Applying the New Technology for Making Pontic Ridge Lap in Posterior Bridge Restoration (대체 신기법을 적용한 구치부 교의치 pontic ridge lap 제작방법)

  • The purpose of this study is to investigate the production method of posterior bridges pontic ridge lap type which prevents the infection in bridge pontic base and is able to cleanse itself, in the process of producing final prothesis that maintains healthy mucous membrane of oral cavity and interproximal papilla, minimizing diastema, is aesthetic and has no effect on pronunciation. New technology is applied to make optimal pontic base which prevent inflammation and clean itself and its products were clinically evaluated in 10 places of dental clinics in busan and gyeongnam. The making of posterior 3 unit bridge pontic base, it was presented as the new technology of forming ridge lab type and to carry out clinical validation, existing conventional method and the new technology were compared. Pontic base made with the existing conventional method cause infection and other periodontal disease by 96% but the pontic base made with the new technology cause infection and other periodontal disease by 3%. Remains of food cause infection and other periodontal disease 100% by the existing conventional method and 91% by the new technology, showing a distinct difference. However, after a gargle, the new technology had low 13%. Additionally, the pontic base made with the existing conventional method showed 71% of chance, the new technology method showed 8% of chance in terms of self-cleansing.

  • Osteomyelitis means inflammation of the bone marrow. It usually begins in the medullary cavity, involving the cancellous bone; then it extends and spreads to the cortical bone and eventually to the periosteum. The cause is usually thought to be microbiological. But there still are factors that predispose to produce a possible bone infection such as injuries, syphilis, actionomycosis, chronic kidney failure, alcoholism, malnutrition, radiotherapy, and chemotherapy. Treatment of modalities have been directed toward eradicating microbes and improving circulation in the early stage. In the case presented, surgical debridement and IV antibiotics were the treatment of choice. Osteomyelitis in children is mainly affected in the mandible. And in childhood, the mandibular condyle is regarded as an important center of mandibular growth. Therefore, in young patients, osteomyelitis involving this region may cause a restraint of mandibular development, resulting in facial asymmetry. So diagnosis in the early stage is important in child with osteomyelitis. Recently, we have encountered an interesting case of osteomyelitis of the mandibular condyle in 9-year-old boy. So we present the case and review the literature about osteomyelitis.

The Effects of Corticosteroid Solutions in OLP Patients (구강편평태선에 대한 스테로이드 가글의 효과비교 연구)

  • Oral lichen planus(OLP) is a chronic inflammatory disease with cell-mediated immune responses, but the exact cause is unknown. The treatment aim of OLP is not complete cure but to alleviate symptoms. In this study, two kinds of corticosteroid gargling solutions used for comparing the effects. From 2002 to 2010, 180 patients diagnosed with oral lichen planus and received topical steroid therapy in the Pusan National University Dental Hospital. Each of two types of solution contained dexamethasone (dexamethasone disodium phosphate) and prednisolone ($solondo^{(R)}$). A period of relief of symptoms and recurrence was recorded. The group using solution containing dexamethasone(dexa gargle) was prescribed to 33 patients(25 female, 8 male) and another group containing prednisolone (solon gargle) included 147 patients (114 female, 33 male). The effect of dexa gargle seemed faster than the solon gargle. There was no significant difference for recurrent rate between the groups using dexa and solon gargle.

  • We selected 100 patients necessitating the surgical or non-surgical treatment among the patients who visited our hospital. We administered the newly developed non-steroid, PROFENID capsule (25mg per capsule), to the selected patients Via per OS and obtained the following results. 1) Cases of 56% at about 15 minutes after the administration of Profenid capsules, 35% at about 30 minutes after the administration of Profenid capsules, 35% at about 30 minutes and 6% at 60 minutes or more presented the initial effects of that drug respectively. 2) In the patients with the comparatively heavy swelling, the appearances of the effect of that drug were relatively delayed. 3) Sexual difference was not seen in that drug effect. 4) Untoward effects of that drug seemed to be not found in this case study.

