The Role of Colonoscopy in Children with Hematochezia (소아 선혈변에서 대장 내시경 검사의 역할)

  • Purpose: This study was performed to evaluate the role of colonoscopy in children with hematochezia. Methods: We retrospectively reviewed the medical records of 277 children who underwent colonoscopy because of hematochezia between January, 2003 and July, 2010. Results: The mean age of the patients was $6.0{\pm}4.4$ (7 days~17.8 years) years. The male to female ratio was 2.2:1. The duration between the 1st episode of hematochezia and colonoscopy was $4.9{\pm}12.1$ months. Characteristics of hematochezia included red stool (65.1%), blood on wipe (12.8%), bloody toilet (11.9%), and blood dripping (10.2%). The most proximal region of colonoscopic approach was terminal ileum (84.5%), cecum (9.5%), hepatic flexure (2.8%), and splenic flexure (3.2%). Eighty five patients (30.6%) had no specific abnormal findings. Major causes of hematochezia were polyp (26.4%), food protein induced proctocolitis (6.9%), infectious colitis (5.4%), lymphofolliculitis (5.7%), non specific colitis (5.7%), and vascular ectasia (5.1%). The hemorrhagic sites included the rectum (24.0%), rectosigmoid junction (18.1%), sigmoid colon (13.5%), ascending colon (14.2%), transverse colon (11.3%), descending colon (7.8%), cecum (8.1%), and terminal ileum (3.1%). The recurrence rate of hematochezia after colonoscopy was 19.1%. Colonoscopy was performed in 262 patients (94.6%) with conscious sedation. Endoscopic hemostasis was performed in 5 patients. Complications of colonoscopy or sedation were not found. Conclusion: The causes and lesional localization of pediatric hematochezia were diverse. Colonoscopy has an important role in the diagnosis and treatment of hematochezia in children. Total colonoscopy is recommended to detect the cause of hematochezia.

Factors related to Quality of Life of Patients with Ulcerative Colitis (궤양성 대장염 환자의 삶의 질 영향요인)

  • Purpose: The purpose of this study was to identify factors related to the quality of life of patients with ulcerative colitis. Methods: A total 116 patients with ulcerative colitis were asked on general characteristics, illness-related characteristics, depression, and quality of life. The collected data were analyzed by t-test, ANOVA, and stepwise multiple regression. Results: As for depression the 81.03% were normal, the 8.62% were mild depression, the 6.90% were moderated depression, and the 3.45% were severe. As for quality of life, the social functions was highest, followed by intestine-related symptoms, systemic symptoms, and emotional functions. Quality of life was positively correlated to subjective health status, age, and body mass index, and negatively to depression. Main factors affecting the quality of life included depression, subjective health status, physician's global assessment, age, days of loss in social life, and present abdominal pain. Conclusion: Physical and psychosocial health problems related to ulcerative colitis affected the quality of life of the patients. Further research is warranted for developing educational programs and psychosocial strategies in order to efficiently handle the illness.

Vision transformers for endoscopic pathological findings classification (내시경 병리소견 분류를 위한 비전 트랜스포머)

  • The endoscopic pathological findings of gastrointestinal tract (GIT) are important in the early diagnosis of colorectal cancer. Deep learning based on convolutional nueral network (CNN) has been implemented to solve the subjective analysis problem and to increase the performance of early detection of pathological findings. However, the desired performance is yet to be achieved and CNNs are computationally complex. To solve these problems, in this paper, we propose a vision transformer based endoscopic pathological findings classification for the early detection of colorectal cancer. Publicly available endoscopic images with three pathological findings, including esophagitis, polyps, and ulcerative colitis, each with 1000 images were used. Using our approach, we have achieved a test accuracy of 98% in classifying the three pathological findings.

Inhibitory Effects of Ginger and Beopje Ginger on DSS-induced Colitis in Mice (생강과 법제생강의 DSS(Dextran Sulfate Sodium)로 유도된 마우스의 대장염 억제 효과)

