The objective of this study was to analyze occlusal contact pattern of 33 young adult males with normal occlusion during lateral excursion using T-scan III system which can measure the dynamic occlusion. Occlusal contact patterns were examined and categorized in non-working side disclusion point(more functionally acceptable position) not canine-to-canine position. Disclusion time, occlusal force, and occlusal force distribution ratio of upper canine at disclusion point during lateral excursion movement was also measured and compared among the groups divided according to contact patterns. The results showed that group function was clearly more dominant than canine guidance and other occlusal contact patterns different with existing patterns were found. There were significantly difference between groups in variables of T-scan measurement. T-scan III system is effective tools to analyze and evaluate occlusal contact patterns, disclusion time, occlusal force, and occlusal force distribution ratio of upper canine at disclusion point.

Trauma from occlusion (TFO) is a pathologic alterations which develop in the periodontium as a result of undue masticatory force. The purpose of this article is to review the controversies about TFO. There are evidences that TFO is a risk factor in the progression of periodontitis. Tooth mobility should be reduced by selective occlusal adjustment. TFO can be developed dependent on the masticatory pattern, occlusion and anterior overbite in sound periodontal conditions. Secondary TFO may aggravate unstable occlusion. If "loss of posterior support" was occur, the problems were worsen. Extrusion, migration, rotation and pathologic deviation can be resulted. Opposite contention is the "shortened dental arch" concept. However, these two concepts persue the occlusal stability together. To treat TFO adequately, exact diagnosis and multi-disciplinary treatment should be needed.
저작운동을 중추신경계와 말초조직의 작용에의해 일생동안 학습과 적응의 가정을 통해 이루어진다. 저작의 양상은 유치가 구강내에 맹출되면서 이루어지며, 상하악 치아의 접촉, 점막과 혀 및 치아상호간에 일어나는 접촉과 치근막의 고유수용기와 악관절 수용기등에 의해 조정되어 최종적으로 에너지소비가 적고 동통이나 불편함이 없이 최대의 효율을 나타내는 양상으로 된다. 그러나 원활한 저작과 하악은동을 이룩하기 위해서는 저작근, 악관절, 상하악 치아의 교합등이 서로 조화를 이루어야 한다. 만일 외상에 의해 악관절의 기능장애가 오면 저작근활동이 변화하고 이에따라 교합장애를 야기한다. 또한 치아를 상실하거나 대합치아간의 조기접촉이 있으면 교합장애가 원인이 되어 근육활동이 변화와 악관절 기능장애를 초래한다. 그리고 때로는 지나친 근육의 활동(이갈이, 지속적 긴장성 수축)에 의해 지나친 치아의 마모로인한 교합장애나 악관절장애를 일으킬 수 있다. 따라서 하악의 기능을 충분히 이해하기 위해서는 교합의 형태학적 관계와 더불어 저작계의 기능적인 면도 고려하여 그 상호관계를 검토하여야 하며 본문에서는 교합에 관련된 저작근의 작용과 이에 영향을 주는 요인 및 교합장애와 근육활동장애의 연관성에 관해 고찰코자 한다.

Purpose: The purpose of this study was to analyze correlation between bruxism and occlusal contacts on balancing side. Materials and Methods: The purpose of this study is to compare the difference of group function and balancing side occlusal contacts according to bruxism and sex, A total of 100 adults that aged 26-37 years (39 bruxers and 61 non-bruxers) were examined. The lateral excursion and balanced occlusal contacts were analyzed to determine the correlation with bruxism. The occlusal contacts were recorded by T-Scan system and articulating paper. Results: The group function was the highest in 61.5% of bruxers and 47.5% of non-bruxers. In comparison between males and females, group function was 58.9% in males and 37.0% in females. Occlusal contacts on non-working side occurred in 48 out of 100 patients. There were 51.2% of the cases in the bruxers and 46.0% in the non-bruxers. Statistically, there was no correlation between the bruxism and occlusal contacts on non-working side. There was no correlation between sex and balancing occlusal contacts. Conclusion: There was no significant correlation between bruxism and occlusal contacts on non-working side. The group function was the highest in bruxers when lateral excursion was occurred.
