Development of Comprehensive Oro-Facial Function Scale (포괄적 구강안면기능척도(Comprehensive Oro-Facial Function Scale; COFFS)의 개발)

  • Objective : This study aimed to develop a Comprehensive Oro-Facial Function Scale (COFFS) that can evaluate oro-facial function in patients with dysphagia. Methods : To verify the item composition and reliability of the COFFS, preliminary items were collected by selecting and analyzing four previous studies, and the Content Validity Ratio (CVR) was derived through a second survey of experts. Cronbach's 𝛼 was calculated for the internal validity of the evaluation items, and the test-retest reliability and inter-rater reliability were calculated using the internal classification coefficients (ICC). Results : The content validity ratio of all items was 0.67; in the case of Cronbach's 𝛼 value for each domain, 0.849 for communication domain, -0.224 for the oro-facial structure and shape, 0.831 for the ability to perform orofacial movements, and 0.946 for mastication and swallowing function. The test-retest reliability was 0.974 and the inter-rater reliability was 0.937, showing high reliability. Conclusion : In this study, the evaluation tool of COFFS was finally selected from 34 items in four areas and developed on a 3-5 point scale according to the evaluation items. In future studies, additional research is needed to prove its validity through correlation with other evaluation tools that measure oro-facial function.

Feeding Disorders in Autistic Spectrum Disorders (자폐 스펙트럼 장애 아동의 섭식장애: 문헌 고찰)

  • Objective : Autistic Spectrum Disorders(ASD) is a developmental disorder characterized by atypical sensory adaptation, communication problem, stereotyped behavior, and feeding disorders. The reasons for ASD feeding disorders are oral sensory motor, cognitive, behavioral, and social problems. Major symptoms include picky eating, selective eating, food refusal, food neophobia, limited food variety, and food aversion. ASD feeding disorders could be accompanied by various problems such as health and nutrition intake problems, feeding development, eating-related sociability, and family and caregiver stress. Feeding problems and disorders in ASD can present from birth. However, ASD is diagnosed by the age of 3, and there might be an appropriate treatment gap. Usually, symptoms of feeding disorders tend to decrease with age. However, the symptoms often remain, so early evaluation, intervention, and periodic checking are necessary. In this study, the general information about the feeding disorder characteristics of ASD, influencing factors, and intervention were described through a literature review. Conclusion : Sensory-based therapy and behavior-based therapies are generally used for feeding disorders in ASD. Sensory-based therapy is effective for food sensitivity and behavior-based therapy for food selection. As the symptoms of feeding disorders in ASD are diverse, a comprehensive approach includes play and participation, oral motor exercise, diet, and daily life. However, appropriate evaluation, intervention protocol, and guidelines for the treatment of feeding disorders in ASD are limited. Therefore, a complex approach based on a more systematic understanding is needed. Feeding rehabilitation specialists, such as occupational therapists, should provide appropriate evaluation and intervention.

The role and implications of dental hygienists in oral rehabilitation in Japan (일본 구강 재활 분야에서의 치과위생사의 역할 및 시사점)

  • Objectives: The purpose of this study was to provide basic data that would inform the direction of oral rehabilitation and how to expand of the role of dental hygienists in Japan. Methods: A systematic literature review was conducted on the role of dental hygienists in the field of oral rehabilitation in Japan. Results: Japan has been making academic and practical developments in the field of oral rehabilitation for over 30 years, and has been gradually implementing a fee support policy since the 1990s. In addition, Japan has been operating a dental hygienist system specializing in oral rehabilitation since 2006. The related work was being carried out with the dental hygienist's expertise in the field of rehabilitation medical treatment secured. Dental hygienists work full-time at long-term care facilities for the elderly in addition to conducting oral care activities under the local comprehensive care system, in the areas of convalescence and acute rehabilitation, as well as in the field of visiting rehabilitation. It can be seen that, in the field of nursing care, they are specialized in oral care tasks for the elderly. Conclusions: In the future, a policy and related fee system should be gradually prepared to expand the role of dental hygienists in the field of oral rehabilitation that can contribute to improving oral health linked to systemic diseases.

