Ultrasonographic Examination of Compression Neuropathy in the Upper Extremity (상지의 압박성 신경병증의 초음파 검사)

  • Compression neuropathy around elbow and wrist are one of the common disturbing problems in the upper extremity. The understanding of normal nerve architectures and pathophysiologic changes in compression neuropathy is important to interpret the ultrasonographic images correctly. Compression neuropathies have characteristic ultrasonographic imaging features of flattened nerve at compression and hypoechoic swollen nerve with loss of fascicular patterns at proximal segments. Dynamic ultrasonographic imagings on motion can show dymanic subluxation of ulnar nerve and medial head of triceps muscle over the medial epicondyle in snapping triceps syndrome. Dynamic compression of median nerve also can be visualized in pronator teres syndrome by dynamic imaging studies. A quantitative measures of cross sectional area or compression ratio can be helpful to diagnose compression neuropathies, such as carpal tunnel syndrome or cubital tunnel syndrome. With the clinical features and electeophysiologic studies, the untrasonographic imagings are useful tool for evaluation of the compression neuropathies in the upper extremities.

A Case Report of Patient with Recurrent Patellar Dislocation Treated by Korean Medicine Treatment in Combination with Intra-articular Bee Venom Injection and Needle-embedding Therapy (관절강내 봉약침과 매선요법을 병행한 슬개골 재발성 탈구 증례보고)

  • The purpose of this study is to observe the effect of Korean medicine treatment in combination with intra-articular bee venom injection and needle-embedding therapy on recurrent patellar dislocation in patient. The Patient, diagnosed as recurrent patellar dislocation, was treated by Korean medicine treatment in combination with intra-articular bee venom injection and needle-embedding therapy. Visual analogue scale (VAS), knee flexion range of motion (ROM), Korean Western Ontario and Mcmaster Universities arthritis index (K-WOMAC) were used to measure changes during treatment. After treatment, VAS, knee flexion ROM, K-WOMAC were improved significantly. The Korean medicine treatment in combination with intra-articular bee venom injection and needle-embedding therapy was proved to be helpful to improve the symptoms of the recurrent patellar dislocation.

Self Management of Pain by Folk Remedies in Patient with Chronic Arthritis (만성 관절염 환자의 민간요법을 이용한 자가통증조절 행태)

  • The purpose of this study was to explore self management of pain by folk remedies in patient with chronic arthritis. The participant of this study were 90 volunteers who were visit C hospital and who want to counsel with researcher for their pain control by convenient sampling method. The results of this study were summarized as follows : The subjects were the 90 patients who had been diagnosed rehumatoid arthritis(52.5%)and osteoarthritis(47.8). 93.3% of participants were women. Mean age of participants was 53.7 and duration of arthritis was 7 years. Mean pain score was 5.5cm. Eighty five kinds of folk remedies were used for pain management. The mean cost for folk remedies was 3,723,207 won and the patients with rheumatoid arthritis paid to folk remedies as much as 3 times. The perceived effect score of pain management by folk remedies was 2.932(the range of perceived effect score were 0-10cm). Percentage of discontinue to use folk remedies was 82.8% and the major reason of discontinue to use folk remedies was no effect to pain control(50.4%). Some participants(30.5%) were experienced side effect such as exacerbation of pain(50.5%). In conculusion, chronic arthritis patients have been experienced various kind of folk remedies for the management of their pain. But the effect of pain control was very low. The important weakness of folk remedies were economical waste and loss of opportunity to treatment for arthritis. The nurse who care for arthritis should be teach about weakness of folk remedies for their pain control to the patients with arthritis.

The Mechanical Properties of Trabecular Bone in Knee Joint (무릎관절 해면뼈의 기계적 물성)

  • In this study, we performed the compressive strength test of trabecular bone in knee joint for measuring the elastic modulus and ultimate strength. The main knee joint is femorotibial articulation between the lateral and medial femorotibial condyle. In the case of osteoarthritis, some patients have only medial condylar osteoarthritis. We performed the mechanical test for comparison the difference of the each condylar strength. We used diamond core-drill and linear precision saw for making the specimens. Specimens were cored from both condyle in distal femur and proximal tibia in fresh cadaver (male 10, female 12), and tested by universal test machine with temperature control saline circulation system. Results of the test in distal femoral parts, averaged elastic modulus was $360.61{\pm}159.40MPa$ for male, $150.89{\pm}70.65MPa$ for female. Averaged ultimate strength was $6.79{\pm}2.91MPa$ for male, $2.89{\pm}1.31MPa$ for female. Male was 2.4 times stronger than female. In the proximal tibial parts, averaged elastic modulus was $108.80{\pm}52.88MPa$ for male, $73.45{\pm}55.06MPa$ for female. Averaged ultimate strength was $2.59{\pm}1.39MPa$ for male, $1.75{\pm}1.16MPa$ for female. Male was 1.5 times stronger than female. In the distal femoral condyle, medial condyle had more strength than lateral condyle at middle region. But lateral condyle had more strength than medial condyle at anterior & posterior regions (p<0.02). In the proximal tibial condyle, medial condyle had more strength than lateral condyle. (p<0.01).

Behavior Analysis of the Treated Femur and Design of Composite Hip Prosthesis (대퇴부 거동 해석 및 복합재료 보철물 설계)

  • The nonlinear finite element program has been developed to analyze the design performance of an artificial hip prosthesis and long term behavior of a treated femur with stems made of composite material after cementless total hip arthroplasty(THA). The authors developed the three dimentional FEM models of femoral bone with designed composite stem which was taken with elliptic cross section of 816 brick elements under hip contact load and muscle farce in simulating single leg stand. Using the program, density changes, stress distributions and micromotions of the material femoral bone were evaluated by changing fiber orientation of stems for selected manufacturing method such as plate cut and bend mold. The results showed that the composite materials such as AS4/PEEK and T300/976 gave less bone resorption than the metallic material such as cobalt chrome alloy, titanium alloy and stainless steal. It was found that increasing the long term stability of the prosthesis in the femur could be obtained by selecting the appropriate ply orientation and stacking sequence of composite.

