The effects and follow-up of early preorthdontic trainer treatment on class II malocclusions (2급 부정교합에서 교정 전 Trainer를 이용한 조기치료 효과와 예후관찰)

  • TRAINER for Kids ($T4K^{TM}$, Myofunctional. Research Co, Australia) is a prefabricated myofunctional orthodontic appliance recommended to ClassII division1 malocclusion patients who have bad oral habits such as mouth breathing, tongue thrusting, inappropriate tongue position, thumb sucking and so on. Trainer has a soft texture and a small volume so that those advantages lead to an increase in the agreement rate of young patients of its use. This presentation is to analyze clinical efficacy of Trainer. The analysis is based on a result of regular follow-up on Class II division1 malocclusion patients who has been completely treated by Trainer in the Sanbon Dental Hospital of Wonkwang university. This case report is to present the satisfactory results gained by using Trainer on Class II patients. First, Trainer was applied in Class II malocclusion patients of mixed dentition with expected space insufficient to gain facial improvement. Second, excessive overjet, overbite were improved. Third, main effects are regarded to have been achieved by development of lingual slant of upper jaw, labial slant of lower jaw, and lower part of jaw bone.

  • Oral appliance therapy is a simple, reversible way for improving snoring and/or obstructive sleep apnea. It may be indicated for the patients who are unable to tolerate continuous positive airway pressure (CPAP) or who have potential risks for surgical intervention. Oral appliance therapy increases airway space by the providing stable anterior positioning of the mandible, pulling out tongue, lifting up soft palate, or changing the muscle activity of the genioglossus. Currently, more than 80 different types of oral appliances have been introduced for snoring and/or obstructive sleep apnea. They are classified by their characteristics such as mode of action, adjustability and material used. This article provides a detailed clinical protocol and treatment procedure for oral appliance therapy.

  • Oral appliances therapy is becoming increasingly recognized as a successful treatment for snoring and obstructive sleep apnea(OSA). Compared with continuous positive airway pressure(CPAP), the gold standard therapy for OSA, oral appliance therapy are less efficacious for severe OSA but are more acceptable and tolerable for patients, which in turn, may lead to a comparable level of therapeutic effectiveness. Nevertheless, the various side effects of oral appliance therapy, such as, increased salivation or dryness, pain or discomfort in the teeth or gums, occlusal discomfort in the morning, temporomandibular disorders, dental and occlusal changes may cause discontinuation of treatment or changes in treatment plan. Therefore, oral appliance therapy should be provided by a qualified dentist who can evaluate oral tissues, occlusion, and temporomandibular joints, and prevent and manage the possible side effects.

  • Sleep-disordered breathing (SDB) is defined as a disturbed breathing during sleep caused by repetitive upper airway collapse. Complete collapse causes a cessation of breathing, known as obstructive sleep apnea (OSA) and snoring can arise from partial collapse. Undiagnosed and untreated OSA means recurrent intermittent hypoxemia and leads to a variety of cardiovascular disorders, disturbed neurocognition, and excessive daytime sleepiness. Various behavioral modalities have been suggested for treating snoring and sleep apnea including changing the sleep position, avoiding alcohol, and weight loss. Until now continuous positive airway pressure (CPAP) therapy is one of effective treatment for patients with OSA, but its discomfort causes less tolerance and compliance. Therefore, clinical effectiveness and convenience for oral appliance have emerged and the role of dentists has become more important in the management of OSA.

  • $T4K^{TM}$(Myofunctional Research Co, Australia) is one of the myofunctional appliance developed to be used in children of mixed dentition. Myofuncitonal appliance stimulate the facial, masticatory and tongue muscle and help to balance the muscular force. Labial bow included in the device exerts strength in excessively labial inclineded upper jaw, Lip bumper blocks strength of the mouth to prevent abnormal strength exerted in lower jaw, Tongue tag secures proper position of tongue, and additional exercise is not required for child patients. For the more, simpler design and softer texture of device prmoted cooperation of patients during use. This case report is to present the satisfactory results gained by using $T4K^{TM}$ on Class II patients. Comment 1. $T4K^{TM}$ was applied in Class II malocclusion patients of mixed dentition with expected space insufficient to gain facial improvement. 2. Excessive overjet, overbite were improved. 3. Bad habits, such as mouth breathing, can also be adjusted.

