A Study on the Characteristics of Vascular Plants in the Stone Retaining Wall of Gudeuljang Paddy Fields in Cheongsan-Island (청산도 구들장논 석축(石築)의 관속식물 특성 연구)

  • This study was carried out as basic data for the conservation and management of plant diversity in the Gudeuljang paddy fields stone retaining wall of Cheongsan-island by identifying the characteristics of vascular plants targeting Gudeuljang paddy fields stone retaining wall in Cheongsan-island. The total vascular plants of the Gudeuljang paddy fields stone retaining wall in Cheongsan-island have been identified with a total of 184 taxa of 60 families, 142 genera, 170 species, 5 subspecies, 9 varieties. Rare plants were 1 taxa, Korea endemic plants were 2 taxa, floristic regional indicator plants were 28 taxa, rock vegetation was 20 taxa, and invasive alien plants were 25 taxa. Among them, Selliguea hastata (Thunb.) Fraser-Jenk., a rare plants, has been identified as a small number of individuals in the Gudeuljang paddy fields stone retaining wall in Buheung-ri and Cheonggye-ri, but it is necessary to conservation the habitat because there is a concern that the species and population will decrease due to damage and neglect of the Gudeuljang paddy fields stone retaining wall.

The Usefulness of Scintigraphy for the Detection of Gastroesophageal Reflux and Pulmonary Aspiration (위식도 역류와 폐 흡인 진단 방법으로서 위식도 역류 신티그래피의 유용성)

  • Purpose: Chronic pulmonary disease may be caused by aspiration of gastric contents secondary to gastroesophageal reflux. At present, there is no gold standard for documenting pulmonary aspiration. The purpose of this study was to investigate the usefulness of radionuclide scintigraphy in the detection of gastroesophageal reflux and pulmonary aspiration. Methods: Thirty-five patients with suspected aspiration pneumonia, and five normal control subjects, were included in the study. All subjects underwent gastroesophageal reflux scintigraphy after the ingestion of a $^{99m}Tc$-tin colloid mixture. Dynamic images to detect gastroesophageal reflux were obtained for 1 hour. Additional static images of the chest, to detect lung aspiration, were obtained at 6 and 24 hours after oral ingestion of the tin colloid. In addition to visual analysis, pulmonary aspiration was quantitated by counting the number of pixels labeled with radioactive isotope in the region of interest (ROI) of both lung fields. Aspiration index (AI) was obtained by subtracting the pixel counts of the background from the pixel counts of the ROI. Results: Among 35 patients with suspected aspiration pneumonia, 23 proved to have gastroesophageal reflux by scintigraphy. One patient showed definite pulmonary accumulation of activity by visual analysis of the 6-hour image. Thirty of 35 (85.7%) patients showed higher AI beyond the upper limit of AI in the healthy controls. When we compared the reflux group with the non-reflux group, there was a significantly higher AI at 6 hours in the reflux group (p<0.05). Conclusion: The results suggest that radionuclide scintigraphy is useful in detecting small pulmonary aspiration in patients with suspected aspiration pneumonia secondary to reflux.

In Vitro Radiosensitization of Flavopiridol Did Not Translated into In Vivo Radiosensitization (마우스를 이용한 생체내 실험에서의 플라보피리돌의 방사선민감화 효과)

  • Purpose: Flavopiridol enhanced radiation-induced apoptosis of cancer cells in our previous in vitro study. The purpose of this study was to assess if flavopiridol could enhance the radioresponse of mouse mammary tumors in vivo. Materials and Methods: Balb/c mice bearing EMT-6 murine mammary carcinoma were treated with flavopiridol only, radiation only, or both for 7 days. Flavopiridol was administered 2.5 mg/kg twice a day intraperitoneally (IP). Radiation was delivered at a 4 Gy/fraction at 24-h intervals for a total dose of 28 Gy. Tumor volume was measured and compared among the different treatment groups to evaluate the in vivo radiosensitizing effect of flavopiridol. Tumors were removed from the mice 20 days after treatment, and TUNEL and Immunohistochemical stainings were performed. Results: Significant tumor growth delay was observed in the radiation only and combined treatment groups, when compared with the control group. However, there was no significant difference between the tumor growth curves of the control and flavopiridol only group or between the radiation only and combination treatment group. Apoptotic cells of different treatment groups were detected by terminal deoxynucleotidyl transferase-medicated nick end labeling (TUNEL) staining. The expressions of Ku70 in tumor tissues from the different groups were analyzed by immunohistochemistry. Similarly, no significant difference was found between the apoptotic rate or Ku70 expression among the different treatment groups. Conclusion: Flavopiridol did not show evidence of enhancing the radioresponse of mouse mammary tumors in this study.

