관성측정장치를 이용한 경추관절 가동범위 측정의 검사 내 반복성 및 검사-재검사 신뢰도 연구 (Test-retest Reliability and Intratest Repeatability of Measuring Cervical Range of Motion Using Inertial Measurement Unit)

  • Objectives : To assess the test-retest reliability and the intratest repeatability in measuring the cervical range of motion of healthy subjects with wireless microelectromechanical system inertial measurement unit(MEMS-IMU) system and to discuss the feasibility of this system in the clinical setting to evaluate the cervical spine musculoskeletal. Methods : 12 healthy people who were evaluated as no- or mild-disability with neck disability index were participated. Their cervical motion were measured with IMU twice in consecutive two days for the test-retest reliability study. Intratest repeatability was calculated in the two tests separately. The calculated intraclass correlation coefficients(ICC) were discussed and compared with the those of the previous studies. Results : Cervical range of motion data were acquired and statistically processed: left rotation($61.64^{\circ}$), right rotation($65.12^{\circ}$), extension($61.98^{\circ}$), flexion($52.81^{\circ}$), left bending($39.31^{\circ}$), right bending($41.08^{\circ}$). ICCs were 0.77~0.98(intratest repeatability) and 0.74~0.93 (test-retest reliability) in the primary motion. In the coupling motion, intratest repeatability ICCs were 0.93~ 0.99(transverse primary plane), 0.88~0.97(saggital primay plane), and 0.77~0.93(coronal primary plane). Test-retest reliability of coupling motion were 0.90~0.97(transverse primary plane), 0.00~0.72(saggital primary plane), and 0.04~0.76(coronal primary plane). Conclusions : Several types of range-of-motion devices are now on use in many fields including medicine, but the practicality of the devices in clinical use is questionable for the convenient and economical aspects. In this study, we presented the reliability of cervical range of motion test with the developed wireless MEMS-IMU system and discussed its potential utility in clinical use.

뇌병변장애인의 일상생활을 위한 맞춤형 보조기구의 설계·제작·적용 사례 연구 (Case Study on Design, Manufacture and Application of Customized Assistive Device for the ADL of Person with Brain Lesions)

  • 본 논문은 뇌병변장애인의 일상생활을 위한 맞춤형 보조기구 설계 제작 적용 사례연구로 대구광역시보조기구센터 장애인 보조기구 사례관리 시범사업의 2010년 일상생활영역의 맞춤 서비스 사례 중 2명을 대상으로 2010년 10월 18일부터 11월 17일까지 1개월간 진행되었다. 대상자 A는 47세 뇌병변장애인으로 양하지의 근력과 기능이 많이 저하되어 바닥에서만 생활하며 스스로 변기에 올라 앉아 배변활동을 할 수 없었다. 이에 대상자 A에게 이동식 좌변기와 맞춤 경사로를 설계 제작하여 상지의 잔존근력으로 경사로를 올라가서 이동식 좌변기에 스스로 앉아 배변활동을 할 수 있도록 적용하였다. 대상자 B는 8세 뇌병변장애인으로 손 기능이 저하되어 스스로 식사를 할 수 없었다. 기존에 나와 있는 여러 가지 식사보조도구들을 1차 적용하였으나 대상자에게 적합하지 않아 대상자 의 관절가동범위(ROM)와 손 크기를 고려하여 맞춤으로 식사보조도구를 설계 제작하였고, 작업치료와 연계하여 지속적인 훈련을 통해 향후 스스로 식사를 할 수 있도록 적용하였다. 대상자 A와 대상자 B는 기존에 나와 있는 제품의 적용이나 개조가 접근할 수 없는 사례였음으로 대상자에게 맞춤형으로 설계 제작되어 적용되었다. 본 사례연구를 통해 맞춤형 보조도구의 설계 제작 서비스의 대한 중요성과 필요성을 인식할 수 있다.

탄력밴드를 이용한 PNF 운동이 유방암 절제술에 속발한 상지 림프부종 환자의 부종, 관절가동범위 및 통증에 미치는 영향 -열린 홀딩과 닫힌 홀딩의 차이- (The Effects of Proprioceptive Neuromuscular Facilitation (PNF) Using Elastic Bands on Edema, Range of Motion, and Pain in Post-Mastectomy Patients with Upper Limb Lymphedema: Differences between Open-Hand and Closed-Hand Grips)

