Convergence between Dental Caries and Obesity in Adolescents: The 6th Korea National Health and Nutrition Examination Survey (청소년 비만과 치아우식증과의 융합연구: 제6기(2013-2015년) 국민건강영양조사를 바탕으로)

  • This study aims to assess the relationship between obesity and dental caries in adolescents. Data were used from the 6th Korean National Health and Nutrition Examination Survey (2013-2015). Data analysis was performed using the SPSS Ver 22.0 program, and complex sample multivariate logistic regression analysis was performed to analyze relationship dental caries and obesity according to gender in adolescents. The subjects of this study were 1,243 adolescents aged 13 to 18 years. The relationship between obesity and dental caries in adolescents according to gender was not confirmed after adjusting for confounding variables. Therefore, obesity prevention programs and oral care programs for adolescents will be necessary.

A study on oral health state of immigrant workers with DMFT-index (외국인 이주 노동자의 구강건강관리 실태 및 우식경험영구치지수)

  • This document is about immigrant worker who use counseling office for human rights in the Daegu Gumin Church. We researched their oral health state so that we can understand their situation and support them properly. General characteristics of study subjects, habits related to oral health, the oral examination and treatment clinic, participated in oral heath education program were studied by designed administered questionaire. And decay, filling(treatment teeth), missing tooth(lost teeth by dental-caries)was counted by oral examination, and calculated DMFT-index. This study was done from the July, 9, 2006 to Aug. 8, 2006. In a total of 289 immigrant worker, 77.9% of them were men and 22.1% of them were women. 55.7% of their salary was from 1,000,000 won to 1,500,000 won and most of them were working for a fiber industry. Many of them are living in korea for more then three years. DMFT index for men was 2.77 and for women was 4.06 so average of DMFT index was 3.06. 46.7% of them said that they are healthy in oral health state. The question for having difficulty using dental clinic in korea, 65.1% of them said "it is difficult". First reason was a communication problem and second was time. Most of them didn't have a oral health education but 85.1% of them said that they are looking forward to attending oral health education. Immigrant worker had better DMFT index then that of korean blue color worker. But still it is quite difficult for them using dental clinic in korea also cost. It is necessary to support them properly that medical insurance system, medical facilities of quality, medical insurance subscriber beside, made by their language, manual for them. At once, medical service improvement a policy is necessary for immigrant worker in korea.

Comparison of oral health index between adequate and inadequate dental care following preventive scaling (치과치료 미충족군과 충족군의 예방적 스케일링 적용에 따른 구강건강지수 비교)

  • Objectives: The objective of the study was to investigate oral health index between adequate and inadequate dental care following preventive scaling in regular dental check-up and management. Methods: The subjects in Busan were assigned to two groups including inadequate dental care (140 patients) and adequate dental care (88 patients) after preventive scaling from January 2014 to June 2015. The study instruments included oral health knowledge, awareness, practice, and community periodontal index of treatment needs (CPITN). Results: CPITN was significantly different between the two groups (p<0.05). Positive correlations were found in oral heath knowledge and awareness (r=0.462, p<0.05) before application of preventive scaling. There was a close correlation in oral health awareness and practice (r=0.205, p<0.05) before application of preventive scaling. After application of preventive scaling, oral health knowledge and awareness showed positive correlation (r=0.643, p<0.05). After application of preventive scaling, there was also a close correlation between oral health knowledge and practice (r=0.453, p<0.05). Oral health awareness and practice (r=0.512, p<0.05) showed positive correlation after the application of preventive scaling. However, oral health practice and CPITN (r=-0.189, p<0.05) showed a negative correlation. Conclusions: Practice made periodontal condition improve. To promote oral health practice in the group without adequate dental care, it is necessary to pay attention to adequate dental care and preventive scaling through education by dental hygienists.

Effect of dietary quality on periodontal diseases in Korean adults: the Korea National Health and Nutrition Examination Survey (2016-2018) (한국 성인의 식생활의 질이 치주질환에 미치는 영향: 국민건강영양조사 제7기(2016-2018) 자료 활용)

  • Objectives: This study investigated the relationship between the Korean healthy eating index (KHEI) and periodontal disease in the Korean adult population. Methods: The data used in the analyses were obtained from the seventh Korean National Health and Nutrition Examination Survey (2016-2018). Data were analyzed by chi-square tests and t-test. Multiple regression analysis was also performed to assess the association between KHEI and periodontal disease. Statistical significance was set at p<0.05. Results: Multiple logistic regression analysis adjusted for socioeconomic variables showed that medical and health behavior variables were significantly related to the KHEI 1 (<63.7, odds ratio [OR]: 1.23, 95% confidence interval [CI]: 1.03-1.46), KHEI 2 (63.7-79.9, OR: 1.14, 95% CI: 0.97-1.34), and risk for periodontal disease. Conclusions: The results showed a significant association between the KHEI and periodontal disease in the Korean adult population.

