  • Objectives : This study was a systematic review of tongue movements in stroke patients with dysphagia. This study aimed to provide a basis for verifying the effects of tongue movement and identifying the tendency of tongue movement. Methods : A systematic review was conducted using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses checklist and flow chart. PubMed, MEDLINE, CINAHL, RISS, and e-articles databases were searched. A total of six documents were investigated, and the PEDro scale was used to evaluate the quality of the papers. Results : Three intervention methods were included in the six papers analyzed. Regarding the type of tongue exercise, three TPRT (Tongue to Palate Resistance Trainings) and two TSAT (Tongue Strength and Accuracy Training) were mediated through the IOPI (Iowa Oral Performance Instrument), and only one study applied TSE (Tongue Stretching Exercise). The treatment effects for each intervention implemented in the literature were confirmed to be effective. However, generalizability of findings is difficult because of the small sample size. Further, no significant difference was found between the experimental and control groups. Conclusions : This study can help occupational therapists provide efficient swallowing rehabilitation treatment by applying tongue exercises to stroke patients with dysphagia. More research should be conducted to determine the effects of tongue exercise.

  • Objective : This study analyzed the correlation between cognitive function and oral health-related quality of life (OHQoL). Methods : Demographic and clinical characteristics were extracted and utilized for subjects aged 45 years or older who participated in the 8th Korean Longitudinal Study on Aging in 2020. The dependent variable was the Geriatric Oral Health Assessment Index, and the independent variable was the level of cognitive function classified by the Mini-Mental State Examination scores. The analysis method used inverse probability of treatment weighting (IPTW). Then, the association between cognitive function and OHQoL was analyzed by multiple regression analysis. Results : Among the participants, 4,367 (71.40%) had normal cognition, 1,155 (18.89%) had moderate cognitive impairment, and 594 (9.71%) had severe cognitive impairment. As a result of analysis by applying IPTW, there was a negative correlation between the cognitive function group and OHQoL (normal vs. moderate: β = -2.534, p < .0001; normal vs. severe: β = -2.452, p < .0001). Conclusion : After propensity score matching, mild cognitive impairment showed a more negative association than severe cognitive impairment. Therefore, patients with cognitive impairment require oral health management education to improve OHQoL regardless of the level of cognitive impairment.

  • The purpose of this study was to investigate relation and effect of PCC(Percentage of Correct Consonant) and nasalance score on the speech intelligibility and acceptability in adults with dysarthria by reading task of standardized passage. Ten adults with dysarthria and sixteen normal adults were participated in this study. PCC and nasalance score were measured through reading task of standardized passage. And, speech intelligibility and acceptability were examined using visual analogue criteria. The result of the study was as follows. First, the nasalance score of adults with dysarthria group is significantly higher than normal adults group in reading sample by standardized passage. Second, the PCC, speech intelligibility and acceptability shows significant correlation. However, the nasalance score doesn't show significant corelation with speech intelligibility and acceptability. These results suggest that PCC is closely related to speech intelligibility and speech acceptability, but nasalance score is not related to speech intelligibility and speech acceptability.

  • Dysphagia is common and serious problems in post-stroke patients. The post-stroke dysphagia with aspiration is associated with dehydration, malnutrition, pneumonia, sepsis and death. Up to date, gag reflex and choking history used to decide the aspiration in clinical. The purpose of this study was to evaluate the aspiration and to choice the proper meal formation using by video fluoroscpic swallowing study(VFSS) with post-stroke dysphagia patients. 58 post-stroke patients and ten normal person participated to perform the VFSS with liquid type, puding, yoplait, rice mixed with barium(Ba). Two rehabilitation medicine doctors and a radiological technologist analysed the phase(oral, pharyngeal, esophageal phase)with video film, and checked the pharyngeal transition time(PTT). 38 patients showed abnormality in pharyngeal phase, 13 patients in oral and pharyngeal phase, 3 patients in oral phase, and 2 patients in pharyngeal and esophageal phase. 43 patients(65.2%) occured the aspiration, but 23 of 43 patients improved by the chin tuck position. Aspiration occured 34 patients in liquid type, 2patients in rice gruel and nothing in boiled rice. After VFSS, 13 of 23 patients change the intaking pathway from nasogastric tube(NGT) to oral, On the contrary with 3 of 42 patients from oral to NGT. Consequently VFSS is clearly effective to evaluate the aspiration with post-stroke dysphagia patients.