  • The purpose of this study was to examine the anti-inflammatory effects of ginger and processed (Beopje) ginger on colitis induced by 2.5% dextran sulfate sodium (DSS) in Balb/c mice. Beopje means a process that herbal medicines are treated by a specific Korean traditional method in order to obtain better pharmacological effects. Mice were fed saline or two different doses of ethanol extracts (ginger and processed (Beopje) ginger) once a day for 14 days. Colitis was induced from day 7 to 14 via administration of 2.5% DSS in drinking water. Experimental animals were divided into four groups: Nor (Normal, 200 ${\mu}L$ of saline without 2.5% DSS-treated group), Con (Control, 200 ${\mu}L$ of saline and 2.5% DSS treated group), G (500 mg/kg of ginger and 2.5% DSS treated group), and BG (500 mg/kg of Beopje ginger and 2.5% DSS treated group). Body weights of both ginger-administered groups increased compared to the control. Colon length increased to 7.6, and 8.0 cm in the G and BG groups, respectively, whereas that of control was 5.7 cm. Histological colon injury induced by DSS-induced colitis was reduced (P<0.05). In serum and DSS-treated colon tissues, mRNA expression levels of IFN-${\gamma}$, IL-6, TNF-${\alpha}$, and IL-12 of the Beopje ginger-treated group were significantly suppressed compared to those of the ginger-treated groups. Expression levels of iNOS and COX-2 of the Beopje ginger-treated group were significantly reduced compared to those of the ginger-treated groups (P<0.05), and BG showed stronger anti-inflammatory effects on colitis. These results indicated that ginger exerted anti-inflammatory effects on DSS-induced colitis in mice, and its effects could be increased through Beopje.

Inhibitory Effects of Sulfur-Fed Duck Extracts on DSS-induced Colitis in Mouse (DSS(Dextran Sulfate Sodium)로 유도된 마우스의 대장염에 대한 유황오리 추출물의 항염증 효과)

  • Anti-inflammatory effects of sulfur-fed duck extract on colitis induced by 2.5% dextran sulfate sodium (DSS) were examined in male Balb/c mice. Animals were divided into eight groups: normal (0.1 mL of PBS without 2.5% DSS), control (0.1 mL of PBS with 2.5% DSS), SD-H (3 mL/kg of high sulfur-fed duck extract), SD-L (1 mL/kg of low sulfur-fed duck extract), GD-H (3 mL/kg of high general duck extract), GD-L (1 mL/kg of low general duck extract), GC-H (3 mL/kg of high general chicken extract), and GC-L (1 mL/kg of low general chicken extract). Mice were fed PBS or six different doses of extracts (sulfur-fed duck, general duck, and chicken), once daily for 14 days. Colitis was induced from day 7 to 14 via the administration of 2.5% DSS in drinking water. The colon length was significantly shortened in mice compared to the control group. The administration of SD-H, SD-L, and GD-L increased colon length and decreased histological colon injury from DSS-induced colitis. However, chicken extracts did not recover any clinical sign of the colitis. SD-L significantly suppressed not only the concentrations of IL-$1{\beta}$, IL-6, TNF-${\alpha}$, IL-17A, and IL-12 in serum but also the mRNA expressions of IL-6, TNF-${\alpha}$, iNOS and COX-2 in DSS-treated colon tissues (P<0.05). The administration of SD-H suppressed the concentrations of IL-6 in serum and the mRNA expressions of IL-6, iNOS, and COX-2 in colon tissues. Administration of GD-L suppressed the concentrations of IL-6, TNF-${\alpha}$, and IL-17A in serum and the mRNA expressions of IL-6, iNOS, and COX-2 in colon tissues. The inhibitory effects of sulfur-fed duck extracts were effective at a dose of 1 mL/kg. Our results indicate that sulfur-fed duck extracts may possess anti-inflammatory effects on DSS-induced colitis mice.

Ulcerative Colitis Mimicking Acute Hemorrhagic Colitis (급성 출혈성 대장염으로 오인된 궤양성 대장염 1예)

  • Ulcerative colitis (UC) is a chronic inflammatory disorder of the gastrointestinal tract that affects the large bowel. Its etiology remains controversial. However, an infectious or immunologic origin is considered the primary cause. The onset of UC is typically slow and insidious, but some patients may present acutely with symptoms mimicking infectious colitis. We report a case of ulcerative colitis mimicking acute hemorrhagic colitis at initial presentation. A 60-year-old man was referred to Yeungnam University Hospital for bloody diarrhea and abdominal pain. Sigmoidoscopy revealed mildly edematous mucosa in the rectum and hyperemic mucosa with petechiae in the sigmoid colon. The patient was treated with antibiotics for several days, and his symptoms improved. However, after one month, his bloody diarrhea relapsed. Follow-up sigmoidoscopy revealed mucosal friability in the rectum and sigmoid colon. He was diagnosed with ulcerative colitis, and his symptoms were improved with mesalazine and a steroid enema.

