FIBRIN SEALANTS IN MAXILLOFACIAL SURGERY : A INTRODUCTORY REPORT (악안면 외과 영역에서의 FIBRIN SEALANTS 의 이용)

  • The fibrin sealant was first designed as an alternative to surgical suture for the purpose of surface-to-surface union especially in parenchymal organs like the liver, spleen and kidney. The clinical application of currently used fibrin sealant was first introduced in 1972. The fibrin sealant consists of principal two components; lyophilized human fibrinogen and bovine thrombin. The fibrinogen component also contains coagulation factor XIII. A solution of aprotinin, an inhibitor of fibrinolysis is used to dissolve the fibrinogen and to provide the first component, and a solution of calcium chloride is also used to provide the second component. From July to December in 1990, during 6 months, we used fibrin sealant in the 28 patients of 33 various cases, in the following ways; supportive application of fibrin sealant after free autogenouse nerve graft for the repair of inferior alveolar nerve, facial nerve or accessory nerve, treament of hemangioma or lymphangioma to thrombosize and lead to the tumor shrinking, skin grafting to stimulate the adhesion and tissue repair, bone grafting in the patients of cleft alveolus, mandibular reconstruction or orthognathic surgery to facilitate the knitting of bone chips, tissue adhesion after tumor resection, radical neck dissection or flap reconstructions, and supportive adhesion of external auditory cannal after TMJ surgery via postauricular approach. No adverse effects were observed, none of the patients developed hepatitis or other blood transmitted disease, and the wound healing were acceptable.

A Study on the Effect of Intermittent Exotropia Vision Training Using Proprioceptive Test (Maindot test) (양안시 고유수용감각검사(Maindot 검사)를 사용한 간헐성 외사시안의 시기능 훈련효과에 대한 연구)

  • Purpose: This study evaluates the effectiveness of intermittent exotropia vision training through follow up of average 3 years using proprioceptive test (Maindot test). Methods: 35 people (18 men, 17 women) with mean age of 13.48 (${\pm}2.45$) with corrected visual acuity of 0.9 or higher and normal binocular vision and control were observed for changes between before, after, and 3 years following the vision training during the period from 2005 to 2009. Results: After training, the correlation between the changed amount of deviation and PRC(diplopia) value and symptoms was not significant (p>0.05). However, it showed a statistically significant (p<0.001) correlation that change of the binocular proprioceptive sense abilities is decreased subjective symptoms. Conclusions: Binocular proprioceptive sense may be used in variety of purposes and applications such as predictive evaluation of binocular symptoms, binocular function performance evaluation after vision training and prism treatment.

Effect of Presurgical Nasoalveolar Molding in Unilateral Cleft Lip and Palate Infants (편측성 구순구개열 신생아에 대한 술전비치조정형장치의 효과)

  • Cleft lip and palate, the most common craniofacial anomalies, are severe congenital defects that have an incidence of 0.28 to 3.74 per 1000 live births. Although there has been great improvement in the field of cleft surgery, surgical approach cannot be the single solution to resolve the various problems encountered in patients with cleft lip and palate. The concept of presurgical infant orthopedics (PSIO) for gradual closure of the cleft gap and simplified surgical performance was first introduced by McNeil in 1950. Recently, there are many attempts not only to approximate the alveolar segments but also to reshape the nasal cartilage. Three infants with unilateral cleft lip and palate were referred from the department of Plastic Surgery for presurgical nasoalveolar molding (PNAM). Maxillary appliances using resin with orthodontic wire were fabricated. Then these appliance was applied until patients underwent lip surgery. In all cases, the patients could wear the appliance all day since they were able to eat even with the appliance on, This resulted in significant improvements in the nasal symmetry were found. Our appliance, namely K-NAM, extends the wearing time within the limited period and as a result it is expected to maximize the treatment effects. Used properly, this appliance would play a major role in enhancing nasal symmetry with satisfactory results.