  • The purpose of this study was to analyze the dental consultation of pediatric inpatients to the department of pediatric dentistry in Yonsei University Severance Hospital, and to investigate the change in patterns. In 2017, 268 pediatric patients (384 cases) admitted to the Severance Hospital were referred to the department of pediatric dentistry. The mean age was 6.6 years, and most of the patients were referred from the Department of Pediatric Hematology Oncology and Rehabilitation Medicine. The chief complaints were as follows: oral examination (31%), dental caries (20%), oral pain (10%), tooth mobility (10%), pre-operative evaluation (9%) and others (20%). 41% of the patients received only oral examination without treatment. Dental caries were the most frequent dental diagnosis of the patients. 28% (111 cases) of patients received operative treatments, 22 cases were treated under general anesthesia. Oral health is closely related to systemic diseases, especially for hospitalized patients. Dental consultations should be encouraged for prevention and early appropriate treatments. For this purpose, it is necessary to establish a referral system and perform dental treatment under general anesthesia.

  • In this study, the effective dose was measured using the PCXMC v2.0 program by examining the conditions used to set the diagnostic reference level for intraoral imaging recommended by the government, and the effect of the Al additive filter was confirmed. In oral imaging, the largest effective dose was calculated from the oral mucosa among 11 organs. The effect of the Al additive filter showed an excellent radiation reduction effect at 2mm rather than 1mm. In the case of children aged 5 years, the overall effective dose was calculated to be high in all 11 organs because they are more sensitive to radiation than adults. And as a result of evaluating the image quality according to the use of an additional filter during intraoral imaging, there was no significant difference in SNR and CNR changes compared to before the additional filter was used. Based on this study, it is thought that additional filter settings can be recommended for intraoral imaging.

  • Objective : The purpose of this study was to systematically review dysphagia rehabilitation treatment for children with feeding disorders. Methods : The articles evaluated in this study were collected from the PubMed, Medline Complete, and CINAHL databases and subsequently reviewed using the PRISMA flow chart and PICOS approach. A total of 13 papers were analyzed for study quality, disease groups, evaluation tools, interventions, and post-intervention effects. Results : Of the reviewed papers, six (46.15%) related to autism spectrum disorder (ASD) and seven (53.85%) to cerebral palsy (CP) with age ranges of between 2 and 8 years for the ASD studies and between 12 months and 18 years for CP. In evaluating the types of feeding disorder involved, the ASD group exhibited predominantly behavioral conditions while the CP subjects had a larger number of functional oral and swallowing issues. In terms of interventions, behavior modifications were used most frequently with ASD while oral-sensory motor, texture modifications, and electrical stimulation were applied at the same frequency with children with CP. All interventions were found to be effective. Conclusion : In this study, research into children with feeding disorders was reviewed according to condition, evaluation tool, and method of intervention. It is expected that this review can be used as basic data for developing a protocol that will allow clinicians to efficiently apply condition-specific interventions for eating disorders without resorting to trial and error.

  • Objectives: We aim to observe the relation of body mass index (BMI) and the indicators of oral health. Methods: 400 subjects participated in the study. The BMI values are calculated from the height and weight. For the tongue diagnosis, we used the tongue imaging device to analyze the color, tongue coating, and tooth marks. We measured the concentration of hydrogen sulfide (H2S) and methyl mercaptan (CH3SH) to evaluate the halitosis. The dry mouth was evaluated through the measurement of saliva secretion and with the questionnaire asking the frequency of dry mouth. Results: The BMI values were significantly higher in the group with light-white and blue-purple colored tongue, and significantly lower for lightly-coated tongue. However, the correlation of BMI and the amounts of saliva secretion was not significant as well as in the correlation of BMI and the concentration of H2S, CH3SH. In tongue diagnosis, the subjects who had blue-purple colored tongue also had significantly higher H2S and CH3SH, but tendency of lower saliva secretion. Conclusion: We obtain data showing that BMI value and the indicators of oral health including tongue diagnosis have meaningful correlation.