Systematic Review of Method for Application of Oral Sensorimotor Intervention for Feeding Disorders in Children with Cerebral Palsy (섭식 장애가 있는 뇌성마비 아동에게 적용된 구강감각운동치료방법에 대한 체계적 고찰)

  • Objective: This study was conducted to comprehensively analyze domestic and international literature on the oral sensorimotor intervention approaches and evaluation/non-instrumental assessment methods for children with cerebral palsy with feeding disorders. Methods: One hundred and seventy-six papers published from January 2009 to December 2018 were screened. Forty-seven papers were selected based on the abstract and title, and five papers were selected through a secondary search. Results: The PEDro scale of the selected papers was high with an average of 7 points, and the therapeutic intervention period was found to be between 2 and 6 months, providing therapeutic interventions once to 5 times a week, at least 15 minutes to 1 hour a day. The treatment approach was used with impairment-based intervention and adaptive-based intervention, and the assessment method was divided into clinical evaluation and non-instrumental assessments. Conclusion: Through this systematic review, we found that there are a variety of oral sensorimotor interventions for children with cerebral palsy with feeding disorders. This study provides support for planning oral sensorimotor intervention programs for occupational therapy in clinical practice for children with cerebral palsy.

The Importance of Video Fluoroscopy Swallowing Study for Nasogastric Tube Removal of Rehabilitation Patient (재활치료환자의 비위관(nasogastric tube)제거에 따른 비디오투시연하검사(VFSS)의 중요성 평가)

  • 뇌경색, 뇌 신경장애 및 뇌졸중환자 중 급성기 환자는 의식이 없어 연하기능장애로 인한 흡인성 폐렴을 유발하기 때문에 비위관(nasogastric tube)을 삽입하고 영양공급을 한다. 재활훈련 후 회복기에 침상선별검사를 시행하지만 임상적 검사를 통해서는 무증상흡인은 발견할 수 없다. 그래서 연하장애로 비위관을 삽입한 연하재활치료 중인 환자 10명을 대상으로 VFSS를 시행하여 식이재료에 따른 삼킴 정도, 자세에 따른 삼킴을 재활의학과전문의가 평가 후, 흡인이 있거나 삼킴 곤란 등이 있으면 연하재활치료를 시행하여 환자의 상태에 따라 약 30일 ~ 50일후에 재평가를 시행한 후 흡인이 없을 시 비위관을 제거 하였다. 이때 VFSS 영상을 기능적 연하곤란척도를 이용, 분석하여 부여된 점수를 통계 산출 하였는데 비위관 유지군은 $49.79{\pm}9.431$을 보여 흡인의 위험을 나타내며, 비위관 제거군은 $11.20{\pm}1.932$로 흡인의 위험성이 낮아 비위관 제거관련 의미 있게 낮은 점수를 보였다. 두 군의 유의성을 평가하기 위해 Mann-Whitney test를 시행한 결과 p<0.001로 통계적으로 유의하다고 평가하였다. 결론적으로 VFSS는 구강, 인두, 식도의 구조적 이상과 움직임을 가장 효과적으로 평가, 기도흡인여부를 즉시 확인 및 환자에게 적합한 식이나 연하자세를 결정할 수 있어 비위관 제거를 위한 연하평가에 가장 확실한 표준검사로 제안할 수 있다.

Review of Clinical Studies Using Oral Appliance in Korean Medicine (구강 내 장치를 이용한 한의 임상연구 고찰)

  • Objectives: The purpose of this review is to analyse the trends of clinical studies using oral appliance in Korean Medicine. Methods: We reviewed Korean Medicine papers by searching Korean web databases 'Korea Traditional Knowledge Portal (KTKP)', 'Korea Medical Information Portal (OASIS)', 'Academic Research Information Service (RISS)', 'Scientific and Technological Information Integration Service (NDSL)'. We classified the papers by the year of publishment, the title of journals, the type of study and contents. Results: 1. Clinical studies using oral appliance have been increasingly published since 2011. 2. Most of the studies were published in the Journal of TMJ Balancing Medicine. 3. 33 research papers were divided in to 6 original articles, 27 case reports. But almost presented a low level of evidence. 4. In the content of the studies, oral appliance were used for various diseases such as nervous system, musculoskeletal system and visceral system. Conclusions: We expect that various studies and clinical application will be actively carried out with the development of technology related to oral appliance.

Inpatient Dental Consultations to Pediatric Dentistry in the Yonsei University Severance Hospital (연세대학교 세브란스 병원 내 입원한 환자의 소아치과 의뢰 현황)

  • The goal of this study was to describe dental consultation of pediatric inpatients to the department of pediatric dentistry at Yonsei University Severance Hospital. 391 dental consultations at Yonsei University Severance Hospital referred to pediatric dentistry in the year 2012 were included in this study. Consultations were categorized according to patients' gender, age, chief complaint, referred department and diagnosis. 288 patients (166 males and 122 females) with an average age of 5.9 were referred to the Department of Pediatric Dentistry. 129 cases (33.1%) from Department of Rehabilitation Medicine, 80 cases (20.5%) from Pediatric Hematology- Oncology, 51 cases (13.0%) from Pediatric Cardiology, and 44 cases (11.3%) from Pediatric Neurology. Chief complaints were ranked from oral examination (39.7%), dental caries (14.0%), pre-operative evaluation (12.8%) and others (33.5%); including oral pain, trauma, tooth mobility, orthodontic treatment, self-injury, fabrication of obturator and etc. Dental consultations should be encouraged as dental care and treatment could affect the control of systemic diseases of admitted patients. Pediatric inpatients have been referred to pediatric dentistry for not only comprehensive oral exam but also various chief complaints. The most frequent dental diagnosis made and treatment performed were dental caries and non-invasive/preventive care respectively.

