Loss of posterior support may cause extrusion of antagonistic teeth and excessive wear of remaining teeth which often leads to the destruction of the occlusal plane. In such cases, it is critical to verify the need to increase vertical dimension of occlusion (VDO). Should you increase VDO, provisionalization is crucial in evaluating function and esthetics. Double scanning technique is a useful method when fabricating definitive restoration that mimic provisional restoration. In this case, a patient with apparently no loss of VDO and insufficient interocclusal space for dental materials due to loss of posterior support and extrusion of antagonistic teeth was rehabilitated using double scanning technique.

Loss of molar support and abnormal jaw relationship lead to occlusal disharmony and cause pathologic signs. Full mouth rehabilitations with reestablishment of occlusal schemes are needed. In this case, the 75 year-old female patient showed posterior bite collapse, irregular occlusal plane and Class II jaw relationship. By observing her profile and interocclusal distance, she was diagnosed as loss of occlusal vertical dimension. Treatment plan is to restore maxillay class I removable partial denture and mandibular fixed prosthesis and to establish vertical dimension and harmonious occlusal plane. Occlusal vertical dimension of 19 mm, which is obtained by 7.5 mm increase between maxillary right lateral incisor and mandibular canine, was established using temporary prosthesis via diagnostic wax-up. Patient adaptation with newly formed vertical dimension was verified during 8 week follow-up period. Within the information of interim prostheses, final restoration was constructed and delivered. The patient showed sound occlusal scheme and esthetic profile.
Purpose. This retrospective study was to investigate the survival and complication rates of posterior single implant according to the implant diameters, lengths and position. Materials and methods. Patients who had been restored single implant prosthesis in the posterior area by the three prosthodontists in the department of prosthodontics, Daejeon dental hospital of Wonkwang university, in the period from February 2014 to May 2018 were selected for the study. A total of 505 patients with 697 implants were observed. The survival and complications of implants were investigated using electronic medical records and radiographs. Fixture diameters, lengths, position, patient's sex and age were assessed as possible factor affecting the survival and complications of implants. Results. 3-year cumulative survival rates of posterior single implants were 98.5% and 5-year cumulative survival rates were 94.4%. 5-year cumulative survival rates were higher in implants with diameter > 4.0 mm (97.0%) than implants with diameter ≤ 4.0 mm (89.5%), and in females (98.8%) than males (92.4%). There were statistically significant differences (P < .05). The mechanical complication rate was 20.1% and the biological complication rate was 4.7%. Complications occurred in order of abutment screw loosening (7.5%), decementation (6.3%), proximal contact loss (3.7%) and so on. Abutment screw loosening occurred more frequently in the lower molar region (10.5%), in males (9.5%) than females (5.1%), and in patients aged < 65 years (9.4%) than patients aged ≥ 65 years (5.1%). There were statistically significant differences (P < .05). Conclusion. The 5-year cumulative survival rates were higher in implants with diameter > 4.0 mm than implants with diameter ≤ 4.0 mm and in females than males. Abutment screw loosening which was the most commonly occurring complication occurred more frequently in the lower molar region, in males than females, and in patients aged < 65 years than patients aged ≥ 65 years. There were statistically significant differences.

The use of stainless steel crowns are indicated for restoration of primary or permanent molars with proximal dental caries, extensive dental caries, or previous pulp treatment with increased danger of tooth fracture. Stainless steel crowns were introduced by Humphrey in 1950. For their improved durability, longevity, and success rate, they have been strongly considered for restoring extensive and multi-surfaced dental caries of molars in pediatric dentistry. However, they also have shortcomings, such as possibility of pulpal exposure or damaging proximal surface of adjacent teeth. In addition, when oversized stainless steel crowns are used, eruption of the adjacent permanent teeth may be disturbed by their prominent margin. As a means to compensate the shortcomings of stainless steel crowns, use of orthodontics bands may be considered. It is an alternative restoration method, where an orthodontic band is placed on a tooth first and cavity is restored with filling material, such as composite resin, glass ionomer, or amalgam. The use of an orthodontic band is indicated for molar restoration with cervical dental caries, extensive dental caries, enamel hypoplasia, or previous pulp treatment. Because it requires shorter chair time compared to stainless steel crown, its application is very useful for children with poor behavior. However, restoration using an orthodontic band requires good oral hygiene after its application. This case report illustrates the conservative restoration of primary molars and permanent molars with extensive dental caries using orthodontic bands.
Cephalometric superimposition is unable to evaluate tooth movement along the occlusal plane and the errors of photographing and superimposition of the occlusogram is also inevitable. The purpose of the present study was to evaluate the rotational movements of the maxillary posterior teeth following space closure after extraction of maxillary first premolars. using 3D dental images. In 19 adult females. analysis of rotational movements of the maxillary posterior teeth was performed between the initial and final 3D dental images superimposed on the maxillary hard palate. The results showed a diversity of rotational movements of posterior teeth. The causes of various rotational movements may be different rotation of molars in each case at the beginning of treatment. individual characters of the dental arch and tooth morphology. The results of the study indicate that accurate evaluation of rotational movements of posterior teeth following space closure after extraction of premolars using 3D dental images can be obtained through the subdivision of tooth movement during the initial aligning, space closure and finishing stages.

