EVALUATION OF CONDYLAR DISPLACEMENT USING COMPUTER TOMOGRAPHY AFTER THE SURGICAL CORRECTION OF MANDIBULAR PROGNATHISM (전산화단층촬영법을 이용한 하악전돌증 환자의 외과적 악교정술후 하악과두 위치 변화 검토)

  • This study was intended to perform the influence of condyle positional change after surgical correction of skeletal Class III malocclusion after orthognathic surgery in 37 patients(male 13, female 24) using computed tomogram that were taken in centric occlusion before, immediate after, and long term after surgery and lateral cephalogram that were taken in centric occlusion before, 7 days within the period of intermaxillary fixation, at the 24 hours later removing intermaxillary fixation and long term after surgery. 1. Mean intercondylar distance was $84.42{\pm}5.30mm$ and horizontal long axis of condylar angle was $12.79{\pm}4.92^{\circ}$ on the right, $13.53{\pm}5.56^{\circ}$ on the left side. Condylar lateral poles were located about 12mm and medial poles about 7mm away from the reference line(AA') on the axial tomogram. Mean intercondylar distance was $83.15{\pm}4.62mm$ and vertical axis angle of condylar angle was $76.28{\pm}428^{\circ}$ on the right, $78.30{\pm}3.79^{\circ}$ on the left. 2. In amount of set back, We found the condylar change(T2C-T1C) which had increasing tendency in group III (amount of setback : 10-15mm). but there was no statistical significance(p>0.05). 3. There was some correlation between condylar change(T2C-T1C) and TMJ dysfunction. It seemed that postoperative condylar change had influenced postoperative TMJ dysfunction, through there was no statistical significance (p>0.05). As we have observed the change of condylar axis in the group that complained of TMJ dysfunction in cases of large amount of mandibular setback. So we consider that the more trying to conserve condylar position will decrease occurrence rate of post operational TMJ dysfunction.

Dentofacial changes of non-orthodontically treated female patients with TMJ disk displacement: a longitudinal cephalometric study (교정 치료를 받지 않은 측두하악관절원반변위가 있는 여성 환자의 두부계측방사선사진을 이용한 안모 및 치열 변화 연구)

  • Objective: The purpose of this study was to investigate the changes in dentofacial morphology of non-orthodontically treated female patients with TMJ disk displacement. Methods: The sample consisted of 25 Korean female patients with bilateral TMJ disk displacement who visited the Department of Orthodontics, Seoul National University Dental Hospital from 1996 to 2006. Disk displacements were diagnosed using the magnetic resonance imaging (MRI) of both TMJs. Baseline (T1) and follow-up (T2) lateral cephalograms were analyzed. The mean age of samples at T1 was $18.1{\pm}3.5$ years (range 14.2 - 25.8 years) and at T2, $21.1{\pm}3.5$ years (range 16.2 - 28.0 years). The mean observation period was $3.0{\pm}1.9$ years. Descriptive statistics for each variable were calculated at baseline (T1) and follow-up (T2) stages, and during the observation period (T2-T1). Results: Skeletal changes were found in 64% of the non-orthodontically treated female patients with TMJ disk displacement during the observation period. The L1 to Mandibular plane distance (mm) increased significantly by 0.8 mm (p < 0.01). But there were no significant differences in the other dental relationship variables (overjet, overbite, U1 to palatal plane) during the observation period. Most patients with skeletal changes showed a backward rotation of the mandible. The ratio of the rotation was a decrease of SNB by $0.43^{\circ}$ for every $1^{\circ}$ increase of FMA (Spearman rho = -0.660, P < 0.01). A few patients showed a distal shift of the mandible without rotation or significant changes in the vertical dimension. Conclusions: During observation periods without orthodontic treatment, non-growing patients with TMJ disk displacement showed dentoskeletal changes, mainly backward rotation of the mandible.

The Relationship between Bone Bruise in MRI and Associated Injuries after the Knee Joint Trauma (슬관절 외상 후 자기 공명 영상에서 관찰되는 잠재성 골 병변과 동반 손상과의 관계)

  • Purpose : The purpose is to analyze the relationship between bone bruise in MRI and associated injuries after the knee joint trauma. Materials and Methods : Total 50 cases were reviewed, divided into two groups according to initial trauma energy. High-energy group (group I), such as traffic accident was 13 cases, low-energy group (group II), such as sports trauma was 37 cases. The type of the lesion was used the classification by Costa-Paz. The site of lesion was analyzed according to femoral/tibial, medial/lateral and anterior/middle/posterior site respectively. Associated injuries were confirmed by physical examination, radiograph, MRI and arthroscopy. Results : In group I there was a various distribution of the bone bruise in the knee joint according to mechanism of injury. The bone bruise with ACL injury was 38$\%$ only and the most frequent type was Costa-Paz type I (52.6 $\%$). In group II more frequent locations were the middle portion of the lateral femoral condyle and the posterior portion of the lateral tibial condyle. Bone bruise associated with ACL injury, was upto 56.8 $\%$ and the frequent type were Costa-Paz type II (48 $\%$), in order type I (42 $\%$). As a result, in the high-energy injury the bone bruise had a various location in both condyle and less frequently associated injury, but in the low-energy injury there was particularly frequent location of bone bruise, associated injury and type. Conclusions : We could assess the associated injury by analysis of the location and type of bone bruise, especially in the low-energy injury, e.g. sports injury. But further study will be necessary with more case analysis.