우리는 매일 반복되는 진료행위를 통해서 개개의 치아와 그 주위조직에 대한 병적 이상을 발견하고 치료하는 일에는 익수해져 있으나, 두 개 혹은 그 이사의 일련의 치아들이 상하악의 대합치들과 어떠한 양상으로 조화를 이루며 교합기능을 유지해 나가고 그것들이 어떻게 보이지 않는 저작계의 통제에 의한 결과인가에 대해서는 거의 등한시 하고 있다. 다시 말해서 교합에 관여하는 기능적 요소와 임상적인 정상교합에 관한 이해, 그리고 이러한 개념을 임상적인 치료에 적용하는 일에 우리의 관심이 더욱 집중되어야 할 것이며, 이러한 연구를 통해서만 복잡하고 다양하게 나타나는 일련의 교합질환(Occlusal Disease)을 다룰 수 있으리라 생각된다. 교합의 임상적인 정의와 기본 개념, 그리고 정상적인 교합이 치주조직에 미치는 영향에 관하여 고찰해 보기로 한다.
치주영역에서의 교합기능은 치주조직의 유지에 필수적이며 교합력이 치주조직의 적응한계를 초과하는 경우 치주조직의 파괴가 유발되며 이러한 현상을 교합성외상, 교합성외상에 의한 병소라고 한다. 교합성외상에 의한 조직파괴양상은 염증성 치주질환과는 많은차이를 보이며 그 원인과 시간에 따라 분류되기도 하며 치주조직의 건강에 영향을 미치는 교합성외상의 영향과 치료과정에 대해 간략하게 살펴보고자 한다.
저작은 교합과 악운동 뿐만 아리라 근신경계, 고위 중추까지 복합적으로 관여하는 기능적 행위이다. 교합 양상은 다양하게 저작 형태에 영향을 끼치며 저작 효율에도 관여한다. 저작 형태는 다양한 모양을 가지나 두가지 전형적인 군 즉, 전방에서 관찰시 그 양상이 수직적이며 chopping운동을 닮은 군과 저작 형태가 주로 측방으로 이루어지며 grinding을 하는 군으로 나눌 수 있다. 본 연구의 목적은 저작 형태의 치아가 교합접촉 및 저작 효율에 미치는 영향을 고찰해 보고자 하는 것이다. 하악운동궤적기록기를 이용하여 정상교합을 가진 치과대학생중 전형적인 2가지 저작형태를 보이는 각 10명씩을 피검자로 선택하였다. 3가지 하악위 즉, 중심위, 작업측 비작업측에서의 교합접촉을 고무형 교합인기재로 기록하여 천공부의 직경이 1mm이하면 1점, 1-2mm또는 직선상이면 2점, 2mm이상이면 3점으로 평가하여 각 점수의 합으로 좌우 소구치 및 대구치의 접촉 지수를 측정하였다. 저작 효율을 평가하기 위해 땅콩 3g(${\pm}0.01g$)을 20회 저작하게 한 후 3회 입을 헹구어 뱉게 하였다. 체눈 크기가 각 0.425, 0.60, 0.85, 2.0, 4.0인 체에 거른 후 $65^{\circ}C$로 오븐에서 세시간 말려 무게를 측정하고 중심 크기$(M_{50})$과 저작효율치(R)를 계산 비교하여 다음과 같은 결론을 얻었다. 1. Chopping형은 grinding형에 비해 중심위에서 더 넓은 교합접촉을 보였다(P<0.01). 2. Grinding형은 chopping형에 비해 측방위에서 더 넓은 교합접촉을 보였다(P<0.01). 3. Chopping형은 중심크기($(M_{50})$)과 저작효율치(R)로 비교하였을 때 더 좋은 저작 효율을 보였다(P<0.01).
