SURGICAL REPAIR OF COMPLETE BONY BILATERAL CHOANAL ATRESIA VIA TRANSPALATAL APPROACH (완전 골성 양측성 후비공 폐쇄 환자의 구개부를 통한 외과적 치험례)

  • Choanal atresia may be membrane or bony, unilateral or bilateral. Approximately 90% of the choanal atresia are bony type. Unilateral choanal atresia often eludes the diagnosis because of the absence of subjective symptoms in the neonatal period. However, bilateral choanal atresia presents at birth with cyclic respiratory distress aggravated by feedings. So complete bilateral choanal atresia is considered as a neonatal emergency. Examinations for the diagnosis of chonal atresia include 1) attempt at passing a rubber catheter or probe through the patient s nose, 2) mirror examination of the nasopharynx, 3) digital examination of the nasopharynx, 4) X-ray examination after installation of radiopaque materal into the nasal cavity. But, computed tomography has become accepted method for evaluation of choanal atresia. Surgical repair of choanal atresia is accomplished via transnasal or transpalatal approach. Advantages of the transpalatal approach are improved exposure and the preservation of mucosal flap along the newly formed apertures. On the other hand, the transpalatal approach carries the risk of injury to the greater palatine neurovascular complex, and requires longer operative time. After careful physical and radiographic examinations, we accomplished the surgical repair of the complete bony bilateral choanal atresia via transpalatal approach without complications.

Seventeen Years' Experience with Ninety-six Esophageal Atresias (선천성 식도 폐쇄증 - 17년간의 96예 치험 분석 -)

  • To study the clinical profiles and outcome of surgery in infants with esophageal atresia, we reviewed 96 esophageal atresia cases who were treated from April, 1978 to June, 1995. There were 51 male and 45 female infants, a ratio of 1.1:1. The low birth weight(<2500g) patients were 32%. Clinical findings at the time of diagnosis included drooling in 57%, choking in 50%, cyanosis in 38%, respiratory distress in 27% and swallowing difficulty in 20%. Gross classification included 6 cases of type A esophageal atresia(6%), 79 cases of type C(82%), 3 cases of type E (3%) and 8 cases of type F(8%). Associated anomalies occurred in 34 infants(35%). Among them, cardiac anomalies were most common(60%). A primary repair of the defect was carried out in 76 patients with type A or C. A staged operation comprising a repair or gastric tube interposition after gastrostomy was performed in 8 patients. In all 3 infants with H-type, a division of fistula was performed. Esophageal resection and anastomosis was done in 8 infants with esophageal stenosis. In one infant, a gastrostomy was performed and he expired before staged operation. Anastomotic complications included leakage in 16 cases(17%), stricture in 37 cases(39%) and recurrent tracheoesopohageal fistula in 3 cases(3%). The mortality rate was 14% and the leading cause of death was pneumonia. The overall survival rate was 86%, and according to Waterston criteria, the survival rates were 93%, 85% and 58% in class A, Band C, respectively. 75 patients were followed up with median follow up 6.4 years. Among them, 93% were uneventful and 7% had frequent pneumonia.

Effect of Damage to Medial Amygdaloid Nucleus on Pancreatic Exocrine Secretion Stimulated by Hydrochloric Acid in the Rat (흰쥐에서 내측 편도핵의 손상이 염산 자극에 의한 췌장 외분비에 비치는 영향)

  • This study was undertaken to investigate the effect of the medial amygdaloid nucleus on the pancreatic exocrine secretion and plasma secretin concentration in 44 male albino rats. Twenty-three rats in which the medial amygdaloid nucleus was damaged bilaterally by radio frequency a.c. through stereotaxically inserted electrodes (medical amygdaloid group, MA) and twenty-one rats which received the same operation without damage (operated control, OC), were prepared. Under urethan anesthesia, 0.01 N hydrochloric acid (HCl) or physiological saline (0.9% NaCl) was infused at a rate of 0.18 ml/min into the duodenum for 20 minutes. Pancreatic jucie was collected for the 20 min infusion period. After collection of pancreatic juice, blood was sampled from the abdominal aorta for the radioimmunoassay of plasma secretin concentration. In the MA group, the exocrine pancreatic secretory response to 0.01 N HCI as well as saline infusion was significantly inhibited compared with that in the OC group. The pancreatic protein output of the MA group significantly decreased after the saline infusion and tended to decrease after the 0.01 N HCI infusion, compared with that of the OC group. However, there was no significant difference in plasma secretin concentration between the two groups. Therefore it is strongly suggested that the rat medial amygdaloid nucleus has a facilitatory influence on both basal and acid-stimulated pancreatic exocrine secretion, but the releasing mechanism of secretin appears not to be involved in the influence.

