohistochemically that the distributions of iNOS, COX-2, MAC387, serotonin, apoptosis and PCNA treated with CSE decreased in the S group more than in the C group (p<0.05). Conclusions : It is confirmed that CSE has cytoprotective effect, so can alleviate inflammation process. Therefore, it is expected to have potential protective effect on colitis.

The Anti-Inflammatory Effect of Lonicera Japonica-Glycyrrhiza Uralensis Decoction on Ulcerative Colitis Induced by DSS in Mice (항염증조절을 통한 금은화-감초 복합 추출물의 DSS 유도 궤양성 대장염 완화 효과)

  • Objectives The purpose of this study is to investigate the anti-inflammatory effect of Lonicera Japonica-Glycyrrhiza Uralensis decoction extracts (LGE) on ulcerative colitis in children and adolescents. Methods Colitis was induced by DSS (Dextran Sulfate Sodium) in C57BL/6 mice. The sample mice were divided into group of four. The mice in the control group were not inflammation-induced. The control group was composed of untreated ulcerative colitis elicited mice. The mice in the experimental group were administered with Pentasa and another experimental group mice were treated with LGE after colitis elicitation. The effects on ulcerative colitis were evaluated by the morphological changes of colonic mucosa, decrease in the effect of pro-inflammatory cytokines ($TNF-{\alpha}$ and $NF-{\kappa}B$) and inflammatory cytokines (iNOS and COX-2) in the mucosa. Results LGE showed protective effects in DSS induced ulcerative colitis. LGE inhibited shortening of colon length and relieved the hemorrhagic erosion in colonic mucosa. LGE decreased pro-inflammatory cytokines ($TNF-{\alpha}$ and $NF-{\kappa}B$) and inflammatory cytokines (iNOS and COX-2). According to the GC/MS analysis, N-methyl pyrrolidone (NMP) was identified. Conclusions The result shows the clinical efficacy of LGE and demonstrates possible treatment options for ulcerative colitis. Further investigations for biological activity and chemical analysis of LGE will be needed.

A study on Pain Express Pattern of Peptic Ulcer Patient (소화성궤양환자의 통증표현양상에 관한 연구)

  • Pain is a subjective and multidemensional concept. Therefore the patient's expression of pain have been referedl to the best believable indicator of pain condition but the support data obtained from the patient considered cultural difference is a deficient condition in determinded on the precise nursing diagnosis. The purpose of this reasearch was to understand multiple pain reponses in cultural difference and sensitivity, to encourage communication between medical teams, and to provide the foundation data of on data of precise nursing assessment for the patient in pain. The research problem was to grasp pain express pattern of Korean peptic ulcer patients. The subjects were 20 peptic ulcer patients in medical unit or OPD of twp university hospitals in Seoul. Data were collected from September 7th to 22nd, 1990 by intensive interviews. Interviews were done by the researcher and all were tape - recorded. The Data analysis was done by Phenomenological method from Van Kaam. Validity assured by confirmation of the internal consistency of the statements and catigory by nursing collegue in educational and clinicions in medical care. From the emic data, 96 descriptive statements were organized in 18 theme cluster. The results of study were summerized as follows. 1. Pain Express Pattern cluster of Peptic Ulcer Patients were “Pain as clogging”, “shallow pain”, “pain as pressing”, “nauseating pain”, “pain as smarting”, “pain as pulling”, “pain as pricking”, “pain as bursting”, “wrenching pain”, “excising pain”, “uncontrollable pain for mind and body”, “awakening pain”, “pain as hollowing” and the other cluster. As above mentioned, Pain Express Pattern of Peptic Ulcer Patient appeared diversely in verbal and they were propered to Korean culture. Therefore they will provide for the foundation data of precise nursing assessment.
