Application of $CO_2$ laser in Minor Surgery of Oral Soft Tissue : Case Reports (연조직 소수술에서 $CO_2$ 레이저의 적용 증례)

  • Conventional surgical therapy for oral soft tissue includes the use of scalpel, diathermy, cryotherapy and electrosurgery. But, these therapies have some surgical problems. Nowadays, laser surgery can be considered as the another option for conventional surgical therapy. Compared to conventional surgical therapies, advantages of laser therapy include maintenance of sterile conditions, promotion of wound healing, reduction of bleeding, less instruments, post operative pain reduction, less scar, saving cost by using fewer materials, staff and time. Carbon dioxide ($CO_2$) laser uses gaseous medium, and has long wavelength about 10,600nm. The first advantage of $CO_2$ laser for surgical treatment of oral soft tissue is hemostasis and visibility improvement by making relatively dry field. These case reports are about cases of minor surgery of oral soft tissue using $CO_2$ laser, and emphasize advantages of laser compared to conventional surgical therapies.

Quality of Life in Korean Patients with Burning Mouth Syndrome (한국인을 대상으로 한 구강작열감증후군 환자의 삶의 질 평가)

  • The purpose of this study was to evaluate the individually perceived quality of life in Korean patients with BMS using two questionnaires : the Medical Outcome Short Form Health Survey Questionnaire (SF-36) and the Oral Health Impact Profile (OHIP-49). This cross-sectional study included sixty subjects diagnosed with BMS and sixty healthy subjects as controls. All subjects in this study completed two questionnaires: the Medical Outcome Short Form Health Survey Questionnaire (SF-36) and the Oral Health Impact Profile (OHIP-49), which had been translated into Korean and subsequently validated for use in Korea. All of the subscales in the SF-36 exhibited significantly lower scores in BMS patients than control groups. Comparison of the mean SF-36 scores between the two groups revealed the greatest differences to be for the subscales of physical pain and role emotional (role limitations due to emotional problems). The mean score on each subscale of the OHIP-49 was significantly higher in BMS patients than control groups. Comparison of the mean OHIP-49 scores between the two groups revealed the greatest difference to be for the subscale of physical pain. These findings demonstrated that BMS had an impact on various components of a patient's quality of life. BMS patients exhibited more impaired results and a poorer quality of life than control groups.

A Case Report of the Allopuinol-Associated Angiokeratoma in the Oral Mucosa (Allopurinol과 연관된 구강내 혈관각화종(Angiokeratoma)의 증례보고)

  • Angiokeratoma is a cutaneous vascular disorder that occurs at any sites as trunk,extremities, fingers and toes etc. Although solitary or multiple cutaneous lesions have been reported, oral lesions have been very care. A 72-year-old man who had an exophtic, essile mass with dark red, black colored which located on buccal mucosa, was treated with excisional biopsy. He had no specific systemic history except for the medication of allopurinol, for treatment of gout since 10 years. Final diagnosis was determined as angiokeratoma by evaluation of clinical and histopathological finding, and the lesion has not been recurred for two months by decrease of allopurinol. It has been emphasized that the relationship between oral mucosal disease and the complication of allopurinol. Allopurinol is widely used for gout treatment, which we will report a case on the allopurinol-associated angiokeratoma in the oral mucosa.

Reactive Hyperplasia of Intraoral Soft Tissue by Mechanical Irritation (기계적 자극에 의한 구강내 반응성 증식)

  • Intraoral soft tissue can easily be injured by weak mechanical irritation. Each symptom by irritation is various, but most of patients show chronic inflammatory lesion. The fibroma is the most frequent disease found in intraoral area followed by pyogenic granuloma, epulis fissuratum, palatal papillomatosis, and epulis granulomatosum. The inflammatory hyperplasia by mechnical irritation is easily different from other disease, but this shows similar to several benign and malignant tumors required differential diagnosis. By microscopic feature, the lesions is divided by granulatory stage, mixed stage, and fibrotic tissue stage. The inflammatory hyperplasia is differently treated by each clinical features.: only removal of cause or, and excisional biopsy is/are required. This is the cases report of reactive hyperplasia of intraoral soft tissue by mechanical irritation that suggests various treatments of each cases.

A Clinical Study on the Taste Threshold of the Patients with Xerostomia (구강건조증환자의 미각역치에 관한 연구)

  • To investigate the change of taste sensitivity which follows after treatment for xerostomia, from September, 2006 to July, 2007, we categorized the patients, who were diagnosed as xerostomia in Oral Medicine, College of Dentistry, Busan National University, as subject group and from September, 2007 to December. The patients who were not diagnosed as systemic disease were categorized as control group. The results were as follows ; 1. Taste threshold comparison showed the differences at tongue lateral, tongue tip, circumvallate papilla between subject and control. 2. Taste threshold comparison showed no difference in male between subject and control. 3. Taste threshold comparison showed decrease in whole tongue except soft palate in female. 4. Taste threshold comparison showed no difference between first and second visit, but there was a tendency to change in circumvallate papilla.