In general, orthodontists make problem lists and treatment plans based on norms of several cephalometric standards. But consideration of dentoalveolar compensation, which tends to maintain normal dental arch relationship in various skeletal jaw relationships, helps orthodontists make more individualized treatment objectives and plans. The purpose of this study was to classify skeletal patterns of normal occlusion samples by cluster analysis and to investigate the dentoalveolar compensation according to skeletal patterns. The subjects were consisted of 125 subjects who were normal occlusion samples at Seoul National University Dental Hospital, Department of Orthodontics. Lateral cephalograms in centric occlusion were traced and digitized. The skeletal patterns of normal occlusion samples were classified into three horizontal groups and three vertical groups by cluster analysis and ANOVA on the skeletal and dentoalveolar measurements among the groups were carried out. The results were as follows ; 1. Anteroposterior and vertical skeletal relationships of normal occlusion samples were very variable. 2. As the mandibular position was anterior to the maxilla, the maxillary incisors inclined more labially, the mandibular incisors more lingually, and the occlusal plane was flattened due to the anteroposterior dentoalveolar compensation. dentoalveolar height was decreased and upper posterior teeth was uprighted to the palatal plane and lower incisors and lower posterior teeth to the mandibular plane. 4. Lower incisors were more strongly associated with the dentoalveolar compensation than upper incisors according to the anteroposterior and vertical skeletal relationship.
Purpose: Most of the former studies about the occlusal contact patterns during the mandibular movement focused on foreigner. The purpose of this study is analyzing the occlusal contacts of young Koreans by using T-Scan system. Materials and methods: The sample size was 87 and the occlusal contacts of each right and left lateral movements were measured from the maximum intercuspation to the 3mm excursive position for three times respectively. All of the occlusal contacts were double checked through the thin metal foil. The results were categorized as two; 1) considering occlusal contact patterns on working side only, 2) considering occlusal contact patterns on working and nonworking sides. Results: The results showed that the nonworking side occlusal contacts play major roles in the lateral mandibular movement. In both cases of considering with and without the nonworking side occlusal contacts, the group function was the most prevalent. In the working side, the contacts were the most frequent in canine and the frequency of contacts was decreased as the distance was increased from canine to molar. In the nonworking side, the contacts were the most frequent in second molar. And the gender factor was statistically significant (${\alpha}$=.05), as females have more nonworking side occlusal contacts in this study. Conclusion: Among the three factors of anterior guidance, the group function was the most dominant factor. The analysis of 87 samples showed that canine contact was the most frequent in the working side and second molar contact was the most common in the nonworking side.
The aim of this study was to evaluate the morphology of the mandibular symphysis and anterior alveolar and skeletal relationship under the influence of the rotational growth pattern of mandible in skeletal Class III malocclusion. A total of U untreated adult subjects were divided into two groups-forward rotational growth pattern group, backward rotational growth pattern group-according to the suggestion of Skieller et al.. The antero-posterior position, vertical relationship, mandibular symphysis and anterior alveolar and skeletal relationship were assessed on lateral cephalometric radiographs. Mandibular symphysis and anterior alveolar and skeletal relationship in each subject were studied and the following conclusions were drawn : 1. Concerning the antero-posterior position, forward rotational growth pattern group showed significantly larger SNA, SNB. Conceming the vertical relationship, all measurements showed statistically significant differences. 2. Forward rotatioal growth pattern group showed significantly larger IMPA, MnAD, backward rotational growth pattern group showed significantly larger MxABH. 3. There was no statistically significant difference in symphysis ratio to mandibular plane between forward and backward rotational growth pattern group. 4. In the correlative analysis of rotational growth pattern of mandible and mandibular symphysis, anterior alveolar and skeletal relationship, statistically significant correlations in overbite, IMPA, MnAD, symphysis width were showed.
