A Case Report of Complete Atrioventricular Block Treatment with Samulanshin-tang-gamibang and Acupuncture (사물안신탕가미방과 침 치료로 임상 증상에 호전을 보인 완전방실차단 환자 치험 1례)

  • Introduction: This study reports the effect of herbal medicine (Samulanshin-tang-gamibang) and acupuncture on complete atrioventricular (AV) block. Case presentation: A 63-year-old female with complete AV block was experiencing dyspnea, palpitation, dizziness, headache, bradycardia, and insomnia, and she was treated with Samulanshin-tang-gamibang and acupuncture for 12 days. To evaluate the treatment, a numeric rating scale (NRS) and the New York Heart Association (NYHA) functional classification was used. The patient's NRS scores decreased from 6 to 2 for dyspnea and palpitation and from 5 to 1 for dizziness and headache. Her NYHA Class improved from Class II to Class I. No side effects were observed during treatment. Conclusion: This study suggests that herbal medicine and acupuncture may be effective in relieving symptoms caused by complete AV block. However, the long-term effects of the treatment were not observed, and so further studies are still needed.

A Case Report of Korean Medical Treatment of a Dyspepsia Patient with Lethargy and Dizziness (기력 저하 및 현기증을 동반한 소화불량 환자에 대한 한의학 치료 1례)

  • Objectives: The aim of this case report was to present the effects of Korean medicine treatment on a patient with dyspepsia symptoms like those of gastroesophageal reflux disease. Methods: A 64-year-old male patient with dyspepsia was treated with Sosiho-tang decoction, Banhasasim-tang soft extract, Gamihyangsayukgunja-tang decoction, and Kracie Sihogayonggolmoryu-tang extract, along with acupuncture, pharmacopuncture, and deep tissue meridian hot pack therapy for five days of hospitalization. Pain from the dyspepsia was assessed using the Numerical Rating Scale and the European Quality of Life 5 Dimensions Scale. Results: Within five days of treatment, dyspepsia was relieved from a Numerical Rating Scale score of 7 to a score of 3. The European Quality of Life 5 Dimensions Scale score increased from 0.455 to 0.816. Conclusion: These results show that dyspepsia can be relieved with Korean medicine treatment. However, further well-designed studies are required to confirm these findings.

Case Report of Chemotherapy-Induced Peripheral Neuropathy Treated with Korean Medicine (항암화학요법 유발 말초신경병증에 대한 한방치험 1례)

  • Introduction: The aim of this study is to report on the effectiveness of Korean medicine to improve symptoms of chemotherapy-induced peripheral neuropathy (CIPN). Some patients are treated with medication, but medications may have no effect in some patients. Korean medicine treatments have been used in such patients, but few reports exist. Case Presentation: A 58-year-old female patient with CIPN reported symptoms of bilateral limb pain and tingling sensations as well as cold sensations in bilateral lower legs and hands, fatigue, and bilateral limb weakness. To reduce the patient's symptoms, we provided Korean medical treatment, including herbal medicine (Uchashinki-hwan), acupuncture, and moxibation. To evaluate the results of this treatment, we used a numeric rating scale (NRS), the manual muscle test (MMT), the National Cancer Institute Common Toxicity Criteria for Adverse Events (CTCAE), and the European Organization for Research and Treatment of Cancer quality-of-life questionnaire about CIPN (EORTC-QLQ-CIPN20). After 49 days of treatment, the NRS scores decreased for bilateral limb pain and tingling, for bilateral lower leg and hand cold sensation, and for fatigue. The MMT results for the upper and lower extremities were improved. CTCAE Grade and EORTC QLQ-CIPN20 score were also decreased. Conclusion: According to these results, Korean medicine treatment may be considered an effective treatment for CIPN. Prospective studies are needed in the future to confirm and expand these findings.

The effect of taping therapy on hemiplegic gait (테이핑 요법이 편마비 환자의 보행에 미치는 영향)

  • ■ Objectives The aim of this study is to analyze the change of gait pattern by the taping therapy in a hemiplegic patient. ■ Methods We conducted the taping therapy for 7 days to affected ankle joint of hemiplegic patient and analyzed the change of gait pattern using Treadmill Gait analysis equipment(Zebris Co.Ltd FDM-T) ■ Results In terms of affected side, cadence, total double support, step length and stance phase decreased, while swing phase and step time increased by taping therapy. In terms of unaffected side, step length increased by taping therapy. Center of pressure intersection point moved to posterior and affected side in cyclogram pattern. ■ Conclusion Taping therapy to affected ankle joint of hemiplegic patient would be advantages to gait improvement in hemiplegic patients.

Change of gait pattern of a patient with ataxic gait by cerebellar infarction (소뇌 경색 환자의 임상양상에 따른 보행의 변화)

  • ■ Objectives The goal of this pilot study is to observe the change of gait pattern according to the clinical status in a patient with ataxic gait by cerebellar infarction. ■ Methods We measured the spatiotemporal gait parameters of a patient with ataxic gait four times, in which, first, the patient was not able to walk independently, second, able to walk independently, third, unable to walk independently by general tremor, and, last, able to walk independently after disappearing of general tremor. ■ Results When a patient with cerebellar ataxic gait was able to walk independently and the cerebellar tremor was disappeared, the change of gait pattern was that step and stride length decreased and total double support and stance phase increased. ■ Conclusion When different clinical characteristics were improved, the change of gait showed same pattern.