  • Purpose: This study was conducted to examine the effects of a PNF intervention using elastic bands on edema, range of motion (ROM), and pain in post-mastectomy patients with upper limb lymphedema, according to their hand grip type. Methods: The subjects were 14 female patients who were at Stage II lymphedema after undergoing mastectomy for Stage II breast cancer. They were randomly divided into an experimental group (n =7) and a control group (n = 7). Both groups participated in a treatment program four times (one time under the therapist's instructions and three times as self-exercise) a week for 4 weeks. Both groups repeated an upper limb PNF pattern exercise for 30 min using elastic bands after receiving a manual lymph drainage treatment for 1 hour. Here, subjects in the experimental group performed the PNF exercise with an open-hand grip by putting their hands into the loops of elastic bands and keeping their fingers spread out. Subjects in the control group performed the PNF exercise with a closed-hand grip by holding the loops of elastic bands with their fingers. In both groups, the subjects' edema, ROM, and pain levels were measured before the intervention and 2, 3 and 4 weeks after the intervention. Results: As a result of the experiment, both groups demonstrated edema reductions, ROM increases, and pain reductions in four areas of the upper limbs. Notably, the experimental group exhibited larger ROM increases in flexion, extension, and abduction as well as greater edema and pain reductions than the control group. In particular, the most significant effects were found in the elbow of the four upper limb areas for edema reductions and in extension for ROM increases. Conclusion: The present study indicates that exercise therapy-based approaches using elastic bands in post-mastectomy patients with upper limb lymphedema can have different effects depending on the type of hand grip (open or closed), which is the body's most distal part. Therefore, these approaches should be based on the conditions of the distal parts of the patient's body for their effective applications in clinical practice.

젊은 성인에서 능동 움직임을 결합한 근막감압치료 적용과 정적 적용이 관절가동범위, 근력, 기능적 움직임에 미치는 영향 (The Effects of Active Movement Myofascial Decompression Therapy and Static Myofascial Decompression Therapy on Range of Motion, Muscle Strength, Functional Movement in Young Adults.)

  • Purpose : Myofascial decompression is frequently mentioned as a method applied to cupping. The purpose of this study is to evaluate and compare active range of motion (AROM), muscle strength, and functional movement by applying myofascial decompression to the hamstrings. Methods : This study evaluated AROM, muscle strength, and functional movement by applying active movement myofascial decompression and static myofascial decompression to the dominant leg, respectively, in a crossover design conducted with normal adults (n=21) in their average 20s enrolled at G University in G city, Gyeongsangbuk-do. Active movement myofascial decompression was implemented for five minutes at a rate of 100 bpm to make the beats in flexion and extension respectively. Static myofascial decompression was only performed for five minutes while at rest. All of these interventions were performed at a cupping depth of two mm. After a one-week washout period, static was applied again to compare the same dependent variables. Results : Regarding AROM and muscle strength, both groups showed significant differences in the before and after results (p<.05). However, in the Functional Reach Aspect and Single Leg Hop test, the active movement myofascial decompression group showed better results. There was no statistical difference between the Active movement myofascial decompression group and Static myofascial decompression group in any dependent variable (p<.05). Conclusion : As a result of this experiment, both active movement myofascial decompression and static myofascial decompression had a positive effect on dependent variable. Therefore this study is meaningful in that it is easier and simpler to see the effect on flexibility, muscle strength, and functional movement just by implementing movement myofascial decompression.

진동을 동반한 슬링 운동이 어깨 손상 환자의 관절가동범위, 근력, 통증, 기능장애 수준에 미치는 영향 (Effects of Sling Exercise With Vibration on Range of Motion, Muscle Strength, Pain, Disability in Patients With Shoulder Injuries)

  • Background: Sling exercises are frequently used for the rehabilitation process of patients with shoulder joint injuries, but research on the significant frequency intensity and appropriate treatment duration for sling exercises with local vibration stimulation is lacking. Objects: The aim of this study was to investigate the effects of sling exercise with vibration on shoulder range of motion (ROM), muscle strength, pain, and dysfunction in patients with a medical diagnosis of shoulder joint injury. Methods: Twenty-two patients were randomly assigned to the experiment and control groups. Six sling exercises with and without 50 Hz vibrations were applied in the experiment and control groups, respectively. Each exercise consisted of 3 sets of 5 repetitions performed for 6 weeks. The assessment tools used included shoulder joint range of motion, muscle strength, pain level, and shoulder pain and disability index for functional disability. We conducted re-evaluations before and 3 and 6 weeks after intervention. The changes in the measurement variables were analyzed and compared between the two groups. Results: The ROM of the external rotation of the shoulder joint had a significant interaction between the group and the measurement point (F=3.652, p<.05). In both groups, we found a significant increase in external rotation angle between the measurement points (p<.05). The flexor strength of the shoulder joint significant interaction between the group and the measurement point (F=4.247, p<.05). Both the experiment (p<.01) and control groups (p<.05) showed a significant increase in shoulder flexor strength at the measurement points. After 6 weeks of the interventions, both the groups showed significantly improved VAS (p<.01), SPADI (p<.01), and orthopedic tests (p<.01). However, there was no significant difference between the group and the measurement point in terms of the clinical outcomes observed. Conclusion: The sling exercise with local vibration of 50 Hz affected the external rotation of the shoulder range of motion and improved shoulder flexor strength in the patients with shoulder injuries. Therefore, we propose the use of the sling exercise intervention with vibration in the exercise rehabilitation of patients with shoulder joint injuries.