Exercise Pattern and Influencing Factor of Exercise Barrier in Patients with Osteoarthritis (골관절염 환자의 운동양상과 운동장애 영향요인)

  • Exercise is an important strategy for health promotion in patients having osteoarthritis. But, lots of patients with osteoarthritis were underexercised. Exercise pattern and influencing factors of exercise barrier are not well-known. To address this issue, we studied the exercise pattern and influencing factors of exercise barrier in patients with osteoarthritis. The subjects of the study were 463 adult osteoarthritis (Mean age = 61.63 years) who had diagnosed osteoarthritis by rheumatologist. Data were gathered from May 1999 to February 2000 using a questionnaire and exercise barrier(Sallis et al, 1989), exercise pattern(Lee et al., 2000), physical status by WOMAC(Bellamy, 1989), socail support(Sallis et al., 1989), fatigue and pain using graphic rating scale, depression by CES-D(Radloff,1977). Data were analyzed with the SPSS win 6.0 using frequency, ANOVA, Stepwise multiple regression. The results of this study were as follows; 1) 56.4% of sample was 'do not exercise at all', 'longer rest than exercise', was 15.9%, 'longer exercise than rest' was 7.2%, 'exercise regularly' was 20.5%. 2) Social support (F=10.349, p=0.000) and exercise barrier(F=4.455, p=0.004) were showed significantly difference by exercise pattern. 3) Influencing factors of exercise barrier were depression and social support. Thoses explained 13.3% of exercise barrier. In conclusion, half of osteoarthritis patient did not do exercise and it was shown that depression and social support were major influencing factors to exercise barrier. The results of this study can be applied to develop the health promoting educational program for patients with osteoarthritis.

  • In this paper, we propose gesture recognition based 3D Serious Games to prevent dementia. These games are designed to enhance the effect of preventing dementia by helping increase brain usage and physical activities of users by the entire body gesture recognition. The existing cameras used for gesture recognition technology are limited in terms of recognition ratio and operation range. For more stable recognition of the body gestures, we recognized users with a 3D depth camera, obtained joint data of users, and analyzed joint motions to recognize gestures of the body. Game contents were designed to practice memory, reasoning, calculation, and spatial recognition focusing on the atrophy of brain cells as a major cause of dementia. Game results of each user were saved and analyzed to measure how their recognition skills improved.

Robust Walking Algorithm of Biped Robot on Uneven Terrain (비평탄 지형에서 이족로봇의 강인한 보행 알고리즘)

  • Biped robot with high DOF has instability in mechanism. Therefore, it is important to guarantee walking stability of biped robot. Biped robot can stably walk on the flat ground using static walking patterns. However, walking stability of robot becomes increasingly worse on the uneven terrain. In the paper, we propose a robust walking algorithm of biped robot with motion stabilization to solve the problem The proposed algorithm was designed to stabilize walking motions based on the inclination of robot body using a gyro sensor and a accelerometer equipped in the center of the upper body. If unstable motions are recognized, angles of each joints are modified to increase stability by using compensation of angles of lower legs. The experimental results show that biped robot performs stable walking on the uneven terrain.

  • The purpose of this study is to explore the treatment pattern of treatment of patients with arthritis. The grounded theory approach methodology was used in this study. The purposive sampling was conducted. 16 subjects who experienced RA, lived in middle sized city in Korea, and all women The results of this study were as follows : 1. The process of treatment was composed of the stage of symptom experience and the stage of sick role experience. The naming of the symptom was conducted in the stage of symptom experience through, the doctor shopping. The sick role experience was patterned by the choice of the treatment mode. 2. The treatment modes were roughly devided by western medicine and oriental or ethnic medicine. Several factors which affected the choice of the treatment mode were patient's economic state, educational level, religion, the acknowledgement of the cause of illness, duration of illness, lay referral system, the relationship of medical staff, and the acculturation of medical professionalism. The key component of the decision of the treatment was the level of the acculturation of medical professionalism. To enhance the acculturation of medical professionalism, we have to provide the information of cause and the prognosis of the disease to the patients and the variety of communication channel between medical staff and patients, and we should understand the medical culture according to the ages, gender, locations in Korea.

The Relationship among Self-Efficacy, Pain, Depression, and ADL in Chronic Arthritis (만성 류마티스 관절염 환자의 자기효능감, 우울 및 일상활동과의 관계)

  • Many arthritis patients experience physical and psychological impairments. Two hundred and three patients were examined to identify the relationships between the variables such as pain, ADL depression, self-efficacy and the effect of self-efficacy on pain, ADL and depression. It was found that ADL is significantly negatively related to depression and pain, and positively related to self-efficacy. Pain was significantly negatively related to self-efficacy, and positively related to depression and ADL. Then stepwise multiple regression was performed to Identify the variables to predict ADL, pain and depression. Self-efficacy predicted negatively depression and positively ADL. Depression predicted positively pain. So it was revealed that self-efficacy decreased pain indirectly by influencing depression. Also Pain predicted negatively ADL. ADL was predicted directly by pain and self-efficacy. So it is suggented that nursing intervention improving self-efficacy would be useful for patients with depression, pain, and ADL limitation. Moreover, these subjects gained 44 score in depression which means severe depressed state. So it is very important to implement nursing intervention to increase self-efficacy. Recommendations for future research was suggested.
