A STUDY ON THE VOLUMES AND FORMS OF THE PALATE FOR DECIDUOUS AND PERMANENT DENTITION (유치열과 영구치열의 구개 형태에 관한 연구)

  • The purpose of this study was to clarify the palatal forms through palatal curvatures of right to left, anterior to posterior direction, and difference of right and left palatal volumes in the primary and permanent dentition. Samples were consisted of normal occlusion of both dentition(50 males and 50 females each). Their upper plaster casts were used, measuring points were decided, through 3-dimensional laser scanning(3-D Laser Scanner, DS4060, LDI, U.S.A.), and fitting standard horizontal plane were made for measuring the height and sectioned volumes of palate. The results were as follows: 1. Palatal volume and height were greater at the right side of the palate in the primary and permanent dentition of male and female, but there was no significance. 2. Palatal height was greater in male compared to female, especially, there was significant difference at intercuspid, inter-second premolar area in the permanent dentition(P<0.05). 3. To the height of A-P direction of mid-palatal area, the highest point was 20mm in the primary dentition, 30mm in the permanent dentition from interdental papilla of central incisors. 4. Palatal height of inter-cuspid and inter-second premolar became shallow and broad, high and broad each, compared to inter-deciduous canine and inter-second deciduous molar.

The effect of palatal height on the Korean vowels (구개의 높이가 한국어 모음 발음에 미치는 효과에 관한 연구)

  • Purpose: The purpose of this study was to analyze the influence of palatal height on Korean vowels and speech intelligibility in Korean adults and to produce baseline data for future prosthodontic treatment. Material and methods: Forty one healthy Korean men and women who had no problem in pronunciation, hearing, and communication and had no history of airway disease participated in this study. Subjects were classified into H, M, and L groups after clinical determination of palatal height with study casts. Seven Korean vowels were used as sample vowels and subjects'clear speech sounds were recorded using Multispeech software program on computer. The F1 and the F2 of 3 groups were produced and they were compared. In addition, the vowel working spaces of 3 groups by /a/, /i/, and /u/ corner vowels were obtained and their areas were compared. Kruskal-Wallis test and Mann-Whiteny U test were used as statistical methods and P < .05 was considered statistically significant. Results: There were no significant differences in formant frequencies among 3 groups except for the F2 formant frequency between H and L group (P = .003). In the analysis of vowel working space areas of 3 groups, the vowel working spaces of 3 groups were similar in shape and no significant differences of their areas were found. Conclusion: The palatal height did not affect vowel frequencies in most of the vowels and speech intelligibility. The dynamics of tongue activity seems to compensate the morphological difference.

A STUDY ON THE RELATIONS OF VARIOUS PARTS OF THE PALATE FOR PRIMARY AND PERMANENT DENTITION (유치열과 영구치열의 구개 각부의 관계에 관한 연구)

  • The purpose of this study was to clarify the palatal arch length, width and height in the primary and permanent dentition. Samples were consisted of normal occlusions both in the primary dentition(50 males and 50 females) and in the permanent dentition(50 males and 50 females). With their upper plaster casts were used and through 3-dimensional laser scanning(3D Scanner, DS4060, LDI, U.S.A.), cloud data, polygonization, section curve and loft surface, fit and horizontal plane were based to measure the palatal arch length, width and height(Surfacer 10.0, Imageware, U.S.A.). T-tests were applied for the statistical analyze of the data. The results were as follows : 1. In the measurement values, the values of the male were higher than those of the female except primary anterior palatal height. There were not only statistically significant differences in anterior palatal width(p<0.05) and posterior palatal width(p<0.01) in primary dentition but palatal width(p<0.05), anterior palatal length(p<0.01), middle and posterior palatal length(p<0.05) in permanent dentition between male and female. 2. In the indices of palate, there were statistically significant differences in height-length index(p<0.05) and width-length index(p<0.01) between male and female in primary dentition. In permanent dentition, there was statistically difference between male and female. 3. In the measurement values, posterior palatal width was increased most greatly. Posterior palatal height, anterior palatal width and anterior palatal length were followed by descending order. On the other hand, anterior palatal height and posterior palatal length were decreased. 4. In the indices of palate, the height-length index, the width-length index and posterior height-width index were increased, but the others were decreased.

Patients' satisfaction on the obturators with different extension heights into defects after maxillectomy (높이가 다른 폐쇄장치에 대한 상악절제술 환자의 만족도)

  • Purpose: The purpose of this study was to compare the functional abilities of the low bulb obturators with those of high bulb obturators in terms of patients' evaluation. Material and methods: This study included 11 maxillectomy patients who underwent postoperative prosthodontic rehabilitations. Two obturators of the same design except for different bulb heights, were fabricated for each of the maxillectomy patient. After two months of alternate use, the functions of the obturators were measured by investigating the patients' subjective evaluations in terms of convenience, speech, nasality, leakage, and mastication and identifying their preferred prostheses. Wilcoxon signed rank test was used as a statistical method (P < .05). Results: There were no significant differences in patient evaluations of low and high bulb obturators (P >.05). And patients' preferences varied. Conclusion: In extreme situation such as in mouth opening limitation, the use of low bulb obturators can be recommended and result in comparable speech function to that of obturators with high lateral walls.

