THE ERUPTION GUIDANCE OF AN IMPACTED DILACERATED MAXILLARY CENTRAL INCISOR (변위 매복된 상악 중절치의 맹출유도)

  • Tooth impaction is defined as a cessation of the eruption of a tooth at the level of the oral mucosa or alveolar bone by any causes. Any tooth in the dental arch can be impacted, but the teeth frequently involved in a descending order are the mandibular and maxillary third molars, the maxillary canines, the mandibular and maxillary second premolars, and the maxillary central incisors. In these teeth, impaction of maxillary incisor occurs in about 0.1-0.5% and major causes are trauma, supernumerary teeth and periapical inflammation of primary maxillary incisor. Delayed eruption of a maxillary central incisor results in midline shift, the space's being occupied by an adjacent tooth and different levels of alveolar height. Treatment options are observation, surgical intervention, surgical exposure and orthodontic traction, transplantation and extraction. These cases were about the patients with delayed eruption of maxillary central incisor. We surgically exposed impacted tooth and guided it into normal position by the orthodontic traction. At the completion of traction, the maxillary central incisor was positoned fairly within the arch and complications such as root resorption were not observed.

Evaluation of Tightness of Proximal tooth Contact on Implant Prostheses (임프란트 보철수복물에서의 인접치간 접촉강도의 평가)

  • The aim of this study was to evaluate the tightness of proximal tooth contact(TPTC) using a novel device at rest state on implant prostheses. Ten healthy young adults with class I normal occlusion consented to participate in the study and twenty patients were restored with a total 20 single-implant crowns in the left maxillary and mandibular second molars for 10 single-implant crowns, respectively. Test area were divided by 4 groups. UM describes the contact between the upper natural left first molar and natural second molar; LM the contact between the lower natural left first molar and natural second molar; IUM the contact between the upper natural left first molar and implant second molar and ILM the contact between the lower natural left first molar and implant second molar. The TPTC was measured at rest state in each area. The mean TPTC of the UM, LM, IUM and ILM was 1.48(${\pm}0.44$) N, 1.78(${\pm}0.40$) N, 1.14(${\pm}0.37$) N and 1.30(${\pm}0.32$) N respectively. These results indicate that the TPTC was less between natural tooth and implant prosthesis than between natural teeth.

  • This case report described a technique utilizing a computer-aided design (CAD)/computer-aided machining (CAM) - guided surgical implant placement and prefabricated temporary fixed prosthesis for an immediately loaded restoration. The advantages of CAD/CAM guided implant procedures are flapless, minimally invasive surgery and shorter surgery time. With this technique, less postoperative morbidity and delivery of prosthesis for immediate function would be possible. A patient with an edentulous maxilla received 8 implants in maxilla using CAD/CAM surgical templates. Prefabricated provisional maxillary implant supported fixed prosthesis were connected immediately after implant installation. Provisional prosthesis was evaluated for aesthetics, function during 6 months. Definitive implant supported fixed porcelain fused metal bridges were fabricated.

  • In the implant prosthetic procedure, the soft tissue reaction was varied with the material and surface treatment of the abutment. It may be the cause of the peri-implantitis, and hence it can affect the long-term prognosis of the implant prosthesis. Titania and zirconia abutment presented superior biocompatibility and stable soft tissue reaction, while gold alloy abutment showed unfavorable reaction sometimes. A soft tissue reaction can be differed by the surface characteristics even in the same material type. Because rougher surface induces a bacterial attachment, the part contacting a soft tissue should have smooth surface. Additional surface treatment can enhance the cellular response without increasing bacterial attachment. Repeated removal and insertion of the abutment and the shape of the abutment may affect the soft tissue reaction, also. Ultrasonic cleaning and argon plasma cleaning are effective way to clean the retained micro-dust on the customized abutment.