한국원자력연구소 방사선방어기술 개발 및 연구 현황

  • 1959년 한국원자력연구소가 창립됨과 동시에 &Health Physics&, 즉 보건물리라고 하는 명칭과 조직이 탄생되어, 방사선안전관리의 실무와 보건물리의 연구가 시작되었다. 최초 10년간은 선진제국의 보건물리분야의 연구와 기술을 추적하여 우리나라의 방사선안전관리 기술의 기초를 다지는 시기로서 개인방사선모니터링기술, 환경방사선(능) 모니터링기술 및 방사선방어용계측기기의 교정기술 개발에 중점을 두고 연구개발이 추진되었으며, TRIGA Mark-II 연구용원자로의 가동에 따라 원자로 생체차폐체의 건전성 검증에 관한 유익한 방사선량 측정자료도 얻게 되었다. 즉 이 기간은 방사선안전관리의 체제정비 및 기초기술 확립에 노력한 기간이었다. 1970년대는 원자력 연구개발에 대한 기본방향과 정책의 변경등으로 보건물리 연구조직은 방사선안전관리, 환경연구 그리고 방사화학분야로 분산되었으며, 그로인하여 연구개발활동은 거의 정체되어 겨우 방사선안전관리 실무만이 그 명맥을 유지하였다. 그 결과 우리나라 방사선안전관리 및 그와 관련된 연구개발의 기반이 흔들리게 되었으나, 그러한 환경하에서도 방사선량측정평가기술, 방사선차폐설계기술 및 원자로사고시 피폭선량평가기술의 선진화에 필요한 지식을 얻었으며, 방사선 안전관리에 유익한 실무경험도 축적하게 되었다. 1980년대는 통합된 원자력 연구개발체제의 구축으로 방사선작업종사자 및 일반공중의 피폭저감화 기술개발에 필요한 각종 최신기술을 도입하였고, 관리업무에 있어서도 측정의 정확도와 신뢰성향상 및 새로운 관리기술의 개발에 많은 노력을 한 결과, 유익한성과를 얻게된 기간이다. 특히, 이 기간은 방사선안전관리기술의 선진화를 위한 지식이 축적되어 90년대의 방사선안전관리기술자립화를 위한 전환기로서, 이와같이 축적된 기술은 원자력의 평화적 이용에 크게 기여할 것으로 기대된다.서 dithiothreitol를 투여한 군에서는 우라늄단독투여군에 비해 cretinine의 배설이 상당히 증가하였다(P<0.05). 6. 우라늄오염에 의한 신장의 소견에 있어 우라늄단독투여군은 근위곡세뇨관상피의 공포화 및 종창, microvilli와 brush border의 손실, 세뇨관 상피의 괴사가 관찰되었으며, 간장의 충혈, 중심성 괴사 및 모세관 확장증도 관찰되었다. 그리고 sodium bicarbonate와 생리적 식염수를 병행투여한 군과 우라늄을 투여하고 30분이 지나서 dithiothreitol를 투여한 군에서는 우라늄 단독투여군에 비해 높은 방호효과가 관찰되었으나 다른 실험군에서는 큰 효과가 없는 것으로 나타났다. 결론적으로 우라늄의 체내오염시에는 sodium bicarbonate와 생리적 식염수를 가능한 빨리 병행투여하거나 dithiothreitol을 체내오염후 30분이 지나서 투여하는 방법이 우라늄오염에 대한 제염에 매우 유효할 것으로 생각되며, 특히 우라늄에 의한 인체장해를 유의하게 경감시켜줄 것으로 사료되었다.내의 어떤 부위와도 관계가 되는 것으로 간주되는데 이것이 $(^3H)$ QNB가 $(^3H)$ NMS보다 높은 최대 결합능력 $(B_{max})$을 나타낼 이유이다. Study on Determination of Boron using the PGAA Facility at HANARO Research Reactor (하나로의 즉발감마선 방사화분석 장치를 이용한 붕소의 정량에 대한 연구)

