Full mouth rehabilitation through re-establishment of occlusal plane in partially edentulous patient with reduced vertical dimension accompanied by loss of posterior occlusal support (구치부 교합지지 상실과 수직고경 감소를 동반한 부분 무치악 환자에서 교합평면 회복을 통한 완전구강회복 증례)

  • The loss of posterior occlusal support leads to further complications such as collapsed occlusal plane and reduced vertical dimension, and it may cause problems such as facial appearance change, reduced chewing efficiency, and temporomandibular joint disorders. In such case, it is necessary to re-establish occlusal plane and vertical dimension properly through accurate diagnosis and predictable treatment plan. This case report presents a 71-year-old female, whose occlusal plane was collapsed and posterior restorative space was insufficient. To perform a patient-friendly full mouth rehabilitation, proper vertical dimension and occlusal plane were decided by evaluation of interocclusal space at her physiologic mandibular rest position, swallowing, pronunciation, facial appearance, and the average length of anterior teeth. And then, the fixed provisional restorations were fabricated with the new occlusal position, and evaluated for 5 months with checking adaptation of masticatory muscles and any kind of clinical symptoms occurs or not. After confirmation of functional stability and esthetic satisfaction with the newly established occlusion, final definitive restorations were fabricated and inserted in the mouth. Through the above process, the treatment result was functionally and aesthetically satisfactory.

AN EXPERIMENTAL STUDY ON THE TENSILE STRENGTH OF POSTERIOR RESIN-BASED COMPOSITES (구치부 복합레진의 인장강도에 관한 실험적 연구)

  • The purpose of this study was to evaluate the tensile strength of light-cured restorative posterior resin-based composites. Five commercially available light-cured composites(Denfil : DF, P60 : PS, Unifil S : US, Z100 : ZH, Z250 : ZT) were used. Rectangular tension test specimens were fabricated in a teflon mold giving 5mm in gauge length and 2mm in thickness. Specimens were subjected to the 5,000 thermal cycles between $5^{\circ}C$ and $55^{\circ}C$ and the immersion time in each bath was 15 second per cycle. Tensile testing was carried out with Instron at a crosshead speed of 0.5mm/min and fractured surface were observed with scanning electron microscope. The obtained results were summarized as follows; 1. The tensile strength of PS was highest. PS was significantly higher than DF, US and ZH(p<0.05) but in the case of ZT was similar to PS(p>0.05). 2. The tensile strength DF was lowest. DF was significantly lower than PS, US, ZH and ZT(p<0.05). 3. The tensile strength of US and ZH were significantly lower than PS and ZT(p<0.05). but were significantly higher than DF(p<0.05). The tensile strength of US and ZH were similar(p>0.05).

Full Mouth Rehabilitation in a Patient with Severely Worn Dentition (심하게 마모된 치열을 가진 환자의 완전구강회복)

  • Inadequate or unstable posterior support cause extrusion of antagonistic teeth and overloading of remaining teeth. Extrusion of antagonistic teeth result in collapsed occlusal plane and decreased prosthetic space. Also, overloading of remaining teeth increase occlusal wear or anterior extrusion of anterior teeth, which is resulting in loss of anterior guidance, disharmony of occlusion and decreased prosthetic space. In this case, careful evaluation of OVD(occlusal vertical dimension) is the general first trial to regain appropriate function and esthetics. Simultaneously, adequate anterior guidance and harmonious occlusal plane should be established in the repeatable reference position of the mandible. We report this case because severely worn dentition achieved functional and esthetic results from full mouth rehabilitation with increased vertical dimension and use of centric relation.

  • Accurate impression taking for the success of implant prosthesis is a very important process. Methods of taking implant impression include the conventional method using impression coping and impression material, and the digital method using an intraoral scanner and scanbody. However, the impression coping or the scanbody must install after remove healing abutment. Because of this, the dentist must repeat the process of removing and installing the healing abutment, the impression coping or the scanbody several times. In addition, the impression coping or the scanbody rises higher than the occlusal surface, so the patient has the inconvenience of constantly maintaining the open state. Recently, a scannable healing abutment, which can be scanned by a intraoral scanner directly, without the need to remove the healing abutment by applying a scannable part of the scanbody to the healing abutment, was introduced. We present a case of single posterior implant prosthesis using a scannable healing abutment.