Seasonal Sedimentary Characteristics and Depositional Environments after the Construction of seawall on the Iwon Macrotidal Flat (방조제 건설 후 이원 대조차 조간대의 계절별 퇴적학적 특성 및 퇴적환경)

  • In order to elucidate seasonal sedimentary characteristics and depositional environment after construction of seawall on macrotidal flat, a seasonal observations of surface sediments (total 450) and sedimentation rates on 4 transects have been investigated for 2 years. The eastern area of Iwon tidal flat, has been changed from semi-closed coast to open coast by construction of seawall, shows general seasonal changes similar to characteristics of open coast type, which represented both fining and bad sorted distribution due to deposition of fine sediments under low energy condition in the summer, and relatively coarser and better sorted distribution because of erosion of fine sediments in the winter. In considering angles of transects, distribution patterns of surface sediments, the northern and southern parts of eastern tidal flat are dominantly influenced by wave and tidal effects, respectively. As time goes by, the eastern tidal flat shows coarsening-trend of surface sediments caused by direct effect of tidal current, were and typhoon. Meanwhile the western area of seawall, which has been re-formed by construction seawall, is sheltered from northwesterly seasonal wind. The seasonal change pattern of western area of seawall is slightly different from that of eastern tidal flat. Mean grain size and sorting of surface sediments during spring is finer and worse than those during summer. This seasonal change pattern maybe influenced by topographic effects caused from the construction of seawall. In consideration of all result, the transport of fine sediments in the study area, which is supplied to limited sediments, shows clockwise circulation pattern that fine sediments are transported from the eastern tidal flat to the western area of seawall because of blocking of seawall in the winter and are transported reversed direction the summer. As a result, many changes have been observed in the study area after construction of seawall; however, this change is still in progress and is expected to need continuous monitoring.

Analysis of golf swing motion for specific properties of club shaft (클럽 샤프트(Club Shaft) 특성에 따른 골프 스윙(Golf Swing)동작 분석)

  • The purpose of this study was to find the rational method to analyze golf swing with specific property of club shaft. Three subjects were filmed by two high speed digital cameras with 500 fps. The phase analyzed was downswing of each subject. The three-dimensional coordinates of the anatomical landmarks were obtained with motion analysis system Kwon3d 3.0 version and smoothed by lowpass digital filter with cutoff frequency 6Hz. From these data, kinematic and kinetic variables were calculated using Matlab(ver 5.0) The variables for this study were angular velocity and accelerations, which were calculated and following conclusions have been made : 1) Golf swing time of stiff club is faster than that of regular club. 2) In shoulder joint motion of swing with the stiff club, x-stiff showed mort rapid negative acceleration than that of regular club. 3) In regular club, the velocity of club head would be more effective velocity, which was increasing, than those of other clubs before impact. 4) In wrist joint motion of swing with stiff club, x-stiff club showed faster than regular club in the downswing and impact more rapid negative acceleration.

A Study of Radiation Incidence Angle in Anteroposterior Cervical Vertebra Examination (경추 정면 검사에서 방사선 입사각에 관한 연구)

  • In anteroposterior projection for cervical vertebra, it is general that the incidence angle of X-ray is $15^{\circ}$ to $20^{\circ}$ degrees to head in order to prevent overlap of mandible and occipital bone and to observe array of cervical interbody and shapes of joints. However, the angle is appropriate for foreigners that was determined by foreign literature review long ago, and there have been few researches of incidence angle for Koreans' body type. The purpose of in this study are to identify the incidence angle appropriate for Koreans and to present methodology. In order to measure the incidence angle, 1,044 patients who visited S Hospital located in Seosan were selected and measured of average length of cervical vertebra, OID, axis angle, and FID. The incidence angle was calculated from the applied formula by measuring average values per age groups and sex (see Formula 1 and 2). The average length of cervical vertebra was 6cm: the length was increased from teenagers to twenties but was decreased since thirties. The difference between males and females was around 1cm (p<.01). The OID was almost the same regardless of age groups and sex. As for axis angle, the slope was increased in teenagers and twenties, but was decreased since thirties. The difference between males and females was around 2 degrees (p<.01). The FID measurements were almost the same regardless of age groups and sex, and when the incidence angle was measured from these values, the teenagers were $15.9^{\circ}$, the twenties were $16.9^{\circ}$, the thirties were $16.6^{\circ}$, the forties were $16.2^{\circ}$, the fifties were $15.9^{\circ}$, and the sixties were $14.5^{\circ}$, indicating that the angle was increased from teenagers to the twenties but decreased since the thirties. While the angles of males and females were measured to be the same in the teenagers, the angle was different between males and females by $2^{\circ}$. When the incidence angle statistically analyzed with measurement of average length of cervical vertebra, OID, axis angle, and FID, all of them were shown to have correlations with the incidence angle (p<.01). Conclusively, it was shown that the incidence angle was measured differently from average length of cervical vertebra, OID, FID, and axis slope, as well as from age and sex. Therefore, it can be suggested that the anteroposterior radiation test for cervical vertebra should be conducted by different incidence angles based on age and sex. The data of this study may be used as reference in determining the incidence angle of cervical vertebra tests for the future.

