  • This study investigated the awareness and purchase status of the private dental insurance. Self-reported survey was conducted with patients over the age of twenties who visited dental institutions located in Seoul Metropolitan City and Gyeonggi Province. The demographic and health related characteristics of respondents were analyzed, and logistic regression was conducted to examine factors affecting the awareness and purchase of private dental insurance. Because only four years had been passed since the introduction of private dental insurance, the awareness and purchase rate was found to be low. However, the number of people considering subscription due to the economic burden of dental care service was relatively high. Factors affecting awareness were satisfaction of the national health insurance, purchase of private health insurance and private dental insurance, self-perceived dental health status, and smoking. The variables affecting purchase of dental insurance were age, awareness, purchase of private health insurance, smoking, number of visits to dental institution. Because qualitative and quantitative change would be made in the dental care utilization due to the rapidly growing dental insurance subscribers, further studies regarding the trend of purchase rate of private dental insurance and the effect of dental insurance on use of dental institution are needed.

  • The author conducted a dental survey of college girl students and compared with that of college students 20 years and 10 years ago which based on the same criteria. The items of this survey included the prevalence of dental caries, DMFT and the kinds of restored materials. College girl students 2,294 were examined in May 1988, with the recommended criteria and method of W.H.O.. Among them, the data of 2,243 were analyzed and compared with the data of 1968 and 1978. The following results were obtained : 1. The average number of present teeth was $28.86{\pm}1.65$, which does not show much difference when compared to $29.00{\pm}1.94$ of 1968 and $28.97{\pm}1.64$ of 1978. 2. The prevalence of dental caries was 86.22%, which increased in comparison to 83.25% of 1968 and 84.92% of 1978. 3. DMF index was 19.98, which considerably increased in comparison to 14.56 of 1968 and 15.51 of 1978. 4. The number of DMFT was $5.75{\pm}5.04$ per student. Compared to $4.23{\pm}3.88$ of 1968 and $4.49{\pm}3.69$ of 1978, there is a considerable increase. I) The average number of decayed(D) teeth was $1.14{\pm}1.63$, which decreased from $1.16{\pm}1.57$ of 1968 and $2.09{\pm}2.16$ of 1978. II) The average number of missing(M) teeth was $0.23{\pm}0.73$, which also decreased from $0.69{\pm}1.11$ of 1968 and $0.32{\pm}0.79$ of 1978. III) The average number of filled(F) teeth was $4.39{\pm}4.91$, which increased from $2.38{\pm}3.33$ of 1968 and $2.09{\pm}3.17$ of 1978. 5. According to the analysis of filling material, filling baby including amalgam, gold inlay, resin, silicate cement and paladium inlay was 90.5%, which increased in comparison to 72.8% of 1968 and 83.6% of 1978. Among them, amalgam occupied 62.93%, which showed the most increase. On the other hand, crown and bridge was 6.5%, 3.0%, respectively, which showed continuous decrease from 12.1%, 15.1% of 1968 and 8.7%, 7.7% of 1978. • Obstructive sleep apnea syndrome(OSAS) is a common disease in the field of otorhinolaryngology and is characterized by repeated upper airway occlusions occurring during sleep. OSAS can occur due to various etiologies of the nasal, oral, pharyngeal and laryngeal airway in adults. Nasal obstruction can be caused by septal deviation, nasal polyps, concha bullosa, choanal atresia, neoplasms, foreign body, postoperative/post-traumatic synechiae, various rhinitis and so on. There are various kinds of surgical treatment of OSAS including nasal surgery, LAUP, UPPP, surgery of tongue base, tracheostomy and so on, but the effect of nasal surgery on snoring and OSAS is controversial. The authors report the case of a patient who had experienced nasal obstruction, moderate snoring and OSAS and who improved after septoplasty and turbinoplasty.

  • Sialolith is one of the most common pathologic conditions found in the salivary glands. The mechanisms responsible for the formation of sialoliths have not been elucidated so far. In this article, the chemical composition and micromorphology of a sialolith of a 58-year old female patient suffering from chronic sialoadenitis of the submandibular gland was analyzed using scanning electron microscopy (SEM) and energy-dispersive X-ray spectroscopy (EDX). In a SEM evaluation, the highly mineralized amorphous core surrounded by lamellar and concentric structures was revealed, however no foreign body, organic material, or signs of microorganism were observed in the core of the sialolith. EDX analysis showed the central core was composed of only Ca, O and P, and that a high level of C was detected near the central area as well. These results indicated that the inorganic composition of the sialolith was hydroxyapatite crystals, and that inorganic and organic substances existed around the central cores. This study suggests that the sialolith was composed mainly of hydroxyapatite crystals and the formation of the nucleus of the sialolith in the submandibular gland duct was secondary to sialadenitis, which favors the growth of an inorganic crystalline nucleus.