Delayed Diagnosis of Primary Lung Cancer (원발성 폐암의 지연진단에 관한 임상적 고찰)

  • Background : Lung cancer is an important public health problem because of rapidly increasing malignancy in both sexes in relation with high smoking rate in Korea. Despite advances in therapeutic modalities and supportive cares, 5-year survival rate has improved only marginally during the past 2 decades. Therefore, the early detection of lung cancer is strongly needed for better prognosis and we conducted this study to review the clinical factors resulting in delayed diagnosis of lung cancer. Method: The clinical data such as presenting symptoms, duration for diagnosis, disease entities causing misdiagnosis or delayed diagnosis, were analyzed retrospectively in 154 patients with primary lung cancer diagnosed at Chungnam National university hospital from January to December in 1995. Results : 63 patients(40.9%) out of 154 patients were delayed diagnosed with the duration of 6.3 months compared with 3.6 months in patients diagnosed without delay. In delayed diagnosed group, Cough & sputum and dyspnea as presenting symptom were more critical than hemoptysis and chest pain, and doctor's delay was more critical than patient's delay. Tuberculosis(30/63) was most frequent disease entity causing delayed diagnosis, followed by pneumorua(9/63), COPD(6/63), heart diseases(5/63), etc.. Conclusion: It should be emphasized that any respiratory symptom be checked with chest X-ray to differentiate lung cancer and periodic check of chest X-ray be also needed for the longterm patients with any disease, especially with high risk group.

고령 위암 환자에서 술 후 처치 및 영양지원

  • 1) 병원에 입원하는 환자들 중 상당수에서 어느정도의 여양실조가 있는 것으로 보고되므로 수술하기 위해 입원하는 환자에서 영양과 관계있는 측면에서 병력청취 및 이학적 검사상 약 6개월간 평소체중의 $10\%$ 이상 감소, 혈장단백(알부민,트란스페린) 저차, 총임파 구수감소 등이 있으면 철저한 영양평가를 시행하여 중등도 이상의 위험군에서는 술 전 약 $7\∼10$일간 영양지원을 하는 것이 약 $10\%$의 술 후 합병증을 줄이는 것으로 보고되나 이는 수술의 스트레스 정도, 술 후 영양섭취 지연의 기간 및 정도 및 수술의 응급여부에 의해 결정한다. 2) 영양지원이 결정되면 그 경로는 경장이 좋으며 그것이 안될 때 정맥영양을 시행한다. 3) 술 전 영양지원을 받지않은 중등도 이상의 영양실조 환자에서 술 후 조기 영양지원에 대한 연구에서는 TPNrns에서 대조군보다 오히려 약 $10\%$의 합병증 발생률이 높다. 4) 영양실조가 없는 환자에서도 술 후 약 7일 이상의 금식이 예상되면 TPN을 시행하는 것이 좋다.

  • 중증외상 환자의 사망률을 높이는 출혈의 형태에는 정맥손상으로 인한 출혈과 골절 및 장기손상에 의한 동맥손상 출혈, 골수 내 출혈이 있을 수 있는데, 이중에서 동맥손상에 의한 출혈은 진단이 지연되면 다량의 출혈로 인한 생체활력 징후가 불안정하며 사망률이 높아지는 아주 중요한 질환이다. 응급실을 내원한 중증외상 환자로 등록된 환자 중 혈관조영술을 시행 받고, 동맥 파열이 진단되어 동맥색전술을 시행 받은 환자를 대상으로 생존의 영향을 미치는 인자에 대하여 알아보고자 하며 생존 군과 사망 군을 비교하여 동맥촬영 및 색전술이 적절한 치료방법으로 유용성과 적절한 시행시점을 연구하기 위함이다.

  • 체외 충격파 쇄석술 (ESWL)은 인체 외부에서 발생된 충격파를 인체 내부의 결석 부위에 집속하여 결석을 분쇄하고 치료 효과를 얻는 혁신적 치료술이다. 충격파의 압력 및 파형은 결석의 분쇄 효과 즉 치료 효과를 좌우한다. 체외에서 충격파의 압력은 고감도 하이드로폰을 이용하여 측정할 수 있지만, 인체 체내에서, 특히, 비침습적으로 충격파를 측정하기는 매우 어렵다. 본 논문에서는 ESWL 충격파에 의해 활성화된 체내의 기포군으로부터 방출된 음향 신호 (CIAE)를 측정하여 비침습적으로 충격파의 압력을 추정하는 방법을 실험하였다. 충격파 압력의 추정은 측정된 CIAE 신호에서 1차 기포군 파열음과 2차 파열음 간의 시간 지연은 충격파의 압력과 선형적인 관련성 가진다는 실험 결과 (Coleman et al 1996)에 근거하고 있다. 본 논문에서는 충격파 압력 측정 시스템을 구성하여 생체 외 실험을 수행하였고, 개발된 시스템의 임상적인 활용 가능성을 확인하였다.