  • The purpose of this study is to find out the distribution of underground biomass and its soil stabilizing effects. Bamboo and grass were examined and compared. 1. Thirty 'soil & root' samples were collected to the depth of 30cm with Impact-corer, and then divided into three sections(0-10cm, 10-20cm, 20-30cm). Each piece had a volume of $950cm^3$ and the underground biomass was separated from the soil particles by washing with flowing water. The average underground biomass rates of bamboo and grass were 10.8% and 4% of each sample, respectively. The rate of grass root biomass declined with depth, but the bamboo root biomass was at peak at around 20cm depth. 2. The shear strength was measured with con-penetration tester at each sample collecting site. Three measurements were made at each depth(0-10cm, 10-20cm, 20-30cm) and were compared with the impact counts needed to insert the corer to the depth of 30cm. The shear strength has clear correlations with underground biomass. The more underground biomass, the higher shear strength. The shear strength of bamboo was about three times larger than the grass.

  • A multi line electric tube consisted of XL pipes contained with 2~4 hot wires and water in it. The specification of one meter length multi-line electric tube was investigated and the proper number in the multi-line electric tube was determined. A multi line electric tube with three hot wires were found to be the most efficient for the media heating control system. Temperature rise of medium in the rice hulls media was faster than that in the perlite media, showed better insulation effect of rice hulls media. Temperature rise of medium with mulching on the top of the bed was faster than without mulching, resulted in the beneficial effect of temperature rise with mulching. The regression model for the rice hulls media with mulching air temperature of $5^{\circ}C$ were a = -0.1458 and b = -0.1088. Using the model, the temperature rise of medium during low temperature season can be predicted for the various media according to the different depths.

  • Sympathetic reaction to grotesque facial injuries with oral bleeding can trigger confusion and generate inefficiency among emergency room personnel. Regardless of the extent of the injury, or of the sympathy elicated, the victim must be evaluated and treated as a whole patient with multiple trauma. There must be no confusion as to order of importance in the steps of evaluation : 1. airway 2. hemorrhage 3. shock 4. associated injuries 5. local injury 6. triage of facial injuries. The most dangerous aspect of oral hemorrhage is the possibility of its obstructing the upper airway. Swallowing large amounts of blood will usually cause gastric irritation and lead to vomiting, thus further complicating the management of the patient. Once a clear airway is assured and hemorrhage have been controlled, consideration is given to possible associated injuries before undertaking treatment of the facial injuries themselves. Status of the cardiopulmonary, gastrointestinal and neuromuscular systems all have a bearing on decisions concerning facial injury treatment. In this light, treatment priority becomes exceeding important. On the other hand, the importance of facial injuries should never be minimized, especially in a mentally retarded patient with oral hemorrhage. This is a case report about oral hemorrhage control as treatment priority in a mentally retarded patient.

Estimation of the Central Aortic Pulse using Transfer Function and Improvement of an Augmentation Point Detection Algorithm (전달함수를 이용한 대동맥 맥파 추정 및 증강점 검출 알고리즘 개선에 관한 연구)