Change of gait pattern of a patient with cerebral stroke by peroneal nerve stimulation therapy (뇌졸중 환자의 비골신경 자극에 따른 보행 양상의 변화)

  • ■ Objectives The goal of this pilot study is to observe the change of gait pattern in a patient after peroneal nerve electrical stimulation(PNST). ■ Methods We analyzed the gait pattern of stroke patient using treadmill gait analysis system before and after PNST for seven weeks. The PNST was carried out for 20minutes every day except Sunday. In addition, the measurement was carried out every Saturday. At the fifth week, the PNST was not carried out to confirm whether the effect of PNST was disappeared immediately when PNST was not applied. ■ Results After PNST, while heel contact time and heel max force increased and forefoot and midfoot max force decreased, the gait parameters such as cadence, velocity, swing phase, stance phase, total double support, step length, stride length, step time, stride time and forefoot contact time, were not changed. ■ Conclusion Gait of a patient with cerebral stroke was changed positively after PNST.

Correlation between motor performance and displacements of center of pressure in hemiplegic patients (편마비 환자의 운동수행능력과 질량중심 변화의 상관 관계)

  • ■ Objectives The aim of this study is to look into the correlation between motor performance and displacement of center of pressure(CoP) in hemiplegic patients with cerebral stroke. ■ Methods We measured manual muscle test(MMT) as motor performance value and, anterior/posterior position(Ant./Post. position) and lateral symmetry of CoP as displacement of CoP values from 129 stroke patients. The gait analysis was carried out using treadmill gait analysis equipment. ■ Results In terms of motor performance, MMT of upper extremity negatively correlated with Ant/Post position of CoP, however, one of lower extremity did not. In addition, MMT of both extremities negatively correlated with lateral symmetry of CoP. ■ Conclusion Displacement of CoP was negatively correlated with hemiplegic patient's motor performance.

  • ■ Objectives The aim of study is to analyze the change of gait pattern by arm sling in a hemiplegic patient. ■ Methods We analyzed the change of gait pattern under three conditions using Treadmill Gait analysis equipment(Zebris Co.Ltd FDM-T) First, the patient didn't have arm sling on her upper limb, second, the patient have arm sling on her affected upper limb, third, patient have arm sling on her unaffected upper limb. ■ Results In terms of spatiotemporal gait values, swing phase, step time, step length of unaffected lower limb increased. Furthermore, stride time and stride length also increased when the patient had arm sling on her unaffected upper limb. In terms of displacement of Center of pressure(CoP), anterior/posterior position and lateral symmetry of CoP increased. Furthermore, lateral symmetry of decreased when the patient had arm sling on her affected upper limb. ■ Conclusion Arm sling applied on affected side would be advantage to gait improvement in hemiplegic patients.

Effects of Youngseonjeatonguem on Monosodium Iodoacetate-Induced Osteoarthritis in Sprague-Dawley Rats (영선제통음(靈仙除痛飮)이 Monosodium Iodoacetate로 유도한 흰쥐의 골관절염에 미치는 효과)

  • Objectives The anti-inflammatory and anti-arthritic effects of Youngseonjeatonguem (靈仙除痛飮, YSJTU) was evaluated in a cellular model using RAW264.7 macrophages, which are involved in osteoarthritis (OA), and an animal model using Sprague-Dawley (SD) rats, and a possible mechanism of anti-arthritic actions of YSJTU was presented. Methods RAW264.7 macrophages were activated by lipopolysaccharide (LPS). The production of pro-inflammatory cytokines (tumor necrosis factor [TNF]-𝛼, interleukin [IL]-1𝛽, and IL-6) and inflammatory mediators (nitric oxide [NO] and prostaglandin E2 [PGE2]) was determined by ELISA and Griess assay, respectively. Western blot was performed to determine inducible nitric oxide synthase (iNOS) and cyclooxygenase-2 (COX-2) expression. OA was induced by intra-articular injection of monosodium iodoacetate (MIA) into the right knee joint of SD rats. Results In RAW264.7 macrophages, YSJTU reduced LPS-induced production of TNF-𝛼, IL-1𝛽, and IL-6. In addition, YSJTU inhibited LPS-induced production of NO and PGE2 by suppressing iNOS and COX-2 expression. In SD rats, YSJTU improved MIA-induced OA by reducing swelling, skin heat, and cartilage degradation. In addition, YSJTU reduced serum levels of TNF-𝛼, IL-1𝛽, and IL-6, along with its significant decrease in serum levels of NO and PGE2. Conclusions These results suggest that YSJTU may exert anti-arthritic effect, at least in part, by inhibiting macrophage-mediated joint inflammation.

The Dietary Habits and Perception of Vegetable Intake of Elementary Students in Gwangju and Jeonnam (광주·전남 일부지역 초등학생의 채소류 섭취에 대한 인식)

  • The purpose of this study was to analyze the dietary habits and perception of vegetable intake of elementary students in Gwangju and Jeonnam Gokseong county. Data collection was conducted from 5th and 6th grade students of elementary schools in Gwangju and Jeonnam Gokseong county using a structured questionnaire survey. The SPSS program was used for statistics processing and data analysis. The chi-square test was also conducted. In terms of dietary intake habits, female students consumed their meals slower than male students. Information on dietary habits and nutrition was commonly obtained from family, including the mother or father who commonly prepared meals at home. Snacks were commonly consumed less than twice daily, with the Gwangju area having a higher frequency of snacks than the Jeonnam area. Elementary students indicated that vegetables were their least favorite food, with female students having a higher interest in vegetables than male students. The pattern and perception of vegetable intake came when the students (that did not eat vegetables) were lectured by their parents on the nutritive value of vegetables. Most students understood the important nutritional ingredients of vegetables. In the case of an interest in vegetables, the Gwangju area showed significantly more comprehension than the Jeonnam area on the definition and role of dietary fiber, the dental benefits of dietary fiber, and the identification of the environment-friendly certification mark.
