LOCAL ANESTHESIA IN CHILDREN USING JET INJECTION INSTRUMENT (분사식 주사기를 이용한 소아환자의 국소마취)

  • Local anesthesia procedure in pediatric dentistry using needle-tipped syringes is stressful and painful for the child patients The Syrijet Mark II (Mizzy Inc. USA) is a jet injection instrument, which uses high pressure to propel fluids into soft tissue without the use of a hollow neeldle, so it seems to be able to reduce the injection phobia of the child patients. The authors compared the Syrijet with the conventional syringe by assessing the pain level after local anesthetic procedures using CAS(color analogue scale) which was developed from VAS(visual analogue scale). The result showed that the pain was reduced by the use of syrijet. The advantages of syrijet were no use of neeldes, the reuse of the anesthetic cartridge, and the safety to nerves and vessels. The disadvantages were the large size, the high price, and the need of skill to use.

Usefulness of $^{99m}Tc-HMPAO$ SPECT in the Localization of the Epileptic focus in Temporal Lobe Epilepsy: Comparison with EEG, MRI and CT (측두엽성간질의 간질 병소 편측화에서 $^{99m}Tc-HMPAO$ SPECT의 유용성: 뇌파, 자기 공명 영상 및 전산화 단층 영상과의 비교)

  • 뇌 혈류의 기능적 영상화는 간질병소의 국소화에 이용되고 있으며 측두엽성간질의 편측화에 여러가지 진단 방법이 이용되고 있으나 만족할만한 결과를 보이지 못하고 있다. 최근 PET또는 SPECT를 이용하여 측두엽성간질에서 발작 간에 측두엽 병소의 대사율 및 혈류의 감소가 나타나며, 이러한 소견은 발작 유발 병소의 편측화에 매우 유용할것이라는 보고들이 있다. 저자들은 측두엽성간질에서 간질 병소를 편측화 하는데에 $^{99m}Tc-HMPAO$ SPECT의 유용성을 평가 하고자 측두엽성간질 31예에서 발작 간의 $^{99m}Tc-HMPAO$ SPECT 소견, 뇌파, 자기 공명 영상 및 전산화 단층 소견을 비교하였다. SPECT 소견에 따른 나이, 병력 기간과 병발시 나이 등의 임상 지수 간에는 유의한 차이가 없었다. 31예의 환자중 23예에서(74.2%) 국소 뇌 혈류 감소를 보였으며 17예(54.8%)에서 측두엽에 관류 감소가 관찰 되었다. 비인두 뇌파 표준 뇌파는 24예(77.4%)에서 측두엽에 편측화를 보였으며 SPECT와 뇌파 양자가 모두 편측화된 경우 일치도는 8/12예 (66.7%) 였다. 16예에서 시행된 전산화 단층 영상은 모두 편측화를 보이지 못했으며 27예에서 시행된 자기 공명 영상에서는 단지 1예에서 편측화를 보였다. 이상의 결과로서 발작 간의 $^{99m}Tc-HMPAO$ SPECT는 측두엽성간질 병소의 편측화에 유용한 보조 검사로 생각된다.

Automatic Interpretation of F-18-FDG Brain PET Using Artificial Neural Network: Discrimination of Medial and Lateral Temporal Lobe Epilepsy (인공신경회로망을 이용한 뇌 F-18-FDG PET 자동 해석: 내.외측 측두엽간질의 감별)

  • Purpose: We developed a computer-aided classifier using artificial neural network (ANN) to discriminate the cerebral metabolic pattern of medial and lateral temporal lobe epilepsy (TLE). Materials and Methods: We studied brain F-18-FDG PET images of 113 epilepsy patients sugically and pathologically proven as medial TLE (left 41, right 42) or lateral TLE (left 14, right 16). PET images were spatially transformed onto a standard template and normalized to the mean counts of cortical regions. Asymmetry indices for predefined 17 mirrored regions to hemispheric midline and those for medial and lateral temporal lobes were used as input features for ANN. ANN classifier was composed of 3 independent multi-layered perceptrons (1 for left/right lateralization and 2 for medial/lateral discrimination) and trained to interpret metabolic patterns and produce one of 4 diagnoses (L/R medial TLE or L/R lateral TLE). Randomly selected 8 images from each group were used to train the ANN classifier and remaining 51 images were used as test sets. The accuracy of the diagnosis with ANN was estimated by averaging the agreement rates of independent 50 trials and compared to that of nuclear medicine experts. Results: The accuracy in lateralization was 89% by the human experts and 90% by the ANN classifier Overall accuracy in localization of epileptogenic zones by the ANN classifier was 69%, which was comparable to that by the human experts (72%). Conclusion: We conclude that ANN classifier performed as well as human experts and could be potentially useful supporting tool for the differential diagnosis of TLE.

  • Since the usual conventional edge detectors employ the local differential derivatives, the detected edges are not uniform in their widths or some edges are missed out of the detection on magnified images. We employ a mapping from the exactly monotonic intensity distributions of ramp edges to the simple step functions of intensity, which is referred to as perfect sharpening map of ramp edges. This map is based on the non-local feature of intensity distribution and used to introduce a modified differentiation, in terms of which we can construct an efficient edge detector adaptive to the variation of edge width. By adopting the operator MADD in this paper, we developed an edge detector that works stably against the magnification of image or the variation of edge width. It is shown by comparing to the conventional algorithms that the proposed one is very excellent.