Study of The Correlation of Lung-Large intestine-Skin by Ulcerative Colitis-Induced 3 Weeks Old Mice (3주된 생쥐에서 궤양성 대장염 유발을 통한 폐-대장-피부의 상관관계 연구)

  • Objectives The purpose of this study is to understand the correlations between lung, large intestine, and skin of 3-week-old mice in which ulcerative colitis was induced, up on administration of Coptidis rhizome and Glycyrrhiza uralensis mixed extract. Methods Mice were divided into 4 groups as follows; no treatment group (Ctrl group), ulcerative colitis-induced mice group (UE group), ulcerative colitis-induced mice group after administering Pentasa (PT group), ulcerative colitis-induced mice group after administering Coptidis rhizoma and Glycyrrhiza uralensis mixed extract (CGT group). Mice were induced ulcerative colitis by Dextran sulfate sodium (DSS). After 5 days of administration, We obvserved anti-inflammatory effect, alveolar formation, and skin barrier control in the colon mucosa. Results The CGT group was observed arrangement of normal intestinal cells, Infiltration of less inflammatory cells. The CGT significantly decreased positive rseponse of $TNF-{\alpha}$, p-IkB, Caspase 3 in large intestine, and significantly increased positive rseponse of EGF, IGF, catalase, Filaggrin, involucrin, loricrin. Conclusions The results of this study show the correlation of Lung-Large intestine-Skin by administering Coptidis rhizoma and Glycyrrhiza uralensis mixed extract to ulcerative colitis-induced mice.

A Case Report of a Patient with Active Ulcerative Colitis Treated with Do-Che Decoction-based Korean Medicine (도체탕(導滯湯) 위주의 한방치료로 호전된 활동성 궤양성 대장염 환자 치험 1례)

  • Background : Sulfasalazine for treating ulcerative colitis has a clinical limitation due to its adverse effects. This case is about a patient with ulcerative colitis who could not tolerate sulfasalazine and was improved by Do-che decoction-based Korean medicine. Method and Results : Purulent and bloody diarrhea more than 20 times a day, mild fever, chilling and tenesmus were main symptoms of this patient who was diagnosed as ulcerative colitis 10 years ago. Do-che decoction-based Korean medicine which was recommended for purulent and bloody diarrhea in Dong-Ui-Bo-Gam, the classic literature of Traditional Korean Medicine was given to the patient for more than 1 month. After treatment, the frequency of purulent and bloody diarrhea, tenesmus and fever was decreased remarkably, and C-reactive protein, erythrocyte sedimentation rate, white blood cell count and maximum temperature also dropped to the normal range. Conclusion : Do-che decoction-based Korean medicine has a potential effect and may be a alternative therapeutic option in patients with ulcerative colitis who cannot tolerate sulfasalazine due to its adverse effects.

Anti-Inflammatory Effects of Glycyrrhiza glabra Linne Extract in a Dextran Sulfate Sodium-Induced Colitis Mouse Model (감초 추출물의 Dextran Sulfate Sodium 유도 마우스 궤양성 대장염 억제 효과)

  • The aim of this study was to evaluate the anti-inlfammatory effects of Glycyrrhiza glabra Linne extract on ulcerative colitis induced by 3% dextran sulfate sodium in mice. The experimental animals were divided into six groups: control(normal), DSS-induced colitis(control), 1 mg/kg, 10 mg/kg, and 100 mg/kg of Glycyrrhiza glabra Linne extract, and 150 mg/kg 5-aminosalicylic acid(5-ASA)(positive control). We evaluated the pathological disease activity index(DAI), change in weight, colon mucosa damage and myeloperoxidase(MPO) in colon mucosa. Treatment with 10 mg/kg and 100 mg/kg of Glycyrrhiza glabra Linne extract led to significant loss of body weight, the decrease of MPO activity and clinical symptoms such as DAI and histological change. In particular, 100 mg/kg Glycyrrhiza glabra Linne extract led to markedly greater improvement than 150 mg/kg 5-ASA treatment. These results suggest that Glycyrrhiza glabra mediated anti-inflammatory action on colorectal sites may be a useful therapeutic approach to ulcerative colitis.

Effects of Goihwa-san on Ulcerative Colitis Induced by Dextran Sulfate Sodium in Mice (괴화산(槐花散)이 Dextran Sulfate Sodium으로 유도된 생쥐의 궤양성 대장염에 미치는 영향)

  • Objectives : This study was carried out to investigate the effects of Goihwa-san(GHS) against ulcerative colitis induced by DSS (dextran sulfate sodium). Method : The sample group was divided into three. The control group consisted of mice that were not inflammation-induced. The pathological group was composed of untreated colitis elicited mice. The experimental group was administered GHS after colitis elicitation. The effects on ulcerative colitis were evaluated by the morphological change of colonic mucosa, the anti-oxidant effect, HSP 70, NF-${\kappa}$B, COX-1, COX-2 and iNOS. Results : In terms of immunohistochemical changes, the distribution of COX-1 in mice treated with GHS increased noticeably more than that in the pathological group. The distributions of HSP70, NF-${\kappa}$B, COX-2, iNOS in mice treated with GHS more decreased than those in the pathological group. Regeneration of surface epithelial cell and goblet cell in mucosa was observed by optical microscope. The colonic lengths in GHS-treated mice were more elongated than those of DSS only treated mice. Conclusion : GHS is a candidate treatment for ulcerative colitis.