  • Aortic AIx(augmentation index) has been used to measure aortic stiffness quantitatively and even to evaluate ventricular load. However, in order to calculate aortic AIx catheters should be inserted to the subjects' artery, which hampers its clinical usage. To overcome such limitation, aortic AIx has been indirectly calculated by estimating aortic pressure wave from the peripheral arterial pulse by applying transfer functions. In this study, central aortic pressure waves using Millar catheter and radial artery pulse waves using tonometry pressure sensor were measured to establish transfer functions for an estimation of central aortic pressure waves from radial artery pulse waves. Also, an algorithm which detects augmentation point for the calculation of AIx were developed. Developed algorithm for the detection of augmentation point gradually increases the differential order to detect inflection point rather than detects the distinctive point that appears after a specific time. Transfer functions were established using 10th order ARX model and were verified for the stability of the transfer function through residual analysis. Evaluation of an algorithm for the detection of augmentation point were performed by comparing the augmentation points obtained from developed algorithm with the known augmentation points synthesized in various conditions. In addition, developed algorithm for the AIx is proved to provide more accurate results than the ones developed by previous studies. The significance of the study was in two folds. Firstly, the results could provide the basis for the measurement of aortic stiffness using easily-measurable radial artery pulse waves, and secondly, extension of the study may enable the early diagnosis of various vascular diseases.

A Reconfigurable Active Array Antenna System with Reconfigurable Power Amplifiers Based on MEMS Switches (MEMS 스위치 기반 재구성 고출력 증폭기를 갖는 재구성 능동 배열 안테나 시스템)

  • In this paper, a novel frequency reconfigurable active array antenna(RAA) system, which can be reconfigurable for three reconfigurable frequency bands, is proposed by using commercial RF MEMS switches. The MEMS switch shows excellent insertion loss, linearity, as well as isolation. So, the system performance degradation of the reconfigurable system by using MEMS switches can be minimized. The proposed frequency reconfigurable active antenna system is consisted with the noble frequency reconfigurable front-end amplifiers(RFA) with the simple reconfigurable impedance matching circuits(RMC), reconfigurable antenna elements(RAE), as well as a reconfiguration control board(RCB) for MEMS switch control. The proposed RAA system can be reconfigurable for three frequency bands, 850 MHz, 1.9 GHz, and 3.4 GHz, with $2{\times}2$ array of the RAE having broadband printed dipole antenna topology. The validity of the proposed RFA as well as RAA is also presented with the experimental results of the fabricated systems.

Laparoscopy-assisted Total Gastrectomy with Pancreas-preserving Splenectomy for Early Gastric Cancer: A Case Report (조기위암에서 복강경보조 위전절제술 및 췌장보존식 비전절제술 1예)

  • We report our experience with a case of performing laparoscopy-assisted total gastrectomy along with pancreas-preserving splenectomy for treating early gastric cancer. laparoscopy-assisted total gastrectomy was planned for a 62-year-old male patient with a double early gastric cancer located in the upper and lower third of the stomach. Five trocars were placed and we used a harmonic scalpel to dissect the greater curvature. Enlarged splenic hilar lymph node was encountered and they were proved to be metastasis by frozen section biopsy. We then performed total gastrectomy with pancreas-preserving splenectomy for the purpose of completely dissecting the lymph nodes along the splenic artery and splenic hilum. We created a 4 cm sized longitudinal mini-laparotomy below the xiphoid process to remove the specimen, and anastomosis was done via the Roux-en-Y method. The patient was discharged on the 9th postoperative days after an uneventful recovery. Our experience shows that laparoscopy-assisted total gastrectomy with pancreas-preserving splenectomy is a relatively safe procedure for treating upper third early gastric cancer with metastatic splenic hilar lymph nodes.

  • Three clones of Populus alba ${\times}$ P. grandidentata have been tested for susceptibility to Agrobacterium tumefaciens strains A281 and A348. We determined the optimum concentration of kanamycin sulfate for effective selection of leaf disc-derived, transgenic tissues transformed using Agrobacterium binary vector pGA472 containing a neomycin phosphotransferase gene (NPT-II) which confers kanamycin resistance. Of the wild type Ti plasmids contained by the two Agrobacterium strains, pTiBo542 of strain A281 appears to be best suited to serve as a helper plasmid for binary vector systems. A relatively low concentration (10mg/l) of kanamycin sulfate inhibited adventitious shoot initiation from leaf discs on regeneration medium. Transformed kanamycin-resistant calli were obtained by culturing Agrobacterium inoculated leaf discs on selective regeneration medium. The transformed kanamycin-resistant calli continued to grow on regeneration media supplemented with kanamycin sulfate to levels of 50 and 200mg/l. The growth of non-co-cultivated control calli was severely inhibited on regeneration medium containing 50mg/l kanamycin sulfate.