Prosthetic rehabilitation of a patient with tongue cancer using palatal augmentation prosthesis and mandibular implant-retained overdenture: A case report (구개 증대 보철물과 임플란트 피개의치를 이용한 설암 환자의 보철수복 증례)

  • The tongue, especially its lateral part, is the most common site of oral tumors. Patients who undergo glossectomy for the treatment of tongue cancer may experience difficulty in proper functioning for pronunciation, chewing, swallowing, and oral hygiene maintenance; therefore, a palatal augmentation prosthesis can be used to restore function of the tongue. In this case, an implant overdenture was used in a patient who had residual ridge resorption and obliteration of alveololingual sulcus after undergoing glossectomy for tongue cancer treatment. In addition, a palatal augmentation prosthesis with a metal framework, support, and retention part was fabricated. The palatal vault was reduced, so that even with limited tongue movement, adequate tongue-palate contact could be achieved. After placement of the definitive prostheses, the patient showed improvement in the functions of chewing, swallowing, and pronunciation.

Anatomical characteristics of the midpalatal suture area for miniscrew implantation using CT image (정중구개봉합부의 해부학적 구조에 관한 전산화 단층촬영을 이용한 연구)

  • There is lack of data for the amount of vertical bone in the midpalatal region for miniscrew implantation. The purpose of this study was to measure the structure of the midpalatal suture area using CT image and V-works 4.0 program (Cybermed Inc, Seoul, Korea). CT images of 14 male and 14 female adults were reconstructed. In detail, it was 1) to measure the length of maxilla on the midsagittal plane 2) to measure vertical bone height in the midpalatal area 3) to establish the zone of safety for miniscrew implantation. The following results were obtained. The mean length of ANS-PNS was 51.08mm in males and 47.34mm in females. There was a statistically significant difference between males and females (p<0.05). The vertical bone height of the midpalatal suture area was above 0mm except for 6mm posterior from the central Point of ANS-PNS in males The zone of safety was located 19.43mm posterior from the ANS in males while it was 17.62mm in females along the palatal plane. These results support that the safety zone of the midpalatal area is suitable for screw implantation. Midpalatal miniscrew implantation is a powerful tool in modern orthodontics Through many applications. it can expand the modern orthodontic field.

Effects of Horseshoe Expander (Horseshoe Expander의 확장 효과에 관한 연구)

  • Horseshoe Expander is one of Slow Maxillary Expansion(SME) which aims to accommodate the contra- lateral expansion and midpalatal suture expansion or the palate. The appliance consists of skeleton type strew embedded in split Horseshoe appliance. It is the objectives of the presentation to manifest the changes in dental & craniofacial components subsequent to the application of Horseshoe Expander. The subjects for this study consisted of 32 patients (mean age : 12.7). frontal, lateral cephalometric headfilm were taken and study casts were fabricated before and after expansion. 24 items were measured, compared preexpansion with postexpansion. Especially, palatal volume was measured by means of 'Hydro-measurement method'. Tooth axis measurement on the dental casts were made with Universal bevel protractor, and Horseshoe Expander group were compared with RME group. This study of changes to maxillary expansion with Horseshoe Expander revealed the following significant results. 1. Triangular-shaped expansion pattern appeared in frontal cephalometric headfilm. 2. Palatal plane, occlusal plane, mandibular plane and upper incisor to FH increased in lateral cephalometrir headfilm. 3. Palatal volume increased significantly. A slight bite opening, reduction of occlusal contact points showed in dental casts. 4. A 2.2:1 ratio of the amount of intermolar width in maxilla(orthodontic movement) to maxillary width (orthopedic movement) was determined. 5. Horseshoe Expander group has less buccal tipping tendency than RME group, by taking high correlation coefficients in the upper second premolar and first molar. It was suggested that Horseshoe Expander showed less orthodontic changes, less buccal tipping tendency. In addition, it was effective in maxillary expansion.

A study on the changes of the posterior segments between before, after treatment and postretention period through maxillary expansion therapy (상악궁 확장술을 통한 치료전, 후 및 보정후 상악 구치부 변화에 관한 연구)

  • The purpose of thus study was to measure the changes of arch and dentition of maxillary posterior teeth before and after treatment and postretentios in patients treated with Quad-helix. Measurements and analysis were performed on study model with carlipers. Seventeen cases, eight boys and nine girls, were selected with average ages of 12.7 years. Mean retention period was 4 months, and mean wearing time was. 127 days. The result were as follows 1. The interpremolar widths and intermolar width were significantly increased in maxillary first piennolar and molar when compared between before and after treatment and postretention. The maxillary second premolar resulted in significant increase in interpremolar between after tretment and post retention. 2. The long axis of maxillary first premolar and first molar accompaning buccal tipping was significant increased before and after Tx and post retention. Significant increase of the lingual inclination of maxillary second premolar was obsered both in before and after Tx, and before Tx and post retention. 3. The distance between the height of tooth contour was significantly increased in the first and second premolar before and after Tx, and was significant increased in the first molar before and after Tx and post retention. 4. Intermolar width at the palatal groove was increased in before and after Tx. and post retention. The palatal depth was significant increased in before and after Tx. In conclusion expansion of maxillary dental arch showed no relapse and results were stable retention