MR Evaluation of Tendinous Portions in the Subscapularis Muscle (견갑하근의 건 부분에 대한 자기공명영상을 이용한 분석)

  • Purpose: The purpose of this study was to document the structural features of the tendinous portions within the non-pathologic subscapularis muscle by performing high resolution MR imaging of the shoulder. Materials and Methods: Between April 2007 and May 2010, we retrospectively obtained the MR scans of 88 consecutive young patients (88 shoulders) who were in their twenties. MRI and MR arthrography were performed using a 3.0-T system for the evaluation of glenohumeral instability and nonspecific shoulder pain. None of the patient in this study had any evidence of injury to the tendon or muscle belly of the subscapularis. On MR images, we recorded the transverse length of a stout tendinous band and the total tendinous portion of the subscapularis. In addition, we recorded the number of intramuscular tendinous slips of the susbscapularis. Results: The mean transverse length of the tendinous band was 15.0 mm (range: 8 to 20 mm). The mean transverse length of the total tendinous portion was 48.9 mm (range: 40 to 60 mm). The number of intramuscular tendinous slips on the base of the glenoid fossa was 3 in 20 (22.72%), 4 in 45 (51.14%) and 5 in 23 shoulders (26.14%). On the lateral portion, the intramuscular tendinous slips became gradually rounder and thicker and they gave converge in the superior direction. Conclusion: In this study, the structural features of the tendinous portions of the subscapularis on the MR scans were identified. This will in return give good justification for the lines to be pulled during biomechanical stimulation and also for the surgical approach to restore the biomechanical function.

Validity experiment of dipping method in the manufacture of metal-ceramic substructure (금속 도재 하부구조물 제작 시 침적법의 타당도 실험)

  • Purpose: The main purpose of this study is to suggest application plan of dipping method by comparing & analysing the difference between addition method and dipping method in the manufacture of metal-ceramic substructure of maxillary central incisor (#1), followed by assessing the work efficiency. Materials and Methods: Master die and hard plaster-copied one were produced and then a total of 20 copies, 10 copies for each through addition method and dipping method, were manufactured and experimented. Copings were fixed on the abutment model and invested in epoxy mounting cup. Samples were cut in labial and lingual direction, using cutter and then rubbed on sandpaper, whose 4 points were measured by using digital microscope. Results: The comparison of mean values by using t-test, parametric statistical method, shows overall significant difference (P < 0.05). Conclusion: The result of this study can be suggested as an application plan, since there is no significant difference between addition method and dipping method in the manufacture of metal-ceramic substructure.

Experimental Study on the Evolution of Tip Vortex Structures Generated by a Two-Bladed Rotor (2개의 블레이드로 구성된 회전익 끝와류들의 간섭 특성)

  • In order to observe the wake interaction between tip vortices generated by a two-bladed rotor with slightly different pitch angles, the velocity components of the tip vortices were measured by using a two-dimensional LDV system. It was observed that the swirl velocity components of the ensuing blade deviated from the Vatistas' n = 2 vortex model and the axial velocity components of the preceding blade deviated from the Gaussian profile. It was also found that in the wake-age range of $200^{\circ}$ to $240^{\circ}$, the filament of the ensuing blade tip vortex was stretched as result of the closing in of two vortices. The results from these observations suggest the possibility that a similar wake interaction is generated in actual rotor blades, especially, in the ones with articulated hubs.

  • The purpose of this paper is to analyze lower extremities pattern of daily activities and walking on the treadmill with passive body support system. The experiments will be used for basic research of developing active harness system. The experimental procedure has been validated on 5 healthy male subjects and we measured foot pressure and 8 section of lower limb muscles activities. The EMG results of ADL indicate that there have specific muscle activity patterns followed by each activities. The vastus lateralis muscle activities were highly seems on the activities that requires flection and extension of knee joint. The foot pressure value of stair descent activity was the highest due to the fact that it is come down to the direction of gravity. The results with the passive body weight support percentages show that the EMG and foot pressure values were declined according to increasement of the body weight support percentages. Therefore the body weight support system could apply gait rehabilitation system for various patients by changing the percentages of the body weight support.