Effects of the Dental Preventive Health Services for Children on Oral Health-Related Quality of Life (서울시 학생치과주치의 사업이 구강 건강과 관련된 삶의 질에 미치는 영향)

  • The purpose of this study was to investigate the effects of public preventive dental healthcare services named the "Dental Preventive Health Services for Children" (DPHSC) on oral health-related quality of life (OHRQoL) in Korean children. Of the total 20,802 beneficiaries in 2018, 110 10-year-old elementary school students were randomly selected and were requested to complete a survey using the Korean version of Child Oral Health Impact Profile (COHIP) questionnaire before and after participating in DPHSC. Overall, 107 participants completed the pre- and post-survey. COHIP scores and clinical results such as numbers of decayed and missing teeth, malocclusion, and oral hygiene status from dental examination were statistically analyzed and the reliability and the validity of the responses were verified. There was a significant clinical increase in mean overall COHIP scores following DPHSC (p < 0.001, effect size 1.05). Children with decayed and missing teeth demonstrated poor OHRQoL. In conclusion, DPHSC can improve the OHRQoL of 10-year-old children.

Research on the status of children's dental health following mothers' dental health knowledge and behavior (일부 어머니의 구강보건지식 및 행태에 따른 자녀의 구강건강상태에 관한 연구)

  • Objectives : This research analyzed the general characteristics that closely affect the children's dental health and the mothers' dental health knowledge and dental health behavior to identify the correlation of the latter with the status of children's dental health in order to provide the basic data for the development of dental health business that would target children. Methods : Research subjects were selected arbitrarily from three nursery schools located in Busan metropolitan city, targeting 186 children between the ages of four to six and their mothers. The children were subjected to dental Inspection, and the status of their dental caries was studied whereas the mothers were subjected to the surveys on the general characteristics, dental health knowledge and dental health behavior. Results : 1. Level of mothers' dental health knowledge is higher when the mothers' educational level is higher(p=0.02) and when the household's monthly income is higher(p=0.009). 2. When the level of mothers' dental health knowledge is higher, children tended to brush their toothbrushing using proper method(p=0.025). Moreover, when the level of mothers' dental health knowledge is higher, they tended to take their children to dental clinic mostly for preventive measures than for treating cavity(p=0.023). 3. When the level of mothers' dental health knowledge is higher, children's dmft index was significantly low(p=0.02). When the mothers use fluoride-containing toothpaste, children's children' dmft index was even lower(p=0.02). 4. As the children tended to brush their teeth more often, dmft index was lower(p=0.003). When the reason that the children visited dental clinic was more to prevent, than to treat cavity, dmft index was even lower(p=0.000) Conclusions : When the above mentioned results are summarized, it is possible to know that the mothers' dental health knowledge and dental health behavior significantly affect children's dental health. Accordingly, it is necessary to develop maternal and child dental health program that factors in both the mothers and children in order to prevent children's dental caries and to increase their dental health, and continued care is required to discover and treat dental caries early on.

Research on plaque removal by sonic toothbrush for patients with a fixed orthodontic appliance (고정식 교정장치 부착환자에서 음파칫솔의 치면세균막 제거 효과에 관한 연구)

  • The purpose of this research is to evaluate and compare the plaque removal ability of sonic brushing and manual brushing in order to present a method for continuous and effective oral hygiene control during orthodontic treatment. Nonextracted adolescent patients (male: 23 persons, female: 37 persons) and adult patients (male: 15 persons, female: 45 persons) outfitted with a fixed orthodontic appliance from their incisors to second molar, in both the maxilla and mandible, were divided into manual brushing and sonic brushing groups. A Philips oral healthcare HX-4401 sonic toothbrush was used, and the application of brushing for the two groups was standardized at two minutes. After brushing, the plaque score was measured with a modified Wilkin's PHP. The following results were achieved after comparing each group's plaque score. 1. In patients with a fixed orthodontic appliance when age was not accounted for, the application of sonic brushing exerted a beneficially decreased effect on the plaque score. (adolescents : p<0.01, adults: p<0.05) 2. When comparing the adolescent and adult groups in regard to plaque score, the adolescent group showed a more decreased effect. 3. When using the same toothbrush type, there was no difference between the adolescent and adult groups.