The aim of this study was to evaluate the proximal caries detecting ability and identify the optimal cut-off values of two types of laser fluorescence (LF) devices; classic type (DD) and pen type (DDpen). The number of proximal surfaces participated in this study were 164 surfaces in primary dentition and 438 surfaces in permanent dentition. Each tooth surface was sequentially assessed by two types of LF devices, and bitewing radiograph. The radiographs were classified into 3 groups in primary dentition ($pR_0$, $pR_1$, $pR_2$), and 4 groups in permanent dentition ($PR_0$, $PR_1$, $PR_2$, $PR_3$) according to the depth of caries, and used as gold standard. In primary dentition, the area under the curve (AUC) values of DD were 0.851 and 0.890, and those of DDpen were 0.883 and 0.917, respectively in enamel caries and dentin caries. In permanent dentition, the AUC values of DD were 0.762 and 0.886, and those of DDpen were 0.828 and 0.958, respectively in enamel caries and dentin caries. When detecting proximal caries in posterior teeth with LF devices, DDpen is more useful than DD in both primary and permanent dentition. However, in primary dentition, DD can also be useful to detect proximal caries.

The loss of posterior occlusal support leads to further complications such as collapsed occlusal plane and reduced vertical dimension, and it may cause problems such as facial appearance change, reduced chewing efficiency, and temporomandibular joint disorders. In such case, it is necessary to re-establish occlusal plane and vertical dimension properly through accurate diagnosis and predictable treatment plan. This case report presents a 71-year-old female, whose occlusal plane was collapsed and posterior restorative space was insufficient. To perform a patient-friendly full mouth rehabilitation, proper vertical dimension and occlusal plane were decided by evaluation of interocclusal space at her physiologic mandibular rest position, swallowing, pronunciation, facial appearance, and the average length of anterior teeth. And then, the fixed provisional restorations were fabricated with the new occlusal position, and evaluated for 5 months with checking adaptation of masticatory muscles and any kind of clinical symptoms occurs or not. After confirmation of functional stability and esthetic satisfaction with the newly established occlusion, final definitive restorations were fabricated and inserted in the mouth. Through the above process, the treatment result was functionally and aesthetically satisfactory.
This study was undertaken to investigate the changes of tooth contacts following orthodontic treatment , using T-Scan system. Occlusal contacts in maximum intercuspation were examined in 26 patients at the end of the active phase of orthodontic treatment and again 3 months into the retention phase to abbess the initial posttreatment occlusal changes. The obtained results of this study were as follows : 1. The mean total number of contacts increased from 11.27 at the end of orthodontic treatment to 13.15 at the fellow up visit, which was found to be statistically significant (P<0.05). 2. The change of number of contacts on anterior teeth was unchanged. 3. The change of number of contacts on posterior teeth was found to be statistically significnat (P<0.01). 4. The change of fetal number of teeth in contact was significant (P<0.05). 5. The number of ant. teeth in contact was slightly increased. but Which was not found to be statistically significant (P>0.05). 6. The number of post. teeth in contact significantly increased, which was statistically significant (P<0.05). 7. A statistically significant relationship was found between the total number of contacts at the end of treatment and the change in the contacts over time. The fewer the number of tooth contact was at the end of orthodontic treatment, the greater number of tooth contact increased over time(r=-0.38, P<0.05).
Purpose: This study was to assess clinically the incidence of abutment screw loosening of posterior implant-supported fixed prosthesis and its affecting factors. Materials and methods: 391 implant-supported crowns restored from January 2013 to January 2016 were included in this study. All restorations were fabricated with either a single crowns or a splinted crown, and cemented with temporary cement. The incidence of abutment screw loosening is investigated and gender, restoration position, opposing teeth, restoration type, abutment connection type were assessed as possible factors affecting abutment screw loosening. Results: During the observation period (2 - 5 years), abutment screw loosening was found in 29 restorations (7.4%). It took 3 to 48 months (means 19.5 months) to loose the screw, and three of these implants were fractured. Among the factors considered, there were statistically significant differences at abutment screw loosening rate between molar group (9.4%) and premolar group (2.6%) (P<.019). According to the type of opposing teeth, there were statistically significant differences between nature teeth (74.7%) and implant (25.0%), removable denture (3%) (P<.019). The other possible factors did not have a significant effect on loosening of the abutment. Conclusion: The incidence of abutment screw loosening in posterior restoration was 7.4%. Abutment screw loosening were more likely to occur in molars group than premolar group, and according to the opposing teeth, there were the greatest frequency in nature teeth than implant and removal denture. There was a statistically significant difference.

The purpose of this study is to investigate the production method of posterior bridges pontic ridge lap type which prevents the infection in bridge pontic base and is able to cleanse itself, in the process of producing final prothesis that maintains healthy mucous membrane of oral cavity and interproximal papilla, minimizing diastema, is aesthetic and has no effect on pronunciation. New technology is applied to make optimal pontic base which prevent inflammation and clean itself and its products were clinically evaluated in 10 places of dental clinics in busan and gyeongnam. The making of posterior 3 unit bridge pontic base, it was presented as the new technology of forming ridge lab type and to carry out clinical validation, existing conventional method and the new technology were compared. Pontic base made with the existing conventional method cause infection and other periodontal disease by 96% but the pontic base made with the new technology cause infection and other periodontal disease by 3%. Remains of food cause infection and other periodontal disease 100% by the existing conventional method and 91% by the new technology, showing a distinct difference. However, after a gargle, the new technology had low 13%. Additionally, the pontic base made with the existing conventional method showed 71% of chance, the new technology method showed 8% of chance in terms of self-cleansing.