  • 본 연구는 시각장애 아동 군과 청소년 군이 가지는 정보요구의 주제, 자주 이용하는 정보원, 정보를 찾는 데 있어 느끼는 어려움, 그리고 학교도서관 이용행태에 대해 파악하고자 하였다. 연구는 국내시각장애 특수학교에 재학 중인 아동과 청소년 25명을 대상으로 한 개별심층면담을 통해 이루어졌다. 연구 결과, 시각장애 아동은 취미, 학업 등에 대한 정보요구가 있었으며, 이를 해결하고자 인적정보원과 인터넷을 주로 이용하는 것으로 나타났다. 아동들은 정보를 획득할 때, 기기 이용에 있어 제한된 환경으로 인해 어려움을 느끼는 것으로 나타났다. 시각장애 청소년은 취미, 진로 등에 대한 정보요구가 있었으며, 이를 해결하고자 인터넷을 주로 이용하였다. 청소년들은 정보를 획득 할 때, 장애 등으로 인해 어려움을 느끼는 것으로 나타났다.

The Effect of Eisenia bicyclis Extracts on Antioxidant Activity and Serum Lipid Level in Ovariectomized Rats (대황 추출물의 in vitro 항산화 활성 및 난소를 절제한 흰쥐의 혈중 지질함량에 미치는 영향)

  • Hormone replacement therapy (HRT) is an effective regimen that has been found to prevent these diseases in postmenopausal women. However, HRT is accompanied by an increased risk of unfavorable outcomes. This study was conducted to evaluate the effects of Eisenia Bicyclis extract on lipids in ovariectomized rats. Fifty 7-week-old female Sprague-Dawley rats were randomly assigned to four groups: sham-operated rats (SHAM), ovariectomized rats (OVX-CON), and ovariectomized rats that were treated with Eisenia bicyclis extracts. The extract-treated diets were fed to the rats for 6 weeks after operation. Antioxidant effects were measured by DPPH free radical scavenging activity. Antioxidant activity of the ethanol extract increased in a dose-dependent manner and was about 55.9% in a concentration of 100 ${\mu}g/ml$. We measured the total cholesterol content, triglyceride content, HDL-cholesterol content, LDL-cholesterol content, atherosclerotic index, cardiac risk factor in serum, and anti-platelet aggregation and blood rheology. The total cholesterol and triglyceride concentration in serum increased for the OVX-control group, but supplementation with the E. bicyclis extract caused these factors to decrease. Notably, the serum LDL-cholesterol concentration in the OVX-EB200 group was significantly lower than the OVX-CON group. In addition, the blood passage times in rats that received the E. bicyclis extract were more rapid than the times in the untreated group (OVX-CON). Microscopic evaluation revealed that whole blood passed more smoothly through the microchannels in rats in the E. bicyclis extract supplement groups. Our results clarified the effects of E. bicyclis extract on serum lipid content in ovariectomized rats, and consequently we expect positive effects from providing E. bicyclis extract to postmenopausal women with cardiovascular disease.

THE MANDIBULAR GROWTH EFFECT OF THE BOTULINUM TOXIN TYPE A INJECTION INTO UNILATERAL MASSSTER MUSCLE OF GROWING RAT (성장기 쥐의 편측 교근에 주사한 보툴리눔 A형 독소가 하악골 성장에 미치는 영향)

  • The purpose of this study was to assess the effect on mandibular growth of botulinum toxin type A (BTXA) injection into the unilateral massester muscle of growing rats at three different growing stages. Thirty six male Sprague-Dawley rats were divided into three groups according to the age (group 1: 4 week-old, group 2: 5week-old, group 3: 6week-old). Then each group was randomly divided into 3 subgroups (control group, unilateral injection group, bilateral injection group). Experimental animals were sacrificed after 4 weeks. Then the jaw measurements were evaluated. The results were as follows: 1. In the group 1, mandibular body length, condylar height and coronoid process height of the unilateral group(BTXA side) and the bilateral group were shorter than those of the control group (p<0.05). 2. In the group 2, anterior region height, condylar height, coronoid process height of the unilateral group(BTXA side) and the bilateral group were shorter than those of the control group (p<0.05). 3. In the group 3, mandibular body length, condylar height, coronoid process height of the unilateral group(BTXA side) and the bilateral group were shorter than those of the control group (p<0.05). 4. There was no significant difference in mandibular measurements between the control side and the injection side in the unilateral group in all age groups (p>0.05).