운동 후 극저온 냉각치료 적용이 운동유발성 근육 손상의 최대등척성근력과 관절가동범위에 미치는 효과 (The effect of whole-body cryotherapy intervention after an exercise on MVIC and ROM of EIMD)

  • Background: The purpose of this study was to investigate the effects of WBC (whole-body cryotherapy)on the MVIC (maximal voluntary isometric contraction)and active ROM (range of motion) recovery of after EIMD (exercise-induced muscle injury). Design: Randomized Controlled Trial. Methods: Thirty subjects who are student in their 20s at a university participated in this study, these subjects were assigned into three groups, a control group (n=10), experiment group I(n=10), and experiment group II (n=10). The subjects in the experimental group II were intervened by WBC (-130℃, 3 minutes) before induced EIMD, the experimental group were intervened by WBC (-130℃, 3 minutes) after induced EIMD, and the control group weren't by any intervened after induced EIMD. Results: First, In the comparison of the MVIC, there were significant variations with the lapse the time in three groups (p<.001) and there was a significant interaction of time and group (p<.001). In the among group comparison, the MVIC of experimental group II was significantly larger than those of other groups (p<.001). Second, In the comparison of the active extension angle, there were significant variations with the lapse the time in three groups (p<.001) and there was a significant interaction of time and group (p<.001). In the among group comparison, the active extension angle of experimental group II was significantly smaller than those of other groups (p<.001). Third, In the comparison of the active flexion angle, there were significant variations with the lapse the time in three groups (p<.01) and there was a significant interaction of time and group (p<.001). In the among group comparison, the active flexion angle of experimental group II was significantly larger than those of other groups (p<.05). Conclusion: The above results revealed that the WBC intervention after an exercise had a positive effect of muscle function after EIMD. Therefore we can consider the WBC as a considerable intervention method to prevent or reduce an exercise injury.

Kinect Sensor 기반의 개인 맞춤형 운동 처방 시스템 개발 (Development of Personalized Exercise Prescription System based on Kinect Sensor)

  • 본 연구는 Kinect Sensor 기반의 개인 맞춤형 운동 처방 시스템을 개발하고, 개발 시스템의 사용성을 평가하는 것이다. 개발 시스템을 개인 맞춤형 운동 처방 시스템을 모션캡쳐 도구로서 이용할 수 있을지에 대한 가능성 검증을 위해서 스마트 거울 시스템에 부착된 키넥트 센서(Kinect sensor)에서 측정된 인체 움직임 데이터와 적외선 모션캡쳐 장비에서 측정된 인체 움직임 데이터를 비교하여 타당성과 신뢰성을 분석하였다. 타당성 검증 결과 상관계수 r=0.871~0.919로 높은 양의 상관성을 보였고, 예측가능정도가 88%로 높게 나타났다. 신뢰성 검증 결과 r=0.743~0.916 높은 양의 상관성을 보였고, 반복 측정에 대한 일관성도 ICC=0.937로 매우 높게 나타났다. 결론적으로 본 연구에서 개발한 키넥트 센서기반의 운동 처방 시스템에서 인간 골격에 대한 특징 벡터를 통한 관절의 가동범위 평가 및 자세측정평가가 운동 처방을 제공하는데 있어서 하나의 기준이 될 수 있다는 가능성을 보여주었다. 향후 병원, 임상시험센터, 스포츠센터 등의 운동처방사 혹은 물리치료사, 퍼스널 트레이너들에게 전문성 제고에 도움을 줄 수 있을 것으로 사료된다.

