DEA기법을 이용한 함정무기체계의 구조적 효율성 평가방안 연구 (A Study on the Structural Efficiency Evaluation of Naval Weapon Systems through DEA Method)

  • 자료포락분석(DEA: Data Envelopment Analysis)기법은 민간 및 국방 분야에 있어서 조직 및 사업의 효율성 분석을 위해 폭넓게 활용되고 있다. 특히, 최근 국방분야에서는 방위산업체, 군병원 그리고 군시설공사에 DEA기법을 활용한 연구가 진행되었다. 본 논문에서는 DEA기법을 활용하여 함정무기체계 구조적 효율성 평가를 수행하였다. 함정무기체계의 효율성 평가를 통해 무기체계 획득시 ROC 결정이나 기종결정 등에 객관적인 의사결정 정보를 제공할 수 있을 것으로 기대된다.

  • 오늘날의 병원들은 환자가 의료기관을 선택하는 구매자 시장으로 변하게 되었고, 군 병원도 2004년부터 현역병의 의료보험적용이 가능해지면서 민간병원처럼 선택 받아야 하는 위치에 놓이게 되었다. 그러므로 군병원도 민간병원의 흐름과 함께 병원 이용고객들이 인식하는 의료서비스 품질, 고객만족도 및 고개충성도에 대한 관심이 필요하다. 따라서 정과 중심의 SERVPERF모델을 이용하여 지각된 군 병원의료 서비스 품질과, 고객만족 및 충성도에 관한 연구로 빠르게 변하고 있는 의료서비스 환경에서의 국민으로부터 신뢰를 받으며 군 의료서비스품질의 발전 방안을 모색하기 위해 본 연구를 시도하였다.

군병원 만성질환자의 처방전분석 및 약물상호 작용 검토 (A Study on Chronic Outpatients' Prescriptions and Drug Interactions Based on Prescriptions in Army Hospital)

  • This study has collected prescriptions of individual chronic outpatients in an army hospital, analysed them, and investigated the possible drug interactions. It also reviewed the mechanism of drug interactions. Out of total of 42 outpatients with chronic diseases, the percentages of populations having hypertension, hypertension with diabetes, uncomplicated diabetes, hyperlipidemia, hypertension with cardiac insufficiency, and ventricular septal defect were 62%, 19%, 10%, 5%, 2%, and 2%, in the corresponding order. The average number medications prescribed for the outpatients were 2.5 with the highest frequency of five medications in two patients. The number of drug-drug interactions detected was 456 prescriptions out of total of 1104 prescriptions during the study period, accounting for 41.3%. The most frequent drug-drug interaction was between beta-blockers and calcium channel blockers with 132 prescriptions followed by one between beta-blockers and cimetidine with 89 prescriptions. Based on the high incidence of possibly dangerous drug interactions, much attention needs to be aid to the drug-drug interactions in the pharmacotherapy for the treatment of outpatients with chronic diseases in army hospital setting.

군병원에서의 심혈관계 수술 114례에 대한 임상적 고찰 (Clinical Experience of Cardiovascular Surgery in Military Hospital - 114 Cases -)

  • One hundred and fourteen patients were underwent cardiovascular operation from the 11th June 1985 to 12th July 1988 in the department of cardiovascular surgery of Seoul District Hospital. There were one hundred and one open heart surgery patients and thirteen non-open heart surgery patients. Of open heart surgery cases, seventy-four were acyanotic, twenty four were cyanotic congenital heart disease and three were acquired valvular heart disease. VSD was most common[55 cases] among acyanotic group and TOF[21 cases] was most common among cyanotic group. Of non-open heart patients, there were eleven cases of PDA and two cases of shunt[modified Blalock-Taussig shunt using PTFE graft] for TOF. There were one operative death[0.9%] due to inadequate relief of right ventricular outflow tract obstruction and peripheral pulmonary artery stenosis for TOF patient. No Patients were dead during follow-up.