  • Basic research for the determination of boron content in biological sample has been carried out using the PGAA facility of the 24MW research reactor(HANARO). For investigation of characteristics for the measurement condition, neutron flux and its homogeneity were measured at irradiating geometry. The size of thermal neutron beam collimated from beam guide is $2{\times}2cm^2$ at the sample position. The neutron flux measured was the range of $1.0{\sim}6.5{\times}10^7n{\cdot}cm^{-2}{\cdot}s^{-1}$, and flux distribution from center within the radius of 4.5 mm and 9.0 mm was $5.77{\pm}0.71{\times}10^7n{\cdot}cm^{-2}{\cdot}s^{-1}$ and $4.68{\pm}1.64{\times}10^7n{\cdot}cm^{-2}{\cdot}s^{-1}$, respectively. Accordingly, sample size is adjusted within 10 mm for a homogeneous irradiation of high quality. Measurement system is designed to reduce the background source by Compton scattering and to improve the analytical sensitivity. To investigate the energy calibration and Compton suppression effect of gamma-ray counting system, the background conditions on both of Compton and single-mode were measured using NaCl standard. On the other hand, degree of spectral interference for sodium 472 keV peak as a matrix effect in the sample is established for an accurate boron analysis, and then boron content in three certified reference materials (NIST SRM 1570a, 1547, 1573a) was measured by using two modes and the results were compared with each other.

Effectiveness of Acid Injection as a Method to Remove $\textrm{HCO}_2$ in Hydroponic Water (양액재배 용수 중의 $\textrm{HCO}_2$ 이온 제거를 위한 산 첨가의 효과와 효과적인 산 처리방법)

  • High concentrations of HCO$_3$$^{[-10]}$ (bicarbonate ion) in hydroponic water lead to high pH and to change in solubility, and consequently inhibition of absorption of available iou. An adequate and practical method is needed to remove HCO$_3$$^{[-10]}$ in the water fur hydroponics. to compare the efficiency of HCO$_3$$^{[-10]}$ removal, the effect of injecting HNO$_3$, H$_3$PO$_4$ or H$_2$SO$_4$ was tested. Acid injection was effective to remove HCO$_3$$^{[-10]}$ Based on the assumption that an equivalent of HCO$_3$$^{[-10]}$ ion is neutralized by an equivalent of acid, KHCO$_3$ was dissolved in a double distilled water at 50, 100, 150, 200 or 250 mg.L$^{[-10]}$ concentration and either HNO$_3$, H$_3$PO$_4$ or H$_2$SO$_4$ was injected at a certain ratio, and the resulting pH change and HCO$_3$$^{[-10]}$ ion removal was measured. According to the results obtained, HCO$_3$$^{[-10]}$ in hydroponic water was titrated, and concentration of the residual HCO$_3$$^{[-10]}$ ion well correlated with the amount of acid injected.

The Effects of Small Group Learning Using Smart Devices in Science Classes (과학 수업에서 스마트 기기를 활용한 소집단 학습의 효과)

  • The purpose of this study is to investigate the influences of small group learning using smart devices in science classes on students' achievement, learning motivation, attitude toward science lessons, and perception of small group learning using smart devices. Four 11th-grade classes (N=133) at a coed high school in Seoul were randomly assigned to a control group and a treatment group. The intervention of small group learning using smart devices emphasized collaborative writing on activity sheet. The students were taught about acid, base, and neutralization reaction for six class periods. After the instructions, an achievement test, the learning motivation test, the attitude toward science lessons test, and a questionnaire on the perception of small group learning using smart devices were administered. Two-way ANCOVA results revealed that there was a statistically significant interaction effect by their previous chemistry achievement in the achievement test scores. Only low-level students in small group learning using smart devices significantly improved their achievement probably by having the opportunities to get help from high-level students. The adjusted means of the treatment group were significantly higher than those of the control group in learning motivation and attitude toward science lessons. Students' perceptions of small group learning using smart devices tended to be positive. Educational implications of this study are discussed.