Synergistic Effect of Methanol Extract of Salvia Miltiorrhiza and Antibiotics against Dental Caries Pathogens (치아우식증유발세균에 대한 단삼 메탄올추출물과 항생제와의 병용효과)

  • Salvia miltiorrhiza Bunge (S. miltiorrhiza) is a traditional Korean medicine that is commonly used for the treatment of inflammatory diseases such as edema, arthritis, and hepatitis. The present study investigated the antimicrobial activity of methanol (MeOH) extract of S. miltiorrhiza roots against oral bacteria using broth the microdilution method and the checkerboard and time-kill methods evaluated the synergistic effects of treatment with antibiotics. The MeOH extract was demonstrated as a higher antibacterial activity (MICs, 8 to $64\;{\mu}g/mL$; MBCs, 16 to $64\;{\mu}g/mL$) against all tested oral bacteria. Additionally, the extract was observed to have a synergistic effect with ampicillin or gentamicin. A time-kill study evaluating the effects of the extract indicated that the extract treatment in combination with ampicillin or gentamicin showed rapid bactericidal activity. The results suggest that MeOH extract of S. miltiorrhiza could be employed as a natural antibacterial agent against dental caries.

  • Dermatomyositis (DM) is an idiopathic inflammatory connective tissue disorder and a systemic autonomic immune disease which shows a progressive muscle weakness and characteristic rash. It is identified by a characteristic rash accompanying, or more often preceding muscle weakness. Pathognomonic skin lesions are a blue-purple discoloration on the upper eyelids with edema (heliotropic rash), a flat red rash on the face and upper trunk, and erythema of the knuckles with a raised violaceous scaly eruption (Gottron's papule). The myopathy represents inflammatory and degenerative changes primarily affecting proximal muscles. DM often involves GI tract and respiratory system with as risk of 15-25% internal malignancy. It's managed with sun protection since muscle weakness as well as a rash could be aggravated by sun exposure. Systemic corticosteroid is an initial therapy and other immunosuppressive agent has been used as alternatives. Facial muscles are unaffected and masticatory muscles are rarely affected in DM. We present trismus close to muscle contracture in a patient with DM. Therefore, it needs continuous mouth-opening exercise to prevent progressive muscle contracture and to ensure normal mouth opening.

  • Behcet's disease (BD) is a systemic vasculitis characterized by recurrent oral and genital ulcers, and ocular inflammation, and which may involve the joints, skin, central nervous system and gastrointestinal tract. Although the exact pathogenesis for BD is not completely understood, it has been suggested that the disease is triggered in genetically susceptible individuals by environmental factors, such as microbial agents. It is noted that multiple genes, including MHC (major histocompatibility complex) and non-MHC genes, are implicated in the pathogenesis of BD. This study tries to determine whether IL-6 gene polymorphisms are associated with susceptibility to Behcet's disease in Koreans. Gene polymorphisms were typed by VNTR (variable number of tandem repeat), RFLP (restriction fragment length polymorphism), DHPLC (denaturing high performance liquid chromatography).There were no evidences for genetic association conferred by the IL-6prom polymorphism. However, significant differences in the IL-6vntr genotype and allele frequencies were found between patients with BD and controls. The IL-6vntr*C allele appeared to be an additional susceptibility gene to Korean BD. Further studies in other populations and gene are required to confirm these results.