군병원 간호조직문화와 조직성과에 관한 연구 (A Study on the Relationship between Nursing Organizational Culture of Military Hospital and Organizational Performance)

  • Purpose: This research was, by investigating the relations between the cultural types of a nursing organization in military hospital and performance, to ascertain the cultural types to be able to improve performance. Method: The date were gathered from 255 nurse officers who were in 19 military hospitals by using the self-report type of questionnaire. The period of data collection was from April 20, 2003 to July 15, 2003. For this research, the following tools were used: the tools for measuring the organizational characteristics, organizational culture, and job satisfaction, the tool for measuring organizational commitment. For data analysis the SPSS Win 12.0 program was used. Result: 1) Most of the cultural types of a nursing organization in military hospital is Relation-oriented. 2) In the relation between general the characteristics of subject and the organizational cultural type, there was a difference in the innovation-oriented, relation-oriented, hierarchy-oriented, and task-oriented culture according to nurse officers careers, hospital types, year in hospital, marital status, and unit(p<0.05). 3) In the relation between general characteristics of subject and organizational performance, there was a difference in the job satisfaction, affective commitment, transactional commitment, and normative commitment according to nurse officers careers, education, madrigal status and unit(p<0.05). 4) In the relationship between the of a military hospital were correlated with the type of each culture(p=0.00), 5) In the relation between the organizational culture type of military hospital and its performance, there was a positive correlation among job satisfaction and innovation-oriented culture, relation-oriented culture, task-oriented culture(p<0.05). And hierarchy-oriented culture showed that they had a weak negative correlation with job satisfaction(p<0.05). There was a positive correlation among affective commitment and innovation-oriented culture, relation-oriented culture, task-oriented culture(p<0.05). And hierarchy-oriented culture showed that they had a weak negative correlation with affective commitment(p<0.05). There was no culture type significantly related to continuance commitment and there was a weak positive correlation among normative commitment and innovation-oriented culture, relation-oriented culture, task-oriented culture. The types to have an influence upon nurse officers' job satisfaction were relation-oriented culture and innovation-oriented culture(p=0.00). And relation-oriented culture and innovation-oriented culture were major variances for affective commitment and only relation-oriented culture was influential variance for normative commitment(p=0.00). Conclusion: The organizational culture type was found which had an influence upon nurse officers' job satisfaction and organizational commitment. These result are very significant in having showed the persons in charge of nursing administration a basic data for creation of an effective organizational culture.

군병원 환자의 근골격계손상과 군대활동과의 연관성에 관한 조사 (Causal Relationship between Military Activities and Musculoskeletal Injuries)

  • The current study investigated the causality between the musculoskeletal injuries of the patients who visited our military hospital and their military-related activities. The surgeon diagnosed the patients and let them answer the questionnaire on pain and causes from April 1, 2008 to May 30, 2008. The included 287 male patients were mean 21 years old(mean height 175 cm and mean weight 69.4Kgf). The visiting time after injury was mean 53 days. The visual analog scale of pain was mean 5.1 points. The 30% of visiting patients answered the most common cause of injury was the military training. The Most common site of injury was the knee, in 38% of patients(111 of 287). Seventy two percent of patients suspected the causality between their injuries and military activities. In military activities, a special stretching program before training, especially for the knee, is required to reduce musculoskeletal injuries. And, the injured military people have to access the medical treat as soon as possible.

3차 군병원 응급실에 내원한 국군 병사들의 파상풍 항체의 정성적 조사 (Qualitative Analysis of the Tetanus Antibody in Korean Army personnel after Visiting a Tertiary Armed Forces Hospital)

  • Purpose: In the Korean armed forces, vaccination against tetanus is done when personnel join the military service, but we do not know how many military personnel are vaccinated and how many soldiers have protection against tetanus. We performed a qualitative analysis of the tetanus antibody in Korean military personnel by using the TQS (Tetanus Quick Stick) Methods: This study used a prospective collection method for military personnel visiting to the emergency department of a tertiary armed forces hospital from July 2005 to January 2007. We performed an analysis by using the TQS and asked whether the personnel had been inoculated during military service. Results: The number of enrolled military personnel was 474. Among them, 412 had been vaccinated against tetanus after entering the military service. The positive rate of TQS was 91.3% (373 patients) after vaccination for tetanus. Conclusion: Many military personnel were inoculated with tetanus toxoid during military service and have protective antibodies for tetanus. According as TQS should be used for military personnel who visit the emergency department of an armed forces hospital. Also, anti-tetanus immunoglobulin should not normally be used a positive TQS for tetanus prophylaxis.

