A STUDY ON THE EFFECTS OF CHEWING PATTERNS TO OCCLUSAL CONTACT POINTS AND CHEWING EFFICIENCY (저작 형태가 교합 접촉및 저작 능률에 미치는 영향에 관한 연구)

  • 저작은 교합과 악운동 뿐만 아리라 근신경계, 고위 중추까지 복합적으로 관여하는 기능적 행위이다. 교합 양상은 다양하게 저작 형태에 영향을 끼치며 저작 효율에도 관여한다. 저작 형태는 다양한 모양을 가지나 두가지 전형적인 군 즉, 전방에서 관찰시 그 양상이 수직적이며 chopping운동을 닮은 군과 저작 형태가 주로 측방으로 이루어지며 grinding을 하는 군으로 나눌 수 있다. 본 연구의 목적은 저작 형태의 치아가 교합접촉 및 저작 효율에 미치는 영향을 고찰해 보고자 하는 것이다. 하악운동궤적기록기를 이용하여 정상교합을 가진 치과대학생중 전형적인 2가지 저작형태를 보이는 각 10명씩을 피검자로 선택하였다. 3가지 하악위 즉, 중심위, 작업측 비작업측에서의 교합접촉을 고무형 교합인기재로 기록하여 천공부의 직경이 1mm이하면 1점, 1-2mm또는 직선상이면 2점, 2mm이상이면 3점으로 평가하여 각 점수의 합으로 좌우 소구치 및 대구치의 접촉 지수를 측정하였다. 저작 효율을 평가하기 위해 땅콩 3g(${\pm}0.01g$)을 20회 저작하게 한 후 3회 입을 헹구어 뱉게 하였다. 체눈 크기가 각 0.425, 0.60, 0.85, 2.0, 4.0인 체에 거른 후 $65^{\circ}C$로 오븐에서 세시간 말려 무게를 측정하고 중심 크기$(M_{50})$과 저작효율치(R)를 계산 비교하여 다음과 같은 결론을 얻었다. 1. Chopping형은 grinding형에 비해 중심위에서 더 넓은 교합접촉을 보였다(P<0.01). 2. Grinding형은 chopping형에 비해 측방위에서 더 넓은 교합접촉을 보였다(P<0.01). 3. Chopping형은 중심크기($(M_{50})$)과 저작효율치(R)로 비교하였을 때 더 좋은 저작 효율을 보였다(P<0.01).

Elastic properties of addition silicone interocclusal recording materials (부가중합형 실리콘 교합인기재의 탄성 특성)

  • Objectives : In this study, contact angle and shore D hardness were measured, and a shark fin test was conducted after selecting five addition silicon(Blu-Mousse, BM; EXABITE II, EX; PERFECT, PF; Regisil$^{(R)}$ Rigid, RE; Silagum$^{(R)}$, SI) in order to figure out the properties of elastomeric interocclusal recording materials and reduce errors at interocclusal recording. 8) Methods : A contact angle was measured using a contact angle analyzer. After placing a drop of liquid on the surface of the specimens of interocclusal recording materials, a contact angle was photographed with a CCD camera on the equipment. In terms of a shark fin test, interocclusal recording materials were mixed for the time proposed by the manufacturer and inserted into the split ring of the Shark fin device. Twenty (20) seconds exactly, a metal rod was removed to make the materials slowly absorbed. Once they hardened, fin height was measured with a caliper after separating molds and trimming the specimens. The shore D hardness was measured with a shore D hardness tester(Model HPDSD, Hans Schmidt & Co. Gmbh, Germany) in sixty (60) minutes after fabricating specimens. In each experiment, five specimens, mean and standard deviation were calculated. A one-way ANOVA test was performed at the p>0.05 level of significance. In terms of correlation among the tests, Pearson correlation coefficient was estimated. For multiple comparison, Scheffe's test was carried out. Results : A contact angle was the highest in EX with $99.23^{\circ}$ (p<0.05) while the result of the shark fin test was the longest in RE with 5.45mm. SI was the lowest (0.27mm) with statistical significance. Among the interocclusal recording materials, significant difference was observed in terms of means (p<0.05). The shore D hardness was the highest in SI with 31.0 while RE was significantly low with 16.4 (p<0.05). Among the materials, statistically significant difference was observed in terms of means when compared to the rest materials (RE), BM, RE and SI (PF and EX) and the remaining materials (BM and SI) (p<0.05). In terms of correlations among the tests, a negative correlation occurred between shore D hardness and shark fin test(r=-0.823, p=0.000). Conclusions : According to the study above, it is necessary to understand the properties of interocclusal recording materials and consider contact angle, shark fin test and properties of shore D hardness to select appropriate materials.