Safety and Efficacy of the Mandibular Advancement Device 'Bioguard' for the Treatment of Obstructive Sleep Apnea : A Prospective, Multi-Center, Single Group, and Non-Inferiority Trial (폐쇄성 수면무호흡증의 치료에 사용되는 하악전방이동장치-'바이오가드'의 안전성과 유효성 검증을 위한 전향적, 다기관, 단일군 및 비열등성 연구)

  • Objectives: The purpose of this study was to estimate the safety and efficacy of a mandibular advancement device (MAD), 'Bioguard,' for the treatment of obstructive sleep apnea (OSA). Methods: In this 5-week prospective, multi-center, single group, and non-inferiority trial, patients who chose 'Bioguard' as their treatment option were evaluated using both questionnaires (Pittsburgh Sleep Quality Index (PSQI), Epworth sleepiness scale (ESS)) and polysomonography (PSG) (apnea hypopnea index (AHI), oxygen saturation). All patient data, including clinical records, PSG studies (both pre- and post-treatment), and adverse events (AEs), were reviewed and analyzed. Results: Results were obtained for 59 of 62 patients (95.16%). No significant difference in success rate was found between the MAD treatment and surgical treatment (95% CI). AHI, PSQI, ESS and oxygen saturation demonstrated significant improvement (p < 0.001) after MAD treatment, and 39 of 62 patients (62.9%) reported 85 AEs. 79 of the 85 AEs (91.8%) were mild cases, and there were no severe AEs related to the MAD treatment. Conclusion: The MAD 'Bioguard' should be considered as an alternative treatment option for OSA patients.

Zygoma-gear appliance for intraoral upper molar distalization (Zygoma-gear를 이용한 구치부 후방이동을 통한 비발치 치험례)

  • The aim of this report is to present an intraoral upper molar distalization system supported with zygomatic anchorage plates (Zygoma-gear Appliance, ZGA). This system was used for a 16-year-old female patient with a Class II molar relationship requiring molar distalization. The system consisted of bilateral zygomatic anchorage plates, an inner-bow and heavy intraoral elastics. Distalization of the upper molars was achieved in 3 months and the treatment results were evaluated from lateral cephalometric radiographs. According to the results of the cephalometric analysis, the maxillary first molars showed a distalization of 4 mm, associated with a distal axial inclination of $4.5^{\circ}$. The results of this study show that an effective upper molar distalization without anchorage loss can be achieved in a short time using the ZGA. We suggest that this new system may be used in cases requiring molar distalization in place of extraoral appliances.

A Case Report of Phantogeusia Managed by Postural Yinyang Correction of Temporomandibular Joint with Customized TMJ Balancing Intraoral Appliance (맞춤형 턱관절 균형 구강내 장치를 활용한 턱관절 자세 음양교정술의 환상미각증 치험 1례)

  • Objectives: The purpose of this study was to report the effect of Postural Yinyang Correction of the Temporomandibular joint (Functional Cerebrospinal Therapy, FCST) for phantogeusia. Methods: A patient with phantogeusia was treated at the Department of Acupuncture & Moxibustion, Dong-Eui University Korean Medicine Hospital from Oct 10th, 2018 to Dec 4th, 2018. A patient received 24 times of FCST with Customized TMJ Balancing Intraoral Appliance, upper cervical manipulation. To estimate the efficacy, Visual Analogue Scale (VAS), Oral Health Impact Profile-14 (OHIP-14) and questionnaire. Results: The patient showed a certain degree of improvement in phantogeusia and other subject symtoms within 8 weeks. Adverse effects were not reported. Conclusions: These results showed that FCST using an Customized TMJ Balancing Intraoral Appliance, may be useful for reducing the symptoms of phantogeusia.

  • Panfacial fractures require complex multidisciplinary approaches for treatment. Functional stability of bilateral condylar-disc complex should be the goal of the treatment. A patient with complex clinical panfacial fractures, including a bilateral condylar fractures visited our clinic. Facial asymmetry, insufficient vertical space and multiple missing teeth of the patient were major problems. Closed reduction and splint treatment were tried for stable condylar position. A functional and esthetic rehabilitation was accomplished by using implants and full mouth rehabilitaion. Potential possibilities of unstable occlusion should be prevented with night guard and periodic occlusal adjustment.

  • 음식물을 씹거나 삼키는 정상적인 하악운동이외에 악안면 근육의 활동에 의해 상, 하악 치아가 접촉하거나, 비정상적인 근육활동이 있을 때 이 현상을 이갈이(Bruxism)이라 합니다. 이갈이 습관은 대개 수면중에 나타나나 깨어 있는 상태에서도 비정상적인 근육수축을 관찰할 수 있읍니다. 이러한 현상을 저작근의 근전도(Electromyogram, EMG)를 기록하거나, 구강내에 장치된 원격계측장치로 치아접촉을 관찰함으로써 정상적인 저작이나 연하작용과 비교할 수 있습니다. 이갈이의 발생빈도는 학자들에 따라 서로 다르게 보고되고 있으며, 수면중 이를 가는 사람들은 대부분 이갈이 습관을 알지 못하며, 단지 10%정도만이 이가는 소리를 알 뿐입니다. 본문에서는 이갈이와 수면중의 생리적 현상과의 관계, 저작근에 미치는 영향과 이갈이 발생에 대한 중치신경의 작용을 검토함으로써 이갈이를 이해하는데 도움이 되고저 합니다.