A Survey of Oral Health State, Oral Health Awareness and Dental Treatment perception for Middle Schoolers in Hwaseong (화성시 중학생의 구강건강 실태, 인식 및 치과치료에 관한 개별반응 조사)

  • The purpose of this study was to examine the oral health state, oral health awareness and oral health behavior of middle school students in a bid to raise concerns about oral health problems in a local community and help ensure the efficiency of oral health programs. The subjects in this study were 662 seventh graders in 14 different middle schools located in the city of Hwaseong. The findings of the study were as follows: Frist, In terms of indices related to dental caries, DMFT index numbered 2.0. Secondly, as to subjective oral health state, 22 percent of the boys and 10.9 percent of the girls found their own oral cavity to be healthy, and the gap between them and the others was statistically significant different(p=.000). Thirdly, in regard to tooth-related habits, the girls brushed their teeth more often than the boys(p=.000). Fourthly, for snack-intake habits, the largest number of the boys ate between meals once(42.1%), and the second largest group of them did it two or three times(31.3%). Fifthly, regarding how they felt about dental treatment, the greatest number of the boys and girls replied there's nothing special when they were asked about their mood in case of seeing a dentist next day. The second largest group answered they would get a little nervous. As to mood while waiting for treatment, the greatest number of the boys and girls answered they would get a little nervous. In regard to mood while lying on a dental chair, the largest group replied they would be nervous. Concerning mood while lying on a dental chair for scaling, most of them answered they would get nervous.

A study on the comprehensive oral prophylaxis care at G university (포괄적치면세마처치 사례연구)

  • The purpose of this study was to provide basic information for improving oral health and dental hygiene through comprehensive dental hygiene care. The subjects in this study were 54 patients who had been treated for 4 weeks in clinic of the Dental Hygiene Department of G University under comprehensive dental hygiene control and care and measured their oral condition, intensity of oral malodor, Simplified Oral Hygiene Index (S-OHI), and Modified Personal Hygiene Performance Index (PHP-M), and surveyed their oral health behavior and oral health consciousness. The results of the study were as follows: 1. The intensity of malodor decreased by 16.49ppb from 75.33ppb on the first visit to 50.84ppb on the last visit when the comprehensive dental hygiene care had been finished. 2. As to change in S-OHI according to gender between the first visit and the last visit for comprehensive dental hygiene care, S-OHI decreased from 2.89 on the first visit to 1.16 on the last visit, and the difference was statistically highly significant. 3. As to change in PHP-M index by tooth according to gender and age between the first visit and the last visit, PHP-M index of the first 6 teeth decreased very significantly from 0.50 on the first visit to 0.34 on the last visit. 4. As a whole, significant improvement was observed in every tooth. According to gender and age, PHP-M index for Tooth No. 26, 36 and 32 decreased significantly in men and women, and those aged under 30 and those aged 30 or over, but for Tooth No. 13 and 15 and 44 it decreased significantly only in men and those aged 30 or over, and for Tooth No. 44 only in men. As presented above, the oral health behavior and oral health consciousness of the visitors to the oral prophylaxis practice room are very important, and it is necessary to enhance their interest in and knowledge of how to improve oral health. Furthermore, comprehensive dental hygiene care required for improving the visitors' oral health.

DENTAL MANAGEMENT OF A PATIENT WITH MUSCULAR DYSTROPHY UNDER GENERAL ANESTHESIA: CASE REPORT (근이영양증(muscular dystrophy) 환자의 전신마취 하 치과치료 : 증례보고)

  • Muscular dystrophy (MD) is a heterogeneous group of inherited neuromuscular disorders, characterized by progressive muscle weakness. Severity of the disease ranges from mild to severe, and the disease is mostly caused by mutations in a number of genes. These genetic mutations cause lack of proteins which are essential for muscle cell stability. Muscle fibers are gradually replaced by fat and fibrous tissue. The muscles of the head and neck are affected in several types of MD that manifest as altered craniofacial morphology and dental malocclusion. A 3-year-10-month old, 15.0 kg boy with MD presented to Seoul National University Dental Hospital, Seoul, South Korea because of extensive carious teeth. A number of dental caries in primary dentition were identified during clinical oral examination. Due to dental anxiety and underlying systemic disease, general anesthesia was considered. General anesthesia was induced and maintained with intravenous anesthetics, propofol and remifentanil. Caries treatments - resin restoration, pulpectomy, zirconia crown restoration, stainless steel crown restoration - were performed. Under general anesthesia, successful dental procedure was done. Total intravenous anesthesia (TIVA) was performed instead of inhalation anesthesia in order to avoid risk of complications such as malignant hyperthermia and life-threatening rhabdomyolysis. With decreasing muscle function, plaque control becomes more difficult and leads to gingivitis. Especially, the open-mouth posture worsens gingivitis and can leads to malocclusions and problems in swallowing. Regular and periodic dental care is essential for maintaining oral health for patients with MD.