Chemical Saturation Breath-hold Fast MR Imaging for characterization of Regional Fatty Changes in Liver (화학적 포화 호흡정지 급속 자기공명영상에서 국소적 간지방병변의 특성화)

  • Purpose: To assess the usefulness of breath-hold fast MR imaging of liver with fat suppression (FS) by application of chemical saturation technique in the diagnosis of regional fatty changes suspected in sonography. Materials and Methods: Thirteen patients who had focal lesions with diffuse, homogeneous signal changes after FS through chemical saturation technique without additional changes of imaging parameter during MR imaging of liver were selected. T1-weighted fast low-angle shot and T2-weighted turbo spin-echo sequences were obtained with or without FS during each single breath-holding session. Subjective changes of signal intensity between the pre-FS and the FS images were compared with the sonographic findings in each lesion. Results: Seven lesions of decreased signal intensity after FS on T1 or T2-weighted images, including three lesions only at FS T1 images, were regarded as focal fat infiltration. All seven lesions had compatible sonographic findings as homogenously echogenic areas. Another six lesions of subjectively increased signal intensity including two lesions only at FS T2 images were regarded as focal fat sparing. All six lesions had sonographic findings as homogenous echo poor areas suggesting focal fat sparing. In cases regarded as fat infiltration, score changes were more prominent at FS T1 images than FS T2 images(p=0.0002). In cases regarded as fat sparing, score changes were more prominent at FS T2 images than FS T1 images(p=0.042). Conclusion: Breath-hold fast T1 and T2-weighted MR imaging with and without chemical saturation pre-pulse may be sufficient for characterization of regional fatty changes in the differential diagnosis of focal hepatic lesion found at sonography.

A Nonuniform Sampling Technique and Its Application to Speech Coding (비균등 표본화 기법과 음성 부호화로의 응용)

  • For a signal such as speech showing piece-wise linear shape in a very short time period, a nonuniform sampling method based on the inflection point detection (IPD) is proposed to reduce data rate. The method exploits the geometrical characteristics of signal further than the existing local maxima/minima detection (MMD) based sampling method. As results, the reconstructed signal by the interpolation of the IPD based sampled data resembles the original speech more. Computer simulation shows that the proposed IPD based method produces about 9~23 dB improvement over the existing MMD method. To show the usefulness of the IPD technique, it is applied to speech coding, and compared to the continuously variable slope delta modulation (CVSD). The nonuniformly sampled data is binary coded with one bit flag set "1". Noninflection samples are not sent, but only flag bits set 0 are sent. The method shows 0.3 ~ 9 dB SNR and 0.5 ~ 1.3 mean opinion score (MOS) improvements over the CVSD.

Parametric Images of Standardized Uptake Values using P-18-FDG Attenuation Corrected Whole Body PET (F-18-FDG감쇠보정 전신 PET을 이용한 표준섭취계수 추정과 매개변수 영상의 구성)

  • Background and Purpose : Standardized uptake value(SUV) has been used as a quantitative index for differentiating benign and malignant tumors with F-18-FDG PET In this study, we produced whole body parametric images of SUV(WBPIS) by body weight normalization, and validated the values by comparison with SUV's calculated with regional scans. Subjects and Methods : Whole body scans were followed by regional scans sequentially on 23 patients. In whole body study, transmission and emission scans were acquired for 2 minutes and 6 minutes for each bed position, respectively. In regional study, transmission and emission scans were acquired for 20 minutes. Measured and segmented/ smoothed attenuation correction were applied using these 2 min transmission scans in whole body studies. The effects of attenuation correction on SUVs were evaluated quantitatively using F-18 filled cylindrical phantom. The mean and peak SUVs obtained from WBPIS were compared with SUVs of the regional scans. Results : In phantom studies, with any method of attenuation correction using regional or whole body studies of phantom, SUVs were nearly consistent. In whole body scan, SUV obtained using measured attenuation correction method was a little higher than SUV of regional scan. SUV obtained using segmented/smoothed attenuation correction method was a little lower. In patient studies, WBPIS using segmented/smoothed attenuation correction method was much smoother and more readable. SUVs of WBPIS obtained with both methods of attenuation correction were well correlated with SUVs of regional scans(r=0.9). SUVs of WBPIS with measured attenuation correction method were 5% lower than SUVs of regional scans. SUVs of WBPIS with segmented/smoothed attenuation correction method were 10% lower than SUVs of regional scans. The differences of SUVs of WBPIS by the two attenuation correction methods were relatively small compared with the possible differences derived from biological characteristics of tumors. Conclusion : We concluded that WBPIS could be useful in the quantification of tumor as well as in localization of whole body lesions, which were often outside the field of view in regional scan. WBPIS made using segmented/smoothed attenuation correction method could be used in clinical routines and SUVs from attenuation corrected F-18-FDG PET could be used interchangeably with SUVs of regional studies.

  • Regional odontodysplasia is a relatively rare developmental anomaly of dental hard tissue with characteristic clinical, radiographic and histologic features. It requires a continuous and multidisciplinary approaches, and the aim of treatment for these patients should include aiding mastication, improving aesthetics, maintaining normal vertical dimension and space, allowing normal jaw growth and eruptional management of affected teeth. This report describes three cases of regional odontodysplasia with 2-5 years of follow-up. Conservative treatment is chosen to preserve the affected teeth as long as possible, and periodic radiographic and clinical examination was done. During this time, all teeth except one showed progressive development. An interesting finding observed in our cases was that each tooth even in the same person showed different degree of tooth development and eruption rate. Thus, we colcluded that the treatment plan for regional odontodysplasia should be conservative and individualized and based on the assessment of each tooth.