Review of Non-invasive Interventions for Drooling Problems in Children With Cerebral Palsy: Trends and Analysis of Interventions for Drooling (신경발달장애 아동의 침흘림치료를 위한 비침습적 중재방법에 관한 고찰: 경향 및 중재방법 분석)

  • Objective : The purpose of this study was to analyze non-invasive treatments and drooling assessment methods in children with cerebral palsy and developmental disabilities, who drool. Methods : This study searched two hundred papers published in 2005-2019. Forty-four papers were selected based on their abstract and title, and ten papers were finally selected following a secondary search. Results : The PEDro Scale of the selected papers was high with an average of seven points. As a result of analyzing the overall trends, the study participants were primarily patients with cerebral palsy, and recently, the therapeutic intervention of oral sensory exercise was more actively studied than behavioral modification. Studies of behavioral modification and oral sensory exercise intervention methods were found to have differences in participant age and, cognitive level, number of participants, research design, treatment time, and duration. Studies to confirming the frequency and severity of the drooling measurement method were found to be the main factor. Conclusion : This study analyzed typical behavioral modification and oral sensory exercise interventions as examples of non-invasive therapeutic interventions for children with cerebral palsy and developmental disabilities and provided information to help select appropriate therapeutic intervention methods when planning non-invasive therapy using behavioral modification and oral sensory exercise therapy.

Understanding Assessment for Feeding Disorders in Autistic Spectrum Disorders: A Literature Review (자폐 스펙트럼 장애 섭식장애 평가의 이해: 문헌 고찰)

  • Objective : Children with autism spectrum disorder (ASD) commonly suffer from feeding disorders. Major feeding problems include mealtime behavior problems, picky eating, and a lack of food variety can lead to nutritional problems, developmental and social limitations, and stress for the caregivers. A review of the latest literature was conducted to gain an in-depth understanding of assessment tools for feeding disorders in children with ASD. Method : This study analyzed assessments to identify feeding problems in ASD based on previous studies searched through keywords such as ASD, ASD feeding problem, and ASD feeding evaluation. Results : The ASD feeding disorder assessment was divided into direct and indirect assessments. Indirect assessment, in which caregivers measure a child's situation using questionnaires, is mainly used. The assessment of feeding disorders in children with ASD was divided into 1) mealtime behavior, 2) sensory processing, 3) food consumption, and 4) others. Conclusion : As the main feeding disorder characteristics of children with ASD are very diverse, a comprehensive evaluation is necessary but is still limited. Swallowing rehabilitation experts, such as occupational therapists, should apply comprehensive assessment tools based on a basic understanding of the feeding problems, behaviors, and sensations in ASD.

Analysis of Research Trends in Traditional Chinese Medicine Treatments for Pes Anserine Bursitis from 2014 to 2024: Using the Search Results of China National Knowledge Infrastructure (거위발 점액낭염의 중의학적 치료에 대한 2014-2024년도의 연구 동향 분석: China National Knowledge Infrastructure를 중심으로)

  • Objectives The aim of this study is to investigate the research trends in traditional Chinese medicine (TCM) treatment for pes anserine bursitis using the search results from the China National Knowledge Infrastructure (CNKI). Methods We searched for clinical studies on pes anserine bursitis in the CNKI electronic database. We analyzed the publication year, study type, participant demographics, treatment methods and duration, evaluation criteria, adverse events, and the results of each article. Results Seventeen studies published from January 1, 2014 to January 1, 2024 were selected. Among the 17 selected studies, 14 were randomized controlled trials and 2 were case reports. The most frequently used treatment intervention was acupuncture. Nine types of acupuncture were identified, including electro-acupuncture, pharmacopuncture, acupotomy, floating acupuncture, and other new types of acupuncture. The most commonly used evaluation outcome was effective rate. Conclusions Various TCM treatments for pes anserine bursitis were introduced, and we could identify the effectiveness of TCM in treating pes anserine bursitis. However, it is anticipated that more well-designed and high-quality evaluation studies will be required in the future.