Infection control of dental implant hand drivers using ethanol solution (에탄올을 이용한 치과 임플란트 핸드 드라이버의 감염 관리에 대한 연구)

  • Purpose: The purpose of this study was to study the effects of the utilization of ethanol solution in infection control of dental implant hand drivers, a common practice in dental prosthodontic clinics. Materials and Methods: Infection control methods were divided into two groups. One swabbed with 83% ethanol gauze and the other immersed in 83% ethanol solution for 30, 60, 90, 120, 150, 180 and 300 second intervals after inoculation of the dental implant hand drivers with Staphylococcus aureus. After measuring the number of colony forming units and analyzing the optical density, the effects of infection control in the experimental group were compared with the positive control group without infection control after inoculation with bacteria and the negative control group without inoculation with bacteria after sterilization. Results: The number of colony forming units and optical density analysis showed a statistically significant difference compared to the positive control. On the other hand, there was no statistically significant difference between the negative control and the group immersed in the 83% ethanol solution for more than 150 seconds. Conclusion: It is recommended to use the ethanol solution as a pre-cleaning process before sterilization, since the intermediate-level disinfection method using ethanol solution alone for the infection control of the dental implant hand driver cannot clinically secure the sterility.

A Study on the Factors Giving Effects to Patient's Satisfaction in the Selection of Implant Operation (임플란트 시술선택에 있어 환자 만족도에 영향을 미치는 요인)

  • The study performed questionnaire to the patients receiving implant operation, who visited dental clinics and hospitals(2 hospitals and 5 clinics) from Jul. 1, 2009 to Sep. 12, 2009 in order to study the factors giving effects to patient's satisfaction in the selection of implant. 145 questionnaire papers collected from patients were analyzed with SPSS12.0 program. The result revealed that the satisfaction with implant operation was usually normal when implant was operated because of tooth extraction due to dental caries and periodontal disease and it was usually high when implant was operated because of wound(P<0.05). When considering each cause of tooth extraction, the satisfaction with pronunciation function was usually normal(P<0.001), the satisfaction with functional part including foreign body sense was very low when the cause of operation was periodontal disease(P<0.01). The satisfaction with hospital environment depending on the cause of hospital selection was usually high when hospital was selected because of doctor's fame. It was low when hospital was selected because it was close to home(P<0.001). The satisfaction with the number of hospital visits depending on the number of implanted teeth was usually low when many teeth were implanted(P<0.01). The analysis on the satisfaction with tooth shape and color showed that the satisfaction was high when '1~2' teeth and 'more than 6' teeth were implanted(P<0.05). In order to improve patient's satisfaction with implant, it is necessary to recognize individual's oral status and request correctly. It is determined that proposal of operation period, number of hospital visits, function and expected aesthetic satisfaction degree will contribute to the improvement of mutual reliance between doctor and patient and post-operation satisfaction degree.

Changes of Pre-Auricular Cutaneous Blood Flow and Skin Temperature after Dry Heat Therapy and Moist Heat Therapy (건열요법과 습열요법 적용후 전이부 표층부 혈류량과 피부온도의 변화)

  • The purpose of this study was to assess the effects of the superficial heat therapy on the cutaneous blood flow and the skin temperature at pre-auricular region. Two types of the superficial heat therapy-moist hot pack & infrared lamp- were applied to 20 healthy subjects(male: 10, female: 10). For each subject, the two parameters of cutaneous blood flow and skin temperature were measured before and after heat therapy, using laser doppler flowmetry(LDF). The author analyzed the differences of the effects between the two therapies and also characteristics of responsiveness between the two parameters. The results were as follows : 1. The two parameters were significantly increased after both superficial heat therapies. 2. Skin temperature showed a maximum peak immediately after both superficial heat therapies, but cutaneous blood flow showed a maximum peak 4 minutes after both superficial heat therapies. 3. Increased cutaneous blood flow after application of moist hot pack lasted longer than infrared lamp. 4. Increased skin temperature after both superficial heat therapies lasted for 60 minutes, but increased skin temperature after infrared lamp decreased more rapidly than moist hot pack. 5. Amount of changes in cutaneous blood flow after infrared lamp was larger in female than in male, but no significant gender difference was found since 20 minutes after infrared lamp. Both moist hot pack and infrared lamp showed favorable effectiveness in raising cutaneous blood flow and skin temperature. Moist hot pack was slightly superior in maintaining this effect.