간호과정 용어체계를 이용한 간호기록 분석 - 군병원 정형외과 재원환자 기록 대상으로 - (Analysis on Military Hospital Nursing Records by NANDA, NIC, NOC System)

  • Purpose: This study was to construct a useful nursing language system on military nursing field. Method: Military hospital nursing records were analyzed using NANDA(North American Nursing Diagnosis Association), NIC(Nursing Interventions Classification), and NOC(Nursing Outcomes Classification) systems. All kinds of nursing statements from 80 sets of orthopedics inpatient's records were deduced. All nursing statements were mapped to 167 NANDA diagnoses, 433 NIC interventions, and 260 NOC outcomes. Result: 14,744 nursing statements were extracted. Among the extracted nursing statements, 11.75% were linked with NANDA diagnosis, 83.62% were connected with NIC intervention, and 0.96% was tied to NOC outcome. 3.66% of nursing statements were not linked with NANDA-NIC-NOC system. In the nursing statements, 18 diagnoses of NANDA, 63 interventions of NIC, 8 outcomes of NOC were used. Conclusions: The majority of those nursing statements focused on nursing intervention of the nursing process; few nursing plans or goals were found in nursing records. Therefore, it's difficult to make the nursing process network with the nursing statements. Documenting nursing records using a nursing process will contribute to strengthen nursing practice in patient care and to develop nursing as science. Continuous further researches related to nursing records are needed to provide basic data for developing nursing language system and nursing record system.

군병원 간부들의 개인적 성향에 따른 직무 만족도 (Job Satisfaction Influenced Personal Traits among Officers and Noncommissioned Officers Working in Army Hospital)

  • The purpose of this study was to find out and analyze the satisfaction degree related to job by general characteristics, satisfaction factors. This survey was based on the questionnaire method. The sample consisted of 51 noncommissioned officers, 72 administrative officers and 160 nurse officers from army hospitals. The data were collected from March 14 to April 18, 1994 and statistically analysed by percentage, mean and $x^2$ test. Factors of job satisfaction consist of 6 categories; job pristige, interaction, autonomy, task requirements, pay, and organizational requirements. The overall job satisfaction degree was $62.8\%$ in NCOs, $54.2\%$ in administrative officers and $33.3\%$ in nurse officers. The job satisfaction of NCCs increased in factors of job prestige (p<0.01), autonomy (p<0.05), task requirement (p<0.05), and was higher as age. There were statistically significant in the factors of pay in school background, Job prestige in rank (p<0.05). 'I'm OK and You're OK' type was $76.5\%$ which was the highest rate and A major personal traits also showed the highest job satisfaction $(63.7\%)$. There were significant difference between major personal traits and interaction (p<0.01), autonomy (p<0.05), organizational requirement (p<0.01), pay (p<0.05) and task requirement. The job satisfaction of administrative Officers was represented significant differences in factors of pay (p<0.05), task requirements (p<0.05) by school background. Long term Workers showed the highest job satisfaction and significant difference in factors of job prestige and organizational requirement (p<0.01). Job satisfaction in 'I'm OK but you're not OK' type was slightly high, and CP major personal traits also showed the highest job satisfaction. The job satisfaction of nurse officers was represented significant difference in factors of autonomy (p<0.01), job prestige (p<0.05), organizational requirement (p<0.05), and pay by age. In CP major personal traits. job satisfaction showed the highest rate (75.0)

일부 간호장교의 직무만족도와 관련요인 (Job Satisfaction and Related Factors Among Nursing Officers)

  • 본 연구는 간호장교의 인구사회학적 및 건강관련 행위 특성, 직업관련 특성 및 직무스트레스에 따른 직무만족도 수준을 알아보고, 특히 직무스트레스가 직무 만족도에 미치는 영향을 규명해 보고자 시도하였다. 조사대상은 군병원에 복무중인 육군 간호장교 350명으로 하였으며, 조사는 2011년 5월 30일부터 2011년 6월 15일까지의 기간 동안에 구조화된 무기명 자기기입식 설문지를 이용한 설문조사에 의하였다. 연구결과, 조사대상 간호장교들의 직무만족도는 연령이 높을수록, 음주군일수록, 직위가 높을수록, 근무경력이 길수록, 근무부서가 병동근무 군, 업무에 만족한다는 군, 업무의 요구도가 낮은 군, 업무의 자율성 및 상사의 지지도가 높은 군일수록 높았으며, 특히 직무스트레스 요인은 직무만족도와 높은 관련성이 있었다. 따라서 간호장교가 더욱 열심히 즐겁게 일할 수 있도록 내적 동기를 불러일으키는 직무만족도를 높여 주기 위해서는 직무스트레스 요인을 개선시키기 위한 노력이 필요할 것으로 생각된다.