  • The bite collapses due to posterior teeth loss or wear results in inadequate space for restoration and esthetic concerns. Increasing the occlusal vertical dimension to improve space deficiency rotates the mandible posteriorly, creating a gap between the maxillary and mandibular anterior teeth, leading to loss of anterior guidance. To solve this problem, the prosthodontics or orthodontics treatments are the commonly used methods for proper anterior guidance. However, it is reasonable to assume that the anterior teeth can naturally relapse to their original position when the occlusal force is eliminated. Therefore, this case report aimed to test whether natural relapse could recover the lost anterior guidance to develop a less invasive and more convenient treatment method. Digital superimposition was used to evaluate the changes in anterior teeth alignment to confirm the change of the recovered anterior guidance. The appropriate indications for this new treatment method were defined and applied clinically.

  • In edentulous patients, the advantages of prosthodontic therapy using oral implants include both stability and comfort. Advantages suggested for this approach include the prevention of mandibular bone resorption and increased masticatory function. Implant hybrid prostheses place 4 to 6 implants between the mental foramens, and cantilevers are used to replace the posterior teeth at both ends. The 85 - year - old female patient visited our clinic with complete edentulism in the maxilla and mandible. This article reports a satisfactory clinical and esthetic outcome of full mouth rehabilitation using removable complete denture in the maxilla and implant hybrid prosthesis in the mandible.

Implant overdenture using milled bar and attachment in partially edentulous mandible: a case report (하악 부분 무치악 환자에서 Milled Bar와 부착장치를 이용한 임플란트 피개의치 수복 증례)

  • Excessive crown height space increases can cause crestal bone loss and screw loosening after prosthesis is placed. Milled bar and implant overdenture can be used as a treatment method for partially edentulous patients who have severe alveolar bone loss and excessive crown height space. Milled bar can provide primary splinting effect and stability between implants. Also, milled bar with additional retention device such as Advanced Dental Device-Treatment Of Choice (ADD-TOC) and magnet can provide additional retention force for implant overdenture. In this case, the patient has a partially edentulous mandible that has severe alveolar bone loss and multiple number of teeth loss after excision due to leiomyosarcoma. Because of the long-term loss of mandibular molars, the opposing teeth were extruded. Maxillary left molars were corrected to the occlusal plane through molar intrusion, and mandibular left molar region were treated with implant overdenture, using milled bar with ADD-TOC and magnet after implant placement. The clinical result was satisfactory on the aspect of esthetic and masticatory function.

  • As patients' esthetic expectations increase, there is an increase in demand for cosmetic restorations of anterior and posterior teeth that resemble the natural tooth morphology and color. An example of high-strength all-ceramic systems is zirconia with CAD/CAM application. This case report describes a full-mouth rehabilitation using zirconia all-ceramic crowns supported by upper and lower implants and by a minimal increase in the occlusal vertical dimension in a patient with severe abrasion due to loss of posterior teeth.

  • Digital dentistry is becoming a mainstream of dental practice. We are already used to intraoral scan, guided implant surgery, manufacture of prosthesis (CAD-CAM) in our daily practice; those are not sensational anymore. However, treatment planning with digital simulations is not spotlighted enough. In this report, a digital mockup was utilized to visualize the final goal of maxillary rehabilitation including all anterior and posterior teeth, and persuade a patient who wanted restoration of only anterior teeth. We will also discuss the use of digital mockup and its actual copy as the guide of altered vertical dimension through the clinical process.

  • The collapse of the posterior occlusion destroys the normal occlusal plane and causes excessive wear reducing the vertical dimension. Reduced vertical dimension of occlusion causes not only aesthetic and functional problems but also overloading on the temporomandibular joints and abnormalities of muscle nerve system. In order to improve the collapsed occlusal relationship, it is necessary to consider the change of the vertical dimension. It is necessary to make a precise diagnosis and analysis before the treatment and to evaluate the adaption of patient to the new vertical dimension of occlusion. A patient with excessive overbite often has occlusal problems of tooth wear and tooth eruption. Considering these considerations, overall prosthodontic restoration is required to solve the problem. A patient of 68 year old man in this case who suffered major tooth wear and maxillary posterior teeth loss was treated with elevation of vertical dimension of occlusion by maxillary removable dental prosthesis and mandibular fixed prosthesis.