  • Aims: The purpose of this study was to evaluate the relationship between temporomandibular joint pain and the relative signal intensity (RSI) of retrodiscal tissue on T1-, and T2-weighted MRI images. Materials and Methods: This study was based on 122 TMJs of 61 patients who complain of TMJ pain in only one side but were revealed to have disc displacement in both TMJs according to MRI findings. The signal intensities of regions of interest (ROIs) in retrodiscal tissues were measured using T1-, and T2-weighted MRI images. The RSIs of retrodiscal tissues were referenced to the signal intensities of the ROIs of brain gray matter. The relationships between the RSI of retrodiscal tissue and joint pain, joint effusion, condylar degenerative change, and degree of disc displacement were examined. In addition, the relationships between joint pain and joint effusion, condylar degenerative change, and degree of disc displacement were examined. Results: On T1-weighted MR images, the painful TMJs showed significantly higher retrodiscal tissue RSI than non-painful TMJs. In addition, there is an association between joint pain and the degree of disc displacement. However, on T2-weighted MR images, the RSIs of retrodiscal tissues didn't show any significant differences with regard to joint pain, joint effusion, condylar degenerative change, and degree of disc displacement. Conclusions: The signal intensity of retrodiscal tissue can be used as a diagnostic marker for painful TMJ. However, the overall results suggest the signal intensity of retrodiscal tissue has a limited diagnostic significance in determining the pathologic status of TMJ.

  • Tooth impaction is defined as a cessation of the eruption of a tooth at the level of the oral mucosa or alveolar bone by any causes. Any tooth in the dental arch can be impacted, but the teeth frequently involved in a descending order are the mandibular and maxillary third molars, the maxillary canines, the mandibular and maxillary second premolars, and the maxillary central incisors. In these teeth, impaction of maxillary incisor occurs in about 0.1-0.5% and major causes are trauma, supernumerary teeth and periapical inflammation of primary maxillary incisor. Delayed eruption of a maxillary central incisor results in midline shift, the space's being occupied by an adjacent tooth and different levels of alveolar height. Treatment options are observation, surgical intervention, surgical exposure and orthodontic traction, transplantation and extraction. These cases were about the patients with delayed eruption of maxillary central incisor. We surgically exposed impacted tooth and guided it into normal position by the orthodontic traction. At the completion of traction, the maxillary central incisor was positoned fairly within the arch and complications such as root resorption were not observed.

  • The aim of this study was to evaluate the tightness of proximal tooth contact(TPTC) using a novel device at rest state on implant prostheses. Ten healthy young adults with class I normal occlusion consented to participate in the study and twenty patients were restored with a total 20 single-implant crowns in the left maxillary and mandibular second molars for 10 single-implant crowns, respectively. Test area were divided by 4 groups. UM describes the contact between the upper natural left first molar and natural second molar; LM the contact between the lower natural left first molar and natural second molar; IUM the contact between the upper natural left first molar and implant second molar and ILM the contact between the lower natural left first molar and implant second molar. The TPTC was measured at rest state in each area. The mean TPTC of the UM, LM, IUM and ILM was 1.48(${\pm}0.44$) N, 1.78(${\pm}0.40$) N, 1.14(${\pm}0.37$) N and 1.30(${\pm}0.32$) N respectively. These results indicate that the TPTC was less between natural tooth and implant prosthesis than between natural teeth.

  • This case report described a technique utilizing a computer-aided design (CAD)/computer-aided machining (CAM) - guided surgical implant placement and prefabricated temporary fixed prosthesis for an immediately loaded restoration. The advantages of CAD/CAM guided implant procedures are flapless, minimally invasive surgery and shorter surgery time. With this technique, less postoperative morbidity and delivery of prosthesis for immediate function would be possible. A patient with an edentulous maxilla received 8 implants in maxilla using CAD/CAM surgical templates. Prefabricated provisional maxillary implant supported fixed prosthesis were connected immediately after implant installation. Provisional prosthesis was evaluated for aesthetics, function during 6 months. Definitive implant supported fixed porcelain fused metal bridges were fabricated.
