Comparison of Behavioral Response between Intranasal and Submucosal Midazolam Adminstration (소아 진정 치료 시 구강 점막 하와 비점막 Midazolam 투여의 행동 반응 비교)

  • Purpose. The objective of this study was to evaluate the behavioral response and assess the effectiveness of additional intranasal (IN) and submucosal (SM) administration of midazolam during pediatric sedation for dental procedure. Material and methods. Thirty-three cases of healthy (ASAⅠ), uncooperative children aged from 24 to 72 month old at pediatric dental clinic of Ewha Womans University Hospital were selected for this study. Children received oral chloral hydrate 50 mg/kg with hydroxyzine 1.0 mg/kg. After waiting for 45 minutes, midazolam 0.2 mg/kg was administrated via IN route and via SM route randomly maintaining 50% of $N_2O$. A pulse oximeter and a capnograph were used for measuring vital signs ($SpO_2$, PR, RR, $EtCO_2$) throughout the sedation. Behavioral response was evaluated as Quiet (Q), Crying (C), Movement (M) or Struggling (S) in every 2 minutes for 40 minutes. Results. There were also no statistically significant differences in vital signs of the two groups. The behavioral response for the first ten minutes during sedation was a statistically significant difference (P < 0.05) between the two groups. After the first ten minutes, it was revealed that there was no significant difference. Conclusion. This study demonstrated that the addition of IN midazolam to the combination of oral chloral hydrate with hydroxyzine and nitrous oxide/oxygen inhalation is as safe and effective as that of SM midazolam in pediatric sedation for dental procedure.

  • Burning mouth syndrome (BMS) is defined as the xerostomia, burning sensation and various discomfort of tongue and oral mucosa. BMS can occur in both men and women, but is more frequent in middle-aged menopausal women. Because exact cause can't be identified clearly and it is hard to make diagnosis in clinic, the purpose of the treatment have been to relieve symptoms. Etiology of BMS is divided into local, systemic, and psychological factors. ${\alpha}$-lipoic acid, clonazepam, supplemental therapy and cognitive behavior therapy can be prescribed for BMS. Nowdays, many experts focus attention on effect of combination therapy. It is necessary to solve the symptoms of the patients by combination of pharmacological approach and psychotherapy with cognitive behavior therapy considering the factors in various aspects.

  • 본 연구는 치과에 내원한 환자를 대상으로 치아우식증과 치주질환을 제외한 실제 구강건강도에 미치는 요인과 그 비중을 알아봄으로써 개인의 예방줌심 계속구강건강관리제를 운영하는데 기초자료로 이용하고자 한다. 2010년 7월 1일 부터 2010년 7월 30일까지 한 달간 유아기, 아동기, 청소년기, 청년기, 장년기, 노년기를 구분하여 각 연령별 40명씩 조사하여 6단계 연령층으로 총 240명을 대상으로 구강검사와 설문조사를 실시하였다. 연구결과, 치과 내원 환자들은 연령이 적을수록 구강건강도가 높음을 알 수 있다. 전신질환이 없을수록, PHP가 높을수록, 설태가 없을수록, 구강보조용품을 적게 사용할수록, 음주/흡연을 하지 않을수록 TMJ가 높을수록, 구강감염/점막질환이 없을수록, 저작능력이 높을수록 구강건강도가 높음을 알 수 있다.

Self-Perceived Oral Health Status according to Regulation of Blood Glucose in the Type 2 Diabetic Patients (제2형 당뇨병 환자에서 혈당조절에 따른 구강건강인식도)

  • The aim of this study was to investigate the oral health problems among type 2 diabetes patients and suggest basic data for the promotion of their body and oral health by emphasizing the correlation between hemoglobin A1c and subjective oral health status. For 174 patients with type 2 diabetes and who were older than 40 years old, the questionnaire and measurement of hemoglobin A1c were conducted from January 9, 2012 to March 9, 2012. The results of the study were as follows. 1. They tended to be most aware of the following; 'inflammation on the oral mucosa' and 'pain on the oral mucosa' among the symptoms of oral mucosa, 'gum bleeding when brushing teeth' among the symptoms of periodontal disease, and 'feels dry in general' among the symptoms of xerostomia. 2. The patients with longer duration of diabetes showed greater recognition with regards to the symptoms of oral mucosa disease, periodontal disease, and xerostomia (p<0.05). 3. The group with regular meal showed lower level of hemoglobin A1c (p<0.001). 4. The level of hemoglobin A1c was higher in the group that recognized 'burning sensation', 'gum bleeding', 'gum recession' and 'bad breath' (p<0.05) as well as in the group that responded 'get up at night to drink' among the symptoms of xerostomia (p<0.05). Especially the symptoms of periodontal disease were revealed to be a factor that showed the 3rd strongest correlation with hemoglobin A1c.