Effects of Shiitake mushroom extract on antimicrobial activity against periodontopathogens and inflammatory condition of human gingival fibroblast (치주 질환관련 세균의 항균 및 세포 염증에 대한 표고버섯 추출물의 효과)

  • Purpose: The purpose of this study was to investigate antimicrobial activity of extracts from shiitake mushroom against periodontopathogens and its cytotoxicity for human gingival fibroblast. Materials and Methods: Shiitake mushroom was soaked in water and acetone, and the supernatant was dried to collect its extract. The susceptibility of periodontopathogens for the extracts was investigated. Human gingival fibroblast was treated with the extracts, and the cell viability was measured CCK-8 solution. Results: The water extract from shiitake mushroom significantly reduced the growth of periodontopathogens at 2.5 mg/ml (P < 0.05). The acetone extract significantly inhibited the growth of Porphyromonas gingivalis and Tannerella forsythia at 0.32 mg/ml and Treponema denticola growth at 0.64 mg/ml (P < 0.05). The cytotoxicity of the extract was shown at a concentration of 2.5 mg/ml. The extracts with a concentration of 1.25 mg/ml appeared to be reduce cell viability after 4 h. Conclusion: The extracts of shiitake mushroom have antimicrobial activity against periodontitis-causing bacteria and relieving inflammation. Therefore, the extracts may be a candidate for preventing and treating periodontal disease.

Anti-inflammatory and Antioxidative Effects of Lotus Root Extract in LPS-PG-Stimulated Human Gingival Fibroblast-1 Cells (치주염 원인균 LPS-PG로 유도된 인체 치은섬유아세포에서 연뿌리 추출물에 대한 항염증 및 항산화 효과)

  • Gingival inflammation is one of the main causes that can be related to various periodontal diseases. Human gingival fibroblast (HGF) is the major constituent in periodontal connective tissue and secretes various inflammatory mediators, such as nitric oxide (NO) and prostaglandin E2 (PGE2), upon lipopolysaccharide stimulation. This study is aimed at investigating the anti-inflammatory and antioxidative activities of Lotus Root extract (LRE) in Porphyromonas gingivalis derived lipopolysaccharide (LPS-PG)-stimulated HGF-1 cells. The concentration of NO and PGE2, as well as their responsible enzymes, inducible NO synthase (iNOS), and cyclooxygenase-2 (COX-2), was analyzed by Griess reaction, ELISA, and western blot analysis. LPS-PG sharply elevated the production and protein expression of inflammatory mediators, which were significantly attenuated by LRE treatment in a dose-dependent manner. LRE treatment also suppressed activation of Toll-like receptor 4 (TLR4)/myeloid differentiation primary response gene 88 (MyD88) and nuclear factor-κB (NF-κB) in LPS-PG-stimulated HGF-1 cells. In addition, one of phase II enzyme, NAD(P)H quinone dehydrogenase (NQO)-1, and its transcription factor, Nuclear factor erythroid 2-related factor 2 (Nrf2), were significantly induced by LRE treatment. Consequently, these results suggest that LRE ameliorates LPS-PG-induced inflammatory responses by attenuating TLR4/MyD88-mediated NF-κB, and activating NQO-1/Nrf2 antioxidant response element signaling pathways in HGF-1 cells.

  • Infections involved with the oral and maxillofacial area are associated with various anatomical structures. If the proper treatment is not done in an immediate period, the infections will be quite fatal. The causes of the infections are numerous, but the most common cause of odontogenic infections in children is a dental caries. It is known to lead to some kinds of diseases such as periapical abscess, cellulitis, osteomyelitis, Ludwig's angina, toxic shock syndrome and so on. The common pathogenic sequence of fascial abscess is a necrotic pulpal inflammation in the form of dentoalvelor abscess which spreads over and gradually penetrates into the fascial membranes through the cortical bones and finally contracts the potential fascial spaces. If the infections of oral maxillofacial area were penetrated into the surrounding soft tissues, then they would diffuse into the directions of the least tissue resistance along with the connective tissues and the fascial spaces. These infections can be properly cured by tooth extraction, endodontic therapy, surgical treatment including Incision & drainage and antibiotics. The purpose of the cases is to report the satisfactory treatment results in the patients derived from the canine fascial space abscesss or buccal fascial space ones of the odontogenic origin.