Vascular leiomyoma of the palate (구개에 발생한 혈관성 평활근종의 보고)

  • Leiomyoma is benign neoplasm of smooth muscle, but it is rare in the oral cavity. The most common type of leiomyoma in the oral cavity is vascular leiomyoma. Clinically, vascular leiomyoma usually grows slowly and is generally a small, painless, sessile, firm and superficial nodule like lesion, and the length of time before patients seek medical attention may span months to years. The diagnosis of oral leiomyoma is possible with only histological studies with special stains because there are no specific clinical and radiological signs. We experienced a 53-year-old woman with palatal mass. She underwent surgical excision and the final pathological result confirmed it to be vascular leiomyoma. We report this case with a review of the related literatures.

A Study on the Taste of Burning Mouth Syndrome Patients (구강작열감증후군 환자의 미각에 관한 연구)

  • The purpose of this study was to investigate whether there were any changes in taste function with buring mouth syndrome(BMS). 133 subjects (31 males and 99 females) were included for the study and they were categorized into 2 groups BMS 33 persons, control 100 persons) and investigated in the Dept. of Oral Medicine, Pusan National University Hospital from April,2005 to February,2006. The electrical taste thresholds were measured using an electrogustometer of 4 different sites in oral cavity (tongue tip, tongue lateral, circumvallate papillar and soft palate). The results were as follows : 1. The electrical taste threshold in BMS group was very significantly low. 2. The electrical taste thresholds in male and female of BMS group were significantly lower each other than those of control group 3. The electrical taste threshold of BMS group according to single/multiple lesions and chronicity were not different. 4. After treatment, electrical taste threshold and NAS decreased more significantly.

Expectations of the First Visit to Orofacial Pain Clinic for the Patients with Temporomandibular Disorders (구강안면통증클리닉에 처음 내원한 턱관절장애 환자들의 내원 목표)

  • Understanding patients' expectation for health visits and providing appropriate care may increase patients' satisfaction with health care, leading to more positive treatment outcome. The study aimed to investigate expectations of the patients with temporomandibular disorders (TMD) at their first visit to university-based orofacial pain clinic and to evaluate any relation with duration of pain, presence of previous treatment, pain severity and pain interference. Inclusion criterion was patients aged over and 18 years old and diagnosed as TMD during 3 months' period from Aug to Nov 2010. They were asked to complete the questionnaires for patients' expectation and the Brief Pain Inventory (BPI) at waiting room prior to consultation. 322 TMD patients participated in the study(M:F=1:1.5, mean age=36 years old). The study indicated that the most important top 3 expectations were 'cure of pain', 'understanding their problem' and 'doctor-patient communication' in order. This finding was not affected by gender, duration and previous treatment history but affected by sub-category of TMD and BPI pain severity and pain interference. 'Pain relief' and 'understanding their problems' were relatively highlighted in the patients with muscle disorders and combination (joint-muscle) disorders of TMD than those with joint disorders who wanted communication and further investigation relatively more (p=0.000). While expectation for pain relief was expected more with increase of pain severity and interference, patients with mild level of pain severity and interference expected communication and further investigation relatively more (p=0.000, 0.017, respectively). Based on the results of the study, though pain relief was the primary concern for TMD patients suffering from pain, their satisfaction with care may be increased by explanation for etiology and mechanism of TMD to make them understand their problems better and doctor-patient communication and collaborative decision-making for treatment. Importance of patient-centered consultations and availability of written material or web sites for patient information should be stressed out.

SIALOLITHIASIS ON THE MOUTH FLOOR IN A CHILD WITH INTELLECTUAL DISABILITY (지적장애를 가진 장애아동의 구강저에 발생한 타석증)

  • A salivary stone on mouth floor was removed in a 10-year-old girl with cerebral palsy and epilepsy. If left untreated, sialolithiasis can lead to inflammation of whole salivary gland with severe pain and inflammation that spread to other side of salivary gland causing damaging of teeth, abscess, and nerve disorders. Special needs patients have difficulty to express their pain or discomfort. To overcome communication problem and offer appropriate treatment in proper time, a careful and comprehensive clinical examination including soft tissue and tooth should be prioritized in special needs patients visiting the dental clinic.

Evaluation of Taste in the Patients with Oral Lichen Planus by Electrogustometer (전기미각측정기를 이용한 구강편평태선환자의 미각평가)

  • The purpose of this study was to investigate whether there were any changes in taste sensitivity with oral lichen planus (OLP). Sixty subjects (26 males and 34 females) were included for the study and they were categorized into 2 groups (oral lichen planus 30 persons,control 30 persons), oral lichen planus group was investigated in the department of Oral Medicine, College of Dentistry, Pusan National University from April, 2005 to February, 2006 and control group was investigated in the clinic at Cheongju City from february, 2006 to april, 2006. The electrical taste thresholds were measured using an electrogustometer of the 4 different sites (tongue tip, tongue lateral, circumvallate papilla and soft palate) in oral cavity. The results were as follows; 1. The electrical taste threshold showed a tendency to decrease in the OLP group. 2. The electrical taste threshold showed a tendency to decrease in female group, but showed a tendency to increase in male group of the OLP group. 3. The electrical taste threshold showed a tendency to increase in tongue tip, tongue lateral of the multiple OLP group, but showed a tendency to decrease in circumvallate papilla, soft palate of the multiple OLP group. 4. The electrical taste threshold showed a tendency to increase in tongue tip, tongue lateral of the acute OLP group, but showed a tendency to decrease in circumvallate papilla, soft palate of the acute OLP group. 5. After treatment, electrical taste threshold was significant lower in soft palate of the OLP group than control group. 6. After treatment, NAS was significantly lower in soft palate of the OLP group than control group.