The effects of some additives on Methane Fermentation of Paper Mill Sludge treated with Alkali (알칼리 처리된 제지슬러지의 메탄발효에 미치는 몇몇 첨가제의 효과)

  • In order to elevate the efficiency of methane fermentation using the paper mill sludge, this experiment was conducted at two temperature conditions($35^{\circ}C$ and $60^{\circ}C$), and overlooked the addition effects of ethyl acetate as a substrate, nickel as a constituent of $F_430$, and sulfur as a cell growth factor and reductant. The cellulose of paper mill sludge was degraded to lower molecular materials by heating at $60^{\circ}C$ and NaOH treatment. Methane forming rates were 4.8% from NaOH-treated paper mill sludge added with ethyl acetate, 16.5% with sodium sulfide, 19.8% with nickel trioxide, 31.9% with mixture, and 9.6% with control at $60^{\circ}C$, but 0.21% with ethyl acetate, 2.14% with nickel acetate, 3.02% with nickel sulfate, 3.34% with nickel trioxide and 0.62% with control at $35^{\circ}C$. Therefore, methane yield was increased by approximately 10-fold at $60^{\circ}C$ than $35^{\circ}C$, and fermentation liquid added with mixture(nickel trioxide+ethyl acetate+sodium sulfide) at $60^{\circ}C$ showed the medium pH(7.0), higher COD value and lower nitrogen content.

Effects of Schroth 3-Dimensional Exercise on Cubb's Angle, Abdominal Endurance, Flexibility and Balance in Adolescents with Idiopathic Scoliosis (특발성 측만증 청소년의 Schroth 3차원 운동이 콥스각과 복부근지구력 및 유연성, 평형성에 미치는 영향)

  • The purpose of this study was to investigate the effects of 8-week schroth 3-dimensional exercise and ball-sling complex exercise treatment on cobb's angle, abdominal muscle endurance, flexibility and balance in adolescents with idiopathic scoliosis. Thirty subjects with scoliosis were random assignment into one of two experimental groups, either schroth treatment(n=15) or ball-sling complex exercise treatment(n=15). Exercise program was to perform for 8-week and 90min and 3 times per week. The results of this study were as follows; First, cobb's angle was significantly decreased after 8-weeks in schroth treatment(p<.001). And also abdominal muscle endurance(p<.05), flexibility(p<.001) and balance(p<.001) were significantly improve in pre vs. post treatment. Second, the ratio of cobb's angle change was significantly higher in schroth treatment compared to ball-sling complex exercise treatment following 8-weeks(p<.05). These results suggest that although both the schroth treatment and bal-sling complex exercise treatment in adolescents with idiopathic scoliosis can improve on cobb's angle, abdominal muscle endurance, flexibility and balance, but schroth treatment was better than ball-sling complex exercise treatment improving effect of cobb's angle. Therefore, we consider that schroth treatment has more effect of prevention and therapy in idiopathic scoliosis in adolescents.

Experimental Studies on Dissolution Characteristics of a Heavy Metal(As) in Mining Waste (광산매립지에서 중금속(As)의 용출 특성에 관한 실험적 연구)

  • This study investigates the contamination mechanism of soil by drainages including acid rains around mining waste sites, and suggests the quantitative methods of prevention against soil contaminations and its alternatives. For these purposes, the dissolution of arsenic in soils, which is one of toxic heavy metals, has been examined experimentally using the artificial acidic solution. Also, in order to prevent dissolution of arsenic by acid rain, the effects of limestone for the neutrality method on the soil were investigated. The arsenic in soil specimen was dissolved by strong acidic solution below pH1.0. The maximum amount of dissolved arsenic increased with decreasing pH value. Furthermore, it was found very effective to use limestones for the neutrality method. The neutralization of limestones in acidic solution was found to follow the equation of chemical reaction-controlling formulation in unreacted-core models.