Correlation of Korean Elderly Dental Health Capacity and Preferred Foods (한국 노인의 치아건강도와 선호식품과의 관련성)

  • The purpose of this study is to examine whether any correlation exists between the dental health capacity and preferred foods of Korean senior citizens over the age of 65 years. The 5th Korean National Health and Nutrition Survey were used. Tissue health index (T-health), Sound teeth (ST), Functioning teeth index (FS-T), Present teeth (PT), and Missing teeth (MT) were used as variables to assess the dental health capacity of the elderly. Preferred foods of the elderly included 63 foods that were categorized as cereals, pulses and roots, meat and poultry, fish, vegetables, sea algae, fruits, milk and dairy products, cream and sugar, and other foods. For data analysis, the weighted average was taken into consideration to generate planning files, and then complex sample analysis were conducted. For statistical analysis, frequency analysis, t-test, one-way ANOVA, and compound specimen linear regression analysis were conducted. T-health score was significantly high in the group with high preference for cereals, fruits, and other foods. In terms of age and economic status, 65~69 years, 70~74 years, and mid to low range in the income ranges scored high in T-health. ST score was significant in the group that preferred cereals, other foods; the corresponding demographic profiles represent 65~69 years, 70~74 years, and the mid-range income communities. FS-T was significant in relation with a preference for fruits, creams and sugars, other foods; the scores were also high for 65~69 years, 70~74 years, and mid-low to low income groups. PT and MT were significant in the group that preferred cereals and fruits; the same applied for 65~69 years, 70~74 years, and mid-low to low income individuals (p<0.05). Food preferences seemed to vary depending on the dental health state of the elderly, and the dental health state of the elderly may act as a risk factor for nutritional imbalance.

Rehabilitation with implant-supported fixed dental prostheses using digital duplication technique on customized artificial tooth, interim denture and implant surgical template: A case report (디지털 복제로 만든 맞춤형 인공치로 제작한 임시의치와 임플란트 수술용 템플릿을 통한 임플란트 지지형 고정성 보철 수복 증례)

  • Bone and soft tissue conditions are important for successful implant treatment. But, the placement itself is also very important. Implants which is installed in the wrong position result in the biological, esthetical and mechanical problems. In order to place an implant in the correct position, the final restoration and diagnostic wax-up should be considered prior to the surgery. If the artificial teeth for the interim denture are directly transferred from the diagnostic wax-up, the operator can try the form of diagnostic wax-up in the mouth. If the surgical template is produced by duplicating the interim denture, the implant can be placed in the planned position. In this case, the polymethyl methacrylate (PMMA) artificial tooth was precisely milled by the digital duplication of diagnostic wax-up. And interim denture was fabricated by using these milled teeth. After the patient adapted for a sufficient period, the implant was placed at the planned position with surgical template produced by duplicating the interim denture. After confirming sufficient osseointegration, the final prostheses were made to reflect the shape of diagnostic wax-up. Through this procedure, the satisfactory functional and esthetic outcome could be acquired.

DENTAL TREATMENT OF A PATIENT WITH LOWE SYNDROME UNDER GENERAL ANESTHESIA: A CASE REPORT (Lowe syndrome 환아의 전신마취 하 치아우식 치료: 증례보고)

  • Oculo-cerebro-renal syndrome, known as Lowe syndrome, is an X-linked gene disorder characterized by congenital cataracts, brain abnormalities and renal dysfunction. Tooth crowding, taurodontism, delayed eruption in the permanent dentition and over-retained primary teeth with ectopic eruption of the permanent teeth have been reported as dental findings. Because of the high incidence of poor cooperation, patients with Lowe syndrome have difficulties in maintaining good oral hygiene, which may require dental treatment. We present a case of dental treatment for the uncooperative child with Lowe syndrome under general anesthesia. A 4-year-old, 11.2 kg boy with Lowe syndrome visited Seoul National University Dental Hospital for gingival swelling. The patient had multiple caries requiring dental treatment. Because of his past history of malignant hyperthermia during inhalation induction, anesthesia was induced and maintained with total intravenous anesthesia (TIVA) after medical consultation. Dental restorative treatments were successfully performed and no complications were observed during and after the procedure. Safe and effective dental management of the patients with Lowe syndrome could be performed with the help of general anesthesia and careful monitoring.