12주간 필라테스 매트 운동이 임산부의 신체구성, 요부근력, 요통 및 임신 스트레스에 미치는 영향 (The Effects of 12 Weeks Pilates Mat Exercise on Body Composition, Lumbar Muscle Strength, Low Back Pain and Pregnancy Stress in Pregnant Women)

  • 임신 기간 중 임산부는 체중 증가 및 근골격계 변화로 요통과 골반 통증을 유발한다. 필라테스는 유, 무산소성 복합운동으로 자세 교정 및 근 기능을 강화시키는 것으로 보고되고 있다. 따라서 이 연구는 필라테스 매트 운동이 임산부의 신체구성, 요부근력, 요통 및 임신 스트레스에 미치는 영향을 규명하는데 목적이 있다. 본 연구를 수행하기 위해 임신 중기(16~25주) 임산부 19명을 대상으로 필라테스 매트 운동 실험집단(n=11)과 비교집단(n=8)으로 구분하였으며, 실험집단은 하루 60분, 주 2회 12주간 필라테스 매트 운동을 실시하였다. 신체구성 중 체수분량, 체지방량, 체질량지수, 체지방률 및 기초대사량에는 집단 간 변화가 없는 것으로 나타난 반면 실험집단에서는 골격근량에서 유의한 증가가 나타났다. 또한 요부근력의 경우 엉덩이관절 신전근과 요추 신전 가동범위는 필라테스 매트 운동을 수행한 집단에서 유의하게 증가하였다. 요통 수준과 임신 스트레스 수준을 분석한 결과 집단 간 차이는 없는 것으로 나타났다. 따라서 필라테스 매트 운동은 임산부의 근육량을 증가시키고, 요부근력에는 긍정적인 영향을 나타내어 임산부에게 안전하게 적용할 수 있는 운동이라고 할 수 있다.

테이핑과 손목 안정화 운동의 병행훈련이 출산 후 여성의 손목건강과 삶의 질에 미치는 효과 (Effect of Wrist Stabilization Exercise Combined with Taping on Wrist Health and Quality of Life in Postpartum Women with Wrist Pain)

  • 본 연구는 4주간 테이핑을 병행한 손목 안정화 운동이 출산 후 여성의 손목건강과 삶의 질에 미치는 효과를 규명하기 위하여 실시되었다. 단일맹검 및 사전 무작위 임상시험으로 진행되었으며, 17명의 손목통증 환자는 무작위로 실험군(n=8)과 대조군(n=9)으로 배정되었다. 모든 대상자는 손목에 테이핑을 적용하였다. 추가적으로 실험군은 손목 안정화 운동을 하루 2번, 주 5회, 4주간 시행하였고, 대조군은 같은 기간 동안 관절가동범위 운동을 시행하였다. 통증은 시각사상척도(VAS)를 사용하여 측정하였고, 기능장애는 Disabilities of the Arm, Shoulder and Hand (DASH), 삶의 질은 Short-Form 36 items (SF-36)을 사용하였다. 훈련 후 실험군이 대조군에 비해 통증과 기능장애, 삶의 질이 유의하게 개선되었다(p<.05). 본 연구는 테이핑을 병행한 손목 안정화 운동이 출산 후 여성의 손목 건강과 삶의 질을 효과적으로 향상시킴을 증명하였다.

키네시오 테이프의 장력 차이가 젊은 대학생들의 발목 근력, 관절가동범위 및 균형에 미치는 영향 (Effect of tension difference of kinesio tape applied to the tibialis anterior muscle on muscle strength, joint range of motion, and balance of the ankle joint in young college students)

  • Background: The purpose of this study was to investigate the effects of differences in the tension of Kinesio tape applied to the tibialis anterior muscle on muscle strength, joint range of motion and balance of ankle joint in young adults. Design: Randomized Controlled Trial. Methods: 41 young college students participated in this study. And the subjects were allocated randomly to two group. The experimental group had a tension difference (10%G, n=20). And there was no difference in tension in the control group (0%G, n=21). Jtech power track II, goniometer, and Biorescue were used to measure muscle strength, joint range of motion and balance. The paired t-test was performed to examine the differences within the groups before and after taping, and the independent t-test was performed to examine the differences between the groups. Result: 1) As a result of measuring muscle strength of each group, all measurements except 0%G of plantar flexion showed insignificant results. 2) Measurement of joint range of motion each group showed that neither 0%G nor 10%G was significant. 3) As a result of measuring the baladncing ability of each group there was a significant decrease in length, and average speed of the balancing capability (p<.05). 4) Comparisons between groups showed significant differences in the area among balance capability (p<.05). Conclusions: Based on these findings, the tension difference of Kinesio tape applied to the tibialis anterior muscle shows little effect on the muscle strength, range of motion, and balance of ankle joints. Given the current lack of research on the tension of kinesio tapes many studies are needed in the future to establish clear theories.