Effectiveness of clinical remounting improving balanced occlusion of complete dentures (총의치 균형교합에 영향을 미치는 진료실재부착의 효과)

  • Clinical remounting of complete denture is performed to refine occlusal harmony in maxillo-mandibular relation. It has been reported that patients who used adjusted dentures with clinical remounting felt less complications such as pain and discomfort in mastication. The purpose of this study was to assess effects of clinical remounting with case series. Seven patients with existing complete prosthesis were included. Clinical remounting procedure was done through interocclusal relation recording. In addition, occlusal force was measured with pressure indicating sensor and occlusal contact areas were evaluated with photo occlusion analysis. Occlusal contact areas of prosthesis were enlarged, while bite pressure was not increased. Hit and slide phenomenon of prosthesis was reduced concurrently. Clinical remounting procedure improved denture stability and increased occlusal contact area. Therefore, clinical remounting should be considered.

Analysis of Occlusal Contacts Using Add-picture Method (Add-picture 방법을 이용한 교합접촉점 분석)

  • The purpose of this study was to analyze the area of occlusal contact points using visual method. One subject was selected who had Angle Class I, normal dentition, without dental caries, periodontal disease and temporomandibular disorders. Forty times PVS impressions were taken and 10 pairs casts were fabricated using dental super hard stone. After mounting the casts with customized loading apparatus, 78.9kg/f force was loaded as a maximum biting force. In T-Scan method, occlusal contact points measurement was repeated twice. Then, using Photoshop program (Adobe photoshop CS3, Adobe. San Jose, USA), the pixels which indicated occlusal contact points by color was recognized, and the distribution of recognized pixels were calculated to area. In Add picture method, polyether bite material applied to the occlusal surface of the casts. Then, the image of the translucent areas was recorded and classified $0{\sim}10{\mu}m$, $0{\sim}30{\mu}m$, $0{\sim}60{\mu}m$ area by the amount of transmitted light. To acquire occlusal surface, the numbers of pixels from the photograph of the contact area indicated cast converted to $mm^2$. The mean occlusal contact area by two methods was statistically analyzed (paired t-test). Part of the red and pink area in T-Scan image were almost equivalent to the $0{\sim}10{\mu}m$, $0{\sim}30{\mu}m$, $0{\sim}60{\mu}m$ area in Add picture image. The distribution of occlusal contact points were similar, but the average area of occlusal contact points was wider in T-scan image (P<.05). Pink and red area in T-scan image was wider than $0{\sim}10{\mu}m$, $0{\sim}30{\mu}m$ area in Add picture image (P<.05), but similar to $0{\sim}60{\mu}m$area in Add picture image (P>.05). Occlusal contact points in T-scan image did not indicate real occlusal contact points. Occlusal contact areas in T-scan method were enlarged results comparing with those in Add picture method.

Accuracy of conventional and digital mounting of dental models: A literature review (치과용 모형의 모형 부착 과정에서 발생하는 오차에 대한 문헌 고찰)

  • Accurate transfer of the maxillo-mandibular relationship to an articulator (i.e., mounting) is critical in prosthetic treatment procedures. In the current study, a PubMed search was performed to review the influencing factors for the maxillo-mandibular relationship's accuracy. The search included digital mounting as well as conventional gypsum cast mounting. The results showed that a greater amount of displacement was introduced during positioning the maxillary and mandibular models to interocclusal records rather than the dimensional change of registration material. Most intraoral scanners resulted in an accurate reproduction of the maxillo-mandibular relationship for posterior quadrant scanning; however, the accuracy was declined as the scan area increased to a complete arch scan. The digital mounting accuracy was also influenced by the image processing algorithms and software versions, especially for complete arch scans.
