THE EFFECT OF DISTAL MOVEMENT OF UPPER MOLAR USING THE PENDULUM APPLIANCE (Pendulum 장치의 상악대구치의 원심이동에 대한 효과)

  • Treatment of class II malocclusions require distalization of maxillary molars into class I relationship. Intraarch distal molar movement techniques have recently assumed an important role in young patients. In this study, the dental and skeletal effects of the pendulum appliance were evaluated by means of cephalometric radiographs. The samples were consisted of 19 patients: 11 females and 8 males, mean age $11.68{\pm}1.52$ years. Measurements were obtained from cephalometric prior to and the day of removal of the pendulum appliance. Treatment changes were analyzed. The following results were obtain. 1. The pendulum appliance produced $2.94{\pm}1.54mm$ distal molar movement with a mean intrusion of $1.17{\pm}0.97mm$, mean period $18.13{\pm}7.95$ weeks. 2. The anchor tooth was $1.34{\pm}1.40mm$ forward movement and $0.48{\pm}0.99mm$ extrusion, and labial tilting of incisors. 3. The angle between palatal plane and mandibular plane increased significantly. 4. There was no significant difference in according to 2nd molar position. 5. Total movement was consisted of 74% distal movement of 1st molar and 26% forward movement of the anchor tooth.

Comparison of palatal bone thickness between 3D model and lateral cephalometric radiograph (성인 구개부 측모두부계측방사선 사진과 3D model의 골두께 비교)

  • Objective: This study compared the bone thickness of the palate between lateral cephalogram and 3D model measurements. Methods: The subjects consisted of 30 adults (15 men,15 women) with a normal skeletal pattern and occlusion. The CT images were transformed to a 3D model, and were compared with the cephalometric image. Descriptive statistics for each variable were calculated. Results: In the 3D CT model, the mid-palatal area was the thickest part. It became thinner as the palate tapered laterally. In the male group, the thinnest portion was positioned 6 mm away from the mid-palate, while in the female group the thinnest portion was 8mm away from the mid-palate. Correlation analysis between the lateral cephalometric and 3D CT model revealed a significant correlation except in the mid palatal area and the area 2 mm lateral to the mid-palate in men, whereas there was a significant relationship in every area in the women. In both men and women, the highest correlation appeared in the area 8 mm lateral to the mid palate. Conclusions: Using regression analysis, an actual prediction of the bone thickness between the measured bone thickness of the lateral cephalometric radiograph and 3D model was made. This will provide useful information for mini-implant length selection when inserting into the palate.

A cephalometric investigation on the craniofacial configurations of Class ll division 1 and 2 in Korean (한국인 II급 1류 및 2류 부정교합자 두개안면형태의 차에 대한 측모두부방사선계측학적 연구)

  • Numerous studies have revealed the similarities and discrepancies in two divisions of class II malocclusion, since these malocclusion groups have been postulated to be disparate criterion, much as classified under one diagnostic umbrella. This study was undertaken to describe the craniofacial configurations of class II division 1 and 2, and consequently to discriminate the morphologic differences between the two malocclusion groups in Korean sample. Lateral headfilms of 34 class H division 1 and 29 division 2 were employed, while those of 142 adults of normal occlusion served as a control. The landmarks were digitized and 26 variables were statistically analyzed for one way ANOVA. 1. There manifested no statistically significant difference in maxillary position anteroposteriorly. Normal occlusion group exhibited most anteriorly positioned mandible, whereas class II division 1 showed the most retroposition. Class II division 1 disclosed clockwise rotation tendency of mandible, which resulted in position of the chin Posteriorly. 2. Class II division 1 showed greater in SN to MP, SN to PP significantly than other groups. 3. Class II division 2 showed smaller genial angle and larger mandibular body length than other groups. 4. Class II division 1 revealed greater anterior lower face height than other groups, whereas division 2 dictated significantly greater posterior face height. 5. Class II division 2 expressed the most retroclined lower incisor, while division 1 manifested the most proclination. The largest interincisal angle resided in Class II division 2 group. There were no significant differences in upper molar position anteroposteriorly.

A study on the dental arch characteristics of bialveolar protrusion patients using a three-dimensional digital model (3차원 디지털 모형을 이용한 양악 치조골 전돌자의 치열궁 특성 연구)

  • Recent beauty trends have caused an increase in the number of bialveolar protrusion patients seeking treatment of lip protrusion. But studies of these patients are not common. Studies using their dental models are especially rare. Dental models have been measured manually or through a computer by digitizing two dimensional images of models. Nowadays, we are able to study dental models more easily and accurately by utilizing the three-dimensional (3-D) laser scanner in dentistry. An investigation was carried out to evaluate the characteristics of the dental arch in a bialveolar protrusion group in comparison with a normal group using 3-D digital models. The normal group was composed of 20 subjects who were selected from students of the School of Dentistry, Kyungpook National University. The bialveolar protrusion group was composed of 20 subjects who visited for treatment at the Department of Orthodontics, Kyungpook National University Hospital. Tooth size, arch width and arch length were measured digitally, and arch shape and the shape of the palate were drawn. Based on the results of this study, the differences of the arch characteristics in the bialveolar protrusion group were that the size of the teeth was larger, mandibular canine width and 1st premolar width were wider, and arch length was longer than in the normal group. And there were differences in the arch shape and the shape of palate between the bialveolar protrusion and normal groups.

A STUDY ON THE PATTERN OF SKELETAL CHANCE FOLLOWING CERVICAL HEADGEAR THERAPY IN GROWING CHILDREN (성장기 아동에서 Cervical Headgear사용시 골격적 변화 양상에 대한 연구)

  • This study was to investigate the horizontal & vertical bone change pattern when using cervical headgear in Class II malocclusion of growing children and compared the skeletal features between the group with increased lower facial height and the group without increase in lower facial height. The results are as follows ; 1. Forward growth of maxilla was inhibited, downward tipping of anterior palatal plane could be seen and distal movement of maxillary first molar was observed. 2. There was relative forward movement of Mandible against the Maxillary cranial base, and relative forward movement of mandibular 1st molar against the Maxilla and vertical increase due to alveolar growth of Mandible. 3. There was significant increase in anterior and posterior facial heights but the ratio of facial height showed no significant difference. 4. The group with increased lower facial height has shorter ramus length, than the smaller palatal plane angle, and more distal movement of Maxillary 1st molar than the group without increase Ha-young Hyun