  • Recurrent aphthous stomatitis is common oral disease in the world. It is characterized by multiple, recurrent, painful ulcer with circumscribed margins, erythematous haloes and yellow or grey floors. Patients with recurrent aphthous stomatitis suffer from its painful ulcer. But unfortunately, its etiology and pathogenesis is not clear and still unknown. So we review etiology and pathogenesis of recurrent aphthous ulcer and wish to propose direction of the future study.

  • Epidermoid cysts are rare benign lesions in the oral cavity that may be either congenital or acquired. The cysts are usually slow-growing and asymptomatic until becoming secondarily infected or large enough to interfere with mastication and speech. Consequently, diagnosis is often delayed. The condition is also uncommon in newborns and infants. Most of the lesions occur in the floor of the mouth and rarely in the upper lip. This report describes the case of a 29-month-old girl with a palpable mass in the inner mucosa of the upper lip. The lesion was surgically enucleated using an intraoral approach and histopathologically diagnosed as epidermoid cyst.

EFFECT OF ULTRAVIOLET RADIATION ON THE ORAL MUCOSA OF THE RATS (자외선 조사가 백서구강점막에 미치는 영향에 관한 실험적 연구)

  • Present study investigated the effect of Nuva-Lite irradiation on the labial oral mucosa of the albino rats. The specimens were embedded in paraffin wax and stained with histological and histochemical procedures such as hematoxylin-eosin stain, Feulgen reaction, methyl green-pyronin stain, alloxan Schiff reaction, -SH group reaction, PAS reaction, colloidal iron reaction and toluidine blue stain. The specimens 24 hours to 3 days after ultraviolet irradiation exhibited mild intracellular edema in the prickle cell layer. On 6th and 10th day after irradiation rete peg hyperplasia was prominent. Ultraviolet irradiation caused diminution of Feulgen reacted DNA. However, sulfhydril and ${\alpha}$-amino acid radicals were increased at the upper layer of stratified squamous epithelium after irradiation.

Preparation of Poloxamer-based Hydrogels Using Electron Beam and Their Evaluation for Buccal Mucoadhesive Drug Delivery (전자빔을 이용한 폴록사머 하이드로젤의 제조 및 구강 점막부착성 약물전달을 위한 특성 분석)

  • In this study, poloxamer hydrogels were prepared by electron beam irradiation and evaluated for potential application as a buccal mucoadhesive drug delivery system. Poloxamer, one of typical thermoresponsive polymers, was modified to have vinyl end groups for crosslinking reaction, and its hydrogels were fabricated by irradiation crosslinking reaction. Carbopol as a mucoadhesive polymeric additive was introduced to improve the mucoadhesive property of the hydrogels and its effect on the mucoadhesion and drug release properties was investigated. The results showed that the end group modification of poloxamer and the addition of carbopol improved mucoadhesive force and mechanical properties and led to a sustained drug release behavior.

The Associated Factors with Xerostomia in Adults Aged 30 Years and Over (일부 만 30세 이상 성인에서 구강건조증 관련요인 분석)

  • The aim of this study was to analyze factors related to xerostomia in adults aged 30 years and over. The data were analyzed using the t-test, one-way ANOVA and multiple linear regression analysis in the SPSS version 12.0 program. The results were as follow. 1. The respondents who were older than 51 years old, unemployed and had less than 3 million won of average monthly income were more aware of xerostomia (p<0.05). 2. The respondents who answered poor and moderate for their general and oral health and the group with duplicate medication and comorbidity were more aware of xerostomia (p<0.05). 3. The respondents that had problems in chewing, communication, ordinary activities and complained of pain discomfort and suffered from anxiety depression were more aware of xerostomia (p<0.001). 4. The respondents that answered frequent dryness on their skin, eyes, lips, and nasal mucosa were more aware of xerostomia (p<0.001). 5. Xerostomia showed highest correlation with quality of life ($\beta$=0.436) followed by the number of medications ($\beta$=0.239), sense of entire body dryness ($\beta$=0.200), feeling of hopelessness ($\beta$=0.160) and number of oral mucosa disease symptoms ($\beta$=0.099) (p<0.05). According to the results of the study, xerostomia may cause deterioration in quality of life. Thus, it is advised to improve the patient management system among dental professions to prevent various complications caused by xerostomia and conduct regular health education on the cause and management method of xerostomia.
